The continuous emergence of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) variants has provided insights for updating current coronavirus disease 2019 (COVID-19) vaccines. We examined the neutralizing activity of Abs induced by a BA.4/5-containing bivalent mRNA vaccine against Omicron subvariants BN.1 and XBB.1.5. We recruited 40 individuals who had received a monovalent COVID-19 booster dose after a primary series of COVID-19 vaccinations and will be vaccinated with a BA.4/5-containing bivalent vaccine. Sera were collected before vaccination, one month after, and three months after a bivalent booster.Neutralizing Ab (nAb) titers were measured against ancestral SARS-CoV-2 and Omicron subvariants BA.5, BN.1, and XBB.1.5. BA.4/5-containing bivalent vaccination significantly boosted nAb levels against both ancestral SARS-CoV-2 and Omicron subvariants. Participants with a history of SARS-CoV-2 infection had higher nAb titers against all examined strains than the infection-naïve group. NAb titers against BN.1 and XBB.1.5 were lower than those against the ancestral SARS-CoV-2 and BA.5 strains. These results suggest that COVID-19 vaccinations specifically targeting emerging Omicron subvariants, such as XBB.1.5, may be required to ensure better protection against SARS-CoV-2 infection, especially in high-risk groups.

Background:
Several cases of pediatric acute hepatitis of unknown etiology related to adenoviral infections have been reported in Europe since January 2022. The aim of this study was to compare the incidence, severity, possible etiology, and prognosis of the disease with those in the past in Korea.
Methods:
The surveillance group collected data between May and November 2022 using a surveillance system. Acute hepatitis of unknown etiology was defined in patients aged < 16 years with a serum transaminase level > 500 IU/L, not due to hepatitis A-E or other underlying causes. For comparison, data from 18 university hospitals were retrospectively collected as a control group between January 2021 and April 2022.
Results:
We enrolled 270 patients (mean age, 5 years). The most common symptom was fever. However, the incidence was similar between 2021 and 2022. Liver function test results, number of patients with acute liver failure (ALF), liver transplantation (LT), death, and adenovirus detection rates did not differ between the two groups. None of the adenoviruspositive patients in either group experienced ALF, LT, or death. In the surveillance group, adenovirus-associated virus-2 was detected in four patients, one of whom underwent LT. Patients with an unknown etiology showed significantly higher bilirubin levels, a lower platelet count, and a higher LT rate than patients with a possible etiology.
Conclusion
The incidence of pediatric acute hepatitis of unknown etiology and adenovirus detection rate have not increased in Korea.
3.Retrospective analysis of palliative chemotherapy for the patients with bladder adenocarcinoma: Korean Cancer Study Group Genitourinary and Gynecology Cancer Committee.

BACKGROUND/AIMS: Because of rarity, role of chemotherapy of bladder adenocarcinoma are still unidentified. Therefore, we performed a retrospective analysis of the clinical features and chemotherapy outcomes of bladder adenocarcinoma. METHODS: Eligible patients for this retrospective analysis were initially diagnosed with bladder adenocarcinoma and presented with a clinically no other primary site of origin. The collected data included age, gender, performance status, stage, hemoglobin, albumin, initial date of diagnosis, treatment modality utilized, response to treatment, presence of relapse, last status of patient, and last date of follow-up. RESULTS: We retrospectively reviewed 29 patients, who were treated with chemotherapy for bladder adenocarcinoma at 10 Korean medical institutions from 2004 to 2014. The median age of patients was 58 years (range, 17 to 78) and 51.7% of the patients were female. Urachal adenocarcinoma was identified in 15 patients. Of 27 symptomatic patients, 22 experienced gross hematuria. Twelve patients were treated with 5-f luorouracil based chemotherapy, five were gemcitabine based, three were taxane and others. Thirteen of them achieved complete response (10.3%) or partial response (34.5%). Median progression-free survival (PFS) and overall survival (OS) for all patients were 10.6 months (95% confidence interval [CI], 9.5 to 11.6) and 24.5 months (95% CI, 1.2 to 47.8), respectively. The cases of urachal adenocarcinoma exhibited worse tendency in PFS and OS (p = 0.024 and p = 0.046, respectively). CONCLUSIONS: Even though bladder adenocarcinoma had been observed moderate effectiveness to chemotherapy, bladder adenocarcinoma is a highly aggressive form of bladder cancer. PFS and OS were short especially in urachal carcinoma.

4.Risk Factors of Cut-Out in Treatment of Femoral Intertrochanteric Fractures by Proximal Femur Nail.

PURPOSE: The purpose of this study is to evaluate the risk factors for the occurrence of cut-out of proximal femoral nail by a lag screw as the treatment for intertrochanteric fractures. MATERIALS AND METHODS: A total of 151 patients (76 males and 75 females; mean age, 73.7±12.1 years), who were diagnosed with intertrochanteric fracture at Gyeongsang National University Hospital between January 2011 and March 2016, with a follow-up of at least for 6 months were included in this retrospective study. Various risk factors, such as demographic data, osteoporosis, collodiaphyseal angle (CDA) (≤130° or >130°), tip-apex distance (TAD) (≤25 mm or >25 mm), and the position of lag screw in the femur head (quadrant) related to the occurrence of cut-out were taken into consideration. The strength of association for each factor was determined through the calculation of the odds ratio (OR), within the 95% confidence interval (CI). First, we performed univariate logistic regression analyses for all variables; then, we performed a multivariate logistic regression analysis, using only the significant variables that had resulted from the univariate analysis. RESULTS: Among the 151 cases, the occurrence of cut-out was observed in 14 cases (9.3%). In a univariate analysis, the fracture patterns based on the AO/OTA classification (p=0.045), CDA (p<0.001) and the position of lag screw in the femur head (quadrant) (p=0.001) showed a significant association with the occurrence of the cut-out. However, TAD was not significantly associated with the cut-out (p=0.886). Various factors, which were significant in univariate analyses, were included in multivariate analyses. In multivariate analyses, CDA (OR, 12.291; 95% CI, 2.559-59.034; p=0.002), and quadrant (OR, 7.194; 95% CI, 1.712-30.303; p=0.007) were significantly associated with the cut-out. CONCLUSION: Valgus reduction and proper position of lag screw were critical for the prevention of occurrence of cut-out when treating intertrochanteric fracture using proximal femur nail.

5.A Validation Study of Korean Version of Personality Beliefs Questionnaire-Short Form (PBQ-SF).

OBJECTIVES: The Personality Belief Questionnaire-Short Form (PBQ-SF) is a self-report instrument for assessment of dysfunctional beliefs based on Beck's cognitive formulations of personality disorders. The aims of this study was to examine the reliability and validity of the Korean version of PBQ-SF in clinical samples. METHODS: The Korean version of PBQ-SF was examined in 115 participants (50 patients with personality disorder and 65 patients without personality disorder). All participants were assessed for personality disorder using the semi-structured clinical interview of the Personality Assessment Schedule. The construction validity was examined by correlation with Beck Depression Inventory, Spielberger State-Trait Anxiety Inventory, and Neuroticism-Extraversion-Openness Five-Factor Inventory neuroticism scales. Twenty four randomly sampled patients were examined for the test-retest reliability. RESULTS: The Korean version of PBQ-SF showed good internal consistency [Cronbach's alpha=0.73 (schizoid)-0.92 (paranoid)] and test-retest reliability [r=0.74 (narcissistic)-0.92 (paranoid)]. The PBQ-SF was correlated with depression, anxiety, and neuroticism. The overall subscales of PBQ-SF were correlated with Diagnostic and Statistical Manual of Mental Disorders based diagnosis of personality disorders. CONCLUSION: Consistent with previous findings using the Korean full version of PBQ as well as the English version of PBQ-SF, our results support that the Korean version of PBQ-SF is a reliable and valid instrument for assessment of dysfunctional beliefs associated with personality pathology.

6.Risk Factors of Cervical Spondylosis in Workers Requiring Neck Flexion and Extension Actions in Farming and Fishing Communities.

PURPOSE: The purpose of this study is to determine the relationship between the repetitive flexion or extension posture on the cervical spine on labor and degenerative change of the cervical spine, and the factors affecting degenerative change of the cervical spine. MATERIALS AND METHODS: To determine the factors affecting degenerative change of the cervical spine, age, sex, height, weight, body mass index, smoking, diabetes mellitus (DM), time engaging in labor, and cervical spine posture (flexion or extension) required repetitively on labor were investigated in the subjects. In addition, to evaluate the level of degenerative change of the cervical spine on 83 people in the flexion group (flexion strain) and 83 people in the extension group (extension strain), cervical degenerative index (CDI) in the simple cervical spine lateral radiograph was used to score (0-60 points) the degenerative severity. RESULTS: A total of 166 subjects (flexion group: 83 people, extension group: 83 people) participated in this study, and for the CDI, the cervical spine flexion group scored 7.8±6.2 points, and the cervical spine extension group scored 12.2±6.0 points to show that the cervical spine extension group had significant degenerative change in the cervical spine. In the multiple linear regression test performed to verify the risk factors affecting the degenerative change of the cervical spine, age (p=0.004), contraction of DM (p=0.029), and extension posture of cervical spine (p<0.001) influenced the degenerative change of the cervical spine. CONCLUSION: Repetitive extension posture on the cervical spine on labor and contraction of diabetes affected degenerative change of the cervical spine, therefore, training in medical care and posture on labor are required to prevent the progression of degenerative change in the cervical spine.

7.Met inactivation by S-allylcysteine suppresses the migration and invasion of nasopharyngeal cancer cells induced by hepatocyte growth factor.

PURPOSE: Past studies have reported that S-allylcysteine (SAC) inhibits the migration and invasion of cancer cells through the restoration of E-cadherin, the reduction of matrix metalloproteinase (MMP) and Slug protein expression, and inhibition of the production of reactive oxygen species (ROS). Furthermore, evidence is emerging that shows that ROS induced by radiation could increase Met activation. Following on these reports of SAC and Met, we investigated whether SAC could suppress Met activation. MATERIALS AND METHODS: Wound healing, invasion, 3-[4,5-dimethylthiazol-2-yl]-2,5-diphenyltetrazolium (MTT), soft agar colony forming, western blotting, and gelatin zymography assays were performed in the human nasopharyngeal cancer cell lines HNE1 and HONE1 treated with SAC (0, 10, 20, or 40 mM) and hepatocyte growth factor (HGF). RESULTS: This study showed that SAC could suppress the migration and invasion of HNE1 and HONE1 cell lines by inhibiting p-Met. An increase of migration and invasion induced by HGF and its decrease in a dose dependent manner by SAC in wound healing and invasion assays was observed. The reduction of p-Met by SAC was positively correlated with p-focal adhesion kinase (p-FAK) and p-extracellular related kinase (p-ERK in both cell lines). SAC reduced Slug, MMP2, and MMP9 involved in migration and invasion with the inhibition of Met-FAK signaling. CONCLUSION: These results suggest that SAC inhibited not only Met activation but also the downstream FAK, Slug, and MMP expression. Finally, SAC may be a potent anticancer compound for nasopharyngeal cancer treated with radiotherapy.

OBJECTIVES: The aims of this study were to evaluate acceptability and usefulness of the Korean version of Self-report Standardized Assessment of Personality-Abbreviated Scale (SAPAS-SR) as an instrument for screening patients with a personality disorder. METHODS: The Korean version of the SAPAS-SR was administered to a non-random sample of 186 psychiatric patients (155 patients with a personality disorder and 31 patients with no personality disorder). The International Classification of Diseases 10th version Personality Assessment Schedule was used as a gold standard in diagnosis of personality disorder. Receiver-operant-characteristics and validity indicators were determined. In addition, the SAPAS-SR was administered to 22 healthy men to examine the test-retest reliability. RESULTS: The area under the curve for the SAPAS-SR was 0.69 (95% confidence interval 0.59-0.79). The SAPAS-SR score of 4 or more correctly classified 67.2% of patients with a personality disorder. Sensitivity (0.67) and specificity (0.68) were slightly lower compared with the original English version. CONCLUSION: This study provides preliminary evidence of the usefulness of the Korean version of the SAPAS-SR as a self-administered instrument for screening personality disorders in the clinical population.

9.Social Function in Patients with Personality Disorder Diagnosed by Single Dimensional Severity Using Korean Version of Social Functioning Questionnaire.

OBJECTIVES: The aims of this study were to examine validity and reliability of the Korean version of the Social Function Questionnaire (SFQ) and evaluated social function with SFQ in patients with personality disorder. METHODS: The SFQ was administered to 186 psychiatric patients (155 patients with personality disorder and 31 patients without personality disorder), and 22 healthy men were recruited to examine the test-retest reliability of SFQ. The severity of personality disorders was determined using the proposed the International Classification of Diseases (ICD)-11th revision (ICD-11) personality disorders. All participants completed the NEO-Five Factor Inventory, Beck Depression Inventory, and Spielberger State and Trait Anxiety Inventory to examine the convergent validity of SFQ. RESULTS: The Korean version of the SFQ showed good internal consistency (Cronbach's alpha=0.811) and test-retest reliability (r=0.746). Patients with personality disorder had more social dysfunction than those without personality disorder. A graded increase in social dysfunction was observed with increasing severity of personality disorder. Social dysfunction showed a strong linear relationship with the 5 factor model. CONCLUSION: The Korean version of the SFQ has good psychometric properties. The results of our study support the severity classification of personality disorder integrated to upcoming ICD-11.

10.Serum Magnesium Level Is Associated with Type 2 Diabetes in Women with a History of Gestational Diabetes Mellitus: The Korea National Diabetes Program Study.

Gestational diabetes mellitus (GDM) is a strong predictor of postpartum prediabetes and transition to overt type 2 diabetes (T2DM). Although many reports indicate that low magnesium is correlated with deteriorated glucose tolerance, the association between postpartum serum magnesium level and the risk for T2DM in women with a history of GDM has not been evaluated. We analyzed postpartum serum magnesium levels and development of prediabetes and T2DM in women with prior GDM according to American Diabetes Association (ADA) criteria using the Korean National Diabetes Program (KNDP) GDM cohort. During a mean follow-up of 15.6+/-2.0 months after screening, 116 women were divided into three groups according to glucose tolerance status. Ultimately, eight patients (6.9%) were diagnosed with T2DM, 59 patients (50.9%) with prediabetes, and 49 patients (42.2%) with normal glucose tolerance (NGT) after follow-up. The T2DM group had the lowest serum magnesium level (0.65 [0.63-0.68] mM/L) in the postpartum period, but there was no significant difference between the prediabetes group (0.70 [0.65-0.70] mM/L) and the NGT group (0.70 [0.65-0.70] mM/L) (P=0.073) Multiple logistic regression analysis showed that postpartum HOMA-IR was a significant predictor of both prediabetes and T2DM. Moreover, we found that postpartum serum magnesium level was also a possible predictor for T2DM development. Serum magnesium level in the postpartum period may be a possible predictor for T2DM development in women with a history of GDM.
