1.Effects of Tongguan Capsules (通冠胶囊)on Left Ventricular Remodeling and Gut Microbiota in Patients with Acute ST-segment Elevation Myocardial Infarction: A Randomized Controlled Clinical Study

		                        			ObjectiveTo observe the efficacy and safety of Tongguan Capsule(通冠胶囊)on ventricular remodeling in patients after acute myocardial infarction (AMI), and to explore the possible mechanism. MethodsA total of 53 ST-segment elevation myocardial infarction (STEMI) patients were collected and randomly divided into 26 cases in the treatment group and 27 cases in the control group. The control group was given conventional therapy after percutaneous coronary intervention (PCI) for AMI, and the treatment group was given Tongguan Capsules (4.5 g each time, 3 times a day) on the basis of the control group. The course of treatment was 12 weeks. Echocardiographic data and stool samples were collected from subjects before and after the intervention, and left ventricular ejection fraction (LVEF), left ventricular end-diastolic volume index (LVEDVi), left ventricular end-systolic volume index (LVESVi), left ventricular mass index (LVMi) were measured and calculated, so as to compare the number of cases of left ventricular remodeling in the two groups. At the same time, 16S rDNA sequencing was performed on the stool samples to analyze the diversity of gut microbiota (GM), the composition of the GM, the GM difference between the groups; recording the incidence of major adverse cardiovascular events (MACEs) and adverse reactions of patients in the two groups during the study period; and performing Spearman's correlation analyses of the post-treatment flora data with LVEF, LVEDVi, LVESVi, LVM, and LVMi in the two groups. ResultsOne case fail to follow up in the treatment group and 2 cases fail to follow up in the control group, and 25 cases in each of the two groups were finally included in the analysis. LVEDVi, LVESVi, and LVMi of the control group after treatment were higher than those before treatment (P<0.05); after treatment, LVEDVi, LVESVi and LVMi in the treatment group were lower than those in the control group (P<0.05). Left ventricular remodeling occurred in 5 cases (20.00%) in the treatment group and 13 cases (52.00%) in the control group, and the incidence of left ventricular remodeling in the treatment group was lower than that in the control group (P=0.03). After treatment, the ACE estimation and Chao estimation of bacterial abundance in the treatment group were higher than that before treatment and that in the control group (P<0.05). The treatment group showed an increase in Mycobacterium anisopliae phylum and a decrease in Mycobacterium thickum, Mycobacterium anisopliae phylum, and Mycobacterium patella phylum after treatment (P<0.05). When comparing between groups after treatment, the relative abundance of Prevotella, Agathobacter, Dialister, Eubacterium coprostanoligenes group was up-reuglated and the relative abundance of Enterococcus was down-regulated in the treatment group(P<0.05 or P<0.01). There was no statistically significant difference in the incidence of MACEs and the occurrence of adverse reactions between groups during treatment (P>0.05). The results of correlation analysis showed that Prevotella were positively correlated with LVEF and negatively correlated with LVEDVi, LVESVi, LVM, and LVMi (P<0.01). Agathobacter group were negatively correlated with LVEDVi, LVESVi (P<0.01); Enterococcus group were positively correlated with LVESVi (P<0.05). ConclusionTongguan Capsules can improve ventricular remodeling in patients with acute ST-segment elevation myocardial infarction with better safety; its mechanism may be related to adjusting the enrichment of related bacteria such as Prevotella, Dialister and Enterococcus and other related bacterial genera, increasing the colonization and diversity of beneficial bacteria, and adjusting the structure of GM. 
		                        		
		                        		
		                        		
		                        	
2.Discussion on the medication law of TCM compound patents in treating diabetic retinopathy based on data mining

		                        			Objective:To explore the law of Chinese medicine patent compound prescription in treating diabetic retinopathy (DR).Methods:The patents of TCM compound for the treatment of DR Published in the patent database of China National Knowledge Resource Database (CNKI) from the establishment of the database to September 1, 2022 were searched and screened. The Ancient and Modern Medical Record Cloud Platform V2.0 was used to analyze the frequency of medication, nature, taste, and meridian distribution, and the rules of compatibility of medication were mined and analyzed based on association rules and cluster analysis.Results:A total of 113 compound patents were included, involving 201 kinds of Chinese materia medica. The top five drugs in frequency of use were Rehmanniae Radix, Notoginseng Radix et Rhizoma, Angelicae Sinensis Radix, Ganoderma and Astragali Radix. Their properties were mostly cold and warm, and their tastes were sweet and bitter. The top three meridians were liver meridian, lung meridian and kidney meridian. Six pairs of commonly used drugs were obtained, and four kinds of medicines were obtained by clustering analysis. Complex network analysis showed that core TCM combination was Rehmanniae Radix, Notoginseng Radix et Rhizoma, Angelicae Sinensis Radix, Ganoderma, Astragali Radix, Salviae Miltiorrhizae Radix et Rhizoma, Lycii Fructus, Prinsepiae Nux, and Cuscutae Semen.Conclusions:TCM compound patents for the treatment of DR are mainly used to treat cold and liver meridian, and the medication is mainly used to replenish qi and nourish yin, clear heat and cool blood, promote blood circulation and remove blood stasis, and tonify liver and kidney. The drug pairs are mainly used to supplement qi and nourish yin, clear heat and detoxify, invigorate qi and nourish blood, and invigorate qi and stop bleeding. The modified Danggui Buxue Decoction and Siwu Decoction are often used in treatment, and wind drugs are added as appropriate to soothe liver and improve eyesight, which can provide reference for clinic.
		                        		
		                        		
		                        		
		                        	
3.Clinical Reference Strategy for the Selection of Treatment Materials for Maxillofacial Bone Transplantation: A Systematic Review and Network Meta-Analysis

		                        			 Bone graft materials have mixed effects of bone repair in the field of oral maxillofacial surgery. The qualitative analyses performed by previous studies imply that autogenous odontogenic materials and autogenous bone have similar effects on bone repair in clinical jaw bone transplantation. This retrospective systematic assessment and network metaanalysis aimed to analyze the best effect of clinical application of autogenous odontogenic materials and autogenous, allogeneic, and xenogeneic bone grafts in bone defect repair. A systematic review was performed by searching the PubMed, Cochrane Library, and other journal databases using selected keywords and Medical Subject Headings search terms. 10 Papers (n = 466) that met the inclusion criteria were selected. The assessment of heterogeneity did not reveal any overall statistical difference or heterogeneity (P = 0.051 [ 0.05), whereas the comparison between autogenous and allogeneic bone grafts revealed local heterogeneity (P = 0.071  0.1). Risk of bias revealed nine unclear studies and one high-risk study. The overall consistency was good (P = 0.065 [ 0.05), and the local inconsistency test did not reveal any inconsistency. The publication bias was good. The confidence regarding the ranking of bone graft materials after GRADE classification was moderate. The effects on bone repair in the descending order were as follows: autogenous odontogenic materials, xenogeneic bone, autogenous bone, and allogeneic bone. This result indicates that the autogenous odontogenic materials displayed stronger effects on bone repair compared to other bone graft materials. 4.Advances in the physiological functions of plant lipids in response to stresses.

		                        			Lipids are important components of living organisms that participate in and regulate a variety of life activities. Lipids in plants also play important physiological functions in response to a variety of abiotic stresses (e.g. salt stress, drought stress, temperature stress). However, most research on lipids focused on animal cells and medical fields, while the functions of lipids in plants were overlooked. With the rapid development of "omics" technologies and biotechnology, the lipidomics has received much attention in recent years because it can reveal the composition and function of lipids in a deep and comprehensive way. This review summarizes the recent advances in the functions and classification of lipids, the development of lipidomics technology, and the responses of plant lipids against drought stress, salt stress and temperature stress. 5.Short-term follow-up for unruptured wide-necked intracranial aneurysms treated with Pipeline embolization device

		                        			Objective To evaluate the short-term effect of Pipeline embolization device (PED)for the treatment of unruptured wide-necked intracranial aneurysms. Methods From October 2015 to September 2016,15 consecutive patients with unruptured wide-necked intracranial aneurysm (aneurysm neck and aneurysm body ratio ≥0. 5)treated with PED at the Department of Neurosurgery,the Second Affiliated Hospital of Nanchang University were enrolled retrospectively. Their clinical and imaging data were analyzed. Kamran scale was used to evaluate the embolization rate of aneurysms and the changes of the parent arteries. DSA examination was performed again at 6 -12 months after operation. Results Fifteen PED were implanted in 15 patients with unruptured wide-necked intracranial aneurysms,including 13ophthalmic artery aneurysms,1 posterior communicating artery aneurysm,and 1 cavernous sinus aneurysm. The technical success rate was 100% . Immediately after PED implantation,Karman rating of 15 cases were aneurysm grade 2 embolization,parent artery grade A (grade 2a). DSA examination was performed again at 6 - 12 months after operation showed that 14 patients were aneurysm grade 4,parent artery was grade A (grade 4a). One patient (ophthalmic artery aneurysm)underwent the second DSA examinations at 6 and 12 months after operation showed that the residual development of aneurysms. The aneurysm embolization was grade 3, and the parent artery was grade A (grade 3a). No branch artery occlusion was observed. Non of them had neurological deficit. The modified Rankin scale score was 0 in all 15 patients. Conclusion The use of PED in the treatment of unruptured wide-necked intracranial aneurysms has a higher occlusion rate. 6.Significance of the R.E.N. A.L. nephrometry scoring system in renal tumour of T1 stage.

OBJECTIVETo evaluate the application value of R.E.N. A.L. nephrometry score for surgery type decisions of T1 stage renal tumor.
METHODSClinical data including image data, surgery type and prognosis etc were collected retrospectively for 122 cases from January 2010 to December 2012. There were 76 male and 46 female patients and they were 29-82 years (mean 51 years). The body mass index was (22.8 ± 3.9) kg/m(2). The patients were undergoing surgical excision with renal tumor of T1 stage. The R.E.N. A.L. nephrometry score was analyzed to evaluate their relationships to surgery type (RN or NSS) and the approach of NSS (ONSS or LNSS) using chi-square tests, Fisher's exact tests, and logistic regressions analysis.
RESULTSAll surgery had been completed. The surgery included RN of 45 patients, LNSS of 45 patients and ONSS of 32 patients. The R.E.N. A.L. nephrometry score was significantly associated with the type of surgery (χ(2) = 27.89, P < 0.05), and the NSS approach (χ(2) = 12.87, P < 0.05). When the scores less than 7 points, it is majorly treated by nephron sparing surgery (92.9%), and when the scores more than 9 points, it is majorly treated by radical nephrectomy (69.4%). Individual component scores were analyzed to evaluate that they were all related to surgery type (χ(2) = 7.00-14.57, P < 0.05), and the individual component N associated the surgery type mostly. Furthermore, individual component R,E,N and L were statistically significant predictors of the NSS approach (χ(2) = 4.92-15.07, P < 0.05).
CONCLUSIONThe R.E.N. A.L. nephrometry scoring system provides a simple, useful, and stable system to character the salient renal anatomy of T1 stage, and can provide the best surgery approach.

7.Significance of the PADUA nephrometry scoring system in determining the operative method for T1 stage renal tumour

		                        			Objective To evaluate the efficacy of preoperative aspects and dimensions used for an anatomical (PADUA) scores in determining the surgical approach for T1 stage renal masses.Methods From Jan 2010 to Dec 2012,clinical data of 122 cases (76 males and 46 females),who underwent surgery for T1 stage renal masses,were collected retrospectively.The mean age was 51 years(range 21-81) and mean body mass index was (22.8±3.9) kg/m2.Sixty-three tumors were found in left kidney and 59 in right kidney.Among them,78 patients were diagnosed as T1a stage and 44 patients were T1b stage.In patients with T1a stage,56 received nephron sparing surgery (NSS) and 22 received radical nephrectomy (RN).In patients with T1b stage,21 received NSS and 23 received RN.The PADUA nephrometry score was analyzed to evaluate their relationships to surgical type and the approach of NSS.Results According to the PADUA nephrometry score,the number of low risk,middle risk and high risk patients were 24,62,26,respectively.Inlow risk group,middle risk group and high risk group,the proportion of RN and NSS was 8.3%/ 91.7%,30.6%/69.4%,66.7%/33.3%.In 77 patients received NSS,the unmber of laparoscopic NSS and open NSS was 18 ∶ 4,25 ∶ 18,2 ∶ 10,respectively.The PADUA nephrometry score was significantly associated with the type of surgery (x2 =23.16,P<0.01),and the NSS approach (x2 =13.57,P<0.01).Tumor size (HR =2.79 ; 95% CI,1.29-6.02 ; P< 0.01),percentage of tumor deepening into the kidney (HR =3.82; 95%CI,1.77-8.09; P<0.01),longitudinal (HR=4.00;95%CI,1.83-8.72; P<0.01),tumor relationships with renal sinus(HR=103.13; 95%CI,21.85-486.81 ; P<0.01),tumor relationships with urinary collecting system (HR =15.11 ; 95% CI,5.95-38.35 ; P< 0.01),rim tumor location (HR =3.50 ; 95% CI,1.61-7.59; P<0.01) were closely related with surgery approach.The correlation coefficients of relationship with renal sinus was highest (r=0.70).Conclusions The PADUA nephrometry score provides a simple,useful and stable system to character the salient renal anatomy and guide the surgery.Low risk group should consider the NSS as the first line therapy.NSS could also be chosen in the middle risk group.However,the renal anatomy in those patients should be referred.RN should be chosen in high risk group.
		                        		
		                        		
		                        		
		                        	
8.Resting energy expenditure in patients with well controlled type 2 diabetes mellitus

		                        			Objective To investigate the features of resting energy expenditure (REE) in patients with well controlled type 2 diabetes mellitus (T2DM). Methods Totally 45 T2DM patients with stable blood glucose were enrolled. The general conditions, biochemical indicators, measurements of REE ( MREE), and basal energy expenditure (BEE) calculated with Harris-Benedict formula (HBEE) and Owen formula (OBEE) were recorded and compared. Results MREE had no significant difference with HBEE or OBEE in T2DM patients with stable blood glucose (P > 0. 05). Correlation analysis showed that REE was significantly correlated with gender, age, body weight, body height, body surface area, and fat-free mass ( all P < 0.05 ), but was not correlated with body mass index, fat mass, fasting plasma glucose, postprandial plasma glucose, haemoglobin Alc, total cholesterol, triglyceride, total protein, albumin, and haemoglobin (P > 0. 05 ). Multiple regression analysis showed that fat free mass and age had the closest correlation with REE. Conclusions REE does not increase in T2DM patients with well controlled blood glucose. Factors that influence their REE are similar with healthy individuals.
