1.Evaluation of the quality of Chinese guidelines and expert consensuses on nursing published in 2022

Objective To evaluate the scientificity,transparency and applicability of Chinese guidelines and expert consensuses on nursing published in 2022,in order to improve the quality of guidelines and consensuses.Methods Databases including Medline,Embase,Web of Science,CBM,CNKI,WanFang database,Chinese Medical Journal,and related websites were electronically searched,as well as China Hong Kong,Macao and Taiwan medical journals,to collect Chinese guidelines and expert consensuses on nursing from January to December 2022.STAR tool was used to evaluate the quality of each guidelines and consensuses by 3 assessors independently.Total score,scoring rate of each domain and item were adopted to analyze the outcomes.Results A total of 3 guidelines and 33 expert consensuses were included.The total guidelines and expert consensuses STAR score(33.5±14.3).The quality of guidelines and consensuses was low.The quality of guidelines was moderate with average score of 55.1,and the quality of consensuses was low with average score of 31.5.The included guidelines and consensuses had a highest score rate(52.4%)in the domain of recommendation.Among 39 items of STAR tool,the top 4 items including listing participants and institutions,explaining additional instructions for implementation,describing consensus method,and listing references for recommendations had a high score rate of 100%,83.3%,77.8%,75.0%respectively.However,the items of registration,providing registration information,protocols being searched on public platforms and explaining the role of funding had a low score rate,urgent need for attention and upgrading.Conclusion The overall quality of the Chinese guidelines and expert consensuses on nursing published in 2022 was low.As a medical and nursing practice guidance document,the quality of guidelines and expert consensuses should be improved by encouraging registration,strengthening management of interest conflict,enhancing the rigor of guideline developing process,and expanding the dissemination.
2.Effect of PCK1 on proliferation and migration of mouse vascular smooth muscle cells and its underlying mechanism

AIM:To investigate the role of phosphoenolpyruvate carboxykinase 1(PCK1)in the proliferation and migration of mouse vascular smooth muscle cells(VSMCs)and the underlying mechanism.METHODS:The prolif-eration and migration of mouse VSMCs were induced by platelet-derived growth factor(PDGF)-BB.The cells were divided into a vehicle group and a PDGF-BB group.The expression of PCK1 was detected by Western blot and immunofluores-cence staining.The mouse Pck1 siRNA(si Pck1)were transfected into mouse VSMCs to silence PCK1.The cells were di-vided into the vehicle,si Pck1+vehicle,PDGF-BB and si Pck1+PDGF-BB groups.The protein level of PCK1 was detected by Western blot.The proliferation was explored by Ki-67 immunofluorescence staining and the viability was detected by CCK-8 assay.The migration was determined by a scratch test.Mitochondrial dynamics were observed via transmission electron microscopy.A lentivirus carrying dynamin-related protein 1(Drp1)gene(lenti-Drp1)was transfected into VSMCs to induce them to overexpress DRP1.The cells were divided into the PDGF-BB,si Pck1+PDGF-BB,lenti-Drp1+PDGF-BB and lenti-Drp1+si Pck1+PDGF-BB groups.Proliferation,migration and mitochondrial dynamics were measured as described above.RESULTS:PDGF-BB increased the protein expression of PCK1 and DRP1,cell viability,the per-centage of Ki-67-positive cells,the wound healing rate and mitochondrial division in VSMCs.These effects were sup-pressed when PCK1 protein expression was silenced.After DRP1 was overexpressed,the inhibitory effects of PCK1 silenc-ing on cell viability,the percentage of Ki-67-positive cells,the wound healing rate and mitochondrial division were signifi-cantly reversed.CONCLUSION:PCK1 promotes the mitochondrial division,proliferation and migration of VSMCs in mice by upregulating the expression of DRP1.
3.Evidence-based practice of ankle pump exercises in preventing perioperative lower limb deep vein thrombosis in joint replacement patients

Objective:To summarize the best evidence for the use of ankle pump exercises in preventing perioperative lower limb deep vein thrombosis (DVT) in patients undergoing joint replacement surgery and to evaluate its clinical application.Methods:This study utilized an evidence-based continuous quality improvement model. A systematic search was conducted for relevant clinical practice guidelines, expert consensus, systematic reviews, and randomized controlled trials (RCTs) from both domestic and international sources. The best evidence was evaluated and summarized, and audit indicators were developed for baseline assessments. Facilitating and hindering factors were analyzed based on the baseline results. From December 2022 to April 2023, the evidence was applied in the Department of Orthopedics at Henan Provincial People's Hospital. The pre-evidence group for baseline assessments included 36 joint replacement patients and 18 nurses from the orthopedic unit between December 2022 and January 2023, while the post-evidence group included 36 joint replacement patients and the same 18 nurses between March and April 2023. The knowledge level of nurses, the effectiveness of ankle pump exercises, the execution rate, and patient compliance were compared before and after evidence-based practice.Results:A total of 12 articles were included in the study, comprising one clinical practice guideline, five expert consensus papers, one systematic review, and five RCTs. Ten best practices were summarized in terms of applicable populations, exercise assessment, exercise positions, timing, methods, frequency, and patient education. After implementing evidence-based practice, the knowledge scores of nurses on ankle pump exercises significantly improved from (70.56±9.98) to (82.78±8.26) ( P<0.01). The compliance rates of audit items 2, 3, 4, 5, and 6 among nurses increased from 0, 22.2%, 0, 27.8%, and 11.1% respectively, to 94.4%, 94.4%, 83.3%, 100.0%, and 100.0% respectively ( P<0.01). On the patient side, the compliance rates for audit items 7 and 8 increased from 5.6% and 0 to 86.1% and 94.4%, respectively ( P<0.01) . Conclusions:Implementing evidence-based ankle pump exercises to prevent DVT in joint replacement patients can significantly improve nurses' knowledge of evidence-based practice, standardize the practice of ankle pump exercises, and explore balanced educational strategies that enhance patient compliance, ensuring clinical practices are evidence-based.
4.Efficacy of tirofiban in treatment of acute cerebral infarction beyond the time window and with the type of anterior circulation artery atherosclerosis

Objective To evaluate the efficacy of tirofiban injection in treatment of acute cerebral infarction beyond the time window and with the type of anterior circulation artery atherosclerosis.Methods A total of 66 patients with acute cerebral infarction with the type of anterior circulation artery atherosclerosis admitted to the Department of Neurology of Wenzhou Central Hospital from November 2021 to December 2022 were selected as study objects,and they were divided into treatment group and control group by random number table method,with 33 cases in each group.The control group was given aspirin enteric-coated tablets.The treatment group was given intravenous infusion of tirofiban injection for 48 hours,bridging conventional antiplatelet therapy.The course of treatment was 2 weeks and the follow-up was 6 months.Before and after treatment,National Institutes of Health stroke scale(NIHSS)score,modified Rankin scale(MRS)score,modified Barthel index(MBI)score,thromboelasmogram and cerebral infarction hemorrhage transformation of two groups were compared.Results After 14 days of treatment,the NIHSS score in treatment group was significantly lower than before treatment(P<0.01),and significantly lower than that in control group(P<0.05).After 14 days of treatment and 6 months of follow-up,MRS scores in both groups were significantly lower than before treatment(P<0.01).After 6 months of follow-up,MRS scores in treatment group were significantly lower than those in control group(P<0.05).After 7 and 14 days of treatment and 6 months of follow-up,the MBI scores of patients in treatment group were significantly higher than before treatment(P<0.05).After 14 days of treatment and 6 months of follow-up,MBI scores in treatment group were significantly higher than those in control group(P<0.05).After treatment,the platelet aggregation inhibition rate in treatment group was significantly higher than that in control group(P<0.01),and the platelet activity was significantly lower than that in control group(P<0.01).No cerebral infarction hemorrhage transformation occurred in both groups.Conclusion Tirofiban bridging conventional antiplatelet therapy can prevent the neurological deterioration of acute cerebral infarction,greatly reduce the rate of disability,improve patients'life ability,and do not increase the risk of cerebral hemorrhage.
5.Clinical effect of indobufen combined with clopidogrel on acute cerebral infarction

Objective To explore the clinical effect of indobufen combined with clopidogrel on acute cerebral infarction and analyze the drug safety.Methods Using a randomized and controlled clinical study method,72 patients with acute cerebral infarction selected in Wenzhou Central Hospital from June 2021 to December 2022 were included in the study.They were randomly divided into control group and experimental group.The experimental group was given indobufen tablets 200mg(twice daily),and clopidogrel 75mg every day.The control group was treated with clopidogr el 75mg every day.The two groups were treated with conventional internal medicine in the department of neurology.They were given Xingnaojing Injection 20mg every day,and Butylphthalide Injection 100ml(twice daily).And they were given symptomatic support treatment with drugs to improve cerebral circulation and drugs to reduce blood fat and stabilize plaque.In addition,we control patients'basic diseases such as hypertension and hyperglycemia.The indexes of cerebral infarction related scale were evaluated before treatment and after 3 weeks.Results After 3 weeks of treatment,the scores of stroke scale in the experimental group and the control group were lower than those before treatment.The total effective rate in the experimental group was higher than that in the control group,the difference was statistically significant(P<0.05).The score of Quality of Life in the experimental group was significantly higher than that before treatment(P<0.001).The score of Quality of Life in the control group was higher than that before treatment(P<0.05).Compared with the control group,the Quality of Life score of the experimental group was higher,with statistical significance(P<0.05).The incidence of stomachache and acid reflux in the two groups was low,and there was no statistically significant difference between the two groups(P>0.05).No gastrointestinal bleeding and intracranial hemorrhage occurred in the two groups.Conclusion Indobufen combined with clopidogrels can improve the treatment effect and Quality of Life score of patients with acute cerebral infarction,and is more obvious than clopidogrel alone.In addition,compared with clopidogrel monotherapy,indobufen combined with clopidogrel did not significantly increase the incidence of adverse reactions in patients.
6.Risk factors for the occurrence and treatment failure of peritoneal dialysis-associated E. coli peritonitis

Objective:To investigate the clinical characteristics of E. coli peritoneal dialysis-associated peritonitis (PDAP) and the risk factors for its occurrence and treatment failure.Methods:The clinical data of patients with episodes of PDAP in four general hospitals in Jilin Province from 2013 to 2019 were retrospectively reviewed. According to the pathogenic bacteria, the patients were divided into E. coli and non- E. coli groups. The incidence of E. coli PDAP in the last seven years was calculated and the clinical characteristics were compared between two PDAP groups. Logistic regression was used to analyze the risk factors for the occurrence and treatment failure of E. coli PDAP. Results:A total of 693 PDAP episodes/cases were enrolled in this study, including 100 episodes/cases in the E. coli group and 593 episodes/cases in the non- E. coli group. The incidence rate of E. coli PDAP in the four hospitals showed a decreasing trend during 2013 to 2019. Compared with the non-E.coli group, the proportion of diabetic patients and the average blood albumin levels in the E. coli group were lower ( χ2=5.006, Z=-2.992, P<0.05), while the proportion of refractory peritonitis was higher, and the duration of antibiotic therapy was longer ( χ2=6.350, Z=-2.779, P<0.05). Multivariate Logistic regression analysis showed that history of PDAP ( OR=1.577, 95% CI: 1.015-2.448) and low baseline serum albumin level ( OR=0.958, 95% CI: 0.923-0.995) were independent risk factors for the development of E. coli PDAP, while concomitant diabetes was an independent protective factor for E. coli PDAP ( OR=0.538, 95% CI: 0.330-0.876). Moreover, long-term dialysis was an independent risk factor for treatment failure of E. coli PDAP ( OR=1.047, 95% CI: 1.018-1.076). Conclusion:The incidence rate of E. coli PDAP in study institutions has declined in the past 7 years, but the rate of refractory PDAP is still high. The history of PDAP and low blood albumin level are independent risk factors for the occurrence of E. coli PDAP, while concomitant diabetes is an independent protective factor for the occurrence of E. coli PDAP. Long-term dialysis is an independent risk factor for treatment failure of E. coli PDAP.
7.Overview of systematic reviews on effectiveness and safety assessment of intermittent pneumatic compression devices on preventing VTE

Objective:To carry out the overview of systematic reviews on effectiveness and safety assessment of pneumatic compression devices (PCD) on preventing venous thromboembolism (VTE) .Methods:Systematic reviews or Meta-analysis comparing PCD with other physical prophylaxis and drug prophylaxis were retrieved by computer in the Cochrane Library, PubMed, Embase, China National Knowledge Internet (CNKI) , China Biological Medicine, WanFang data from building the database to 31st March 2019. Two researchers independently implemented the literature screening, data extraction and quality evaluation. The Assessment of Multiple Systematic Reviews (AMSTAR) tool was used to quality assessment of methodology for included literatures. The Grading of Recommendations Assessment, Development and Evaluation (GRADE) system was used to quality evaluation for outcome indexes.Results:Totals of 15 systematic reviews or Meta-analysis were included with 8 in Chinese and 7 in English. There were 11 literatures published after 2012. Quality assessment of methodology showed that 3 literatures met 11 items of AMSTAR, and other researches had the defect existed in different aspects. Evidences with the medium quality displayed the effectiveness of intermittent pneumatic compression devices in VTE prevention which could reduce the incidence of adverse events caused by separate drug prophylaxis, such as bleeding, through combining with drug prophylaxis. Application of intermittent pneumatic compression devices for VTE prevention should be based on clinical guidelines. At present, there was limited evidence guiding selection of specific device and device type.Conclusions:Intermittent pneumatic compression devices are safe and effective in VTE prevention which still needs to be demonstrated by researches with the large sample and high of quality.
8.Analysis of MRI features and prognosis for minor stroke and nonGminor stroke of anterior circulation in plateau

Objective To investigate the MRI manifestations and analyze the prognostic factors of patients with anterior circulation minor stroke and nonGminor stroke in Qinghai plateau.Methods 41 6 cases of the first admission,including 1 9 2 patients with minor stroke and 224 patients with nonGminor stroke.MRI and MRA examinations of the head were completed in all patients within 72 h of admission.Patients were followed up for one year to observe the recurrence of stroke,and the quality of life was evaluated with the help of modified Rankin Scale (MRS)scores.Results (1)MRA showed that 36.98% of the minor stroke and 58.93% of the nonGminor stroke had the stenosis of the responsible artery at the infarction site.The difference was significant (χ2= 1 9.94,P< 0.00 1 ).(2 )MRI showed that the initial infarction sites of minor stroke and nonGminor stroke were different (χ2=4.47 ,P<0.005 ).(3 )The recurrence rate was 10.42% in minor stroke and 12.05% in nonGminor stroke.There was no significance between the two groups (χ2= 0.28,P>0.05).(4) Among patients with poor prognostic outcomes (whose MRS≥3),there were 1 9 cases of minor stroke and 6 1 cases of nonGminor stroke,and the difference was significant (χ2=20.00,P<0.0 1 ).Conclusion LesionGrelated vascular stenosis in patients with minor stroke is mild and the primary infarction is more common in isolated subcortical or deep white matter.The stenosis is severe in patients with nonGminor stroke,and the infarct lesion is often subcortical with or without cortical or deep white matter.There is no difference in recurrence risk between minor stroke and nonGminor stroke.The prognosis of minor stroke is better than that of nonGminor stroke.
9.Meta-analysis of enteral nutritional support for pressure ulcer prevention

Objective Nutrition becomes a focus to professionals as an important factor during the development of pressure ulcers.However, there is no evidence for the effectiveness of eternal nutrition support for pressure ulcer prevention .This research aimed to evaluate the effectiveness of eternal nutrition support for pressure ulcer prevention and provide evidence for patients with the risk of pressure ulcer . Methods Key words such as pressure ulcer prevention,eternal nutritional supportwere used to search information in the Cochrane Library , JBI Database of Systematic Reviews , MEDLINE, Web of Science, EMbase, CBMdisc, CNKI and Wanfang Database to collect randomized controlled trials (RCTs).The quality of RCTs was critically appraised and data were extracted by 3 reviewers independently .Meta-analyses were conducted for the eligible RCTs using RevMan 5.0 software, and Stata12.0 software was applied to detect bias or sensitivity analysis to determine the source of heterogeneity if necessary . Results 7 RCTs were includ-ed, among which 1 RCT was excluded by sensitivity analysis .6 RCTs ( n=1553 ) were finally included for meta analysis ( n=728 in trail group, n=825 in control group), and all the patients were given oral nutrition support for 7days to 26 weeks.The results showed that the ulcer incidence in the intervention group reduced by 16%in comparison to the control group (P=0.006).The protein and energy intake in intervention group were higher than those in control group , and the length of stay in hospital was shortened by 16 days. No significant difference was found in the albumin values . Conclusion The enteric nutrition support can reduce the incidence of pressure ulcer obviously with the highest level of evidence , IA .
10.Role of GSK-3βactivity and microglial TLR4 receptor in POCD

Objective To explore glycogen synthase kinase -3β( GSK-3β) activity and Toll-like receptor 4 ( TLR4 ) proteins expression of microglia were tested in vitro experiments, and the possible mechanism of postoperative cognitive dysfunction(POCD).Methods The cell morphology of primary culture microglia was observed by inverted microscope;microglia were identified by glial fibrillary acidic protein ( GFAP ) immunofluorescence;the best POCD modeling conditions of microglia injury induced by lipopolysaccharides( LPS) were screened ; microglia vigor was assayed by MTT ; the proteins expressions of GSK-3βand TLR4 of microglia were detected by Western blot.Results GFAP immunofluorescence showed a positive result that primary culture of rat microglia was successful;MTT result showed that the best PODC modeling conditions of microglia injury induced by LPS (100 ng/mL) was 7h; Western blot results showed that the preotein expressions of GSK-3βand TLR4 of microglial cells were up-regulated by LPS compared with the control group,and there were significantly differences (P<0.01).Conclusion PODC pathogenesis may be associated with LPS that could up-regulat the protein expression of GSK-3βand TLR4 in microglial cells.
