1.Treatment of Granulation Tissue Hyperplasia after Tracheostomy Based on the Kenang (窠囊) Theory

Granulation tissue hyperplasia after tracheotomy is a common clinical complication. Endoscopic treatment can temporarily relieve airway obstruction, however, it is associated with a high recurrence rate and poor long-term prognosis. Based on the traditional Chinese medicine (TCM) Kenang (窠囊) theory and combined with modern pathological mechanisms, this paper explores its correlation with the pathogenesis of post-tracheotomy granulation tissue hyperplasia. Drawing from clinical experience in applying the Kenang theory for treatment, this paper proposes that the fundamental pathogenesis of this condition lies in qi deficiency and organ dysfunction, while phlegm and blood stasis interlocking serve as the symptomatic manifestations. The treatment focuses on resolving phlegm and promoting blood circulation, dispersing nodules and eliminating stagnation, regulating qi flow, and reinforcing the body's vital energy while expelling pathogenic factors. This approach aims to dissolve phlegm and blood stasis, dissipate the Kenang, and ultimately prevent and treat granulation tissue hyperplasia.

To summarize Professor WANG Jianmin's experience in cathartic colon from "keeping sweet to return liquid". It is believed that the key to the pathogenesis of cathartic colon is fluid consumption and intestinal dryness, including yin depletion of spleen earth, and lack of sources for body fluids production; discordance of water and fire in kidneys, and irregular distribution of body fluids; and closure of the lungs and liver leads to inability of the flow of fluids. The treatment is based on the principle of "keeping sweet to return liquid", using sweet medicinals mainly, assistant with sour, bland and acrid medicinals, and self-prescribed Lipi Shengjin Decoction (理脾生津汤), Wenshen Runchang Decoction (温肾润肠汤), Kaifei Shunchang Decoction (开肺顺肠汤), Rougan Tongbian Decoction (柔肝通便汤) could be used to regulate spleen yin by the sweet and bland, and establish qi and promote fluid production; the sweet and warm medicinals can replenish water and fire, transform into qi, and distribute body fluids; the acrid and sweet can open lung depression, descend qi, and flow the body fluids; the sour and sweet can emolliate liver, move qi, and transform fluids.
3.Zhongfeng Xinglou Tongfu Capsule Improves Inflammatory Damage in Mice with Cerebral Hemorrhage by Mediating CCK-8 to Regulate the IL-10/STAT3 Signaling Pathway

Objective To explore the regulatory effect and mechanism of Zhongfeng Xinglou Tongfu Capsule on the brain inflammatory response of mice with cerebral hemorrhage through cholecystokinin octapeptide(CCK-8)mediated the interleukin 10(IL-10)/signal transduction and transcriptional activator 3(STAT3)signaling pathway.Methods A total of 110 C57BL/6 mice were divided into sham operation group,cerebral hemorrhage group,Zhongfeng Xinglou Tongfu Capsule low-,medium-,and high-dose groups according to the random number table method,with 22 mice in each group.The intracerebral hemorrhage model was established by collagenase method.At 2 hours after the establishment of the model,Zhongfeng Xinglou Tongfu Capsule suspension was intragastrically administered at a dose of 10 mL·kg-1.The sham operation group and the cerebral hemorrhage group were intragastrically administered with the same dose of normal saline.The neurofunctional score and left-turn rate of the mice were recorded.The dry and wet weight method was used to detect brain water content on the 72 h after surgery,and the Western Blot method was used to detect tumor necrosis factor-α(TNF-α),interleukin 1β(IL-1β),interleukin 6(IL-6),interleukin 10(IL-10),and the relative expression levels of JAK-1,STAT3,and p-STAT3 pathway proteins around the hematoma of mice at 72 h after surgery.On the 3rd and 5th day after surgery,Elisa method was used to detect the relative expression level of CCK-8,qPCR method was used to detect the expression levels of IL-10 and STAT3 mRNA.Results Compared with the cerebral hemorrhage group,the NDS scores of mice in the low-,medium-,and high-dose groups of Zhongfeng Xinglou Tongfu Capsules were significantly reduced on the 3rd and 5th days after intervention treatment(P<0.01).And the left-turn rate in the low-dose group on the 3rd and 5th days after intervention treatment was reduced(P<0.05),the left-turn rate in the medium-dose group was reduced on the 3rd day(P<0.05)and the 5th day(P<0.01).Furthermore,the left-turn rate was obviously reduced on the 3rd and 5th day in the high-dose group(P<0.01).The brain water content of mice in the low-dose group was reduced(P<0.05),and the brain water content of mice in the medium-and high-dose groups was significantly reduced(P<0.01).In the low-dose group,the expressions of pro-inflammatory factors TNF-α,IL-6 and JAK-1,STAT3,and p-STAT3 pathway proteins were significantly reduced(P<0.01),while the expression of IL-1β was reduced(P<0.05),the expression of IL-10 increased significantly(P<0.01).The expression of pro-inflammatory factors TNF-α,IL-1β,IL-6,as well as JAK-1,STAT3,and p-STAT3 pathway proteins in the medium-and high-dose groups was significantly reduced(P<0.01),the expression of anti-inflammatory factor IL-10 increased significantly(P<0.01).The content of CCK-8 in the low-dose group was increased on the 3rd day after treatment(P<0.05),and the content increased significantly on the 5th day(P<0.01).The content of CCK-8 in the medium-and high-dose groups obviously increased on the 3rd and the 5th day after treatment(P<0.01).The relative expression level of IL-10 mRNA was significantly increased after treatment(P<0.01),but the relative expression level of STAT3 mRNA was significantly decreased after treatment(P<0.01).Conclusion Zhongfeng Xinglou Tongfu Capsule can target the content of brain-gut peptide CCK-8 and regulate the IL-10/STAT3 signaling pathway to reduce the secondary inflammatory reaction and cytotoxic brain edema after cerebral hemorrhage.

An effective therapeutic regimen for hepatic fibrosis requires a deep understanding of the pathogenesis mechanism. Hepatic fibrosis is characterized by activated hepatic stellate cells (aHSCs) with an excessive production of extracellular matrix. Although promoted activation of HSCs by M2 macrophages has been demonstrated, the molecular mechanism involved remains ambiguous. Herein, we propose that the vitamin D receptor (VDR) involved in macrophage polarization may regulate the communication between macrophages and HSCs by changing the functions of exosomes. We confirm that activating the VDR can inhibit the effect of M2 macrophages on HSC activation. The exosomes derived from M2 macrophages can promote HSC activation, while stimulating VDR alters the protein profiles and reverses their roles in M2 macrophage exosomes. Smooth muscle cell-associated protein 5 (SMAP-5) was found to be the key effector protein in promoting HSC activation by regulating autophagy flux. Building on these results, we show that a combined treatment of a VDR agonist and a macrophage-targeted exosomal secretion inhibitor achieves an excellent anti-hepatic fibrosis effect. In this study, we aim to elucidate the association between VDR and macrophages in HSC activation. The results contribute to our understanding of the pathogenesis mechanism of hepatic fibrosis, and provide potential therapeutic targets for its treatment.

5.Effects and mechanisms of SNP-9 on Aβ25-35-induced damage in bEnd.3 cells

In order to investigate the effects of neuroprotective peptide SNP-9 which is derived from silk fibroin hydrolysate on the injury of the blood-brain barrier in Alzheimer′s disease (AD), Aβ25-35 was used to damage brain microvascular endothelial cells bEnd.3 to establish AD injury model and drug intervention was performed.MTT assay was used to detect the effects of SNP-9 and Aβ25-35 on cell viability.RT-qPCR was used to determine the effects of SNP-9 and Aβ25-35 on the mRNA levels of tight junctions (TJs)-related ZO-1, occludin and claudin-5.Western blot was used to detect the effects of SNP-9 and Aβ25-35 on the protein levels of TNF-α, phosphorylated NF-κB, NF-κB, IκBα and RAGE.The results showed that SNP-9 reduced bEnd.3 cell damage induced by Aβ25-35, and improved the abnormal mRNA levels of ZO-1, occludin and claudin-5 in model cells.It alleviated the abnormal protein levels of TNF-α, phosphorylated NF-κB, IκBα and RAGE induced by Aβ25-35. These results suggest that SNP-9 may regulate the levels of TNF-α in model cells by influencing RAGE/NF-κB pathway, and then ameliorate TJs-related abnormalities and alleviate bEnd.3 cell injury induced by Aβ25-35.
6.Systematic analysis on expression quantitative trait loci identifies a novel regulatory variant in ring finger and WD repeat domain 3 associated with prognosis of pancreatic cancer

Background::Pancreatic adenocarcinoma (PAAD) is an extremely lethal malignancy. Identification of the functional genes and genetic variants related to PAAD prognosis is important and challenging. Previously identified prognostic genes from several expression profile analyses were inconsistent. The regulatory genetic variants that affect PAAD prognosis were largely unknown.Methods::Firstly, a meta-analysis was performed with seven published datasets to systematically explore the candidate prognostic genes for PAAD. Next, to identify the regulatory variants for those candidate genes, expression quantitative trait loci analysis was implemented with PAAD data resources from The Cancer Genome Atlas. Then, a two-stage association study in a total of 893 PAAD patients was conducted to interrogate the regulatory variants and find the prognostic locus. Finally, a series of biochemical experiments and phenotype assays were carried out to demonstrate the biological function of variation and genes in PAAD progression process.Results::A total of 128 genes were identified associated with the PAAD prognosis in the meta-analysis. Fourteen regulatory loci in 12 of the 128 genes were discovered, among which, only rs4887783, the functional variant in the promoter of Ring Finger and WD Repeat Domain 3 ( RFWD3), presented significant association with PAAD prognosis in both stages of the population study. Dual-luciferase reporter and electrophoretic mobility shift assays demonstrated that rs4887783-G allele, which predicts the worse prognosis, enhanced the binding of transcript factor REST, thus elevating RFWD3 expression. Further phenotypic assays revealed that excess expression of RFWD3 promoted tumor cell migration without affecting their proliferation rate. RFWD3 was highly expressed in PAAD and might orchestrate the genes in the DNA repair process. Conclusions::RFWD3 and its regulatory variant are novel genetic factors for PAAD prognosis.
7.Progress in detection and modeling of quorum sensing molecules of foodborne pathogens.

Quorum sensing (QS) plays a major role in the outbreak mechanism of foodborne diseases caused by food poisoning and food spoilage. QS affects the formation of cell membrane and pathogenicity ofpathogenic bacteria. Through the in-depth understanding of QS molecules of food-borne pathogens, we describe here the types of signal molecules produced by Gram-negative and Gram-positive bacteria, and the differences in QS molecules. Meanwhile, we introduce the detection of QS molecules by different technologies. According to the influence of QS on food, we propose also future research needs for the control of foodborne pathogenic bacteria.

8.A finite element analysis of torque capabilities of the DamonQ system using the double wire technique

BACKGROUND:In the orthodontic treatment, double wire technique is commonly used to correct the torque of anterior teeth;however, the underlying mechanical mechanisms remain unclear. OBJECTIVE:To study the torque capabilities of DamonQ system through the three-dimensional finite element method. METHODS:Three-dimensional finite element models of Damon Q brackets with different dimension arch wires were established and were grouped according to arch wire with different dimensions. The torque capabilities of Damon Q system with different dimensions and materials arch wire were analyzed. RESULTS AND CONCLUSION:The Damon Q brackets with single or double wires had the same variation of torque, torque angle curve changes under the condition of different size and material arch wires. Torque moment increased gradually with increasing torque angle. Rectangular-round double wires produced larger torque moment compared with single rectangular wire, whereas double round wires produced smal er torque moment compared with single rectangular wire. The torque moment produced by rectangular-round double wires or by single rectangular wire was approximately 2-8 times or 1-5 times more than that by double round wires. The torque moment that rectangular-round double wires produced was larger than that single wire produced in the DamonQ system. In the early clinical practice, we can use double round wires to control root movement. In the middle-term of the treatment, we can use rectangular-round double wires to control root movement as the mild forces produced by them reduce root absorption. This study lays a foundation for the application of Damon Q bracket with double wire technology in the clinic.
9.A case of cutaneous and systemi c plasmacytosis

A 46-year-old male patient developed scatterred reddish-brown plaques and nodules on the back 6 years prior to the presentation. Then, the lesions gradually spread to the axillary fossa and protothorax, and became indurated with slight itching in winter. Laboratory examination revealed hypergammaglobulinemia. Computed tomography(CT)scan showed multiple nodular or patchy shadows in both lungs, lymphadenectasis in axillary, mediastinal and inguinal regions, and spleen enlargement. Histopathological examination of skin lesions showed granulomatous infiltrates with plenty of lymphocytes, histiocytes and mature plasma cells in the middle and lower dermis with the presence of lymphoid follicle-like structures, but no cell atypia was observed. Immunohistochemical study showed positive staining for CD38, CD138, CD79a, κ and λ light chains. According to clinical manifestations and laboratory examination results, the patient was diagnosed with cutaneous and systemic plasmacytosis.
10.A comparative assessment of torque capabilities between lingual and labial brackets using three-dimension finite element

Objective:To analyse the difference of torque performance between lingual and labial brackets as well as the influence of the archwires of different materials with different dimension on the torque capability of lingual brackets.Methods:The 3D finite mo-del of lingual and labial brackets from the right upper canine to the left upper canine was established,all brackets were with 0.56 mm slot size.The center of the bracket slot of the left upper central incisor was used as the central point and the two brackets were rotated clockwise and counterclockwise with a rotation angle of ±20°along the horizontal axis of bracket slot.The torque load on the left up-per central incisors with different materials and dimensions of the archwires was simulated.Results:The torque force of both lingual and labial brackets increased with the increase of rotation angle and the elastic modulus of archwire.The lingual archwire generated torque force was nearly 3 -5 times more than the labial system under the same torque angle.The torque value generated with 0.48 mm ×0.64 mm archwires was greater than 0.43 mm ×0.64 mm.The lingual bracket generated torque force decreased gradually from nickel-titanium(NiTi),Beta-titanium(TMA)to stainless steel(SS)under the same dimension.Conclusion:Torque moments of lin-gual and labial brackets both increases with the increase of torque angle in each operating condition.The lingual orthodontic brackets produce more torque force than labial brackets under the same torque angle.Torque performance of lingual bracket was affected by the material and dimension of the archwires.
