1.Effect of losartan on acute kidney injury and the relationship with mitochondrial fusion-fission in septic mice

Objective:To evaluate the effect of losartan on acute kidney injury (AKI) and the relationship with mitochondrial fusion-fission in septic mice.Methods:One hundred and twenty-eight SPF male C57BL/6J mice, aged 6-8 weeks, weighing 20-25 g, were divided into 4 groups ( n=32 each) using a random number table method: sham operation group (Sham group), sham operation+ losartan group (Sham+ LOS group), sepsis-associated AKI group (SA-AKI group), and sepsis-associated AKI+ losartan group (SA-AKI+ LOS group). Sepsis was induced by cecal ligation and puncture in anesthetized mice. Sham+ LOS group and SA-AKI+ LOS group received intraperitoneal injection of losartan 5 mg/kg, once a day, for 3 consecutive days, starting from 3 days before sham operation or developing the model. The equal volume of solvent was given instead in Sham group and SA-AKI group. Twenty mice were randomly selected to observe the survival 7 days after surgery. At 24 h after sham operation or establishing the model, serum blood urea nitrogen (BUN) and creatinine (Cr) concentrations were determined by the colorimetric method, and serum concentrations of tumor necrosis factor-alpha (TNF-α), interleukin-6 (IL-6) and high-mobility group box 1 protein (HMGB1) were measured using enzyme-linked immunosorbent assay. Renal tissues were obtained for microscopic examination of pathological changes which were scored and for determination of mitochondrial membrane potential (using JC-1 method) and expression of dynamin-related protein 1 (Drp1) and mitofusin-2 (Mfn2) (using Western blot). Results:Compared with Sham group, the survival rate was significantly decreased, the serum BUN, Cr, TNF-α, IL-6 and HMGB1 concentrations and renal tubular injury score were increased, the ATP content and MMP were decreased, the expression of Drp1 was up-regulated, the expression of Mfn2 was down-regulated ( P<0.05), and pathological changes were found in renal tissues in SA-AKI group and SA-AKI+ LOS group. Compared with SA-AKI group, the survival rate was significantly increased, serum concentrations of BUN, Cr, TNF-α, IL-6 and HMGB1 and renal tubular injury score were decreased, the ATP content and MMP were increased, the expression of Drp1 was down-regulated, the expression of Mfn2 was up-regulated ( P<0.05), and the pathological changes of renal tissues were significantly attenuated in SA-AKI+ LOS group. Conclusions:Losartan can alleviate AKI in septic mice, and the mechanism may be related to promoting mitochondrial fusion and inhibiting mitochondrial fission.
2.Implementation of Wearable Wireless Chest Patch Monitoring Terminal

Objective A wearable wireless chest patch monitoring terminal is designed to realize the acquisition,processing,and wireless transmission of ECG,respiration,and body temperature signals.Methods The analog front-end ADS1292R,which integrates respiratory impedance and ECG front-end,is utilized to collect human ECG and respiratory signals.The body temperature is collected using a low-power,high-precision digital temperature sensor MAX30208.A filter algorithm for signal processing and wireless transmission is designed through a low-power nRF52840 Bluetooth SoC with an Arm Cortex-M4F kernel.Results The experimental results show that the designed monitoring terminal can monitor the ECG,respiration,and body temperature parameters of the human body in real-time and send the monitoring results via Bluetooth,with a continuous working time of more than 13 hours.Conclusion The wearable wireless chest patch monitoring terminal features good portability,long standby time,and high measurement accuracy,and it has promising application prospects in the fields of family health monitoring,mobile medical treatment,and smart healthcare.
3.A meta-analysis of effects of repetitive transcranial magnetic stimulation on cognitive function intervention in patients with schizophrenia

Objective:To explore the intervention effects of repetitive transcranial magnetic stimulation on cognitive function of schizophrenia patients with methods using meta-analysis.Methods:CNKI,Wanfang Data,VIP,PubMed,Web of Science,the Cochrane Library and Embase were searched for randomized controlled trials of repetitive transcranial magnetic stimulation on cognitive function in schizophrenia.Quality assessment was per-formed with the Cochrane Handbook for Systematic Reviews.Effect sizes and heterogeneity were assessed using RevMan 5.3.Results:A total of 31 randomized controlled trials(2 421 patients)were included in this study.Meta-analysis showed that rTMS improved cognitive function in schizophrenia patients(SMD=0.73).Subgroup analyses showed that intervention intensity was less than or equal to 100%RMT,the intervention frequency was 1 Hz,the intervention time was lasted at least 20 min/time,and the intervention period was four weeks,the effect of rTMS was better.Subgroup analyses showed that rTMS was more effective in improving for patients with durations of 5 to 10 years.Conclusion:rTMS has more significant improvement effects on cognitive function in schizophrenia pa-tients.It is recommended to set the intervention intensity 100%RMT,the intervention frequency of 1 Hz,the in-tervention time ≥20 min,and the intervention period of 4 weeks in the clinic.
4.Assessment of left ventricular function using tissue motion mitral annular displacement in patients with cardiac amyloidosis

Objective To observe the value of tissue motion mitral annular displacement(TMAD)technique to assess left ventricular function in patients with cardiac amyloidosis.Methods A total of 34 adult patients with cardiac amyloidosis diagnosed by pathology were retrospectively included as the observation group,and 32 healthy adults were collected as the control group for the same period.Basic data of the subjects were collected,and data of routine ultrasonic parameters of left ventricular function and TMAD parameters were obtained,and then compared between groups.The correlation of TMAD parameters with left ventricular ejection fraction(LVEF)or mitral annular plane systolic excursion(MAPSE)were assessed.Results Compared with the control group,the observation group had higher levels of body surface area(BSA),systolic blood pressure,N-terminal pro-B-type natriuretic peptide(NT-proBNP),creatinine and urea(all P<0.05).The observation group had increased values of ascending aorta(AO),left atrium(LA),interventricular septum(IVS),left ventricular posterior wall thickness in diastole(LVPWD),pulmonary artery(PA),and early diastolic peark velocity of mitral inflow(peak E),while smaller values of left ventricular end-diastolic dimension(LVEDD),LVEF,fractional shortening(FS),early diastolic tissue Doppler velocity E'septal(IVS E')and lateral(LW E')and MAPSE(all P<0.05),and the LVEF in observation group was(58.18±7.09)%.For TMAD patameters,the observation group had smaller values of the following parameters on apical four chamber(A4C)view as medial displacement of mitral valve annulus(A4C MV1),displacement of lateral mitral valve annulus(A4C MV2),displacement of the midpoint of the mitral valve annulus(A4C Midpt)and the corresponding percentage(A4C Midpt%),as well as smaller values of the following paramets on apical two chamber(A2C)view as A2C MV1,A2C MV2,A2C Midpt and A2C Midpt%(all P<0.05).In the observation group,A4C Midpt%showed a moderate positive correlation with LVEF(r=0.488,P<0.05),and A2C Midpt showed a high positive correlation with M APSE(r=0.712,P<0.05),and A4C MV2,A4C Midpt,A4C Midpt%,A2C MV1,A2C MV2,A2C Midpt%all showed a moderate positive correlation with MAPSE(r=0.420 to 0.691,all P<0.05).Conclusion Compared with LVEF,the TMAD parameters might reflect the changes in left ventricular systolic function more sensitively in patients with cardiac amyloidosis.
5.Acute-on-chronic liver failure treated by split liver transplantation:a single-center experience on 9 cases

Objective:To evaluate the short-term efficacy of split liver transplantation (SLT) in patients with acute-on-chronic liver failure (ACLF).Methods:The clinical data of 9 ACLF patients receiving SLT in our center from Mar 2021 to May 2022 were retrospectively analyzed to evaluate its safety and efficacy.Results:The preoperative APASL ACLF Research consortium (AARC) score of the 9 ACLF patients was 8 points in 1 case, 9 points in 3 cases, 10 points in 3 cases, 11 points in 1 case and 12 points in 1 case, 7 cases were in AARC-ACLF grade 2, and 2 cases in grade 3.In-situ liver splitting was performed in 9 deceased donors, including 4 classical split cases, 5 full size split cases. Among these 9 ACLF patients, 2 received left half liver transplantation, 3 received right half liver transplantation, and 4 received extended right lobe liver transplantation. After transplantation, all 9 recipients were discharged fully recovered, 1 case developed Clavien grade Ⅳa complication and 2 cases developed Clavien grade Ⅲb complication.After SLT treatment the median postoperative hospital stay was 27 days, the 1-year survival rate was 100%, and the organ survival rate was 88.9%.Conclusion:Split liver transplantation is a safe and feasible treatment method for ACLF patients.
6.Analysis of strong invasive clinical features and poor prognosis in macrotrabecular-massive hepatocellular carcinoma

Objective:To analyze the correlation between the histological classification of hepatocellular carcinoma (HCC), especially macrotrabecular-massive (MTM), tumor invasiveness, and prognosis.Methods:The clinical and follow-up data of 246 consecutive HCC cases who met the inclusion criteria from 2015 to 2018 were retrospectively analyzed. They were divided into trabecular/pseudoglandular, trabecular/patchy, and MTM types according to the histological classification. The relationship between each type and related clinicopathological features was analyzed. The Kaplan-Meier method was used to plot tumor-free survival (DFS) and overall survival (OS) curves. Log rank tests, COX univariate, and multivariate regression analyses were conducted to analyze the relationship between clinical features, including histological classification, DFS, and OS.Results:Trabecular/pseudoglandular, trabecular/nodular, and MTM type proportions were 44.7% (110 cases), 32.9% (81 cases), and 22.4% (55 cases), respectively. The results of the clinicopathological features showed that MTM-type HCC was significantly more invasive than the other two types, with alpha-fetoprotein (AFP) ≥400 ng/ml, tumor diameter≥8 cm, no tumor capsule, poor differentiation, and MVI positivity proportions, and the differences were statistically significant (P < 0.05).The proportion of MTM-type HCC patients with American Joint Committee on Cancer TNM Stage III to IV and Chinese Liver Cancer Staging (CNLC) II to II was significantly higher than that of the first two types, and the differences were statistically significant (P < 0.05). In addition, the proportion of MTM subtypes undergoing transcatheter arterial chemoembolization was also raised (P < 0.05). The DFS and OS were significantly lower for MTM-type HCC compared to trabecular/pseudoductal-type HCC at 1-, 3-, and 5-years, and the differences were statistically significant (P < 0.05). Univariate analysis indicated that strongly invasive clinical pathological features such as alpha fetoprotein (AFP) ≥400 ng/ml, tumor diameter ≥ 8 cm, no tumor capsule, poor differentiation, positive microvascular invasion, tumor stage, and MTM staging were all risk factors affecting DFS and OS (P < 0.05). Multivariate COX analysis showed that MTM histological staging, AFP ≥ 400 ng/ml, tumor non-encapsulation, satellite nodules, CNLC stages II~III, and TNM stages III~IV were independent risk factors for DFS (P < 0.05), while AFP ≥ 400 ng/ml, tumor non-encapsulation, and CNLC stage II~III were independent risk factors for OS ( P < 0.05).Conclusion:Histological classification is highly correlated with tumor invasiveness and HCC prognosis. Trabecular/pseudoglandular types have lower malignancy and a better prognosis, while MTM types exhibit strong invasive features and a poor prognosis.
7.Clinical Efficacy of Topiramate, Carbamazepine and Sodium Valproate in the Treatment of Epilepsy Secondary to Encephalitis

Objective:To observe and compare the clinical curative effect and safety of topiramate,carbamazepine and sodium valproate on the epilepsy secondary to encephalitis.Methods:80 cases of patients with epilepsy secondary to encephalitis who were treated in our hospital from January 2010 to September 2015 were selected and divided into the topiramate group,carbamazepine group and sodium valproate group,which were treated with topiramate,carbamazepine and sodium valproate respectively.The treatment effect,congnitive scores (including executive function and visual space,name,language,abstract,attention,delayed memory,directional),adverse reaction rate of three groups were compared.Results:The effective rate oftopiramate group was the highest(80.65%,25/31),carbamazepine was the lowest (70.00%,21/30),but there was no significant difference between the three groups (P>0.05).After treatment,the scores of executive ability and visual spatial,naming,abstraction,attention,orientation,language in topiramate group were significantly higher than those of carbamazepine group and sodium valproate group (P<0.05);the incidence of adverse reactions of topiramate group was 12.90%,which was significantly lower than that of carbamazepine group (36.67%) and sodium valproate group (29.62%)(P < 0.05).Conclusions:Topiramate,carbamazepine and sodium valproate have equal therapeutic effect on epilepsy secondary to encephalitis,but topiramate had less adverse reactions and best safety.
8.Research on CT and MRI findings of Hemiballism-hemichorea induced by hyperglycemia

Objective:To summarize the clinicopathologia and imaging features of Hemiballism-hemichorea induced by hyperglycemia and increase the understanding of the disease and improve the accuracy of diagnosis. Methods:The imaging images and clinical manifestation of 5 cases Hemiballism-hemichorea induced by hyperglycemia were analyzed retrospectively. The disease pathogenesis and mechanism relating to the Characteristic neuroradiological findings was obtained by literature review. Results:The common clinical manifestation of Hemiballism-hemichorea induced by hyperglycemia is unilateral limb involuntary movements. CT scans typically show an area of hyperdensity in the basal ganglia and The characteristic finding on the T1-weighted MRI is high signal intensity in the contralateral putamen. Conclusion: Hemiballism-hemichorea induced by hyperglycemia is an unusual but important differential diagnosis in patients with particular neuroradiological findings as prompt diagnosis and treatment of hyperglycemia has an excellent prognosis. The correct diagnosis could be received through integrated the patient's past medical history, presentation, laboratory and imaging inspection into account.
9.Effect of Different Package and Storage Conditions on Effective Components of Astragalus Pieces

This experimental study was aimed to find the effect of different package and storage conditions on the content of effective components of A stragalus pieces. A stragalus pieces were stored under different storage conditions by using different packaging materials and packaging methods. Every three months, the contents of Calycosin-7-glu-coside and astragaloside were determined according to the 2010 version of Chinese Pharmacopoeia. With the extend-edstorage time, the contents of two effective components were significantly decreased. After six-monthstorage, the contents were not consistent with the standard of the pharmacopoeia standards. Room temperature had relatively big influence on the loss of content. The plastic and aluminum paperpackagingwere better than kraft paper packaging. The content ofastragaloside using non vacuum packaging method was relatively higher than the vacuum packaging. Contentunder the conditions of cool storehouse and nonvacuum plastic bags was higher than other packagingmethod. And the changes of both contents were relatively stable. It was concluded that the A stragaluspieces should be packed with non vacuum plastic bags, and stored in a cool and dry place.
