1.The impact of interval between primary cytoreductive surgery with bowel resection and initiation of adjuvant chemotherapy on survival of women with advanced ovarian cancer: a multicenter cohort study

Objective:
Our aim was to determine if the time interval between bowel resection and initiation of adjuvant chemotherapy impacts survival in advanced ovarian cancers.
Methods:
This was a retrospective cohort study using data from two cancer centers, Princess Margaret Cancer Centre in Toronto, Ontario, Canada and Samsung Comprehensive Cancer Center in Seoul, South Korea. Patients with International Federation of Gynecology and Obstetrics (FIGO) stage III or IV ovarian cancer that underwent large bowel resection during primary cytoreductive surgery (PCS) were included.
Results:
Ninety-one women were eligible of which the majority (90.1%) were diagnosed with high-grade serous cancer. The median interval from PCS to chemotherapy for all patients was 21 days (7–86 days). Patients were stratified into 3 groups: 1) Interval ≤14 days, 32 (35.2%) patients; 2) Interval between 15–28 days, 27 (29.6%) patients; and 3) Interval between 29–90 days, 32 (35.2%) patients. Surgical procedures and postoperative outcomes were similar between groups. Multivariate analysis indicated that PCS to chemotherapy interval of 2–4 weeks, younger age, and completion of 4 or more adjuvant chemotherapy cycles were independent prognostic factors of favorable overall survival.
Conclusion
Initiation of adjuvant chemotherapy between 2 to 4 weeks after PCS with bowel resection may improve survival outcomes in women with advanced ovarian cancer by maximizing the benefit of PCS plus adjuvant chemotherapy.
2.Antibacterial Effects of Silver Fluoride and Difluorosilane-based Varnish on Streptococcus mutans

The purpose of this study is to evaluate the antibacterial effects of two liquid fluoride materials on Streptococcus mutans (S. mutans). Riva star aqua™ (SDI, Bayswater, Australia), which consists of silver fluoride (AgF, step 1) and potassium iodide (KI, step 2), and Fluor protector® (FP; Ivoclar Vivadent, Liechtenstein) were used for experimental groups. Experimental groups were divided into 4 groups : AgF, KI, AgF + KI and FP. For the positive control (PC) group, ampicillin was used, and the negative control group (NC) did not receive any additional treatment. Each group was divided into 30 and 50 µL groups by volume of liquid and applied to flat-coated S. mutans. The diameter of the zone of inhibition was measured. The PC and AgF groups showed larger diameters than other materials (p < 0.05), and the AgF group showed no significant difference from the PC group in the 50 µL group (p > 0.05). The FP group showed larger diameters than the AgF + KI group in the 30 µL (p = 0.009). The KI group did not show significant difference from the NC group (p > 0.05). AgF is comparable to ampicillin in antibacterial effects on S. mutans, and better than FP.
3.Caries prevalence by risk factor and tooth type using DMFT-related indices

Objectives:
One of the main causes of tooth loss in Korea is dental caries, a chronic disease affecting individuals of all ages worldwide. The average decayed, missing, and filled teeth (DMFT) index, a measure of the caries experience of the population, has been stagnating in Korea for a number of years. The purpose of this study was to investigate the pattern of caries prevalence by tooth type, and to identify the characteristics and risk factors of the specific affected caries (SaC) and significant caries (SiC) groups in Korea’s 12-year-old children. Data were obtained from the 2018 Children’s Oral Health Survey.
Methods:
Data from a total of 22,303 subjects were included in the study. We grouped the data according to two DMFT-related indices, the SaC index, which includes all subjects who have experienced a carious lesion, and the SiC index, which includes the top one-third of the population by DMFT. We then analyzed the sociodemographic variables and oral health behaviors seen in these groups.
Results:
The average DMFT index for all the 12-year-old children was 1.89, and that of those in the SaC and SiC groups was 3.37 and 4.86, respectively. The teeth that were found to be most likely to develop caries were the mandibular and maxillary first molars, and the tooth associated with the highest mean DMFT index was the mandibular first premolar. Among the variables analyzed, perceived oral health status was the most influential variable in the SaC and SiC groups, followed by gender, and then the presence or absence of calculus. Household income was not a statistically significant variable.
Conclusions
The results of this study revealed the characteristics common to 12-year-old children who are vulnerable to caries. This will provide valuable information when planning dental disease management projects.
4.TGF-β2 downregulates osteogenesis under inflammatory conditions in dental follicle stem cells.

Bone formation is important for the reconstruction of bone-related structures in areas that have been damaged by inflammation. Inflammatory conditions such as those that occur in patients with rheumatoid arthritis, cystic fibrosis, and periodontitis have been shown to inhibit osteoblastic differentiation. This study focussed on dental follicle stem cells (DFSCs), which are found in developing tooth germ and participate in the reconstruction of alveolar bone and periodontal tissue in periodontal disease. After bacterial infection of inflamed dental tissue, the destruction of bone was observed. Currently, little is known about the relationship between the inflammatory environment and bone formation. Osteogenic differentiation of inflamed DFSCs resulted in decreased alkaline phosphatase (ALP) activity and alizarin red S staining compared to normal DFSCs. Additionally, in vivo transplantation of inflamed and normal DFSCs demonstrated severe impairment of osteogenesis by inflamed DFSCs. Protein profile analysis via liquid chromatography coupled with tandem mass spectrometry was performed to analyse the differences in protein expression in inflamed and normal tissue. Comparison of inflamed and normal DFSCs showed significant changes in the level of expression of transforming growth factor (TGF)-β2. Porphyromonas gingivalis (P.g.)-derived lipopolysaccharide (LPS) was used to create in vitro inflammatory conditions similar to periodontitis. The osteogenic differentiation of LPS-treated DFSCs was suppressed, and the cells displayed low levels of TGF-β1 and high levels of TGF-β2. DFSCs treated with TGF-β2 inhibitors showed significant increases in alizarin red S staining and ALP activity. TGF-β1 expression was also increased after inhibition of TGF-β2. By examining inflamed DFSCs and LPS-triggered DFSCs, these studies showed both clinically and experimentally that the increase in TGF-β2 levels that occurs under inflammatory conditions inhibits bone formation.

6.Valproic Acid Modulates the Multipotency in Periodontal Ligament Stem Cells via p53-Mediated Cell Cycle.

Human periodontal ligament stem cells (PDLSCs), a type of mesenchymal stem cell, are a promising source for dental regeneration and are identified in human periodontal ligaments from extracted third molars. Valproic acid (VPA) is a histone deacetylase inhibitor that has been used as a wide-spectrum antiepileptic drug and a medication for mood disorders. VPA has shown several effects on increasing the pluripotency of embryonic stem cells and controlling osteogenic differentiation, besides the prevention of seizures. However, its effect on proliferation and osteogenesis depends on the cell type and concentration. The aim of this study was to investigate the effects of cyclic and constant VPA treatment on PDLSCs. Proliferation and apoptosis of PDLSCs were determined with cyclic and constant VPA treatment. In cemento/ osteogenic differentiation, osteogenic markers decreased significantly after cyclic treatment with 0.5 mM VPA. In contrast, VPA enhanced osteogenic differentiation after constant treatment. With cyclic VPA treatment, p53 levels related to apoptotic pathway decreased to induce proliferation. These findings indicated that VPA has different roles in proliferation and differentiation of PDLSCs in vitro and in vivo via p53-related pathway.

7.Neurogenic differentiation of human dental stem cells in vitro.

OBJECTIVES: The purpose of this study was to investigate the neurogenic differentiation of human dental pulp stem cells (DPSCs), periodontal ligament stem cells (PDLSCs), and stem cells from apical papilla (SCAP). MATERIALS AND METHODS: After induction of neurogenic differentiation using commercial differentiation medium, expression levels of neural markers, microtubule-associated protein 2 (MAP2), class III beta-tubulin, and glial fibrillary acidic protein (GFAP) were identified using reverse transcriptase polymerase chain reaction (PCR), real-time PCR, and immunocytochemistry. RESULTS: The induced cells showed neuron-like morphologies, similar to axons, dendrites, and perikaryons, which are composed of neurons in DPSCs, PDLSCs, and SCAP. The mRNA levels of neuronal markers tended to increase in differentiated cells. The expression of MAP2 and beta-tubulin III also increased at the protein level in differentiation groups, even though GFAP was not detected via immunocytochemistry. CONCLUSION: Human dental stem cells including DPSCs, PDLSCs, and SCAP may have neurogenic differentiation capability in vitro. The presented data support the use of human dental stem cells as a possible alternative source of stem cells for therapeutic utility in the treatment of neurological diseases.

8.Factors Affecting Diabetic Screening Behavior of Korean Adults: A Multilevel Analysis.

PURPOSE: We investigated the role of individual and community level factors on diabetes screening test behavior. METHODS: We used individual-level data from 170,193 adults aged 30 years or older who were not diagnosed with diabetes and participated in the 2009 community health survey. Community-level data includes 253 communities and were collected from various national statistics. Multilevel logistic regression analysis was conducted. RESULTS: The rate of diabetes screening within the year prior to this study was 53.2%. Community variance of Model I, Model II and Model III was 0.236, 0.252 and 0.238, respectively. The proportional change in variance of Model II and Model III was -6.8% and -1.2%. The odds ratio for participation of diabetic screening of areas with bottom financial independence compared to areas with top was 0.84 (95% confidence interval, 0.74-0.96); the odds ratio of areas with top internist compared to areas with bottom was 1.15 (95% confidence interval, 1.01-1.31). CONCLUSION: This study identified a contextual effect influencing the participation of Korean adults in diabetes screening. It is necessary to develop specific policies that consider not only individual factors, but also community factors relating to individual behaviors to increase the likelihood of diabetes screening.

9.Comparison of the Antioxidant Effects of Diallyl Sulfide, Capsaicin, Gingerol and Sulforaphane in H2O2-Stressed HepG2 Cells.

Oxygen is necessary to sustain life, yet cellular oxygen metabolism creates destructive elements called free radicals. Free radicals are chemically unbalanced and carrying free electrons that can damage molecules, potentially damaging the cell itself. For this reason, many antioxidant products, including supplements and functional foods, are being developed. In particular, natural products are rich sources of pharmacologically active compounds. The purpose of this study was to investigate the antioxidant effects of target biomaterials in Korean traditional spices such as diallyl sulfide (DAS), capsaicin (CAP), and gingerol (GGR), and to investigate the response of the antioxidant defense system to oxidative stress by hydrogen peroxide (H2O2) compared to sulforaphane (SFN) in HepG2 cells. After the analysis of the cell viability using Cell Counting kit-8 (CCK-8) assay, we determined that the optimum levels were 200 microM DAS, 25 microM CAP, 50 microM GGR, and 12.5 microM SFN. Antioxidant enzymes were measured and protein expression was detected by Western blotting. All treatments showed a significant decrease in antioxidant enzyme activity such as superoxide dismutase, catalse, and glutathione peroxidase in HepG2 cells. Additionally, DAS, CAP, GGR and SFN increased the antioxidant system-related transcription factor Nrf2 which was found to be regulated by the activation of MAPK-JNK in this study. In conclusion, these results indicate the protective effects of DAS CAP, GGR, and SFN against H2O2-induced oxidative stress.

10.Serologic Markers of Viral Hepatitis of Korea University Medical Center Patients.

BACKGROUND: As an endemic area of viral hepatitis B, many studies on hepatitis B and C have been reported in Korea, but no on all five viral types, A, B, C, D, and E. We surveyed ten serologic markers for the five different viral hepatitis and reviewed the seropositivity of each viral hepatitis and concurrent infection. METHODS: Ten serologic markers of five viral hepatitis (anti-HAV IgM, anti-HAV IgG, HBsAg, anti-HBs, anti-HBc, HBeAg, anti-HBe, anti-HCV, anti-HDV, and anti-HEV IgM) were tested for 260,488 samples requested for viral marker studies at three hospitals of Korea University Medical Centers from January through December, 2003. Anti-HAV IgM, anti-HAV IgG, anti-HDV, and anti-HEV IgM were tested by RIA and HBsAg, anti-HBs, anti-HBc, HBeAg, anti-HBe, and anti-HCV were analysed by ELISA or RIA method. RESULTS: Anti-HAV IgM and IgG seropositivity was 1.2% and 88.0%, respectively. Anti-HAV IgM seropositivity was high in a patient group 20 to 29 years of age. The overall seropositivity of HBsAg was 10.4% and for anti-HBs 60.4%. The seropositivity was 1.3% for anti-HCV, 1.1% for anti-HDV, and 22.2% for anti-HEV IgM. The concurrent positivity of HBsAg and anti-HBs was 4.0%. HBsAg was positive in 7 (0.1%) of anti-HCV positive patients; anti-HEV IgM was positive in 2 (25%) of anti-HAV IgM positive patients. CONCLUSIONS: Hepatitis A infection was rare in children but increased in patient group 20 to 29 years of age. The concurrent infection rate of hepatitis A with hepatitis E was high, suggesting that hepatitis E should be considered in hepatitis A patients. In view of the finding that the concurrent infection of hepatis B and C was detected, though at a relatively low rate, patients with viral hepatitis need to be assessed for the possibility of concurrent infection with other types of hepatitis.
