1.Approach for Advanced Cancer Patients with Bone Metastases by the Bone Metastasis Board: A Single-institution Retrospective Study

Prevention, early diagnosis, and early treatment of skeletal-related events (SREs) are important in the treatment of potential or current cases of bone metastasis. In August 2020, our hospital established the bone metastasis team and the bone metastasis board (BMB) started actively engaging in activities aimed at improving the outcome of bone metastasis. We retrospectively examined whether a combined modality therapy started in the diagnosis of bone metastases could prevent the onset of SREs and whether it could prolong survival and improve activities of daily living. The 75 advanced cancer patients who underwent BMB at our hospital from August 1, 2020 to July 31, 2022 were divided into two groups according to when BMB performed before and after SREs for comparative analysis. Numerical Rating Scale improved, however Performance Status did not improve in both groups, and there was no difference in survival between the both groups (15.3 vs. 9.0 months, HR: 0.74, 95%; CI: 0.42–1.29, p=0.29). In conclusion, patients who suffered from SREs from the time of bone metastasis diagnosis were treated early. However, the incidence of SREs after BMB in our hospital was 22.6%, and it is necessary to actively work to prevent SREs in the future.
2.Effect of Aging on Thermoregulatory and Cardiovascular Changes during Bathing in the Elderly

Background: Aging affects thermoregulation and can potentially cause cardiovascular changes during bathing in the elderly, possibly leading to accidents and, in worst cases, drowning. However, adequate data on this subject are lacking. Therefore, the purpose of this study was to clarify the effect of aging on thermoregulatory and cardiovascular changes during bathing in the older versus younger adults the relationship between body temperature and thermoregulatory and cardiovascular changes. Methods: Ten young (mean age: 20.4 years) and 10 elderly (mean age: 69.7 years) participants were asked to bathe for 15 min using water at 41°C. Tympanic temperature (Tty), skin blood flow (SkBF), sweating rate (SR), blood pressure, heart rate (HR), and double product (DP) were measured. In addition, we measured subjective psychological changes during bathing. Results: The results showed significant changes in SkBF, SR, and HR in the young participants rather than in the elderly participants. However, in the elderly participants, significant change was noted in systolic blood pressure (SBP). Conclusion: These results demonstrate changes in thermal adaptation and SBP owing to structural and functional progression due to aging.

4.Cross-sectional survey of depressive symptoms and suicide-related ideation at a Japanese national university during the COVID-19 stay-home order.

BACKGROUND:
We aimed to estimate the prevalence of depressive symptoms as well as suicide-related ideation among Japanese university students during the stay-home order necessitated by the coronavirus disease 2019 pandemic in Japan, and offer evidence in support of future intervention to depression and suicide prevention strategies among college and university students.
METHODS:
The data for this cross-sectional study were derived from the Student Mental Health Survey conducted from May 20 to June 16, 2020 at a national university in Akita prefecture. Among the 5111 students recruited, 2712 participated in this study (response rate, 53%; mean age ± standard deviation, 20.5 ±3.5 years; men, 53.8%). Depressive symptoms were identified by using the Patient Health Questionnaire-9 (PHQ-9).
RESULTS:
The prevalence of moderate depressive symptoms based on a PHQ-9 score ≥10 and suicide-related ideation based on question 9 of PHQ-9 ≥1, which encompasses thoughts of both suicide and self-harm, was 11.7% and 6.7%, respectively. Multivariable logistic regression analyses showed that risk factors for depression included being a woman, smoking, alcohol consumption, and social network communication using either video or voice. For suicide-related ideation, alcohol consumption was the only risk factor. Exercise and having someone to consult about worries were associated with decreased risk of both depressive symptoms and suicide-related ideation.
CONCLUSIONS
Negative lifestyles of smoking and drinking, and being a woman, may be important risk factors for depressive symptoms, whereas exercise and having someone to consult about worries may be protective factors.

5.Comparison of Cardiovascular and Psychological Responses to a 20-min Bath in Summer and Winter

Background: This study aimed to clarify the effects of bathing on cardiovascular and psychological responses during winter and summer, focusing on three different time points (immediately after entering the bathtub, during bathing, and immediately after leaving the bathtub). Methods: Ten young (29.6 ± 1.5 years) individuals were asked to bathe for 20 min with 40℃ hot water in summer and with 42℃ hot water in winter. The room temperature and humidity in winter were 20.1℃ and 56.7%, while those in summer were 27.0℃ and 58.3%, respectively. Tympanic temperature (Tty) was measured using a thermistor thermometer; systolic blood pressure (SBP) and diastolic blood pressures (DBP) and heart rate (HR) were measured using an auto-sphygmomanometer, and double product (DP) was calculated using the value of HR multiplied by SBP value. Subjective thermal and comfortable sensations were evaluated using a seven-point rating scale. Results: Results showed a significant increases in SBP values in both seasons immediately after bathing. In winter, DP increased significantly after bathing. Meanwhile, no significant differences were observed in all parameters immediately after bathing between winter and summer. On the contraly, significant differences were observed in Tty, HR, and DP value between winter and summer during bathing. HR and DP values in winter were significantly lower than those in summer immediately after leaving the bathtub. A relationship between thermal sensation and comfortable sensation was observed in both seasons, but the strength of the relationship was more relevant in summer than in winter. Conclusion: The effects of bathing in 42℃ hot water on the cardiovascular system in winter are greater than those in 40℃ hot water in summer. The high temperature of the bath water may not produce as much discomfort during winter than that during summer, therefore bathing time may be longer.
6.Changes in Body Temperature and Arterial Blood Flow Rate of 42°C Bathing Comparison of a Hot Spring Plunge Bath and Home Bathtub Bath

【Introduction】One of the effects of the hot spring provides worm temperature. This effect raises temperature, and temperature control function operates and causes increase of the bloodstream. This time, these changes examined the thing by the size of the bathtub and the spring quality of the hot spring. 【Subjects and Methods】The subjects were 10 healthy adult men (mean age: 25.2 years). They bathed for 10 min in bathtubs at 42°C. The enforcement used plunge bath (approximately 1,700 L: simple alkaline hot spring) and home bathtub (approximately 300 L: hot water, 0.1% artificially chlorinated spring). Measurement item of the maximum arterial blood flow rate using the Ultrasonic Rheometer Smart Doppler 45, deep body temperature using the deep body temperature monitor core temperature CM-210, I compared each value 10 min during the bathing, and during a 10-min, 20-min, 30-min resting period after bathing, furthermore, I found the conjugation on each condition resting period after bathing. 【Result】The rise in deep body temperature and maximum arterial blood flow rate showed the result that a hot spring of the plunge bath was more meaningful than the value of the home bathtub after 10 min of bathing. The deep body temperature of the hot spring of the plunge bath significantly rose from bathing 3 min after. In deep body temperature with the resting period after bathing, in the hot spring of the plunge bath, a meaningful rise was maintained in hot water 13 min for population chloride spring 16 min of the home bathtub for 15 min. 【Discussion】In thinks that a population spring let you maintain a temperature rise that it disturbs a drop of the water temperature by abundant quantity of water in the plunge bath that hot spring plunge bath had a bigger deep body temperature rise, maximum arterial blood flow rate than home bathtub, and the deep body temperature rise in the home bathtub was continued.
7.Ultrasound cardiography examinations detect victims' long-term realized and potential consequences after major disasters: a case-control study.

BACKGROUND:
An increase in cardiovascular diseases has been reported following major disasters. Previous work has shown that ultrasonographic findings from ultrasound cardiography examination (UCG) increased until the 44th month after the tsunami caused by the Great East Japan Earthquake. The present study conducted UCG among victims in the tsunami disaster area and investigated the frequency of disaster-related cardiovascular diseases and changes over time until the 55th month after the disaster.
METHODS:
The subjects were residents of temporary housing complexes and neighboring housing in Watari-gun, Miyagi Prefecture, Japan. There were 207 subjects in the 18th month, 125 in the 30th month, 121 in the 44th month, and 106 in the 55th month after the disaster. Data were collected through UCG and self-report questionnaire.
RESULTS:
Significant changes were observed among subjects with clinical findings from the UCG, which increased over the study period-from 42.0 to 60.8, 72.7, and 73.6% beginning in the 18th month after the disaster (p < 0.0001).
CONCLUSIONS:
It is possible that the UCG can become a useful examination to visualize the potential impact of a major disaster on the cardiac function of victims. Victims with clinical findings continued increasing not only during the acute phase after a disaster but also in the long term. We therefore need to keep this in mind, and note that it is important to establish a support system to control cardiovascular diseases from the early stage of disaster.

8.A Safe Surgical Procedure for Old Distractive Flexion Injuries of the Subaxial Cervical Spine.

STUDY DESIGN: Retrospective review. PURPOSE: To describe a safe and effective surgical procedure for old distractive flexion (DF) injuries of the subaxial cervical spine. OVERVIEW OF LITERATURE: Surgical treatment is required in old cases when a progression of the kyphotic deformity and/or persistent neck pain and/or the appearance of new neurological symptoms are observed. Since surgical treatment is more complicated and dangerous in old cases than in acute distractive-flexion cases, the indications for surgery and the selection of the surgical procedure must be carefully conducted. METHODS: To identify a safe and effective surgical procedure, the procedure selected, reason(s) for its selection, and associated neurological complications were investigated in 13 patients with old cervical DF injuries. RESULTS: No neurological complications were observed in nine patients (DF stage 2 or 3) who underwent the anterior-posterior-anterior (A-P-A) method and two patients (DF stage 1) who underwent the posterior method. It was initially planned that two patients (DF stage 2) who underwent the P-A method would be treated using the Posterior method alone; however, anterior discectomy was added to the procedure after the development of a severe spinal cord disorder. CONCLUSIONS: The A-P-A method (anterior discectomy, posterior release and/or partial facetectomy, reduction and instrumentation, anterior bone grafting) is considered to be a suitable surgical procedure for old cervical DF injuries.

9.Seatbelt paths of the pregnant women sitting in the rear seat of a motor vehicle.

PURPOSESeatbelt use during pregnancy is important to improve maternal and fetal survival after motor vehicle collisions. However, because the rear seatbelt of a motor vehicle tends to make contact with the neck, even if it is adequately used, some pregnant women sitting in the rear seat opt not to fasten the belt. The purpose of this study is to explore seatbelt-neck contact for pregnant women sitting in the rear seat of a motor vehicle.
METHODSWe carried out an anthropometric study. Japanese women who were ≥30 weeks pregnant (n = 12) sat in the left side of the rear seat of a typical mid-size passenger sedan and fastened the seatbelt. Seating posture was investigated by measuring the coordinates of the anthropometric data points of the pregnant women (head, shoulder, hip joint, and knee joint). The belt path was analyzed by measuring the clearance between the belt and the sternum or navel.
RESULTSAmong the 12 pregnant women at 33.9 week ± 3.3 week gestation, the shoulder belt deviated to the right side and subsequently contacted to the neck in four pregnant women (Contact group). The height of the Contact group was significantly shorter than that of Non-contact group (152.3 cm ± 3.0 cm vs. 159.0 cm ± 3.3 cm, p = 0.008). Regarding the relative position of the seatbelt to the subject's body, the distances from the top of the sternum to the center of the shoulder belt were significantly shorter in Contact group (3.9 cm ± 3.5 cm) than that in the Non-contact group (8.0 cm ± 1.6 cm, p = 0.03). However, no significant difference was found for the distance from the umbilicus to the center of the lap belt.
CONCLUSIONOur findings show that because of short height and late term of pregnancy with protrusion of the abdomen, the shoulder belt deviates to the right or left, avoiding the protruded uterus, and subsequently makes contact with the neck. Seatbelt systems for rear seats need to be developed to improve passenger safety, especially for pregnant women.
10.Changes in Saliva Cortisol Levels and Emotional Assessment after Walking Programs in Komono Town, Mie Prefecture

Komono Town is a well-known spa and health resort in Mie Prefecture. Komono Town has been seeking ways to promote the activities of hot spring area and health resources in surrounding areas. As part of these efforts, Komono Town has developed town-walk programs to promote the health of local residents. In this study, focusing on effect of walking on relaxation, we compared levels of stress hormones and emotional scores obtained before and after walking. After giving their informed consent, adult participated in two walking programs, each for a distance of approximately 7 km. Salivary cortisol levels were measured before and after intervention. In addition, the Mood Check List-Short form. 2 (MCL-S.2) and a Visual Analog Scale (VAS) were used to rate emotions before and after intervention. In both programs, walking tended to lower levels of salivary cortisol than resting. Low levels of cortisol, an adrenocortical hormone released during a state of predominantly sympathetic nervous activity, are thought to reflect mental relaxation; our result implies that the walking program enhanced relaxation in subjects. In addition, both MCL-S.2 and VAS rating showed that the subjects tended to feel better, more relaxed and less anxious after intervention. These findings suggest that the walking programs are beneficial human body thorough, for example, enhanced relaxation.
