1.Application of Testing-TracingTreatment Strategy in Response to the COVID-19 Outbreak in Seoul, Korea

Background:
Following the coronavirus disease 2019 (COVID-19) outbreak in Wuhan, China, a total of 637 patients had been diagnosed with the disease in Seoul as of May 2, 2020. Our study aimed to describe the impact of the 3T strategies (preemptive testing, prompt tracing and proper treatment) on the epidemiological characteristics of COVID-19 in Seoul.
Methods:
The descriptive and explanatory analysis was carried out on critical indicators such as epidemiological characteristics and key duration of patient status change from January 24 to May 2 in Seoul before and after preemptive testing for patients under investigation associated with COVID-19 clusters.
Results:
Preemptive testing increased the positive test rate (3.9% to 4.2%), an asymptomatic case at diagnosis (16.9% to 30.6%), and reduced the time from symptom onset to quarantine (4.0 to 3.0 days). Prompt tracing decreased unknown sources of infection (6.9% to 2.8%), the mean number of contacts (32.2 to 23.6), and the time-varying reproduction number R(t) (1.3 to 0.6). With proper treatment, only 2 cases of mortality occurred, resulting in a fatality rate of just 0.3%.
Conclusion
In the first wave of the COVID-19 pandemic lasting 100 days, the effect of the 3T strategies flattened the curve and decreased the time during which infected individuals were contagious, thereby lowering the R(t) below 1 in Seoul.
2.Frequency and Clinicohematologic Characteristics of MPL W515 Mutations in Patients with Myeloproliferative Neoplasms.

BACKGROUND: Recently, myeloproliferative leukemia (MPL) W515 mutations have been reported to be molecular markers for myeloproliferative neoplasms (MPNs). We studied the association between MPL W515 mutations and the clinico-hematological features of patients with MPNs. METHODS: Our study included 154 consecutive patients diagnosed with MPNs (31 had polycythemia vera [PV]; 106, essential thrombocythemia [ET]; and 17, primary myelofibrosis [PMF]). MPL W515 mutations were detected by real-time PCR and direct sequencing methods. RESULTS: The MPL W515L mutation was found in 4 patients and the MPL W515A mutation was detected in 1 patient. These 5 patients were diagnosed with JAK2 V617F-negative ET, and they accounted for 12.5% of patients with JAK2 V617F-negative ET. The patients with MPL W515-positive ET showed significantly lower hemoglobin levels and WBC counts than did patients with MPL W515-negative ET or JAK2 V617F-positive ET. CONCLUSIONS: MPL W515 mutation is a useful diagnostic marker for JAK2 V617F-negative MPNs and it is associated with specific hematologic characteristics such as lower hemoglobin levels and WBC counts.

3.Satisfaction with Korean Oncology Advanced Practice Nurses' Role: A Study of Patients, Physicians, and Nurses.

PURPOSE: The purpose of this study was to describe the level of satisfaction with the role of Korean oncology advanced practice nurses (KOAPNs) among cancer patients, physicians, and nurses. METHODS: This study was conducted with 176 patients who had experienced KOPANs' care more than three times, as well as 82 physicians and 190 nurses who have worked with KOAPNs at four hospitals in Korea. RESULTS: The results indicate that 1) Overall satisfaction in the patient group was high (3.61) and they were most satisfied with the 'attentiveness' domain; 2) Overall satisfaction in the physician group was also high (3.61), however, that in the nurse group was relatively low (3.28); 'coordination & cooperation' showed the highest score for satisfaction in both groups, while 'research' domain showed the lowest score; 3) Cancer type in patients, working department in physicians or nurses, and duration working with KOAPNs in nurses showed significant association with satisfaction; and 4) The ratio of KOAPNs-to-patient, time since introduction of the KOAPNs policy, and incentive system were institutional factors showing significant association with it. CONCLUSION: These findings suggest the necessity for identification and standardization of the roles of KOAPNs, as well as promotion of awareness of KOAPNs' care.

4.Molecular Characterization of Norovirus and Rotavirus in Outbreak of Acute Gastroenteritis in Seoul.

Norovirus and rotavirus are important causes of acute nonbacterial gastroenteritis in communities worldwide. Genotypes of norovirus and rotavirus in stool samples, which were collected from patients, was determined by RT-PCR and ELISA. A total 4,484 stool samples (461 outbreak cases) collected during January 2010-April 2013 were analyzed. Four hundred thirty eight of samples were positive for norovirus. Twelve samples were positive for Rotavirus. Norovirus genotypes identified were nine kinds of G I genotype (G I -1, G I -2, G I -3, G I -4, G I -6, G I -7, G I -9, G I -12, G I -13) and nine kinds of G II genotype(G II -2, G II -4, G II -5, G II -6, G II -7, G II -11, G II -12, G II -15, G II -16). Rotavirus genotypes were G2P[4]. The results might be useful information for evaluating the epidemiological status of viral diarrhea in Seoul, and providing a strategy to prevent human norovirus, rotavirus and other threats to public health in Korea.

This condition is associated with old age, childbirth, abdominal surgery, severe coughing, severe sneezing, anticoagulation therapy, and/or coagulation disorders. We report herein a case of RSH and pelvic cavity hematoma that was induced by dalteparin injection in a 77-year-old woman with pulmonary embolism and deep vein thrombosis, and who was successfully treated by conservative management.

7.Chronic Administration of Monosodium Glutamate under Chronic Variable Stress Impaired Hypothalamic-Pituitary-Adrenal Axis Function in Rats.

The hypothalamic-pituitary-adrenal (HPA) axis is the primary endocrine system to respond to stress. The HPA axis may be affected by increased level of corticotrophin-releasing factors under chronic stress and by chronic administration of monosodium glutamate (MSG). The purpose of this study was to investigate whether chronic MSG administration aggravates chronic variable stress (CVS)-induced behavioral and hormonal changes. Twenty-four adult male Sprague-Dawley rats, weighing 200~220 g, were divided into 4 groups as follows: water administration (CON), MSG (3 g/kg) administration (MSG), CVS, and CVS with MSG (3 g/kg) administration (CVS+MSG). In addition, for the purpose of comparing the effect on plasma corticosterone levels between chronic stress and daily care or acute stress, 2 groups were added at the end of the experiment; the 2 new groups were as follows: naive mice (n=7) and mice exposed to restraint stress for 2 h just before decapitation (A-Str, n=7). In an open field test performed after the experiment, the CVS+MSG group significant decrease in activity. The increase in relative adrenal weights in the CVS and CVS+MSG group was significantly greater than those in the CON and/or MSG groups. In spite of the increase in the relative adrenal weight, there was a significant decrease in the plasma corticosterone levels in the CVS+MSG group as compared to all other groups, except the naive group. These results suggest that impaired HPA axis function as well as the decrease in the behavioral activity in adult rats can be induced by chronic MSG administration under CVS rather than CVS alone.

BACKGROUND: This study compared the bronchodilator efficacy and safety of tiotropium inhalation capsules (18microgram once daily) with a ipratropium metered dose inhaler (2 puffs of 20microgram q.i.d.) in patients with chronic obstructive pulmonary disease (COPD). METHOD: After the initial screening assessment and a two-week run-in period, patients received either tiotropium 18microgram once daily or ipratropium 40microgram four times daily over a period of 4 weeks in a double blind, double dummy, parallel group study. The outcome measures were the lung function, the daily records of the peak expiratory flow rate (PEFR), the patients' questionnaire, and the use of concomitant salbutamol. The forced expiratory volume in one second (FEV1) and the forced vital capacity (FVC) were measured 5 minutes before inhalation, and 0.5, 1, 2 and 3 hours after inhaling the study drug on days 0, 14 and 28. RESULT: In 16 centers, 134 patients with a mean (SD) age of 66 (7) years and a predicted FEV1 of 42 (12)% were analyzed. The trough FEV1 response was significantly higher in the tiotropium group than in the ipratropium group after a four-week treatment period. The weekly mean morning PEFR of the tiotropium group was consistently higher than that of the ipratropium group during the 4-week treatment period with differences ranging from 12.52 to 13.88 l/min, which were statistically significant. Tiotropium was well tolerated by the COPD patients during the 4-week treatment period and had a similar safety profile to ipratropium. CONCLUSION: This study shows that tiotropium administrated once daily has a superior bronchodilator effect with a similar safety profile in treating COPD patients compared with ipratropium, inhaled four times daily.

9.The Effects of Intravenous Insulin on Hemodynamics and Autonomic Nervous System Activity in Anesthetized Dogs.

BACKGROUND: Even though the cardiovascular actions of insulin were first described shortly after introduction into clinical practice, the precise physiological role and mechanism of insulin-mediated cardiovascular actions are not known. The aim of the present study was to investigate the changes in hemodynamics after an insulin injection and the role of the autonomic nervous system in mediating the responses to insulin. METHODS: Nine mongrel dogs of the male sex, weighing 20 - 26 kg, were studied. Anesthesia was maintained with pentobarbital and vecuronium after the administration of the loading dose. Femoral and pulmonary artery catheters were placed for obtaining blood samples (ABGA, electrolytes, glucose and plasma catecholamines) and measuring hemodynamic variables. Real time power spectral analysis of R-R interval variability was displayed on the color power spectrum every 30 seconds by a simple connection between the EKG monitor and computer via an A/D converter. After control values were obtained, porcine insulin was administrated intravenously as a bolus injection (2 U/kg). Blood glucose and potassium were maintained within physiological range by simultaneous infusion of 50% glucose (2-4 ml/kg/h) and potassium (0.5-1.0 mEq/kg/h). Parameters were measured respectively in 9 steps; 10min before insulin injection (control), 1, 5, 10, 20, 30, 40, 50 and 60min after insulin injection. RESULTS: Heart rate, mean arterial blood pressure and cardiac output increased and systemic vascular resistance decreased significantly after the insulin injection. No significant changes in plasma norepinephrine and epinephrine levels could be detected. Power spectral density of low frequency and ratio oflow and middle frequency power to high frequency power increased significantly 1min after insulin injection but did not increase thereafter. High frequency power remained significantly below the control value after the insulin injection. CONCLUSIONS: Although catecholamine concentration itself did not show a significant change, PSA data reveals that insulin may exert a stimulatory action on the sympathetic nervous system and a depressive action on the parasympathetic nervous system independent of hypoglycemia immediately after an insulin injection and insulin-induced vasodilation is not related to the autonomic nervous system.

10.Correlation of Radiologic and Pathologic Lymph Non Involvement with TIMP-2 ( tissue inhibitors of metalloproteinase-2 ) in Gastric and Colon Carcinomas.

PURPOSE: To evaluate the correlation between the expression of TIMP-2 (tissue inhibitors of metalloproteinase-2) and negative lymph node involvement of colon and gastric carcinoma. MATERIALS AND METHODS: We studied 26 cases (13 each) of gastric and colon carcinoma specimens along with dissected lymph nodes by immunohistochemical staining to investigate the correlation of the expression of TIMP-2. Lymph nodes involvement by CT scan was defined by size criteria and the presence of central low density. RESULTS: Eight cases with positive lymph node involvement by CT scan showed weak expression of TIMP-2. Ten cases with positive lymph node involvement had weak expression of TIMP-2. Among eight cases with strong expression of TIMP-2 7 patients were negative by radiologic exam. Good correlation between strong TIMP-2 expression and negative lymph node involvement by CT scan was found (<0.05). CONCLUSION: As the expression of TIMP-2 had a good correlation with radiologic involvement of lymph nodes, the study of expression of TIMP-2 in patients with stomach and colon carcinoma might be helpful in planning surgery and predicting the prognosis.
