1.Effect of remimazolam on intraoperative neurophysiological monitoring in spinal surgery

Objective To investigate the effect of remimazolam on intraoperative neurophysiological monitoring(IONM)in spinal surgery.Methods Sixty patients undergoing elective spinal surgery for IONM were selected strictly according to the criteria,33 males and 27 females,aged 18-55 years,BMI 18-24 kg/m2,ASA physical status Ⅰ-Ⅲ.The patients were randomly divided into two groups:remimazolam group(group R)and propofol group(group P),30 patients in each group.Remimazolam was used in group R and propofol was used in group P during induction and maintenance of anesthesia.HR,MAP,and BIS values were recorded when patients entered the room(T0),immediately after endotracheal intubation(T1),at the time of muscle relaxant withdrawal(T2),30 minutes after muscle relaxant withdrawal(T3),and 50 minutes after muscle relaxant withdrawal(T4).The current intensity and amplitude of the first motor evoked potential(MEP)were recorded.The waiting time from drug withdrawal to the first induced MEP was recorded.The amplitude and latency of somatosensory evoked potential(SEP)and MEP at T4 were recorded.Operation time,anesthesia time,intraoperative remifentanil dosage,the use of vasoactive drugs,recovery time,extubation time,and adverse reactions were recorded.Results Compared with group P,HR and MAP were significantly increased at T1-T4(P<0.05),the amplitude of MEP induced for the first time were significantly increased(P<0.05),the amplitudes of SEP and MEP at T4 were significantly in-creased and the latency period was significantly shortened(P<0.05),the dosage of remifentanil was sig-nificantly decreased(P<0.05),the number of bradycardia or hypotension were significantly decreased(P<0.05),and the recovery time and extubation time were significantly shortened in group R(P<0.05).Conclusion Remimazolam can be safely used in spinal surgery requiring IONM,with small circu-lation fluctuation,quick recovery,less postoperative adverse reactions,little effect on SEP and MEP,which is conducive to improving the quality of IONM.
2.Alterations in Spontaneous Brain Activity in Drug-Naïve First-Episode Schizophrenia: An Anatomical/Activation Likelihood Estimation Meta-Analysis

Objective:
The etiology of schizophrenia is unknown and is associated with abnormal spontaneous brain activity. There are no consistent results regarding the change in spontaneous brain activity of people with schizophrenia. In this study, we determined the specific changes in the amplitude of low-frequency fluctuation/fractional amplitude of low-frequency fluctuation (ALFF/fALFF) and regional homogeneity (ReHo) in patients with drug-naïve first-episode schizophrenia (Dn-FES).
Methods:
A comprehensive search of databases such as PubMed, Web of Science, and Embase was conducted to find articles on resting-state functional magnetic resonance imaging using ALFF/fALFF and ReHo in schizophrenia patients compared to healthy controls (HCs) and then, anatomical/activation likelihood estimation was performed.
Results:
Eighteen eligible studies were included in this meta-analysis. Compared to the spontaneous brain activity of HCs, we found changes in spontaneous brain activity in Dn-FES based on these two methods, mainly including the frontal lobe, putamen, lateral globus pallidus, insula, cerebellum, and posterior cingulate cortex.
Conclusion
We found that widespread abnormalities of spontaneous brain activity occur in the early stages of the onset of schizophrenia and may provide a reference for the early intervention of schizophrenia.
3.Regional Homogeneity Brain Alterations in Schizophrenia: An Activation Likelihood Estimation Meta-Analysis

Objective:
Resting state functional magnetic resonance imaging (rsfMRI) provides a lot of evidence for local abnormal brain activity in schizophrenia, but the results are not consistent. Our aim is to find out the consistent abnormal brain regions of the patients with schizophrenia by using regional homogeneity (ReHo), and indirectly understand the degree of brain damage of the patients with drug-naive first episode schizophrenia (Dn-FES) and chronic schizophrenia.
Methods:
We performed the experiment by activation likelihood estimation (ALE) software to analysis the differences between people with schizophrenia group (all schizophrenia group and chronic schizophrenia group) and healthy controls.
Results:
Thirteen functional imaging studies were included in quantitative meta-analysis. All schizophrenia group showed decreased ReHo in bilateral precentral gyrus (PreCG) and left middle occipital gyrus (MOG), and increased ReHo in bilateral superior frontal gyrus (SFG) and right insula. Chronic schizophrenia group showed decreased ReHo in bilateral MOG, right fusiform gyrus, left PreCG, left cerebellum, right precuneus, left medial frontal gyrus and left anterior cingulate cortex (ACC). No significant increased brain areas were found in patients with chronic schizophrenia.
Conclusion
Our findings suggest that patients with chronic schizophrenia have more extensive brain damage than FES, which may contribute to our understanding of the progressive pathophysiology of schizophrenia.

5.Factors associated with postpartum weight retention based on the generalized estimation equation in Kaifu Districtin Changsha.

To investigate the current status of the postpartum weight retention (PPWR) in Kaifu District of Changsha, and explore the influential factors with PPWR based on the generalized estimating equation model.
Methods: A cluster sampling method was applied to select women who gave birth at the health service centers of 3 street communities in Kaifu District of Changsha during 2015. According to the inclusion and exclusion criteria, 783 cases were finally included in the study. Data on PPWR were collected prospectively by using the self-made questionnaire at 1, 3, 6 and 8 months after childbirth. Analysis were performed to investigate the associations between PPWR and its potential factors with generalized estimation equation model.
Results: The mean PPWR of women was gradually decreased with the increase of postpartum time, and 34.6% of them have returned to the pre-pregnancy weight in the 8 months postpartum. Our findings revealed that gestation weight gain (GWG), pre-pregnancy body mass index, feeding patterns, and delivery mode were significantly associated with maternal PPWR (P<0.05). In contrast, maternal age, educational level, per capita income of family, parity and postpartum depression were not contributed to PPWR (P>0.05).
Conclusion: GWG is one of the most important predictors for PPWR. The key to reducing PPWR is to control GWG systematically. Early targeted interventions and health education should be taken to prevent women from excessive PPWR at the first-year postpartum, in particular to the women who underwent cesarean deliveries and breastfed their infants. It is conducive to reduce the risks of overweight or obesity caused by PPWR.

6.Risk factors for delayed breastfeeding initiation based on decision tree model and logistic regression model

Objective:To determine influential factors for delayed breastfeeding initiation.Methods:Using questionnaires to collect the data of 950 pregnant women and infants' feeding practices in Changsha,Hunan,logistic regression and decision tree model were used to determine influential factors for delayed breastfeeding initiation.Results:Mothers with timely breastfeeding initiation were 594 (62.5%),and those with delayed breastfeeding initiation accounted for 356 (37.5%).Mothers with timely breastfeeding initiation were more likely to use exclusive breastfeeding than those with delayed breastfeeding initiation (P<0.05).Four risk factors were found by logistic regression model and decision tree model,including cesarean delivery,using feeding-bottle before breastfeeding initiation,falling ill,and vomiting during pregnancy.Receiver operating characteristic curve showed no significant difference in the predictive effect of the 2 models (Z=0.142,P=0.889).Conclusion:Cesarean delivery,using feeding-bottle before initiation of breastfeeding,and poor physical condition during pregnancy exert negatively effects on breastfeeding initiation.Health workers should help mother to timely initiate breastfeeding and enhance their confidence in exclusive breastfeeding.
7.Continuous Observation and Analysis of Female College Students' Menstrual Period Related to Pulse Condition

Objective: To explore pulse characteristic of the health female college students and their relationship with menstrual cycle. Methods: The subjects were 17 female college students of Jiangxi University of Traditional Chinese Medicine. Pulse test was performed with pulse bracelet. The detection time was every day in the wake and has not yet get up and wake up at any time before going to bed with a pulse-taking bracelet, and recorded the current menstrual cycle information. The pulses of the wake that has not yet get up and wake up at any time before going to bed took consistency analysis of the record with Kruskal Wallis Test. The pulse of menstruation stage analysis was compared with chi-square test. Results: The pulses of the wake that has not yet get up and wake up at any time before going to bed consistency was poor except rapid pulse (P> 0.05) . The occurrence frequency of slippery pulse and string-like pulse were 55.21% and26. 03% respectively. The pulses of premenstrual period, post menstrual period and menstrual period were not much different from others. The occurrence frequency of moderate pulse, string-like pulse, unsmooth pulse, slippery pulse in intermenstrual period respectively were 40.68%, 44.07%, 30.51%, and 40.68%. The occurrence frequency of moderate pulse, string-like pulse, unsmooth pulse, slippery pulse in-non intermenstrual period respectively were 17.38%, 24.69%, 10.45%, 56.30%. The difference was significant (P < 0.05) . Conclusion: The female college students are mainly in terms of slippery pulse. The occurrence frequency slippery pulse in intermenstrual period respectively are less common. The string-like pulse, moderate pulse, unsmooth pulse in intermenstrual period are more common than in-non intermenstrual period.
8.Influential factors for postpartum depression based on the structural equation model in Kaifu District in Changsha

Objective:To investigate the current status of the postpartum depression in Kaifu District in Changsha,and to explore the influential factors for postpartum depression.Methods:We selected the mothers,who had a delivery and met the inclusion criteria in Changsha during 2015,as the participants and established a prospective birth cohort study.The social demographic characteristics and health-related information for the mothers and their families were collected by using self-made questionnaire.The mothers' depression symptoms after one month postpartum were assessed by the Edinburgh Postnatal Depression Scale.We used the structural equation model to analyze the influential factors for postpartum depression.Results:A total of 961 cases were investigated.82 of them whose scores of EPDS were ≥ 9 points,and the incidence of PPD was 8.2%.The results of structural equation model revealed the influential factors and their correlations with postpartum depression.It has been shown that maternal age,maternal education level,per capita income of family,intention of pregnancy,pregnancy-related complications,mode of conception,mode of delivery,feeding patterns,birth weight,baby gender,and infant illness could exert effects on postpartum depression.Conclusion:The incidence of postpartum depression in Kaifu District is at a low level and the structural equation model can be applied to analyze the influential factors and their relations with postpartum depression.
9.The Simulation Experiment of Control System for Left Ventricular Assist Device

Objective To test the accuracy, the reliability and the stability offl ow-controling method of a new percutaneous left ventricular assist device.Methods With the AMEsim to set up a simulation model, analysis the variations of motor speed, flow and pressure by adjusting the size of valve. and we test the improved-theoretical model with outside experiment.Results In the simulation experiment, with the variations of pressure from A, the system can regulate the motor speed to maintain the pump outlet pressure 1.1×105 Pa~1.2×105 Pa and control thefl ow. With the B valve size changes, thefl ow-controling by the system identify and change the motor speed, to maintain the pump inlet pressure from-0.25×105 Pa to-0.10×105 Pa. In outside experiment, with the variations of the size of valve A and valve B, the pump can change the voltage by self-regulation, maintain the entrance and exit of pressure is relatively stable and control thefl ow.Conclusions It is a fast-responding and better robust way, by adding the pressure sensor to adjust the speed of motor, to control thefl ow accurately.
10.Is there a healthy obese subtype in cohort undergoing test for endothelial dysfunction ?

Objective To investigate whether there existed a healthy obese subtype.Methods A total of 116 healthy subjects were recruited.They were divided into 3 groups according to BMI and metabolic disorders:40 cases of normal weight and metabolic normality (NMN),36 cases of obesity and metabolic normality (OMN) and 40 cases of obesity and metabolic abnormality (OMA).Anthropometic parameters as height,weight,waist circumference,hip circumference and blood pressure was recorded.Blood glucose,lipids,insulin,high-sensitivity C-reactive protein (hs-CRP) was detected.Body fat distribution was detected by dual-energy X-ray absorptiometry (DXA).Serum von Willebrand factor (vWF),a marker of endothelial dysfunction,were detected by ELISA.Results Both serum vWF levels in OMN group [(733.6±86.2)U/L] and OMA group[(809.2 ±46.3)U/L] are higher than that in NMN group [(466.9 ±65.3)U/L,P <0.05] with serum vWF level in OMA group is higher than in OMN group (P < 0.05).Among android fat mass percentage (AFM%),BMI,waist height ratio,waist circumference,hs-CRP,weight,hip circumference and trunk fat mass,AFM%,BMI and hs-CRP are main influencing factors of vWF.Conclusions Endothelial dysfunction existed in obese adults regardless of their metabolic status.There is no healthy obese subtype.AFM%,BMI and hs-CRP are the main influencing factors of endothelial dysfunction.
