OBJECTIVES:
To investigate the incidence of extrauterine growth retardation (EUGR) and its risk factors in very preterm infants (VPIs) during hospitalization in China.
METHODS:
A prospective multicenter study was performed on the medical data of 2 514 VPIs who were hospitalized in the department of neonatology in 28 hospitals from 7 areas of China between September 2019 and December 2020. According to the presence or absence of EUGR based on the evaluation of body weight at the corrected gestational age of 36 weeks or at discharge, the VPIs were classified to two groups: EUGR group (n=1 189) and non-EUGR (n=1 325). The clinical features were compared between the two groups, and the incidence of EUGR and risk factors for EUGR were examined.
RESULTS:
The incidence of EUGR was 47.30% (1 189/2 514) evaluated by weight. The multivariate logistic regression analysis showed that higher weight growth velocity after regaining birth weight and higher cumulative calorie intake during the first week of hospitalization were protective factors against EUGR (P<0.05), while small-for-gestational-age birth, prolonged time to the initiation of total enteral feeding, prolonged cumulative fasting time, lower breast milk intake before starting human milk fortifiers, prolonged time to the initiation of full fortified feeding, and moderate-to-severe bronchopulmonary dysplasia were risk factors for EUGR (P<0.05).
CONCLUSIONS
It is crucial to reduce the incidence of EUGR by achieving total enteral feeding as early as possible, strengthening breastfeeding, increasing calorie intake in the first week after birth, improving the velocity of weight gain, and preventing moderate-severe bronchopulmonary dysplasia in VPIs.

2.Kinesio Taping combined with electroacupuncture for the treatment of patellofemoral disorder in middle-aged and elderly exercise enthusiasts.

OBJECTIVE:
To observe the clinical efficacy of Kinesio Taping combined with electroacupuncture in the treatment of patellofemoral disorder in middle-aged and elderly exercise enthusiasts.
METHODS:
From April 2018 to April 2020, 62 middle-aged and elderly exercisers with patellofemoral disorder were selected and divided into observation group and control group. In the observation group, there were 30 cases:12 males and 18 females, aged from 35 to 66 years old, with an average age of (48.32±6.80) years old. The observation group was treated with Kinesio Taping and electroacupuncture . In the control group, there were 32 cases:12 males and 20 females, aged from 35 to 67 years old, with an averageage of (48.67±8.13) years old. The patients in the control group was treated with knee electroacupuncture. All the patients in both groups were treated once every other day, 3 times a week, for a total of 4 weeks. The VAS pain score and knee Lysholm score before and after treatment were compared between the two groups before and after treatment, and statistical analysis was conducted.
RESULTS:
In the control group, 2 patients were not satisfied with the curative effect after 2 weeks of treatment and gave up treatment, while the remaining 60 patients completed all the treatments. After 4 weeks of treatment, the VAS and Lyshlom scores of the control group were 2.73±1.46 and 75.63±8.62 respectively;the VAS and Lyshlom scores of the observation group were 2.02±1.85 and 83.31±5.53 respectively;the VAS and Lyshlom scores of the observation group and the control group were better than those before treatment, and the Lyshlom scores of the observation group were better than those of the control group.
CONCLUSION
Kinesio Taping combined with electroacupuncture is aimed at patellofemoral disorderof middle-aged and elderly sports enthusiasts. It has little trauma, can relieve pain and has a fast onset speed, which is readily accepted by patients, and promotes the rehabilitation of knee joint function of patients, thus providing a favorable reference for the promotion in clinical application.

OBJECTIVE:
To analyze the influence of bone marrow involvement (BMI) in patients with malignant lymphoma (ML) on laboratory indexes, and evaluate the laboratory markers that can be used to predict/diagnose BMI.
METHODS:
The clinical characteristics and laboratory indexes of 137 ML patients were analyzed retrospectively, from which the indexes of BMI in ML patients was studied. The logistic regression analysis and receiver operating curve (ROC) were used to evaluate independent risk factors and predictors of BMI diagnosis in ML patients.

4.Artificial neural network-based analysis of the safety and efficacy of thrombolysis for ischemic stroke in older adults in Taiwan

Background: The risk and benefit of tissue plasminogen activator (tPA) for aged>80 years with acute
ischemic stroke (AIS) are controversial. In this study, we investigated the safety and efficacy of tPA
in this population and utilized the artificial neural network (ANN) to established outcome predictive
models. Methods: We retrospectively reviewed the stroke registry data of patients with AIS, aged >80
years who arrived at the hospital within 3 hours from the onset of symptoms. The characteristics and
the outcomes, presented as modified Rankin Scale (mRS), and mortality rate at 3 months between the
tPA-treated and non-tPA groups were analyzed. An ANN algorithm was applied to establish predictive
models. Results: A total of 80 patients aged>80 years with AIS were identified, and 49 of them received
tPA. After adequate training, our ANN models accurately predicted the outcomes with the area under
the receiver operating characteristic curves of 0.974, and a low error to predict the mRS score at 3
months. After applying our prediction model to those in the non-tPA group, we demonstrated the
potential benefits in those patients if they had undergone tPA therapy.
Conclusions: Our results show that ANN can be a potentially useful tool for predicting the treatment
outcomes of tPA. Such novel machine learning-based models may help with therapeutic decision
making in clinical settings.

BACKGROUND:
Albuvirtide is a once-weekly injectable human immunodeficiency virus (HIV)-1 fusion inhibitor. We present interim data for a phase 3 trial assessing the safety and efficacy of albuvirtide plus lopinavir-ritonavir in HIV-1-infected adults already treated with antiretroviral drugs.
METHODS:
We carried out a 48-week, randomized, controlled, open-label non-inferiority trial at 12 sites in China. Adults on the World Health Organization (WHO)-recommended first-line treatment for >6 months with a plasma viral load >1000 copies/mL were enrolled and randomly assigned (1:1) to receive albuvirtide (once weekly) plus ritonavir-boosted lopinavir (ABT group) or the WHO-recommended second-line treatment (NRTI group). The primary endpoint was the proportion of patients with a plasma viral load below 50 copies/mL at 48 weeks. Non-inferiority was prespecified with a margin of 12%.
RESULTS:
At the time of analysis, week 24 data were available for 83 and 92 patients, and week 48 data were available for 46 and 50 patients in the albuvirtide and NRTI groups, respectively. At 48 weeks, 80.4% of patients in the ABT group and 66.0% of those in the NRTI group had HIV-1 RNA levels below 50 copies/mL, meeting the criteria for non-inferiority. For the per-protocol population, the superiority of albuvirtide over NRTI was demonstrated. The frequency of grade 3 to 4 adverse events was similar in the two groups; the most common adverse events were diarrhea, upper respiratory tract infections, and grade 3 to 4 increases in triglyceride concentration. Renal function was significantly more impaired at 12 weeks in the patients of the NRTI group who received tenofovir disoproxil fumarate than in those of the ABT group.
CONCLUSIONS:
The TALENT study is the first phase 3 trial of an injectable long-acting HIV drug. This interim analysis indicates that once-weekly albuvirtide in combination with ritonavir-boosted lopinavir is well tolerated and non-inferior to the WHO-recommended second-line regimen in patients with first-line treatment failure.

6.Safety and efficacy of extending intravenous thrombolysis treatment for acute ischemic stroke in Taiwan

Recombinant tissue plasminogen activator (rt-PA) is the most effective treatment for acute ischemic
stroke and the exclusion criteria of rt-PA has been revised to extend its application. However, in
Taiwan, National Health Insurance (NHI) did not follow the latest international consensus due to
safety concerns. The present study investigated whether extending the application of rt-PA in Taiwan
was safe and effective. The medical records from the Shuang Ho hospital stroke registry between
August 2009 and December 2016 were retrospectively reviewed. Post rt-PA intracranial hemorrhage
(ICH) and modified Rankin Scale (mRS) score at 3-month after stroke were the primary and secondary
outcomes, respectively. Differences were analyzed through Fisher’s exact test and Student’s t test. A
p-value of <0.05 was considered statistically significant. Overall, there were 243 patients categorized
into two groups: NHI exclusion criteria adherence (n = 160) and non-adherence (n = 83). There
was no significant difference in the risk of post rt-PA ICH (12.50% in adherence group, 4.82% in
non-adherence group, p=0.07). Among the non-adherence group, 10 patients breached the latest
international exclusion criteria and none of them experienced post rt-PA ICH. However, among
patients with moderately severe stroke, the odds of mRS < 2 at 3-month were significantly lower in
non-adherence group. This study demonstrated that extending administration of rt-PA in Taiwan was
safe but the functional outcome after moderate stroke was not as favorable as adherence group. Old
age, long onset-to-treatment time and less efficacy of lower dose of rt-PA were the possible factors
for the difference in outcome.
7.Automated Assessment for Bone Age of Left Wrist Joint in Uyghur Teenagers by Deep Learning

Objective To realize the automated bone age assessment by applying deep learning to digital radiography(DR)image recognition of left wrist joint in Uyghur teenagers, and explore its practical ap-plication value in forensic medicine bone age assessment. Methods The X-ray films of left wrist joint after pretreatment, which were taken from 245 male and 227 female Uyghur nationality teenagers in Uygur Autonomous Region aged from 13.0 to 19.0 years old, were chosen as subjects. And AlexNet was as a regression model of image recognition. From the total samples above, 60% of male and fe-male DR images of left wrist joint were selected as net train set, and 10% of samples were selected as validation set. As test set, the rest 30%were used to obtain the image recognition accuracy with an error range in ±1.0 and ±0.7 age respectively, compared to the real age. Results The modelling results of deep learning algorithm showed that when the error range was in ±1.0 and ±0.7 age respectively, the accuracy of the net train set was 81.4% and 75.6% in male, and 80.5% and 74.8% in female, respectively. When the error range was in ±1.0 and ±0.7 age respectively, the accuracy of the test set was 79.5% and 71.2% in male, and 79.4% and 66.2% in female, respectively. Conclusion The combination of bone age research on teenagers' left wrist joint and deep learning, which has high accuracy and good feasi-bility, can be the research basis of bone age automatic assessment system for the rest joints of body.
8.Tamoxifen and the Risk of Parkinson's Disease in Female Patients with Breast Cancer in Asian People: A Nationwide Population-Based Study.

PURPOSE: Whether tamoxifen affects the risk of neurodegenerative disease is controversial. This nationwide population-based study investigated the risk of Parkinson's disease (PD) associated with tamoxifen treatment in female patients with breast cancer using Taiwan's National Health Insurance Research Database. METHODS: A total of 5,185 and 5,592 female patients with breast cancer who did and did not, respectively, receive tamoxifen treatment between 2000 and 2009 were included in the study. Patients who subsequently developed PD were identified. A Cox proportional hazards model was used to compare the risk of PD between the aforementioned groups. RESULTS: Tamoxifen did not significantly increase the crude rate of developing PD in female patients with breast cancer (tamoxifen group, 16/5,169; non-tamoxifen group, 11/5,581; p=0.246). Tamoxifen did not significantly increase the adjusted hazard ratio (aHR) for subsequently developing PD (aHR, 1.310; 95% confidence interval [CI], 0.605–2.837; p=0.494). However, tamoxifen significantly increased the risk of PD among patients followed up for more than 6 years (aHR, 2.435; 95% CI, 1.008–5.882; p=0.048). CONCLUSION: Tamoxifen treatment may increase the risk of PD in Taiwanese female patients with breast cancer more than 6 years after the initiation of treatment.

Deep learning and neural network models have been new research directions and hot issues in the fields of machine learning and artificial intelligence in recent years. Deep learning has made a breakthrough in the applications of image and speech recognitions, and also has been extensively used in the fields of face recognition and information retrieval because of its special superiority. Bone X-ray images express different variations in black-white-gray gradations, which have image features of black and white contrasts and level differences. Based on these advantages of deep learning in image recognition, we combine it with the research of bone age assessment to provide basic datum for constructing a forensic automatic system of bone age assessment. This paper reviews the basic concept and network architectures of deep learning, and describes its recent research progress on image recognition in different research fields at home and abroad, and explores its advantages and application prospects in bone age assessment.
10.Expression and Clinical Significance of Blood IL-37 in Patients with Chronic HBV

Objective To study the expression levels of IL-37 in serum in chronic HBV(CHB)patients,and investigate their clinical significance in CHB patients.Methods The expression levels of IL-37 in serum from 35 CHB patients and 35 health-y controls were quantified by ELISA.This study also tested ALT,AST,HBsAg,HBeAg,HBV DNA,TNF-αand IL-10 in the serum of CHB patients,and also analyzed the relationship between the levels of IL-37 and ALT,AST,HBsAg,HBeAg, HBV DNA,TNF-α,IL-10 levels.Results The expression levels of IL-37 was increased in CHB patients 437.5±152.7 pg/ml compared to healthy donors(63.0±2.1 pg/ml).The difference between the two groups was statistically significant(t=2.45,P=0.017).The levels of IL-37 was found to correlate with ALT(r=0.406,P=0.019)and IL-10(r=0.433,P=0.035)but not AST,HBsAg,HBeAg,HBV DNA and TNF-α.Conclusion This study reveals the levels of IL-37 in CHB patients was higher than healthy controls,and IL-37 may play an important role in liver damage and HBV chronic infection.
