2.The effect of diclofenac sodium suppository combined with tramadol sustained-release tablets on postoperative pain and recovery in patients with mixed hemorrhoids

		                        			Objective:To explore the effect of diclofenac sodium suppository combined with tramadol sustained-release tablets on postoperative pain relief and recovery in patients with mixed hemorrhoids.Methods:Sixty patients with mixed hemorrhoids who underwent external dissection and internal ligation at the Shenzhen Traditional Chinese Medicine Proctology Hospital from September to October 2023 were selected and randomly divided into A group and AS group using a random number table method, with 30 patients in each group. The group A patients received oral tramadol sustained-release tablets 4 hours after surgery, while the group AS received diclofenac sodium suppositories for anal canal therapy in addition to the group A. Two groups were compared for postoperative pain [Visual Analog Scale (VAS)] and patient comfort [Numerical Rating Scale (NRS)], cumulative tramadol consumption, supplementary rescue pethidine needs, and adverse reactions at 4, 8, 24, and 48 hours.Resultsl:The VAS of the AS group were lower than those of the A group at 4, 8, 24, and 48 hours after surgery. The NRS scores of the group A patients at 4, 8, and 24 hours were significantly better than those of the group AS. The cumulative consumption of tramadol in the AS group at 4, 8, 24, and 48 hours after surgery was significantly lower than that in the A group; The demand rate for postoperative rescue of pethidine in the group A was significantly higher than that in the group AS. The incidence of nausea in the group A was significantly higher than that in the group AS.Conclusions:The combination of postoperative diclofenac sodium suppositories and oral tramadol sustained-release tablets for mixed hemorrhoid surgery has a significant improvement effect on pain. It can reduce the dosage of tramadol, reduce the need for rescue and supplementary analgesia, and have fewer adverse reactions.
		                        		
		                        		
		                        		
		                        	
3.Case report of unrelated cord blood transplantation for the treatment of recurrent refractory Epstein-Barr virus-associated hemophagocytic lymphohistiocytosis with intestinal perforation and literature review

		                        			Objective:To examine the clinical experience and efficacy of unrelated cord blood transplantation (UCBT) in the treatment of recurrent refractory Epstein-Barr virus-associated hemophagocytic lymphohistiocytosis (EBV-HLH) in children.Methods:The clinical data of a patient with recurrent refractory EBV-HLH and intestinal perforation who was treated by UCBT in Department of Pediatrics, the First Affiliated Hospital of Zhengzhou University in September 2015 and finally cured were retrospectively analyzed.Meanwhile, literature was reviewed.Results:The patient, male, 1 year and 6 months, was admitted to the hospital with " fever for 15 days, rash for 9 days" as the main complaint, mainly manifested as high fever, large liver, spleen, lymph nodes, rapidly progressing pancytopenia, liver function damage, phagocytic blood cells on bone marrow smear, diagnosed as EBV-HLH in September 2015.The patient received chemotherapy according to the HLH-2004 protocol developed by the International Association of Cell Societies.During the treatment, he suffered two recurrence during the maintenance period, and a second-line rescue treatment was adopted, namely, " Pegaspargase, Doxorubicin liposome, Etoposide and Methylprednisolone" (L-DEP regimen) chemotherapy.The complete relief of diagnostic indexes for hemophagocytic lymphohistiocytosis was evaluated after chemotherapy.The patient developed sudden intestinal perforation and underwent emergency surgical surgery, enteroenterostomy.After the condition was stabilized, the patient was pretreated with the " Fludarabine+ Busulfan+ Cyclophosphamide" (Flu+ BU+ CY) therapy and then treated with UCBT, with intravenous nutritional support provided during the entire process.Neutrophil and platelet implantation was implemented on day 13 and day 35 after transplantation, respectively.The chimeric rate was 100%, and the implantation was a success.Hepatic veno-occlusive disease, fungal pneumonia and skin graft-versus-host disease (GVHD) Ⅱ occurred on the 15 th day, 22 nd day and 26 th day after transplantation, respectively.The corresponding symptoms improved after treatment.On day 49 after transplantation, phase Ⅱ " enterostomy fistula" was performed.The patient was followed up to 70 months after transplantation, and generally in good condition.His symptoms relieved, and no chronic GVHD and other comorbidities occurred. Conclusions:Allogeneic hematopoietic stem cell transplantation is the only possible effective means of treating relapsed refractory EBV-HLH in children.In the absence of a suitable sibling or unrelated donor, unrelated cord blood stem cells can be used as a graft source.Enterostomy after intestinal perforation is not contraindicated for transplantation.

		                        			Common deletions and variants of the globin genes were detected by Gap-PCR and next generation sequencing (NGS). Suspected mutations were verified by Sanger sequencing.

5.Effect of high-throughput sequencing for the prevention and control of thalassemia.

		                        			OBJECTIVE:
		                        			To assess the value of next generation sequencing (NGS) for the prevention and control of thalassemia.
		                        		
		                        			METHODS:
		                        			NGS was used to sequence 3083 clinical blood samples suspected for thalassemia during initial screening. Retrospective analysis was conducted on blood samples detected with rare genotypes of thalassemia and abnormal hemoglobin.
		                        		
		                        			RESULTS:
		                        			NGS analysis of the 3083 samples has found 1089 subjects with thalassemia genotypes (alpha-thelassemia genotype: 26.01%, beta-thalassemia genotype: 6.71%, and alpha-compound-beta genotype: 2.59%), which yielded a positive detection rate of 35.32%. Rare alpha-thalassemia genotypes including HBA2 c.123delG, HBA1 c.354_355insATC and Fusion gene, and rare beta-thalassemia genotypes including HBB c.-100G>A and HBB c.316-90A>G, were discovered. In addition, 19 patients were found to have abnormal hemoglobin, mainly including Hb Hamilton, Hb Hekinan II, Hb Shizuoka, Hb Owari, Hb New York, Hb J-Bangkok and Hb Port Phillip.
		                        		
		                        			CONCLUSION
		                        			NGS can play a crucial role for improving of the prevention and control of thalassemia and formulating a screening system with better efficacy.

7.Clinical significance of fetal weight estimation by modiefied uterine fundal height

		                        			Objective To investigate the accuracy of fetal weight estimation by modiefied uterine fundal height.Methods Totally 547 pregnant women admitted to maternity ward of the Second Affiliated Hospital of Chongqing Medical University from December 1,2013 to April 30,2014 with childbirth were chosen as the subject.The modified uterine fundal height were measured by the uterus inclination and modiefied fundal height admeasuring apparatus,normal uterine fundal height and and abdomen circumfer-ence were measured by tape.The fetal weight was estimated by utilizing modified uterine fundal height.Results The modified fun-dal height was(31.7±2.30)cm,which was significantly higher than that normal fundal height(34.7 ± 2.46 )cm(t = 54.25,P <0.01).The accuracy rate of modified uterine fundal height for fetal weight estimation with empirical formulas,Yuan′s,Zhuo′s,Lin′s,Zen′s,Gu′s were 68.4%,69.7%,59.4%,67.8%,69.3%,78.4%,respectively.The accuracy rate of normal uterine fundal height for fetal weight estimation with the same formulas were 53.4%,34.4%,54.3%,46.1%,38.8%,45.9%,respectively.The accuracy rate of modiefied uterine fundal height for fetal weight estimation with empirical formulas,Yuan′s,Lin′s,Zen′s,Gu′s were significantly higher than that of utilizing normal uterine fundal height estimation with the same formulas(P <0.01).Conclusion Modified uterine fundal height has better accuracy in estimating fetal weight,it has a important clinical significance.

		                        			Objective To investigate the effects of down-regulation of Beclin 1, which is an autophagy regulatory molecule, expression induced by RNA interference on the proliferation and apoptosis in skin fibroblasts of naked mole rat. Methods The expression levels of Beclin 1 were detected after starvation or H2 O2 treatment.The fibroblasts were transi-ently transfected with specific siRNA targeting Beclin 1 and then screened by real-time PCR and Western blot.Cell prolifer-ation and apoptosis were determined using CCK-8 detection kit and flow cytometry ( FCM ) .The expressions of related genes were detected by Western blot.Results The expression of Beclin 1 gene at mRNA and protein levels was signifi-cantly lower in fibroblasts of the naked mole rat.Starvation and H2 O2 treatment induced changes of the Beclin 1 expression. Inhibition of Beclin 1 gene expression can inhibit cell proliferation and induce early and late apoptosis.The protein levels of p53, BAX, Bcl2, LC3B, p-AKT and mTOR were reduced after transient transfection with Beclin 1-siRNA.Conclusions The expression of Beclin 1 in fibroblasts of naked mole rat are changed in response to starvation or H2 O2 stimulation.Inhi-bition of Beclin 1 gene expression can inhibit cell proliferation and induce apoptosis.Therefore, Beclin1 gene may play a regulatory role in autophagy, proliferation and apoptosis in the skin fibroblasts of naked mole rat.
		                        		
		                        		
		                        		
		                        	
9.Effect of different interval time of repositioning on the pressed skin of elderly patients with chronic heart failure

		                        			Objective To discuss the effect of gradually prolonging interval time of repositioning on the pressed skin of elderly patients with chronic heart failure .Methods Convenience sampling was used to choose 60 elderly patients with chronic heart failure , who were randomly divided in the observation group and the control group, each with 30 cases.The control group was repositioned once for 120 min according to the conventional method , while the observation group was repositioned once for 120 min for a week and then 30 min prolonged the next 3 weeks.Incidence rate of pressure ulcers and pressure on both scapular and sacrococcygeal regions with interval time of 120 min, 150 min, 180 min and 210 min were compared between two groups . Results The pressure had no statistically significant difference in the observation group among interval time of 120 min, 150 min and 180 min, however , the pressure on both scapular and sacrococcygeal regions with interval time of 210 min was respectively (53.67 ±3.22),(52.71 ±4.19),(95.34 ±0.48)mmHg, higher than those with other interval times, and the differences were statistically significant (P<0.05).There was no statistically significant difference of pressure on both scapular and sacrococcygeal regions during the first , second and third week between two groups .The pressures, however, were higher in the observation group than in the control group [(48.10 ±2.67)(47.94 ±5.10)(90.91 ±1.27)mmHg] during the fourth week, and the differences were statistically significant (P<0.05).The incidence rate of pressure ulcers was 20.00% in the observation group and 16 .67%in the control group , and both groups had no phase Ⅲpressure ulcers , with no statistically significant difference (Z=0.074, P>0.05).Conclusions Prolonging the interval time of repositioning to 180 min can effectively improve the skin tolerance of elderly patients with chronic heart failure , reduce their discomfort as well as nurses ’ workload without incidence of pressure ulcers .
