1.Reference range and impact factors of left ventricular trabecular and papillary muscle mass in Chinese adults explored by cardiac MRI

Objective To establish normal reference values for left ventricular trabecular and papillary muscle mass(TPMM)in Chinese adults using MRI and to explore its impact factors.Methods A total of 168 healthy Chinese adults were retrospectively included,and compacted and total left ventricular myocardial mass(LVM)were measured using traditional and dedicated methods,respectively.TPMM was calculated from the difference between total and compacted LVM.Independent sample t-tests and analysis of variance were used to explore the differences in TPMM among genders and age groups,while multiple linear regression was used to explore the independent correlation between TPMM and age,gender,heart rate,systolic blood pressure(SBP),fasting blood glucose(FBG),and body mass index(BMI).Results TPMM for men was significantly larger than that for female(P<0.001).TPMM in the elderly group was significantly larger in female(P<0.05),but not in men.Multiple linear regression showed that BMI and SBP were both independently positively correlated with TPMM,and female and heart rate were independently negatively correlated with TPMM(P<0.05).Conclusion This study provides age-and gender-specific normal reference values for TPMM in Chinese adults.Gender,heart rate,BMI,and SBP are all independently associated with TPMM.
2.Optical coherence tomography angiography manifestations of different degrees of nonproliferative diabetic retinopathy

AIM: To investigate the differences in varying stages of non-proliferative diabetic retinopathy(NPDR)using optical coherence tomography angiography(OCTA).METHODS: Cross-sectional study. A total of 77 cases(77 eyes)of diabetic patients were included, and they were divided into non-diabetic retinopathy(NDR; 23 eyes)and non-proliferative diabetic retinopathy(NPDR; 54 eyes)groups, further subdivided into mild NPDR(20 eyes), moderate NPDR(20 eyes), and severe NPDR(14 eyes). Foveal avascular zone(FAZ)area, superficial and deep capillary plexus densities(SSP and DSP), and visual acuity(LogMAR)were compared between NDR and NPDR groups. Furthermore, the visual acuity, FAZ area and levels of SSP and DSP were compared in different degrees of NPDR. Correlation analysis were conducted to elucidate relationships between FAZ area, visual acuity, SSP, DSP, and severity of the disease.RESULTS: Compared with the NDR group, the visual acuity(LogMAR)and macular FAZ area increased, while SSP and DSP were decreased in the NPDR group(P<0.05); there were significant differences in visual acuity, FAZ area and SSP and DSP levels in different degrees of NPDR(P<0.05). Visual acuity(LogMAR)and FAZ area displayed a positive correlation with the severity of disease, while SSP and DSP showed a negative correlation.CONCLUSION: With the progression of NPDR, the visual acuity(LogMAR)and FAZ area increased, and the SSP and DSP decreased.
3.Status quo of screen exposure and its determinants in 3- to 6-year-old children in Minhang district, Shanghai

【Objective】 To investigate the screen exposure status in 3- to 6-year-old children in Minhang district and to analyze its determinants, so as to provide a basis for scientific intervention of screen exposure in children. 【Methods】 A total of 1 035 children aged 3 to 6 years were selected from 30 classes in 10 kindergartens in Minhang district by multi-stage cluster sampling in October 2022. An online questionnaire survey was conducted by their parents. Screen exposure was difined as spending more than one hour on video products per day. Multivariate Logistic regression model was adopted to analyze the determinants of children screen exposure. 【Results】 Among 1 035 children, 730 experienced screen exposure (70.53%). The average time of daily screen exposure was (1.67±0.98)h. The daily screen exposure time of children aged 3, 4, 5 and 6 years at weekends was (1.77±1.13), (1.76±1.04), (1.98±1.10)h and(2.08±1.22)h, respectively, and the difference was statistically significant (F=3.98, P<0.01). Multivariate Logistic analysis showed that age group of 5 years old (OR=1.79, 95%CI:1.19 - 2.68), the caregivers using video products for 1 - 2hours/day (OR=2.61, 95%CI:1.90 - 3.60) and >2hours/day (OR=2.10, 95%CI:1.35 - 3.27) when accompanying children, parents supporting children using video products(OR=1.59, 95%CI:1.17 - 2.15), children using video products before bedtime(OR=1.94, 95%CI:1.32 - 2.86), and unsupervised use of video products for children(OR=1.94, 95%CI:1.36 - 2.77) were independent risk factors for children′s screen exposure(P<0.05). Father′s education level of bachelor(OR=0.61,95%CI:0.43 - 0.89), master and above(OR=0.49, 95%CI:0.34 - 0.73) was a protective factor for children′s screen exposure(P<0.05). 【Conclusions】 Attention should be paid to the problem of screen exposure of 3 -to 6 - year-old children. Targeted efforts should be made to popularize the knowledge of the harm of early screen exposure to children among caregivers, caregivers are advocated for the rule formulation of screen behavior, scientifically regulate children′s screen exposure behaviors, and increase parent-child communication, so as to reduce the adverse effects of screen exposure on children′s health.
4.The occurrence and influencing factors of vascular calcification in non-dialysis chronic kidney disease patients of stage 3-5

Objective:To explore the prevalence and independent associated factors of vascular calcification (VC) in non-dialysis chronic kidney disease (CKD) patients of stage 3-5.Methods:It was a single-center cross-sectional observational study. Non-dialysis stage 3-5 CKD patients ≥18 years old who were admitted to the Department of Nephrology, the First Affiliated Hospital of Sun Yat-sen University from May 1, 2022 to December 31, 2022 with VC evaluation were enrolled. The patients' general information, laboratory examination and imaging data were collected. Coronary artery calcification (CAC), thoracic aorta calcification (TAC), abdominal aorta calcification (AAC), carotid artery calcification and aortic valve calcification (AVC) were evaluated by cardiac-gated electron-beam CT (EBCT) scans, lateral lumbar x-ray, cervical macrovascular ultrasound and echocardiography, respectively. The differences in clinical data and the prevalence of VC at different sites of patients with different CKD stages were compared, and the prevalence of VC at different sites of patients in different age groups [youth group (18-44 years old), middle-aged group (45-64 years old) and elderly group (≥65 years old)] and patients with or without diabetes were compared. Multivariate logistic regression analysis was used to analyse the independent associated factors of VC for different areas.Results:A total of 206 patients aged (51±14) years were included, including 129 (62.6%) males. There were 44 patients with CKD stage 3 (21.4%), 51 patients with CKD stage 4 (24.8%), and 111 patients with CKD stage 5 (53.9%). CKD was caused by chronic glomerulonephritis [104 cases (50.5%)], diabetic kidney damage [35 cases (17.0%)], hypertensive kidney damage [29 cases (14.1%)] and others [38 cases (18.4%)]. Among 206 patients, 131 (63.6%) exhibited cardiovascular calcification, and the prevalence of CAC, TAC, AAC, carotid artery calcification, and AVC was 37.9%, 43.7%, 37.9%, 35.9% and 9.7%, respectively. The overall prevalence of VC in young, middle-aged and elderly patients was 24.6%, 73.6% and 97.4%, respectively. With the increase of age, the prevalence of VC in each site gradually increased, and the increasing trend was statistically significant (all P<0.001). The overall prevalence of VC in CKD patients with diabetes was 92.5% (62/67), and the prevalence of VC at each site in the patients with diabetes was significantly higher than that in the patients without diabetes (all P<0.001). Multivariate logistic regression analysis revealed that age (every 10 years increase, OR=2.51, 95% CI 1.77-3.56, P<0.001), hypertension ( OR=5.88, 95% CI 1.57-22.10, P=0.009), and diabetes ( OR=4.66, 95% CI 2.10-10.35, P<0.001) were independently correlated with CAC; Age (every 10 years increase, OR=6.43, 95% CI 3.64-11.36, P<0.001) and hypertension ( OR=6.09, 95% CI 1.33-27.84, P=0.020) were independently correlated with TAC; Female ( OR=0.23, 95% CI 0.07-0.72, P=0.011), age (every 10 years increase, OR=3.90, 95% CI 2.42-6.29, P<0.001), diabetes ( OR=5.37, 95% CI 2.19-13.19, P<0.001) and serum magnesium ( OR=0.01,95% CI 0-0.35, P=0.014) were independently correlated with AAC. Moreover, age and diabetes were independently correlated with carotid artery calcification, AVC and overall VC Conclusions:The prevalence of VC in non-dialysis CKD patients of stage 3-5 is 63.59%, of which CAC reaches 37.9%, TAC is the most common one (43.7%), while AVC is the least one (9.7%). Age and diabetes are the independent associated factors for VC of all sites except TAC, while hypertension is an independent associated factor for both CAC and TAC.
5.Mechanism study on inhibition of ulcerative colitis by Sirt1 signaling axis mediated by oridonin

Objective:To build mice ulcerative colitis(UC)model by dextran sodium sulfate salt(DSS),and to explore effect and molecular mechanism of oridonin(Ori)on alleviating UC by mediating Sirt1 signaling pathway,as well as to find new drugs for UC treatment.Methods:Sixty BALB/c mice were randomly divided into normal group(NC),model group(DSS),DSS+mesalazine sustained-release granule group(AC),DSS+low-dose Ori group(Ori-L),DSS+medium-dose Ori group(Ori-M)and DSS+high-dose Ori group(Ori-H),with 10 mice in each group.Except NC group,mice in other groups were given 3%DSS aqueous solution free for 7 days.After successful modeling,Ori-L group,Ori-M group and Ori-H group were intragastric with 50,100 and 150 mg/kg Ori suspension,respectively.NC group and DSS group drank distilled water free,AC group was intragastric with mesalazine sustained-release granules 100 mg/kg,lasted for 10 days.Weight changes of mice were recorded and disease active index(DAI)score was performed.After treatment,serum levels of IL-1β and tumor necrosis factor-α(TNF-α)were detected by ELISA.Whole colon tissues were extracted,length was measured,and HE staining was performed.Real-time fluorescence quantitative PCR and Western blot were used to detect expressions of Sirt1,NF-κB and p53 in colon tissues of each group.Results:After successful modeling,compared with NC group,the other 5 groups of mice showed weight loss,DAI score increased and colon length shortened,Sirt1 expression was signi-ficantly inhibited,while expressions of NF-κB and p53 were significantly increased,accompanied by obvious inflammatory pathologi-cal features.Compared with DSS group,Ori-L,Ori-M,and Ori-H groups and AC group had less weight loss during experiment,and DAI scores showed lower disease activity and relatively less colon length shortening,Sirt1 expression was less inhibited,expressions of NF-κB and p53 were also relatively small.Results of AC group could prove that Ori has therapeutic effect on UC.Conclusion:Ori can improve UC in mice,whose effect may be related to regulation of Sirt1 signal.
6.Effect of endoscopic retronasal neurotomy on psychological state of patients with allergic rhinitis

Objective To explore the effect of nasal endoscopic retronasal neurotomy on the psychological status of patients with moderate-severe persistent allergic rhinitis(AR).Methods The enrolled subjects were patients with moderate to severe persistent AR from 2022 to 2023.Patients with endoscopic retronasal neurotomy plus drug therapy were selected as the experimental group and drug therapy as the control group.Rhinoconjunctivitis quality of life questionnaire(RQLQ),self-rating anxiety scale(SAS)and self-rating depression scale(SDS)were used to evaluate the quality of life and psychological status before treatment,6 months and 12 months after treatment,respectively.Results The three scores were significantly lower in the experimental group than those in the control group at 6 and 12 months after treatment,and the three scores were significantly lower at 6 and 12 months after treatment compared with those before treatment,the differences were statistically significant(P<0.05),at 12 months after treatment,SAS and SDS in experimental group were significantly lower than those at 6 months after treatment,and both differences were statistically significant(P<0.05).In the control group,6 months after treatment,only the RQLQ scale was significantly lower than that before treatment(P<0.05),SAS and SDS were not statistically significant when compared with those before treatment(P>0.05),and there were no significant differences in these scores when comparing the scores at 6 months after treatment with those at 12 months after treatment(P>0.05).Conclusion Endoscopic retronasal neurotomy can significantly improve the quality of life of patients with moderate to severe persistent AR,and further improve the psychological state of anxiety and depression of patients.This surgery is easy to locate under nasal endoscopy,with clear field of vision,less trauma,fewer complications,it is economic and effective,as a relatively safe minimally invasive surgery,and it is worthy of clinical application.
7.Application of ANSYS Fatigue Analysis in Type Selection and Evaluation of Bone Plate

Objective By comparing the fatigue strength of type A and type B locking compression plates (LCP) in distal femoral plate, a theoretical evaluation method was provided for type selection of bone plate when testing its bending strength and fatigue performance. Methods Through bending strength performance test and fatigue performance test on bone plates with different types, combined with ANSYS Workbench, the finite element analysis on total deformation, von Mises stress and fatigue service life of bone plates were conducted. Results The fatigue strength of type A plate was 30.7% higher than that of type B plate, the stress of type A plate was lower than that of type B plate, and the minimum fatigue service life of type A plate was 17% higher than that of type B plate. Conclusions The fatigue performance of type A plate is better than that of type B plate, so the failure possibility of type A plate was lower than that of type B plate.The results provide references for assisting selection of different bone plates when testing the performance of two newly developed bone plates.
8.Classification and minimally invasive management of ureteroileal bladder anastomotic stricture after radical cystectomy

Objective:To discuss the classification and treatment of ureteroileal anastomotic stricture (UAS) after radical cystectomy.Methods:The clinical data of 34 patients with UAS after radical cystectomy in the Department of Urology of Tongji Hospital from January 2017 to January 2022 were reviewed and analyzed. There were 25 males and 9 females. The average age was (66.3±7.7)years, including 2 cases of bilateral hydronephrosis and 32 cases of unilateral hydronephrosis. The average time of UAS was detected (14.7±6.5)months after radical cystectomy. There were 32 patients of unilateral hydronephrosis and 2 patients of bilateral hydronephrosis. Two patients had undergone nephrostomy in an external hospital. Three patients had elevated leukocytes in blood routine. Among them, two patients had fever. First, nephrostomy on the hydronephrosis side and anti-infection treatment were performed. After routine blood tests showed that the white blood cells were normal and antibiotics were stopped for 24 hours without fever, the operation was performed. 34 patients had preoperative hydronephrosis of (2.7±0.6) cm. Of the 34 cases in this group, 5 cases were injected with methylene blue through a preoperative nephrostomy tube, and 29 were injected with methylene blue through the renal pelvis using an 18G puncture needle under ultrasound guidance. Using a ureteroscope to observe in the ileal bladder, methylene blue was seen in 4 cases. Methylene blue was used to guide the search for the stenosis and a super smooth guide wire was inserted. Among them, 3 cases were dilated with a 5 mm ureteral dilation balloon catheter, 1 case was dilated with a F14 ureteral access sheath, and then a F6 single J stent was inserted. Methylene blue was not seen in the ileal conduit in 30 cases, of which 16 cases were treated with a flexible ureteroscope through the nephrostomy to locate the stenosis, incised with a 30 W holmium laser. 9 cases were treated with 5 mm ureteral dilation balloon catheter, and 7 cases were treated with a F14 ureteral access sheath, and then an F6 single J stent was inserted. 14 cases were unable to find the stenosis by antegrade method. According to the operation time and patient's condition, it was decided to perform immediate or second stage dual endoscope surgery. Through the nephrostomy, a flexible ureteroscope was used to enter the stenosis along the super slide guide wire. A rigid ureteroscope was used to observe the stenosis through the ileal conduit, and the stenosis was found. The stenosis was found in 10 cases and incised with a 30 W holmium laser. 8 cases were treated with 5 mm ureteral dilation balloon catheter, and 2 cases were treated with a F14 ureteral access sheath, and then an F6 single J stent was inserted. 4 cases were still unable to accurately locate the stenosis using the dual endoscope surgery(one case was bilateral stenosis, and one side was relieved), and continued indwelling nephrostomy. The definition of successful removal of stricture in this study is that an F6 single J stent can be inserted into the ureter.Results:UAS were classified into four types based on the severity of the intraoperative findings: Type Ⅰ, the narrow ureteral lumen is more than 50% narrower than the normal ureteral lumen, but methylene blue can pass through in strands; Type Ⅱ, needle like stricture of the ureteral lumen, allowing only methylene blue filaments to pass through; Type Ⅲ, membranous atresia of the ureter, with a narrow segment of 1 to 3 mm in length, and methylene blue cannot pass through; Type Ⅳ, long segment stenosis. Of the 34 cases in this group, 4 cases were type Ⅰ, and the stenosis was dredged by retrograde method; 16 cases were type Ⅱ, and the stenotic segments were dredged by antegrade method; 10 cases were type Ⅲ, and the stenosis was dredged by the dual endoscope surgery; Four cases were of type Ⅳ (one case was of bilateral UAS, one side was of type Ⅲ, and the other side was of type Ⅳ, which was classified as type Ⅳ). The stenotic segment could not be solved through the above methods. Among the 34 patients, 30 patients were successfully relieved of anastomotic obstruction, and 1 patient with bilateral obstruction was unilaterally relieved of anastomotic obstruction. In the other 3 cases, because the stenosis segment was too long, 2 cases were changed to nephrostomy, and 1 case was changed to open surgery, with a success rate of 88.2%. UAS was classified into 4 types based on the severity of UAS seen during surgery. No serious complications occurred during and after the operation. During the follow-up of 6-24 months, the imaging evaluation of 4 patients showed that hydronephrosis was aggravated, with an average increase in creatinine of (32.5±10.9)μmol/L, requiring replacement of a single J tube. The imaging evaluation of the remaining 26 patients showed that the postoperative hydronephrosis was 0.9 ± 0.6 cm less than the preoperative hydronephrosis 2.6 ± 0.6 cm, with a statistically significant difference ( P<0.01). The quality of life score at 3 months after surgery was (1.9±0.6), which was significantly improved compared to the preoperative indwelling nephrostomy period (5.2±0.7), with a statistically significant difference ( P<0.01) Conclusions:The treatment of UAS after radical cystectomy with retrograde, antegrade, and dual endoscope surgery has a high success rate, which can help some patients avoid the inconvenience of indwelling external drainage tubes and the risk of open surgery. Choosing an appropriate surgical method can achieve the goal of treating UAS with minimal trauma.
9.Effects of different feeding patterns on physical and nutritional status of infants aged 6‒12 months

ObjectiveTo analyze the effects of different feeding patterns on the physical and nutritional status of children aged 6‒12 months, so as to provide reference for promoting scientific feeding and health development of infants and young children. MethodsChildren born between December 2019 and February 2020 and who had completed three follow-up visits at 6‒, 9‒ (8‒10 months) and 12‒ (11‒14 months) months old in all of the 13 communities of Minhang, Shanghai were selected. The subjects’ basic information was investigated by questionnaires. The indicators including feeding pattern, physical development (body weight, body length, head circumference) and nutritional status (the detection rate of overweight, obesity, low body weight, growth retardation, emaciation and iron deficiency anemia) were followed up in the outpatient department, with iron deficiency anemia only monitored at the 6‒ and 12‒ months old. According to different feeding patterns, the groups of 6‒ months old were divided into three groups of exclusive breast feeding (EBF), mixed feeding (MF) and artificial feeding (AF), while 9‒ and 12‒ months old were divided into MF and AF groups. The differences of basic information and follow-up results among the groups were analyzed. ResultsA total of 470 children were included, including 130 (27.66%), 288 (61.28%) and 52 (11.06%) respectively in EBF, MF and AF groups at the 6‒ months old,and 319 (67.87%) and 196 (41.70%) in MF group at the 9‒ and 12‒ months old. There was no significant difference in the other follow-up results among the groups. The detection rate of iron deficiency anemia in 6‒ months old EBF (13.08%) was higher than that in MF group (5.90%) and AF group (1.92%) (χ2=8.40, P=0.010), while it was still higher in 12‒ months old MF group (9.69%) than in AF group (2.92%) (χ2=9.68, P=0.002). ConclusionThere is no significant difference in body weight,body length, head circumference, and the detection rates of overweight, obesity, low body weight, growth retardation and emaciation among the groups of different feeding patterns in the children aged 6‒12 months. The detection rate of iron deficiency anemia in the EBF and MF groups is significantly higher than that in the AF groups of children aged 6‒ and 12‒ months old.
10.Protein expression, purification and mouse antiserum preparation of monkeypox virus A23R.

Objective To express the monkeypox virus (MPXV) A23R protein in Escherichia coli and purify by Ni-NTA affinity column, and to prepare mouse antiserum against MPXV A23R. Methods The recombinant plasmid pET-28a-MPXV-A23R was constructed and transformed into Escherichia coli BL21 to induce the expression of A23R protein. After optimizing the conditions of expression, A23R protein was highly expressed. Recombinant A23R protein was purified by Ni-NTA affinity column and identified by Western blot analysis. The purified protein was used to immunize mice for preparing the A23R polyclonal antibody, and the antibody titer was detected by ELISA. Results The expression of A23R recombinant protein reached the peak under the induced conditions of 0.6 mmol/L isopropyl-β-D-thiogalactoside (IPTG), 37 DegreesCelsius and 20 hours. The purity of the protein was about 96.07% and was identified by Western blot analysis. The mice were immunized with recombinant protein, and the titer of antibody reached 1:102 400 at the 6th week after immunization. Conclusion MPXV A23R is expressed highly and purified with a high purity and its antiserum from mouse is obtained with a high titre.
