1.Correlation between CD4+T lymphocytes and viral load in newly identified human immunodeficiency virus infected individuals in Huangpu District,Shanghai,2023

		                        			ObjectiveTo analyze the detected results of CD4+T lymphocytes and viral load in newly identified human immunodeficiency virus (HIV) infected patients in Huangpu District of Shanghai in 2023, to explore the correlation between them, so as to provide a scientific basis for the development of targeted prevention and control measures and antiviral treatment programs. MethodsThe data of CD4 cell count, viral load and demographic characteristics of the newly infected patients living with HIV in Huangpu District, Shanghai in 2023 were collected and analyzed by using descriptive epidemiological method. ResultsThe mean CD4 cell count of the 67 newly identified HIV infected patients in Huangpu District was (301.22±235.19) cells·µL-1, with a mean viral load of (5.15±1.28) ×105 copies·mL-1.There were statistically significant differences in CD4 cell count and viral load among different age groups (P<0.05), but there were no statistically significant differences by gender and marital status (both P>0.05). The CD4 cell count and CD4/CD8 ratio both were negatively correlated with the lg value of viral load (r=-0.290, -0.378; P=0.027, 0.002). ConclusionThe CD4 cell counts of the newly identified HIV infected patients in Huangpu District in 2023 were generally low, the proportion of patients with high viral load was high, but the risk for elderly infected with HIV was high. The elderly have gradually become the key population for AIDS prevention and control in Huangpu District. It is recommended to expand HIV screening in the elderly to reduce the risk of HIV transmission and increase the rate of early detection and treatment. 
		                        		
		                        		
		                        		
		                        	
2.Optimization of processing technology and quality characterization of wine-steamed Taxillus chinensis

		                        			OBJECTIVE To optimize the steaming and processing technology of wine-steamed Taxillus chinensis, and to characterize its quality. METHODS Using the content of avicularin, quercitrin, quercetin and appearance traits as evaluation indicators, the analytic hierarchy process (AHP)-entropy weight method was used to determine the weights of each indicator, and the comprehensive scores of those indicators were used as response values. Box-Behnken response surface method was used to investigate the effects of solid-liquid ratio (g/mL), soaking time, and steaming time on the processing technology of wine-steamed T. chinensis, optimize the best processing technology, and verify it. Fifteen batches of T. chinensis decoction pieces from different origins were used to prepare wine-steamed T. chinensis using the best processing technology, and their qualities were characterized. RESULTS The optimal processing technology for wine-steamed T. chinensis was to take 100 g of T. chinensis decoction pieces, add 20 mL of yellow wine, seal and moisten for 2 h, steam at normal pressure for 1 h, take out and dry at 50 ℃. The surface of wine-steamed T. chinensis prepared by the optimal processing technology was reddish brown or brownish, and its powder was dark brown, with a hard or brittle texture that was easy to break, and had a slight aroma of alcohol, and an astringent taste. Results of microscopic and thin-layer identification for the stem cross-section of wine-steamed T. chinensis were the same as those of raw T. chinensis. The contents of moisture, total ash and acid-insoluble ash were 3.92%-8.75%, 2.27%-5.08%, and 0.19%-0.82%, respectively; the contents of water-soluble extract were 11.28%-18.56%, and the contents of alcohol-soluble extract were 3.36%-8.58%; the contents of avicularin, quercitrin, and quercetin were 0.22-1.64, 0.26-2.45, and 0.01-0.38 mg/g, respectively. 		                        			The aging disease associated with type 2 diabetes mellitus(T2DM)is a hot research topic in the field of diabetes at present.Sarcopenia has become the third major complication of T2DM after microvascular and macrovascular diseases,which could lead to the occurrence and development of various adverse events such as fracture,disability,and dysfunction.The spleen belongs to the earth,is in the middle jiao,governs transportation and transformation,and governs muscle.The functional activities of the spleen manifesting in normal transformation and transportation,the distribution of cereal essence,and the nourishment of muscles are necessary for normal physiological functions to be exerted.Recent studies have shown that skeletal muscle cell ferroptosis plays an important role in the pathogenesis of T2DM sarcopenia.Based on the theory of"spleen governing transportation and transportation and governing muscle",this study explores the pathogenesis of T2DM sarcopenia from the perspectives of the pathogenesis of"dysfunction of spleen in transportation,deficiency of cereal essence,obstruction of dampness and turbidity,and muscle dystrophy"in traditional Chinese medicine and the pathological mechanism of"skeletal muscle cell ferroptosis"in modern medicine.It summarizes the principles of traditional Chinese medicine prevention and treatment for T2DM sarcopenia based on the spleen,to provide theoretical support for enriching the theoretical connotation of spleen visceral state,as well as basic research and clinical trials on the prevention and treatment of T2DM sarcopenia with traditional Chinese medicine.
		                        		
		                        		
		                        		
		                        	
5.Application of different transbronchoscopic biopsies in the diagnosis of senile central lung cancer

		                        			Objective To study the application value of different transbronchial biopsies in the diagnosis of central lung cancer in elderly patients.Methods The clinical data of 97 elderly patients with central lung cancer diagnosed by pathology from June 2020 to June 2023 in the 923rd Hospital of Chinese People's Liberation Army Joint Logistic Support Force were retrospectively analyzed.According to the different initial transbronchial biopsy methods,the patients were divided into the endobronchial biopsy(EBB)group(n=51)and the conventional transbronchial needle aspiration(cTBNA)group(n=46).The histopathological results were statistically analyzed,and the first biopsy positive rates of EBB and cTBNA in the diagnosis of senile central lung cancer were calculated and compared.At the same time,the differences of biopsy tolerance and complications between the two groups were evaluated.Results The squamous cell carcinoma proportions in both groups were over 50%.There was no significant difference in the first biopsy positive rate between the two groups(P>0.05).The incidence of temporary retreat of the scope due to subjective tolerance in the EBB group was higher than that in the cTBNA group,and the difference was statistically significant(P<0.05).There was a statistically significant difference in the incidence of intraoperative complications of different grades between the two groups(P<0.001).Among them,the incidence of grade 2 and above complications during surgery in the EBB group was significantly higher than that in the cTBNA group(P<0.001).Conclusion For elderly patients with central lung cancer,the success rate of the first biopsy of EBB and cTBNA is roughly equivalent,but the incidence of postoperative complications of the latter is significantly lower than that of the former.cTBNA can be used as the first biopsy method for this population.
		                        		
		                        		
		                        		
		                        	
6.AAMP Promotes Osteosarcoma Cell Metastasis by Regulating YAP Signaling Pathway

		                        			Objective To determine the role of AAMP in osteosarcoma cells and explore the mechanism of invasion and metastasis of osteosarcoma cells regulated by AAMP through the YAP signaling pathway.Methods Public sequencing data analysis was used to explore the correlation between AAMP and osteosarcoma.q-PCR,Western blot,and immunohistochemistry were used to detect the expression levels of osteosarcoma cell-related molecules.CCK-8 was used to detect cell proliferation ability.Transwell and wound healing assays were used to detect the invasive and metastatic abilities of osteosarcoma cells.Immunofluorescence was used to detect the cell localization and expression levels of related molecules.Results High expression of AAMP was negatively correlated with the prognosis of patients with osteosarcoma(P<0.05),and the expression of AAMP in patients with metastatic osteosarcoma increased(P<0.05).Compared with the control group,the AAMP interference group showed significantly decreased migratory,invasive,and EMT activities(P<0.05).The expression of p-CFL1 reduced after the knockdown of AAMP,and the cell plate pseudopods decreased significantly(P<0.05).A positive correlation was found between the expression levels of AAMP and YAP in osteosarcoma cells(P<0.05).Interfering with YAP expression can affect the migration and invasion of osteosarcoma cells.Conclusion AAMP promotes osteosarcoma cell metastasis by regulating the YAP signaling pathway,suggesting that AAMP may be a key molecule in promoting invasion and metastasis of osteosarcoma.
		                        		
		                        		
		                        		
		                        	
7.Association of MTHFR gene C677T polymorphism with problem behavior and inheritance pattern among children with autism.

		                        			OBJECTIVE:
		                        			To assess the association of C677T polymorphism of methylenetetrahydrofolate reductase (MTHFR) gene with autistic behavior and inheritance pattern of children patients.
		                        		
		                        			METHODS:
		                        			Ninety three autism patients were selected as the study group, whilst 93 healthy children were selected as the control group. The C677T genotype of the MTHFR gene was determined, and the correlation between the genotype and the autistic behavior and inheritance pattern were investigated.
		                        		
		                        			RESULTS:
		                        			MTHFR gene C677T locus revealed three genotypes CC, CT and TT. Compared with the control group, the study group had fewer CC genotype but more TT genotype (P<0.05). Individuals with the three genotypes showed a statistically significant difference in the frequencies of four problem behaviors (P<0.05). Regression analysis showed that at least one T allele encoding the degree of 1 and 2 for the 4 problem behaviors that were statistically different. MTHFR gene C677T genotype was associated with autism under the recessive inheritance model and allelic inheritance model (P<0.05).
		                        		
		                        			CONCLUSION
		                        			The C677T polymorphism of the MTHFR gene is associated with autistic behaviors. Children with the TT genotype or T allele are at higher risk of developing autism, particularly direct gaze, complex limb movements, self-injurious behavior and hyperactivity 1 and 2 related with the degree of coding.

8.Association between triglyceride-glucose index and bone mineral density in elderly patients with type 2 diabetes mellitus

		                        			Objective:To explore the relationship between triglyceride-glucose(TyG)index and bone mineral density(BMD)in elderly patients with type 2 diabetes mellitus(T2DM).Methods:Elderly inpatients with T2DM in Department of Geriatrics of the Second Xiangya Hospital of Central South University were enrolled from January 2019 to December 2021.General clinical characteristics and the calculated TyG index were collected.BMDs at the lumbar spine, femoral neck, and total hip were measured using dual-energy X-ray absorptiometry.Multiple linear regression was used to detect the correlation between TyG index and BMDs; Logistic regression analysis was used to analyze the relationship between TyG index and osteoporosis(OP)risk.Results:A total of 822 subjects were included in this study.In elderly men with T2DM, TyG index was significantly positively correlated with BMDs of lumbar spine( β=0.465, P=0.004), femoral neck( β=0.348, P=0.031)and total hip( β=0.317, P=0.022)after adjusting for confounders, but these results were not observed in elder women with T2DM(all P>0.05). Binary Logistic regression analysis showed that in elderly men with T2DM, after adjusting for confounders, the TyG index was a protective factor for osteoporosis( OR=0.349, 95% CI: 0.144-0.846, P=0.020). However, in elderly women with T2DM, the TyG index was not significantly associated with the risk of osteoporosis( OR=1.748, 95% CI: 0.782-3.912, P=0.174). Conclusions:TyG index, which is used to evaluate insulin resistance, is strongly and correlated with BMDs of lumbar spine, femoral neck and total femoral in elderly male patients with T2DM, and is a protective factor for osteoporosis.
		                        		
		                        		
		                        		
		                        	
9.Prevention and Treatment of Atopic Dermatitis with Traditional Chinese Medicine: A Review

		                        			Atopic dermatitis (AD) is a chronic, recurrent and inflammatory skin disease. Modern medical research suggests that AD is related to immune function, genes, skin barrier and other factors, while the specific etiology and pathogenesis remains unclear. The disease has a long course and is prone to reoccur, which seriously affects people’s production and life. Steroids, antihistamines and immunosuppressants are commonly used western medicines for the treatment of AD, which, however, will cause adverse reactions after long-term application. Traditional Chinese medicine (TCM) has a long history, good therapeutic effect and rich clinical experience in the prevention and treatment of AD, and the research on the treatment of AD with TCM has been intensifying. Centering on the theory of TCM, we systematically summarized the research progress related to AD, discussed the etiology and pathogenesis of AD, and summarized the TCM syndrome differentiation of AD from the aspects of eight principles, etiology, Qi-blood-body fluid, zang-fu organs, six meridians, defense-Qi-nutrient-blood and triple energizer. According to the etiology and pathogenesis of AD, we proposed the therapeutic regimens corresponding to the type and stage of the disease. Considering the research progress achieved in the recent years in the prevention and treatment of AD by TCM and the modern pharmacological research on Chinese medicinal materials, we reviewed the classic famous prescriptions, self-made prescriptions and Chinese patent medicines, and expounded the mechanisms of single Chinese medicinal materials in the treatment of AD at the molecular level. The TCM external therapies such as wet compress, medicated bath, gel and ointment are safe and effective. Acupuncture and moxibustion play a role in the prevention and treatment of AD, which is worthy of promotion in clinical practice, and the syndrome differentiation methods of Qi-blood-body fluid and triple energizer are novel in the treatment of this disease. TCM, characterized by diverse therapeutic methods and good clinical efficacy, is worthy of promotion in the treatment of AD, which will contribute to the development of TCM in China. 
		                        		
		                        		
		                        		
		                        	
10.Combination of High-Density Lipoprotein Cholesterol and Lipoprotein(a) as a Predictor of Collateral Circulation in Patients With Severe Unilateral Internal Carotid Artery Stenosis or Occlusion

		                        			and Purpose Collateral circulation is considered an important factor affecting the risk of stroke, but the factors that affect collateral circulation remain unclear. This study was performed to identify the factors associated with collateral circulation, especially blood lipids. 
		                        		
		                        			Methods:
		                        			The study involved patients who had undergone digital subtraction angiography and were confirmed as having severe unilateral stenosis or occlusion of the internal carotid artery (ICA). We classified the collateral circulation status of each patient as good (Grade 3 or 4) or poor (Grade 0, 1, or 2) according to the grading system of the American Society of Interventional and Therapeutic Neuroradiology/American Society of Interventional Radiology. We collected data on patients’ characteristics and identified the factors that affect collateral circulation. 
		                        		
		                        			Results:
		                        			This study included 212 patients. The multivariate logistic regression analysis showed that the high-density lipoprotein cholesterol (HDL-C) concentration and a complete anterior half of the circle of Willis were independent protective factors for good collateral circulation, whereas elevated lipoprotein(a) [Lp(a)] and serum creatinine concentrations were independent risk factors for good collateral circulation. The area under the receiver operating characteristics curve (AUC) was 0.68 (95% confidence interval [CI], 0.61–0.76) for HDL-C and 0.69 (95% CI, 0.62–0.76) for Lp(a). A binary logistic regression model analysis of the joint factor of HDL-C and Lp(a) yielded an AUC of 0.77 (95% CI, 0.71–0.84). 
		                        		
		                        			Conclusions
		                        			In patients with severe unilateral ICA stenosis or occlusion, the combination of HDL-C and Lp(a) is a useful predictor of collateral circulation.
