1.Study on the clinical characteristics of Chinese and Western medicine in 474 children with genetically-related tic disorder

Objective This study aimed to analyze the impact of genetic factors on the clinical presentation of tic disorder and investigate the pathogenesis of tic disorder considering different sources of genetic influence and generations,along with the distribution characteristics of evidence from Chinese medicine.Methods Inclusion of 474 cases of tic disorder was assembled(from October,2020 to October,2023),and clinical data on the children,including gender,age of onset,disease duration,initial symptoms,severity,comorbidities,and family history,and TCM patterns were collected.Firstly,the children were divided into genetic and nongenetic groups based on family history,and their clinical manifestations were observed.Secondly,the age of peak incidence of tic disorder in China(6 or 9 years)was used as the age segmentation point to explore the influence of heredity on clinical manifestations at different ages and the distribution of TCM patterns.Finally,children with genetically-related tic disorder were selected as probands to carry out family studies.The morbidity of the first-degree,second-degree and third-degree relatives of the probands were investigated to analyze the morbidity characteristics and distribution of TCM patterns of children with tic disorder under different genetic types.Results Out of 474 children,226 cases(47.7%)belonged to the genetic group,and 248 cases(52.3%)to the nongenetic group.Compared with the non-genetic group,the genetic group had a younger age of onset(P=0.013),a longer duration of illness(P=0.011),a higher degree of severity(P<0.01),and more comorbid conditions(P=0.016).Children in the genetic group with an age of onset under 6 years accounted for a larger proportion of the liver-hyperactive and wind-driven pattern(26.5%)and a smaller proportion of the qi depression transforming into fire pattern(17.1%),which were significantly different from those in the non-genetic group(P=0.016).In the genetic group,146 cases(64.6%)were inherited from the father,80 cases(35.4%)from the mother,and there was no significant difference in the developmental characteristics of children with tic disorder from different sources of genetic influence.Furthermore,183 cases(81.0%)were inherited from the parent-child generation,26 cases(11.5%)from the second generation,and 17 cases(7.5%)from the third generation;the difference in initial symptoms between different genetic generations was statistically significant(P=0.042).Conclusion Children with genetically related tic disorder have a younger age of onset,their condition is more severe,and they are more likely to be comorbid with other psychiatric disorders.The variation in the distribution of Chinese medicine patterns among children with tic disorder of different ages of onset.
2.Clinical assessment for motor cognitive risk syndrome in patients with enlarged perivascular spaces

Objective To investigate the incidence of motoric cognitive risk(MCR)syndrome and declines in both motor and cognitive functions in elderly patients with enlarged perivascular spaces(EPVS).Methods A total of 113 patients(≥60 years old)with EPVS identified by head MRI admitted to our department from March 1 to November 30,2023 were consecutively recrui-ted,and according to having MCR syndrome or not,they were divided into MCR group(44 cases)and non-MCR group(69 cases).Cognitive function and motor function were assessed with aid of Montreal Cognitive Scale(MoCA),Simple Mental State Examination Scale(MMSE),trail making test,and motor function assessments,including Tinetti Balance and Gait Scale,Short Physical Performance Battery(SPPB),8-meter walk,Timed up and Go test(TUGT),walking and contin-uous subtraction task(WSS),walking and semantic fluency task(WSF),and TUGT and serial subtraction task(TUGSS).The motor and cognitive functions were compared between the two groups,and the risk factors related to MCR syndrome in EPVS patients were analyzed by multi-variate logistic regression.Results Compared with the non-MCR group,the MCR group had low-er MoCA score,total SPPB score,8-meter walk speed,and longer times of TMT-A,TMT-B,WSS,WSF and TUGSS(P<0.05,P<0.01).Multivariate logistic regression analysis showed that in Model 1(including variables with differences in univariate analysis),MoCA score,8-meter walk speed,and TUGSS time were independent risk factors for MCR syndrome in EPVS patients(P<0.05,P<0.01);in Model 2(adjusting for age,sex,education level,and comorbidities),MoCA score(OR=0.621,95%CI:0.432-0.929,P=0.010),8-meter walk speed(OR=1.332,95%CI:1.115-1.591,P=0.002),and TUGSS time(OR=0.393,95%CI:0.232-0.665,P=0.001)re-mained independent risk factors for MCR syndrome in EPVS patients.Conclusion Among elderly EPVS patients,those with MCR syndrome show significantly worse cognitive and motor function compared to those without.MoCA score,8-meter walk speed,and TUGSS time are related factors for MCR occurrence in EPVS patients.
3.Prevalence and risk evaluation of cardiovascular disease in the newly diagnosed prostate cancer population in China: A nationwide, multi-center, population-based cross-sectional study

Background::Cardiovascular disease (CVD) has emerged as the leading cause of death from prostate cancer (PCa) in recent decades, bringing a great disease burden worldwide. Men with preexisting CVD have an increased risk for major adverse cardiovascular events when treated with androgen deprivation therapy (ADT). The present study aimed to explore the prevalence and risk evaluation of CVD among people with newly diagnosed PCa in China.Methods::Clinical data of newly diagnosed PCa patients were retrospectively collected from 34 centers in China from 2010 to 2022 through convenience sampling. CVD was defined as myocardial infarction, arrhythmia, heart failure, stroke, ischemic heart disease, and others. CVD risk was estimated by calculating Framingham risk scores (FRS). Patients were accordingly divided into low-, medium-, and high-risk groups. χ2 or Fisher’s exact test was used for comparison of categorical variables. Results::A total of 4253 patients were enrolled in the present study. A total of 27.0% (1147/4253) of patients had comorbid PCa and CVD, and 7.2% (307/4253) had two or more CVDs. The enrolled population was distributed in six regions of China, and approximately 71.0% (3019/4253) of patients lived in urban areas. With imaging and pathological evaluation, most PCa patients were diagnosed at an advanced stage, with 20.5% (871/4253) locally progressing and 20.5% (871/4253) showing metastasis. Most of them initiated prostatectomy (46.6%, 1983/4253) or regimens involving ADT therapy (45.7%, 1944/4253) for prostate cancer. In the present PCa cohort, 43.1% (1832/4253) of patients had hypertension, and half of them had poorly controlled blood pressure. With FRS stratification, as expected, a higher risk of CVD was related to aging and metabolic disturbance. However, we also found that patients with treatment involving ADT presented an originally higher risk of CVD than those without ADT. This was in accordance with clinical practice, i.e., aged patients or patients at advanced oncological stages were inclined to accept systematic integrative therapy instead of surgery. Among patients who underwent medical castration, only 4.0% (45/1118) received gonadotropin releasing hormone antagonists, in stark contrast to the grim situation of CVD prevalence and risk.Conclusions::PCa patients in China are diagnosed at an advanced stage. A heavy CVD burden was present at the initiation of treatment. Patients who accepted ADT-related therapy showed an original higher risk of CVD, but the awareness of cardiovascular protection was far from sufficient.

5.Effect of Quercus Salicina Extract Capsules on preventing adherent stones on ureteral stent tube after percutaneous nephrolithotomy

Objective:To explore the effect of Quercus Salicina Extract Capsulese on preventing the formation of adherent stones on ureteral stent tube after percutaneous nephrolithotomy (PCNL).Methods:The clinical data of 186 patients who underwent PCNL due to unilateral renal stone in the Affiliated Hospital of Xuzhou Medical University from October 2018 to April 2020 were retrospectively analyzed. All of the patients were indwelling 6 F ureteral stent tube during operation. After postoperative reexamination of kidneys, ureters and bladder, it was confirmed that the postoperative residual stones were clinically meaningless stones (maximum diameter ≤ 4 mm). According to postoperative medication, they were divided into drug group ( n=62) and control group ( n=124). Patients in the drug group were given oral Quercus Salicina Extract Capsules, while patients in the control group did not take the drug. Both groups received the same health education and dietary guidance after operation. The formation of adherent stones on ureteral stent tube was observed when the ureteral stent tube was removed 6 weeks after PCNL. Meanwhile, the adverse reaction, complication and treatment satisfaction of the patients were recorded during the period of taking the drug. Measurement data were expressed as mean ± standard deviation ( ± s), and t-test was used for comparison between groups; the Chi-square test was used to compare the count data between groups. Results:When the ureteral stent tube was removed 6 weeks after PCNL, the weight of adherent stones on ureteral stent tube in the drug group was (334.20±26.65) mg for male, and (336.00±25.64) mg for female. In the control group, the weight of adherent stones on ureteral stent tube was (374.11±42.28) mg for male, (374.42±42.44) mg for female. The weight of adherent stones on ureteral stent tube in the drug group was significantly lighter than that in the control group, and the difference was statistically significant ( P<0.01). The drug group had no obvious serious adverse reaction during the period of taking the drug. At the same time, the complications of the drug group during the intubation period were significantly less than the control group, and the difference was statistically significant ( P=0.040). The satisfaction of patients in the drug group was 93.5%, and that in the control group was 82.3%. The difference was statistically significant between the two groups ( P=0.036). Conclusion:Quercus Salicina Extract Capsules can effectively prevent the occurrence of adherent stones on ureteral stent tube after PCNL, and there are no serious adverse reaction, which is worthy of clinical promotion.
