Investigator initiated trial (IIT) represents a primary format for clinical research in traditional Chinese medicine (TCM). As key implementation sites for TCM-based IIT targeting malignant tumors, western medicine institutions often face unique challenges in conducting such studies, which limit their feasibility and standardization. This paper reviews the registration status of TCM-based IIT for malignancies conducted in western medical institutions and analyzes key difficulties, including complex project initiation and management processes, limited TCM knowledge and skills among western medicine physicians, and relatively low patient acceptance of TCM. From a practical perspective, the study proposes several optimization strategies. These include improving the review and management mechanisms of TCM-related IIT within western medical institutions, establishing multidisciplinary clinical research teams that integrate TCM and western medicine, and enhancing investigators' training in TCM theory and clinical skills. Additionally, the study suggests standardizing IIT operational procedures, objectifying the collection of TCM diagnostic information, refining subject recruitment methods, and increasing TCM involvement in patient follow-up and management. These investigator-oriented, TCM-featured, and operable strategies aim to promote the high-quality development of TCM-based IIT in western medicine institutions and enhance the clinical application of TCM.

ObjectiveTo investigate the immunomodulatory effect of polysaccharides from Astragali Radix-Angelicae Sinensis Radix herb pair(Qi-gui polysaccharides) on lipopolysaccharide(LPS)-induced RAW264.7 macrophages and to characterize the structure of the active component Qi-gui homogeneous polysaccharide(AAPS-4a), and evaluate its protective effect on ulcerative colitis(UC). MethodsThe effects of six Qi-gui polysaccharides(0.01-100 mg·L-1) on the proliferation of RAW264.7 cells were assessed by cell proliferation and activity assay(CCK-8), and enzyme-linked immunosorbent assay(ELISA) was used to investigate the effects of the six polysaccharides(3, 10 mg·L-1) on the secretion levels of tumor necrosis factor(TNF)-α, interferon(IFN)-β, and nitric oxide(NO) in LPS-induced RAW264.7 cells. After screening for active polysaccharides, high-performance size-exclusion chromatography(HPSEC) was used to determine its homogeneity and relative molecular weight, then its characteristic functional groups were identified by Fourier transform infrared spectroscopy(FT-IR), monosaccharide composition was analyzed by high performance liquid chromatography(HPLC), methylation analysis combined with gas chromatography-mass spectrometry(GC-MS) was performed to determine the types and linkage modes of sugar residues, and one- and two-dimensional nuclear magnetic resonance(NMR) were used to identify the sugar residue composition and configuration of the active polysaccharide. Finally, experimental animals were divided into the normal group, model group, AAPS-4a low-dose group(50 mg·kg-1), AAPS-4a high-dose group(100 mg·kg-1), and sulfasalazine(SASP) group (75 mg·kg-1). Except for the normal group, the acute UC mouse model was induced using 3.5% dextran sulfate sodium salt(DSS). Each treatment group was administered the corresponding dose via oral gavage for 7 days, and changes in body weight were recorded. After treatment, the spleen index and disease activity index(DAI) score were calculated, TNF-α and interleukin-6(IL-6) levels in the serum were detected by ELISA, and histopathological changes in colon tissue were observed by hematoxylin-eosin(HE) staining. ResultsAt the cellular level, AAPS-4a exhibited a dose-dependent inhibition of LPS-induced increases in TNF-α, IFN-β, and NO levels(P<0.01). Structural characterization of AAPS-4a revealed that it was a homogeneous polysaccharide with a relative molecular weight of 7.6×103 Da, consisting of mannose(Man), glucose(Glc), and galactose(Gal) in a molar ratio of 1.3∶23.9∶1.0. It was primarily composed of five sugar residues of 1,6-α-D-Glcp, T-α-D-Glcp, 1,3-β-D-Galp, 1,4-α-D-Manp, and 1,2-α-D-Galp. In vivo experiments showed that compared with the normal group, the model group demonstrated markedly increased DAI score and spleen index, significantly reduced colon length, and significantly elevated levels of TNF-α and IL-6(P<0.01). Compared with the model group, the AAPS-4a high-dose group significantly reduced the DAI score and spleen index, as well as TNF-α and IL-6 levels, and improved colonic atrophy(P<0.05, P<0.01). Pathological observations showed that AAPS-4a significantly inhibited inflammatory cell infiltration in colon tissue and alleviated pathological damage. ConclusionAAPS-4a, a neutral homogeneous polysaccharide composed of 1,6-α-D-Glcp, T-α-D-Glcp, 1,3-β-D-Galp, 1,4-α-D-Manp and 1,2-α-D-Galp, is identified as a key bioactive component contributing to the anti-UC effect of the Qi-gui herb pair. Its immunoregulatory and anti-UC properties suggest its potential as a therapeutic agent for UC.

[Objective] To explore the number of examined lymph nodes (ELN) and positive lymph node ratio (LNR) in the prediction of 5-year and 10-year overall survival (OS) and cancer-specific survival (CSS) of prostate cancer (PCa) patients, so as to provide reference for clinical practice. [Methods] Information of PCa patients screened in the Surveillance, Epidemiology and End Results (SEER) database during 2010-2020 were analyzed. A total of 1842 PCa patients were assigned to the training set (n=1290) and validation set (n=552) in a 7∶3 ratio with R 4.3.0 software. Significant factors in the multivariate Cox proportional risk regression model were adjusted, restricted cubic spline plots (RCS) were plotted, the optimal cut-off values of ELN and LNR were determined, and the 5-year and 10-year OS and CSS were analyzed with restricted mean survival time (RMST). [Results] Multivariate Cox analysis showed that there was a 2.9% reduction in the risk of death with an increase of 1-unit ELN and a 3.1% reduction in the risk of cancer-specific death. There was a 481.4% increase in the risk of death with a 1-unit increase in LNR and a 667.5% increase in the risk of cancer-specific death. The risk of overall death and cancer-specific death in ELN and PCa patients showing a non-linear relationship (P<0.001), while in the LNR and PCa patients showing a linear relationship (P>0.05). RMST results showed that the optimal ELN range for evaluating OS was 12-29, the optimal ELN range for assessing CSS was 12-25, LNR>0.152 indicated poor prognosis. [Conclusion] We have clarified the range of ELN and LNR, which can provide reference for the clinical precision diagnosis and treatment of PCa.

Chinese Journal of Trauma 2024;40(5):385-396
Traumatic supraorbital fissure syndrome (TSOFS) is a symptom complex caused by nerve entrapment in the supraorbital fissure after skull base trauma. If the compressed cranial nerve in the supraorbital fissure is not decompressed surgically, ptosis, diplopia and eye movement disorder may exist for a long time and seriously affect the patients′ quality of life. Since its overall incidence is not high, it is not familiarized with the majority of neurosurgeons and some TSOFS may be complicated with skull base vascular injury. If the supraorbital fissure surgery is performed without treatment of vascular injury, it may cause massive hemorrhage, and disability and even life-threatening in severe cases. At present, there is no consensus or guideline on the diagnosis and treatment of TSOFS that can be referred to both domestically and internationally. To improve the understanding of TSOFS among clinical physicians and establish standardized diagnosis and treatment plans, the Skull Base Trauma Group of the Neurorepair Professional Committee of the Chinese Medical Doctor Association, Neurotrauma Group of the Neurosurgery Branch of the Chinese Medical Association, Neurotrauma Group of the Traumatology Branch of the Chinese Medical Association, and Editorial Committee of Chinese Journal of Trauma organized relevant experts to formulate Chinese expert consensus on the diagnosis and treatment of traumatic supraorbital fissure syndrome ( version 2024) based on evidence of evidence-based medicine and clinical experience of diagnosis and treatment. This consensus puts forward 12 recommendations on the diagnosis, classification, treatment, efficacy evaluation and follow-up of TSOFS, aiming to provide references for neurosurgeons from hospitals of all levels to standardize the diagnosis and treatment of TSOFS.

Chinese Medical Journal 2023;136(17):2066-2076
BACKGROUND:
Glutamine synthetase (GS) and arginase 1 (Arg1) are widely used pathological markers that discriminate hepatocellular carcinoma (HCC) from intrahepatic cholangiocarcinoma; however, their clinical significance in HCC remains unclear.
METHODS:
We retrospectively analyzed 431 HCC patients: 251 received hepatectomy alone, and the other 180 received sorafenib as adjuvant treatment after hepatectomy. Expression of GS and Arg1 in tumor specimens was evaluated using immunostaining. mRNA sequencing and immunostaining to detect progenitor markers (cytokeratin 19 [CK19] and epithelial cell adhesion molecule [EpCAM]) and mutant TP53 were also conducted.
RESULTS:
Up to 72.4% (312/431) of HCC tumors were GS positive (GS+). Of the patients receiving hepatectomy alone, GS negative (GS-) patients had significantly better overall survival (OS) and recurrence-free survival (RFS) than GS+ patients; negative expression of Arg1, which is exclusively expressed in GS- hepatocytes in the healthy liver, had a negative effect on prognosis. Of the patients with a high risk of recurrence who received additional sorafenib treatment, GS- patients tended to have better RFS than GS+ patients, regardless of the expression status of Arg1. GS+ HCC tumors exhibit many features of the established proliferation molecular stratification subtype, including poor differentiation, high alpha-fetoprotein levels, increased progenitor tumor cells, TP53 mutation, and upregulation of multiple tumor-related signaling pathways.
CONCLUSIONS
GS- HCC patients have a better prognosis and are more likely to benefit from sorafenib treatment after hepatectomy. Immunostaining of GS may provide a simple and applicable approach for HCC molecular stratification to predict prognosis and guide targeted therapy.

Objective:To investigate the safety and effectiveness of robot-assisted total knee arthroplasty (TKA).Methods:From August 2021 to March 2022, the data of 69 patients who received robot-assisted TKA for inflammatory disease of knee in 6 hospitals were collected, including 24 males and 45 females, aged 52±6 years (range, 46-72 years), including 53 cases of knee osteoarthritis,10 cases of rheumatoid arthritis, and 6 cases of traumatic arthritis. Imaging data examination of patients was completed according to the multicenter specification. Preoperatively, the patient's CT data were extracted before surgery and a personalized prosthesis positioning plan was designed by the robotic system to determine the prosthesis size, lower limb force lines, and femoral and tibial osteotomy volumes; intraoperatively, the osteotomy template was robotically controlled to complete the osteotomy according to the prosthesis positioning plan, and the intraoperative osteotomy volumes were measured to verify the accuracy of the robot-assisted TKA osteotomy with the preoperative planning. Postoperatively, the lateral angle of the distal femur, the medial angle of the proximal tibia and the hip-knee-ankle angle were measured in the coronal plane according to radiographs; the postoperative outcome was evaluated 3 months after operation, using the American Knee Society score (KSS), including the KSS knee score and functional score. Results:The operative time was 97.3±2.3 min (range, 80-110 min) and the amount of bleeding was 320.0±6.2 ml (range, 300-350 ml) in 69 patients. Three months after operation, the mean range of motion of knee joint was increased from 82.2°±1.1° before surgery to 119.7°±0.8° after surgery ( t=27.65, P<0.001), and the line of force of lower limb was improved from 160.9°±0.5° before surgery to 178.0°±0.2° after surgery ( t=32.03, P<0.001). KSS-knee score increased from 54.8±0.7 points before operation to 85.0±0.5 points after operation, and KSS-functional score increased from 56.5±0.7 points before operation to 85.9±0.4 points after operation, the difference was statistically significant ( t=35.45, 36.58, P<0.001). The proportion of patients with intraoperative femoral and tibial osteotomies within 2 mm osteotomy error compared with preoperative planning was 97% for the lateral tibial plateau, 100% for the medial tibial plateau, 100% for the lateral distal femur, 99% for the medial distal femur, 93% for the lateral posterior femoral condyle, and 100% for the medial posterior femoral condyle; The proportion of patients with postoperative anteroposterior X-ray measurement angle error within 3° was: 100.0% for the distal lateral femoral angle, 100.0% for the proximal medial tibial angle, and 100% for the hip-knee-ankle angle. No complications occurred in all patients except for one case in which fat liquefaction occurred in the postoperative wound. Conclusion:Robot-assisted TKA is a safe and effective surgical method for the treatment of inflammatory disease of knee with accurate prosthesis installation and good postoperative recovery of lower limb alignment.
