1.Association of PPIs use with short-term and long-term mortality risk in patients with severe ischemic stroke:a retrospective cohort study based on the MIMIC-Ⅲ database

Objective To investigate the association of proton pump inhibitors(PPIs)use with short-term and long-term mortality risk in patients with severe ischemic stroke.Methods This retrospective study based on the U.S.Medical Information Mark for Intensive Care Ⅲ(MIMIC-Ⅲ)database,ICU patients aged ≥18 years with the first ICU admission and a diagnosis of ischemic stroke were finally included in the study.All enrolled subjects were divided into PPIs group and non-PPIs group according to whether they had used PPIs(pantoprazole,lansoprazole and omeprazole)during hospitalization.Kaplan-Meier survival analyses and Cox regression models were used to analyze the association between the use of PPIs and the risk of ICU death,30 d risk of death,90 d risk of death in patients with severe ischemic stroke.Results A total of 1 015 patients were included,402 cases in the PPIs group and 613 in the non-PPIs group.The ICU-mortality,30 d and 90 d mortality were 15.37%,13.60%and 20.10%,respectively.Kaplan-Meier survival analyses illustrated that the PPIs group survived better than non-PPIs group in ICU mortality analysis(P=0.002).In Cox regression analysis,after adjustment for potential confounders,the hazard ratio(HR)for ICU mortality in the PPIs group relative to the non-PPIs group was 0.671 9(95%CI 0.478 8 to 0.942 8,P=0.021),but there was no significant difference between 30 d and 90 d mortality(P>0.05).Conclusion In patients with severe ischemic stroke,the use of PPIs may be effective in reducing the risk of ICU death,but does not improve 30 d and 90 d risk of death in patients.
2.Research progress in the effects of microRNAs on cardiac electrophysiological properties

The incidence of perioperative arrhythmias is 4%~20%in noncardiac surgery and 10%~30%in cardiothoracic surgery,with the incidence of perioperative arrhythmias in cardiac surgery patients as high as 90%.Perioperative arrhythmias can prolong hospitalization and increase morbidity and mortality during hospitalization.Studies have shown that aberrantly expressed microRNAs in cardiovascular diseases can participate in cardiac auto-maticity,excitability and conductivity by regulating ion channels,gap junction proteins,and intracellular Ca2+ handling proteins,thereby regulating cardiac electrophysiological homeostasis and arrhythmias.In this article,we will review the research progress of microRNAs and arrhythmias mainly from the perspective of cardiac electrophysi-ological imbalance.
3.Correlation analysis between renal ectopic fat deposition and early diabetic kidney disease

Objective To investigate the value of iterative decomposition of water and fat with echo asymmetry and least-squares(IDEAL-IQ)sequence in evaluating the correlation between renal ectopic fat deposition and early diabetic kidney disease(DKD)in patients with diabetes mellitus(DM).Methods A total of 51 patients with T1DM or T2DM were enrolled in this study from the Endocrinology and Metabolism Department in Affiliated Jinhua Hospital,Zhejiang University School of Medicine during January 2022 to July 2023.All the patients were divided into two groups according to the results of urine albumin creatinine ratio(UACR):normal or slightly increased urinary micro albumin group(NAU,UACR<30 mg/g,n=27)and diabetic kidney disease group(DKD,UACR 30~300 mg/g,n=24).Meanwhile,55 healthy subjects in health examination were selected as control group(NC).Pearson correlation analysis was used to analyze the correlation between renal FF and other indicators.Logistic regression analysis was used to analyze the influencing factors of early DKD,and the diagnostic efficiency of renal FF for early DKD was analyzed by the ROC curve.Results Serum creatinine(Scr)and renal fat fraction(FF)value were higher in DKD group than in NC and NAU groups(P<0.05).Pearson correlation analysis showed that kidney FF were positively correlated with UACR and Hcy(P<0.05).Logistic regression analysis showed that after adjusting for confounding factors,renal FF was a contributing factor to early DKD.The ROC curve revealed that model 2 had the highest diagnostic efficiency,with AUC=0.801,sensitivity of 66.7%,specificity of 85.2%,accuracy of 80.0%,and a renal FF cut-off value was 2.46%.Conclusion IDEAL-IQ could non-invasively measure the renal fat content in DM patients,and the renal FF were significantly associated with DKD in early stage.
4.Levels and clinical value of serum sICAM-1,TLR4,and HPT in patients with severe cerebral infarction complicated by nosocomial infection

Objective To analyze the serum levels of soluble intercellular adhesion molecule-1(sICAM-1),toll like receptor 4(TLR4)and haptoglobin(HPT)in patients with severe cerebral infarction complicated by nosocomial infection and their clinical value.Methods A total of 80 patients with severe cerebral infarction who visited the hospital from September 2019 to December 2023 were selected as the study group,and were separated into an infection group(29 cases)and a non infection group(51 cases)based on whether the pa-tients had nosocomial infection.92 healthy individuals who underwent physical examination in the hospital were selected as the health group.Enzyme linked immunosorbent assay(ELISA)was applied to detect serum levels of sICAM-1,TLR4,and HPT in each group,and clinical data of infection group and non infection group were analyzed.Pearson method was applied to analyze the correlation between serum sICAM-1,TLR4,and HPT levels in patients with severe cerebral infarction complicated by nosocomial infection.Multivariate Logis-tic regression was applied to analyze the factors affecting the occurrence of nosocomial infection in patients with severe cerebral infarction.Receiver operating characteristic(ROC)curve was applied to evaluate the di-agnostic value of serum sICAM-1,TLR4,and HPT levels for the occurrence of nosocomial infection in patients with severe cerebral infarction.Results Compared with the health group,the serum levels of sICAM-1,TLR4,and HPT in the study group were all elevated,and the differences were statistically significant(P<0.05).Compared with health group,serum sICAM-1,TLR4 and HPT levels in the non infection group and the infection group were increased,and the differences were statistically significant(P<0.05).Compared with the non infection group,the serum levels of sICAM-1,TLR4,and HPT in the infection group were all elevated,and the differences were statistically significant(P<0.05).The serum sICAM-1 was positively correlated with TLR4 and HPT levels,and TLR4 was positively correlated with HPT in patients with severe cerebral in-farction complicated by nosocomial infection(r=0.473,0.529,0.421,P<0.001).There were significant differences between the infection group and the non infection group in the presence or absence of diabetes and smoking history(P<0.05).Diabetes,smoking history,serum sICAM-1,TLR4,and HPT levels were the in-fluencing factors of nosocomial infection in patients with severe cerebral infarction(P<0.05).The area under the curve(AUC)of combined diagnosis of serum sICAM-1,TLR4,and HPT for nosocomial infection in pa-tients with severe cerebral infarction was 0.991,with sensitivity of 96.55%,and specificity of 94.12%,which were superior to those of the individual diagnosis of sICAM-1,TLR4,and HPT(Z combined diagnosis-SICAM-1=1.972,P=0.049,Z combined diagnosis-TLR4=3.068,P=0.002,Z combined diagnosis-HPT=2.337,P=0.020).Conclusion The serum levels of sICAM-1,TLR4,and HPT are all elevated in patients with severe cerebral infarction complicated by nosocomial infection.The combined detection of the three has high clinical diagnostic value for the occurrence of nosocomial infection in patients with severe cerebral infarction.
5.Efficacy and Safety of Pulse Magnetic Therapy System in Insomnia Disorder: A Multicenter, Randomized, Double-Blind, Placebo-Controlled Trial

Objective:
This study’s objective is to assess the efficacy and safety of Pulsed Magnetic Therapy System (PMTS) in improving insomnia disorder.
Methods:
Participants with insomnia disorder were randomly assigned to receive either PMTS or sham treatment for four weeks (n= 153; PMTS: 76, sham: 77). Primary outcomes are the Insomnia Severity Index (ISI) scores at week 0 (baseline), 1, 2, 3, 4 (treatment), and 5 (follow-up). Secondary outcomes are the Pittsburgh Sleep Quality Index at baseline and week 4, and weekly sleep diary-derived values for sleep latency, sleep efficiency, real sleep time, waking after sleep onset, and sleep duration.
Results:
The ISI scores of the PMTS group and the sham group were 7.13±0.50, 11.07±0.51 at week 4, respectively. There was a significant group×time interaction for ISI (F3.214, 485.271=24.25, p<0.001, ηp 2=0.138). Only the PMTS group experienced continuous improvement throughout the study; in contrast, the sham group only experienced a modest improvement after the first week of therapy. At the end of the treatment and one week after it, the response of the PMTS group were 69.7% (95% confidence interval [CI]: 58.6%–79.0%), 75.0% (95% CI: 64.1%–83.4%), respectively, which were higher than the response of the sham group (p<0.001). For each of the secondary outcomes, similar group×time interactions were discovered. The effects of the treatment persisted for at least a week.
Conclusion
PMTS is safe and effective in improving insomnia disorders.
6.Survey on job satisfaction and analysis of influencing factors of online contracted nurses

Objective:To survey the job satisfaction of online contracted nurses who provide " Internet plus nursing services" for reference of hospital managers in improving their management mechanism in this regard.Methods:Based on the two-factor theory, a questionnaire was designed and a purposive sampling method was used to survey the online contracted nurses in Anhui province in April and May 2022. The motivational factors included such five dimensions as workload, work content, colleague relationship, doctor-patient relationship, and their own development, and the healthcare factors included such three dimensions as salary, job recognition and social status. The questionnaire data and job satisfaction scores were analyzed descriptively, and the correlation between the overall job satisfaction of the online contracted nurses, while each dimension was analyzed by Pearson correlation analysis, and the influence of each dimension on job satisfaction was analyzed by stepwise regression analysis.Results:A total of 335 valid questionnaires were recovered. The mean score of job satisfaction of online contracted nurses was (2.26±0.38), with the highest score of (2.56±0.53) for salary satisfaction and the lowest score of (1.78±0.67) for job recognition, and each dimension was positively correlated with job satisfaction ( r=0.34-0.88, P<0.01). Regression analysis showed that workload ( B=0.07), salary ( B=0.11), job content ( B=0.23), social status ( B=0.12), and self-development ( B=0.15) were the main factors affecting their job satisfaction ( P<0.01). Conclusions:The job satisfaction of online contracted nurses was at a medium level, mainly influenced by workload, salary, job content, social status and their own development. It is recommended that hospitals implement multiple targeted measures to improve the job satisfaction of online contracted nurses and promote the healthy development of " Internet plus nursing services" .
7.Construction of a training course system for online appointment nurses based on job competence

Objective:To construct a training course system for online appointment nurses based on post competence,and to provide references for the training and development of online appointment nurses in China.Methods:From January 2021 to May 2022, based on the theory of post competence, literature review, policy research, questionnaire survey and and expert consultation were used to establish the training course system of online appointment nurses.Results:The effective recovery rates of the two rounds of expert consultation questionnaires were 18/18 and 17/18, respectively; the expert authority coefficient was 0.928 and 0.938, respectively; and the Kendall harmony coefficient was 0.185 and 0.284, respectively, the differences were statistically significant (all P<0.001). The final content of the training course system for online appointment nurses included 4 first-level indicators,which were professional knowledge, professional skills, professional abilities and traits, 19 second-level indicators and 58 third-level indicators. Conclusions:The training course system of online appointment nurses based on post competency is scientific, reasonable and prominent,which can provide reference for training of online appointment nurses in China.

Health behavior decision-making often involves the balance of gains and losses at different time points,which affects the choice and maintenance of health behaviors.As a cognitive processing method of"pre-experiencing"future events,Episodic Future Thinking(EFT)can promote the formation of health intentions and health behavior change by increasing the value of future health outcomes.This paper reviews the concept,intervention model and action effect of EFT in health behaviors,in order to provide references for nurses to innovate health education and behavior management from the perspective of behavioral economics.
9.Lipopolysaccharide induced intestinal epithelial injury: a novel organoids-based model for sepsis in vitro.

BACKGROUND:
Advances in organoid culture technology have provided a greater understanding of disease pathogenesis, which has been rarely studied in sepsis before. We aim to establish a suitable organoids-based intestinal injury model for sepsis.
METHODS:
Stable passaged organoids were constructed and pre-treated with lipopolysaccharide (LPS) to mimic sepsis-induced intestinal injury. The LPS-induced sepsis model was used as a reference. We used quantitative real-time polymerase chain reaction to evaluate the RNA levels of inflammatory factors and antimicrobial peptides. Enzyme-linked immunosorbent assay was used to evaluate the protein levels, hematoxylin and eosin staining was used to evaluate the pathology of the small intestine of mice, and immunohistochemistry and immunofluorescence were used to evaluate the intestinal epithelial barrier function. Perkin Elmer Operetta™ was used to obtain high-resolution images of three-dimensional organoids.
RESULTS:
An LPS concentration >150 μg/mL after 24 h was identified to cause organoid growth restriction. The fluorescence intensity of zonula occludens-1 and occludins at LPS concentrations >100 μg/mL decreased significantly after 24 h. After LPS stimulation for 8 h, the RNA expression levels of interleukin (IL)-1α, tumor necrosis factor alpha, granulocyte-macrophage colony-stimulating factor, IL-6, and regenerating islet-derived protein 3 alpha, beta, and gamma increased. These results resembled those of intestinal epithelial layer alterations in a mouse sepsis model. For IL-10, the RNA expression level increased only when the LPS level >200 μg/mL for 24 h.
CONCLUSIONS
This study provides the primary intestinal in vitro model to study the effects of LPS-induced intestinal injury resembling sepsis. This model provides a platform for immune associated mechanism exploration and effective drug screening.

10.Research Progress of Acquired Resistance Mediated by MET Amplification in Advanced Non-small Cell Lung Cancer.

Mesenchymal-epithelial transition factor (MET) amplification is an important driver of resistance in epidermal growth factor receptor (EGFR)-mutant non-small cell lung cancer (NSCLC), and the combination of MET proto-oncogene (MET) and EGFR-tyrosine kinase inhibitors (TKIs) has shown promise in overcoming this molecularly defined acquired resistance. Emerging data also demonstrate MET amplification as a resistance driver to TKIs-treated anaplastic lymphoma kinase (ALK)-, RET-, and ROS1-fusion NSCLC. Here, we review the literature on recent research progress of MET amplification as a resistance driver to targeted therapy in oncogene-driven NSCLC and summarize the progress of clinical strategies to overcome the resistance mechanism.
