1.Expression and prognostic value of triggering receptor expressed on myeloid cells-1 in patients with cirrhotic ascites and intra-abdominal infection

ObjectiveTo analyze the expression level of triggering receptor expressed on myeloid cells-1 (TREM-1) in serum and ascites of patients with cirrhotic ascites, and to investigate its correlation with clinical features and inflammatory markers and its role in the diagnosis of infection and prognostic evaluation. MethodsA total of 110 patients with cirrhotic ascites who were hospitalized in The Fifth Hospital of Shijiazhuang from January 2019 to December 2020 were enrolled, and according to the presence or absence of intra-abdominal infection, they were divided into infection group with 72 patients and non-infection group with 38 patients. The patients with infection were further divided into improvement group with 38 patients and non-improvement group with 34 patients. Clinical data and laboratory markers were collected from all patients. Serum and ascites samples were collected, and ELISA was used to measure the level of TREM-1. The independent-samples t test was used for comparison of normally distributed continuous data between two groups; the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between two groups, and the Kruskal-Wallis H test was used for comparison between multiple groups; the chi-square test was used for comparison of categorical data between two groups. A Spearman correlation analysis was used to investigate the correlation between indicators. A multivariate Logistic regression analysis was used to identify the influencing factors for the prognosis of patients with cirrhotic ascites and infection. The receiver operating characteristic (ROC) curve was used to evaluate the diagnostic and prognostic efficacy of each indicator, and the Delong test was used for comparison of the area under the ROC curve (AUC). ResultsThe level of TREM-1 in ascites was significantly positively correlated with that in serum (r=0.50, P<0.001). Compared with the improvement group, the non-improvement group had a significantly higher level of TREM-1 in ascites (Z=-2.391, P=0.017) and serum (Z=-2.544, P=0.011), and compared with the non-infection group, the infection group had a significantly higher level of TREM-1 in ascites (Z=-3.420, P<0.001), while there was no significant difference in the level of TREM-1 in serum between the two groups (P>0.05). The level of TREM-1 in serum and ascites were significantly positively correlated with C-reactive protein (CRP), procalcitonin (PCT), white blood cell count, and neutrophil-lymphocyte ratio (r=0.288, 0.344, 0.530, 0.510, 0.534, 0.454, 0.330, and 0.404, all P<0.05). The ROC curve analysis showed that when PCT, CRP, and serum or ascitic TREM-1 were used in combination for the diagnosis of cirrhotic ascites with infection, the AUCs were 0.715 and 0.740, respectively. The multivariate Logistic regression analysis showed that CRP (odds ratio [OR]=1.019, 95% confidence interval [CI]: 1.001 — 1.038, P=0.043) and serum TREM-1 (OR=1.002, 95%CI: 1.000 — 1.003, P=0.016) were independent risk factors for the prognosis of patients with cirrhotic ascites and infection, and the combination of these two indicators had an AUC of 0.728 in predicting poor prognosis. ConclusionThe level of TREM-1 is closely associated with the severity of infection and prognosis in patients with cirrhotic ascites, and combined measurement of TREM-1 and CRP/PCT can improve the diagnostic accuracy of infection and provide support for prognostic evaluation.
2.Bioinformatics analysis of HBV s gene mutation in blood donor

Objective To analyze the infectious indicators,HBV s gene mutations and changes in bioinformatics char-acteristics of HBV DNA+/HBsAg-blood donor.Methods A sample of HBV DNA+/HBsAg-was screened by PCR meth-od,and HBsAg/HBsAb/HBeAg/HBeAb/HBcAb of HBV in the sample were detected by ELISA and chemiluminescence methods.The mutation of the HBVs gene fragment in the sample was sequenced and analyzed,and the bioinformatics analy-sis was performed using analysis software.Results The sample showed HBV DNA+,HBsAg-,HBsAb+,HBeAg+,HBe-Ab-and HBcAb+,with an amino acid unit point mutation(P151L)occurred in the HBV s gene,and the spatial structure changed.Conclusion The spatial structural changes in the gene sequence of HBVs may be the reason for the appearance of serological HBsAg-/HBsAb+/HBV DNA+in the test results.

Objective To investigate the causal association between bronchial asthma and bone mineral density at different sites using a two-sample Mendelian randomization (MR) approach. Methods Summary data for exposure factors and outcome were obtained from different genome-wide association studies.Single nucleotide polymorphisms strongly associated with bronchial asthma were selected as instrumental variables,and those in linkage disequilibrium were excluded.The inverse-variance weighted (IVW) method was used as the primary method for MR analysis,complemented by weighted median,simple mode,weighted mode,and MR-Egger regression methods.Sensitivity analyses were conducted to assess the stability of the results. Objective:To assess impact of radiation from nuclear power on the surrounding environment, a dose assessment model was constructed and relevant dose coefficients were determined through refined animal models.Methods:Zebrafish is one of the most important aquatic model animals in the radiation hazard assessment of nuclear power liquid effluent. A geometric model of zebrafish containing internal bones and visceral organ was established for dose estimation. The internal and external dose coefficients (DC) of the zebrafish model were calculated by Monte Carlo method with seven nuclides as the source term, 3H, 40K, 58Co, 60Co, 110Ag, 134Cs, 137Cs, which are common in nuclear liquid effluents and environmental monitoring. Results:The level of nuclide gamma energy determines dose coefficients for external radiation. The dose coefficients of most nuclides in internal organs was higher than that in whole body, and the internal organ dose of 58Co was 165% higher than that in whole body. The internal radiation dose coefficients of the model established in this study was relatively high, and the internal radiation dose coefficients caused by 60Co was 2.6 times of existing ellipsoid model, which suggested that the different internal materials and the choice of different particle types would affect the energy deposition. Conclusions:Refining of model animals is important. Accurate assessment of the organs dose coefficients of model animals is helpful to assess the radiation effects on non-human species.
5.Correlation between body fat percentage with physical fitness in female college students

Objective:
To reveal the relationship between physical fitness and body fat percentage (BFP) among female college students.
Methods:
During March 2017, BFP and fat free mass were measured among 633 randomly selected healthy female college students. All subjects were divided into five groups based on BFP: low weight, normal weight, high body fat, grade I obesity and grade II/III obesity. Grip strength, leg strength, back strength, vital capacity, maximal oxygen uptake, reaction time, sitandreach, back scratch and standing on one foot with closed eyes were tested.
Results:
There were significant differences between all groups in grip strength, back strength, vital capacity, the absolute and relative value of VO2max, back scratch on both sides and standing on one foot with closed eyes(P<0.05). Grade I obesity group showed significant higher vital capacity, but lower relative value of VO2max, back scratch in both sides compared with normal group. Significant differences were observed in relative value of maximum oxygen uptake and back scratch in both sides between high body fat group and grade I obesity(P<0.05). Moreover, grade II/III obesity group showed significantly higher grip strength, back strength, vital capacity and absolute VO2max, but lower relative VO2max, back scratch in both sides and standing on one foot with closed eyes compared with normal and high body fat groups(P<0.05). There were significant differences in all the indicators except back strength and the relative VO2max between grade II/III obesity and grade I obesity group(P<0.05).
Conclusion
The results showed that physical fitness of female college students with normal body fat percentage is significantly better than that of obese peers. Body fat percentage confers damage for cardiopulmonary endurance fitness and upper limb flexibility fitness when higher than 30% and for balance fitness when higher than 35%.
6.The matching analysis of the tibial osteotomy and the imported prosthesis components of the Han and Uygur populations

Objective To compare the anatomical differences of tibia osteotomy between Han and Uygur populations and to analysis their compatibility with imported prosthesis component.Methods Two hundred patients (400 knees) were selected from the outpatients and healthy volunteers of the non-knee joint disease from September 1,2012 to December 1,2014,with 86 men (172 knees) and 114 women (228 knees).The average age is 44.64-10.8 years (range 21-68 years old).According to the populations,the subjects were divided into Han group and Uygur group.Furthermore,each group was divided into male subgroup and female subgroup.A full-length CT scan of the lower limb was conducted to reconstruct the knee joint in the workstation.The tibial osteotomy was simulated in total knee arthroplasty.The transverse diameter of the tibial cross section,the longitudinal depth of the medial platform and the depth of the lateral platform were measured.The front and back diameter and aspect ratio of the osteotomy were calculated.There parameters were compared between the Han and Uygur groups.Five mm tolerance range method was used to analysis the cover rate of the following three kinds of commercially imported prosthesis,namely Depuy,Link and Zimmer.Results The mean value of the tibial cross section were 72.3±4.3 mm,50.7±3.1 mm,47.8±3.0 mm,49.3±2.9 mm and 0.682±0.021 in Han group,while those were 74.5±4.8 mm,51.4±3.0 mm,48.6±2.8 mm,50.0±2.9 mm and 0.672±0.020 in Uygur group,respectively.Except the aspect ratio,the average value of each parameter in Uygur group was greater than that in the Han group with statistically significant differences between the two populations.The cover rate of the three kinds of prosthesis tibial component were 53.0%,37.0% and 40.0% for Han group,but 61.0%,43.0% and 42.5% for Uygur group.Depuy prosthesis was fit for the Uygur population better.Conclusion The geometry and anatomy of proximal tibia in Chinese showed significant difference between Han and Uygur populations.The mean covering rates of the imported tibial components were low in the dimensions of the resected surface.We can improve the prosthesis and the matching degree by adjusting the parameters or increasing the model set.
7.Recent advances in the diagnosis and treatment of refractory epilepsy

The refractory epilepsy refers to the epilepsy whose seizures couldn't be cured after using two kinds of correctly selected antiepileptic drugs which can be tolerated and enough dosage and duration of monotherapy or combination therapy.The pathogenesis of intractable epilepsy is complex,and there is no established theory at home and abroad.Recently,more and more attention has been paid to the correlation between mitochondrial dysfunction caused by oxidative stress and intractable epilepsy.Based on the summary of common treatment of refractory epilepsy and by searching the related literature at home and abroad,further investigation would be made to explore the diagnosis and treatment of intractable epilepsy theory in order to provide new strategies for diagnosis and treatment of intractable epilepsy.
8.Effect of Aerobic Exercises and Dietary Intervention on Lipid Metabolism in Rats with Metabolic Syndrome and Mechanism Medicated by Peroxisome Proliferator-Activated Receptorα

Objective To study the effect of aerobic exercise and dietary intervention on lipid metabolism in metabolic syndrome rats, and investigate the possible mechanism mediated by peroxisome proliferator-activated receptorα(PPARα). Methods After one-week feed-ing, Sprague-Dawley rats were randomly divided into blank control group (CC group) and model group which were feed in high-fat-and-salt diet for 18 weeks to establish a metabolic syndrome model. Then, the metabolic syndrome rats were randomly divided into model control group (MC), the model high-fat diet group (MHE) and the model general died exercise group (ME). ME and MHE groups were forced to run on a treadmill for twelve weeks at the same time. The weight of perirenal fat, blood free fat acid (FFA), and blood lipid were determined. The expression of PPARαmRNA in myocardium was detected by RT-PCR. Western blotting was applied to detect the protein expression of PPARαin myocardium. Results Compared with CC group, MC group showed significant increase in body weight, perirenal fat weigh, FFA, and blood lipid (P<0.05), and significant decrease in PPARαmRNA and protein expression (P<0.01) in myocardium. Compared with MC group, ME and MHE groups showed significant decrease in body weight, perirenal fat weight, triglyceride (TG), and showed significant in-crease in high-density lipoprotein (HDL), and the expression of PPARαmRNA and protein in myocardium (P<0.05). Compared with MHE group, ME group showed decrease in low-density lipoprotein (LDL) (P<0.05), and increase in the expression of PPARαmRNA and protein (P<0.01). Conclusion Aerobic exercise may activate the expression of PPARα, enhance the utilization of fatty acid, reduce body mass and visceral fat mass, improve the dyslipidemia and then regulate lipid metabolism in metabolic syndrome rats.
9.Effect of hand hygiene comprehensive intervention on incidence of pediatric healthcare-associated infection

Objective To explore the effect of 5M1E quality analysis tool-based hand hygiene(HH) comprehensive intervention measures on pediatric healthcare-associated infection(HAI).Methods HH status of 29 health care workers(HCWs) in the pediatric department of a hospital was monitored, April-June 2015 was baseline survey stage, 5M1E quality analysis tool was adopted to analyze various factors affecting the compliance rate of pediatric HH, intervention measures began to be taken in June 2015, and compared with HH in July 2015-March 2016.The occurrence of HAI between July 2014-March 2015 and July 2015-March 2016 was compared.Results HH compliance rate of HCWs increased from 30.86% in April-June 2015 to 81.94% in January-March 2016;HH correct rate increased from 68.14% to 93.75%;HH compliance rate and correct rate of HCWs gradually increased(X2=2 608.626, 630.798,respectively, both P<0.001).Qualified rate of detection of HCWs' HH sampling increased from 20.69% in April-June 2015 to 89.66% in January-March 2016(X2=31.957,P<0.001).Incidence of HAI decreased from 7.74% in July 2014-March 2015 to 3.62% in July 2015-March 2016(X2=46.717,P<0.001).Conclusion Application of 5M1E quality analysis tool in the investigation, analysis, and comprehensive intervention in HCWs' HH in pediatric department has greatly enhanced HCWs' HH compliance rate, and reduced the incidence of HAI in pediatric patients.
10.Effects of edaravone on the expression of TGF-β1 and myocardialfibrosis in rats

Objective To investigate the effects of edaravone on myocardial fibrosis induced by isoproterenol (ISO) in rats, and to discuss the correlation between the level of transforming growth factor-β1 (TGF-β1) and the myocardial fibrosis. Methods Forty male SD rats were randomly divided into five groups, namely control group, model group and edaravone groups (low, medium and high doses). Isoproterenol was used to establish the rat model of myocardial fibrosis. Edaravone groups were given edaravone [3, 5 and 10 mg/(kg · d)] to intervene for 14 days. The activity of superoxide dismutase (SOD) and the level of malondialdehyde (MDA) were examined after 15-d treatment. The left ventricular mass index (LVMI) and collagen volume fraction (CVF) were examined. The expression of TGF-β1 was detected by Western blot assay and immuno-fluorescence method. Results The content of MDA and LVMI were significantly higher in model group than those of the control group (P<0.01),whereas the content of SOD was significantly lower in model group than that of the control group (P<0.01). Compared with model group, the expression level of MDA decreased with the increased intervention dose of edara-vone (P<0.05), while SOD expression level increased (P<0.05). There was no significant difference in the level of SOD be-tween middle dose edaravone group and the control group. LVMI was decreased with the increased doses of edaravone ( P<0.01). There was no significant difference in LVMI between the high dose of edaravone group and the control group. Com-pared with the control group, the expression level of TGF-β1 was significantly increased in model group (P<0.01). The ex-pression level of TGF-β1 was reduced with the increased doses of edaravone. CVF was significantly increased in model group compared with that of control group (P<0.001). CVF decreased with the increased doses of edaravone in medium and high doses of edaravone groups, but they were higher than that of control group (P<0.01). TGF-β1 was positively correlated with MDA, LVMI and CVF (r=0.931, 0.879 and 0.930, P<0.001). SOD was negatively correlated with TGF-β1 (r=-0.892, P<0.001). Conclusion Edaravone can relieve myocardial fibrosis by inhibiting oxidative stress and TGF-β1 in rats.
