Journal of Korean Diabetes 2025;26(1):39-47
The prevalence of prediabetes has steadily increased in Korea, driven by rising obesity rates and lifestyle changes. A structured lifestyle modification program has been shown to significantly reduce the prevalence of diabetes among adults with prediabetes, with this effect sustained over time. Lifestyle modifications including medical nutrition, exercise, and behavioral psychological therapy are effective for preventing and managing diabetes, with medical nutrition therapy playing a crucial role. This study reviews the latest academic guidelines and medical nutrition therapy for diabetes prevention and to introduce the KDPS-hLSM (Korean Diabetes Prevention Study hospital-based lifestyle modification).

With the increasing use of CAD-CAM (computer-aided design-computer-aided manufacturing) methods for prosthesis fabrication, utilization of virtual articulators in CAD software has also increased. Among the various methods of positioning the maxilla in a virtual articulator, there are techniques that utilize CBCT (cone-beam computed tomography) or facial scans without the use of a facebow. In this case, a full-mouth rehabilitation was planned for a patient who exhibited multiple crown fractures and occlusal plane disharmony through maxillary complete denture and mandibular fixed prosthetic restoration. Radiopaque markers were added to the duplicated maxillary temporary denture to take closed-mouth impression, which was then scanned and positioned on the CBCT. On CBCT, hinge axis connecting the medial poles of both mandibular condyles along with the Frankfort horizontal plane was designated and utilized to perform virtual articulator mounting. Maxillary complete denture and mandibular fixed prostheses were fabricated, resulting in satisfactory aesthetic and functional outcomes.

Recent clinical outcomes of multi-regimen chemotherapy in patients with cholangiocarcinoma (CCC) have shown benefits in terms of overall survival. However, repeated endoscopic biliary drainage (EBD) and serious adverse events negatively affect prolongation of the survival period. The aim of this study was to investigate the prevalence of massive hemobilia and the outcomes of its management with fully covered self-expandable metal stents (FC-SEMSs) in patients with hilum-involving CCC receiving multi-regimen chemotherapy. The methods and effects of FCSEMS placement were retrospectively investigated following the occurrence of massive hemobilia during EBD. A total of 356 patients with CCC received multi-regimen chemotherapy. Among them, 181 patients had hilar invasion, and seven patients (3.9%) developed massive hemobilia during repeated EBD using removable stents. In all cases, the tumor encased the right hepatic artery. In six patients (85.7%), hemostasis was immediately and completely achieved by inserting one or two FC-SEMSs proximal to the hilar invasion area. Therefore, if the tumor encases the right hepatic artery, massive hemobilia is likely to occur during multi-regimen chemotherapy. Thus, prompt placement of a FC-SEMS would be an effective treatment option for massive hemobilia in patients with hilum-involving CCC.
