Background: Syphilis, an infectious and chronic disease primarily transmitted through sexual contact, is caused by the spirochete bacterium Treponema pallidum. Although the introduction of penicillin has significantly reduced its incidence, syphilis continues to have a substantial impact on public health. Objective: The purpose of this study was to investigate trends in syphilis and to analyze its epidemiologic, clinical, and serologic characteristics. Methods: We retrospectively reviewed the medical records of 330 patients who tested positive for the Venereal Disease Research Laboratory (VDRL) test, including 192 patients who received a diagnosis of syphilis and who visited the Dermatology Department of Gachon University Gil Medical Center between 2000 and 2023. Statistical analyses were performed to evaluate trends in syphilis incidence and related variable factors, such as sex, age, and serological results. Results: The average age of the 192 syphilis patients was 34.18±15.81 years, with a male-to-female ratio of 1:1.2. The most prevalent age group was 20∼29 years, comprising 71 patients (37.0%). Over time, the incidence of syphilis demonstrated a decreasing trend (p＜0.001). A total of 105 patients (54.7%) were diagnosed with symptomatic syphilis, with a male-to-female ratio of 1.5:1. The proportion of symptomatic syphilis patients tended to decrease with increasing age (p＜0.05). Maculopapular syphilis was the most frequently observed presentation in secondary syphilis (36.2%). The proportion of symptomatic syphilis patients significantly increased with higher VDRL titers (p＜0.001). Conclusion The incidence of syphilis has shown a consistent decline over the past two decades. However, the relatively higher prevalence among younger age groups indicates the need for continued public health efforts.

Background: Varus malalignment is a risk factor for nonunion and mechanical complications in subtrochanteric femoral fractures (SFFs). Although various reduction techniques have been reported to avoid varus malalignment in SFFs, achieving anatomic reduction remains challenging, often resulting in residual varus alignment (RVA) after reduction. This study aimed to investigate the radiographic and clinical outcomes of a novel method resolving RVA by abducting the ipsilateral hip after cephalomedullary fixation with an intramedullary nail and subsequently inserting distal interlocking screws. Methods: This retrospective study, conducted between March 2016 and March 2022, included patients who underwent hip abduction during intramedullary nailing due to RVA. Demographics and fracture patterns (Arbeitsgemeinschaft für Osteosynthesefragen/Orthopedic Trauma Association [AO/OTA]) and types (typical or atypical) were analyzed. Radiographic outcomes included Baumgaertner reduction quality criteria (BRQC), tip-apex distance (TAD), neck-shaft angle (NSA), lateral cortex residual gap, union, and time to union. Clinical outcomes included ambulatory level using the Palmer-Parker Mobility Score (PPM), complications, and reoperation. Results: This study included 45 patients (mean age, 65.8 years; mean follow-up period, 18.4 months). The most common fracture pattern was 32A2 in 15 patients and 29 were typical and 16 were atypical fractures. The BRQC was good in 36 patients, and TAD was < 25 mm in 43 patients. Pre-abduction NSA (126.0° ± 3.8°) was significantly smaller than post-abduction NSA (129.9° ± 3.4°, p < 0.001). Post-abduction NSA was comparable to contralateral NSA of 128.9° ± 2.8° (p = 0.155). Residual gap was significantly reduced from 6.1 ± 2.9 mm pre-abduction to 1.7 ± 1.0 mm post-abduction (p < 0.001). Union was achieved in 44 patients (97.8%;mean duration, 5.9 months). PPM decreased from 7.8 ± 2.0 pre-injury to 7.0 ± 2.1 1-year postoperatively. One nonunion case required reoperation. Radiographic outcomes did not significantly differ by fracture pattern (p = 0.470 for NSA and p = 0.334 for residual gap). Conclusions Hip abduction during intramedullary nailing corrects alignment and reduces the gap in SFFs with residual varus alignment. This method can be applied to various fracture patterns in a straightforward manner and considered valuable for managing SFFs.

Methods: The medical records of 1,315 ocular trauma patients were reviewed. A total of 927 patients who had orbital or facial computed tomography scans and ophthalmologist evaluations were considered. After applying exclusion criteria, 568 patients with 581 affected eyes were included in the final analysis. The patients were divided into two groups based on the presence of old blowout fractures, identified on computed tomography scan. The clinical characteristics and the frequency of ocular injuries were compared between the two groups. The risk of ocular injury according to the presence of old blowout fracture ware studied using multiple logistic regression after controlling age and sex. Results: Among the 581 eyes examined, 140 (24.1%) had old blowout fractures. The incidence of intraocular complications was significantly higher in the no orbital fracture group compared to the old blowout fracture group (20.4% vs. 2.1%, p < 0.001). Specifically, the incidence of complications such as gross hyphema (p = 0.001), globe rupture (p = 0.006), and vitreous hemorrhage (p = 0.027) was significantly greater in the group without old blowout fractures than in those with them. Multiple logistic regression showed that the presence of old blowout fractures was significantly associated with reduced risk of ocular injury (p < 0.001). Conclusions The patients with old blowout fractures had a lower risk of ocular injuries following subsequent blunt trauma. The findings suggest that old fractures may provide protective effect. In vitro studies using cell models investigated carcinogenetic changes of the intracellular molecules and genes following stimulation with a soluble extract of C. sinensis. The in vitro stimulated cells showed a significant shift to G2/M phage, produced oncogenic molecules, changed expression of oncogenes, increased cell proliferation and suppressed apoptosis. Additionally, the gap-junction proteins between cells, such as connexin (Cx) 43, Cx 26, and Cx 32, were changed significantly, disrupting intercellular homeostasis. These findings suggest that C. sinensis and nitrogen compounds synergistically stimulate the cholangiocytes to become neoplastic. C. sinensis is a biological carcinogen of human CCA, and the World Health Organization guidelines enlist food-borne trematodes as one of target neglected tropical diseases to be eliminated by 2030.
