1.Construction and Verification of Prediction Model of Qi Deficiency and Blood Stasis Syndrome in Chronic Heart Failure

ObjectiveTo construct and validate a clinical prediction model for Qi deficiency and blood stasis syndrome in chronic heart failure (CHF),aiming to assist clinical diagnosis and provide tools and methods for individualized treatment of CHF. MethodsThe clinical data of patients with chronic heart failure treated at Dongzhimen Hospital of Beijing University of Chinese Medicine from January 2022 to January 2024 were retrospectively collected. The patients were randomly divided into a training group and a validation group with a ratio of 7∶3. First, the least absolute shrinkage and selection operator (LASSO) regression analysis was used to preliminarily screen the predictive factors affecting the diagnosis of Qi deficiency and blood stasis syndrome in CHF. Subsequently, the Logistic regression method was applied to conduct a more in-depth and detailed analysis of these factors. Variables with P<0.05 in the results of the multi-factor Logistic regression were carefully selected and included. Based on the regression coefficients obtained from this analysis, a model was constructed, and a nomogram was accurately drawn. Using R software,the receiver operating characteristic (ROC) curve,calibration curve,and decision curve analysis (DCA) were precisely drawn. These analyses were used to comprehensively evaluate the model from three crucial aspects: discrimination,calibration,and clinical applicability. Additionally, the accuracy,specificity,sensitivity,positive predictive value,and negative predictive value of the model were meticulously calculated to conduct a more all-round and comprehensive assessment. ResultsIn total, 168 cases were successfully obtained in the training group, and 71 cases were included in the validation group. After a thorough comparison, it was found that there were no statistically significant differences in the baseline data between the two groups. After being rigorously screened by the LASSO-multivariate logistic regression method, dark red tongue,smoking history,cardiac troponin I,and N-terminal pro-B-type natriuretic peptide (NT-ProBNP) were identified as the influencing factors for diagnosing patients with the Qi deficiency and blood stasis syndrome in CHF. The constructed model demonstrated an area under the curve (AUC) of 0.812 in the training group and 0.719 in the validation group. The calibration curve showed that the predicted curve of the model was close to the actual observed curve. DCA indicated that the model could provide substantial clinical benefits for patients at the decision thresholds ranging from 0.2 to 0.9. ConclusionThe clinical prediction model for Qi deficiency and blood stasis syndrome in chronic heart failure constructed in this study shows good performance. It has certain application value in clinical practice, which may contribute to the improvement of the diagnosis and treatment of CHF patients with this syndrome.
2.Clinical Efficacy of Jiuzi Huichun Decoction Combined with Spleen-strengthening Moxibustion on Asthenospermia Infertility with Spleen-kidney Deficiency Pattern

ObjectiveTo observe the effectiveness and safety of Jiuzihuichun decoction combined with spleen- strengthening moxibustion in patients with asthenospermia infertility with spleen-kidney deficiency pattern. MethodsA total of 82 patients with asthenospermia of spleen-kidney deficiency pattern in Shanghai Seventh People's Hospital were included. The patients were randomly divided into an observation group and a control group, with 41 patients in each group. The control group received oral administration of WuziYanzong pills combined with spleen-strengthening moxibustion. The dosage of Wuzi Yanzong pills was 1 bag each time, and it was taken twice a day. The spleen-strengthening moxibustion was carried out once a week. The observation group, on the other hand, took Jiuzi Huichun decoction orally combined with spleen-strengthening moxibustion. The Jiuzi Huichun Decoction was taken 200 mL each time, twice a day, with one dose in the morning and one in the evening. The spleen-strengthening moxibustion for the observation group was also performed once a week. The treatment course for both groups was 12 weeks, and they were followed up for an additional 12 weeks. During the treatment process,12 cases were either lost to follow-up or excluded. Eventually, 70 cases were available for evaluation,with 35 cases in the control group and 35 cases in the observation group. The pregnancy status of the patients' spouse within 6 months was recorded. The traditional Chinese medicine (TCM) syndrome scores of spleen-kidney deficiency pattern before and after treatment were evaluated. The semen volume,semen routine parameters,normal sperm morphology,sperm DNA fragmentation index,seminal plasma fructose,seminal plasma acid phosphatase, and seminal plasma α-glucosidase levels of the two groups were detected before and after treatment. In addition, the safety indicators related to liver and kidney functions of the two groups were detected before and after treatment. ResultsDuring the 6-month observation period, when compared with the situation before treatment in their respective groups,the semen volume of the observation group and the control group increased. In contrast, the sperm concentration,sperm motility,proportion of a+b-grade sperm,normal sperm morphology,seminal plasma fructose,seminal plasma acid phosphatase,seminal plasma α-Glucosidase,the proportion of a-grade sperm,linear sperm motility,linear sperm concentration, and linear sperm count all increased significantly(P<0.05). At the same time, the sperm DNA fragmentation index and the TCM syndrome scores of the spleen-kidney deficiency pattern decreased significantly(P<0.05). When the observation group was compared with the control group after treatment, the clinical efficacy of the observation group was better(Z=-2.276,P<0.05). The pregnancy rate of the observation group's spouses was 14.3%,which was higher than the 2.9% of the control group. The sperm motility, the proportion of a+b-grade sperm,seminal plasma fructose,the proportion of a-grade sperm,normal sperm morphology,α-glucosidase, and linear sperm motility in the observation group were higher than those in the control group (P<0.05). Moreover, the sperm DNA fragmentation index and the TCM syndrome scores of spleen-kidney deficiency pattern in the observation group were lower than those in the control group (P<0.01). No serious adverse reactions occurred in the two groups,and no abnormalities were found in the safety indicators after treatment. ConclusionJiuzi Huichun decoction combined with spleen-strengthening moxibustion can enhance sperm viability and sperm concentration. It can also improve the TCM-related symptoms of asthenospermia of spleen-kidney deficiency pattern and sperm morphology. Additionally, it can reduce the sperm DNA fragmentation index and regulate the level of seminal plasma bioenzyme in patients with male asthenospermia infertility of spleen-kidney deficiency pattern. Therefore, it is worthy of further promotion and application in clinical practice.
3.Modern Medicine Insights into Strengthening the Spleen and Resolving Dampness for Treating Spleen Deficiency with Dyslipidemia Based on the Theory of "the Spleen Governs the Dispersion of Essence"

The traditional theory of "the spleen governs the dispersion of essence" refers to the spleen's pivotal role in distributing refined nutrients throughout the body. In traditional Chinese medicine (TCM), lipids are categorized under "gaozhi (膏脂)", and their transportation and metabolism via apolipoproteins are believed to be closely related to the spleen's dispersing function. The liver, which synthesizes apolipoproteins, is functionally linked to the spleen system in TCM. Impaired dispersion of essence by the spleen and disrupted transportation of gaozhi constitute the pathological mechanism of dyslipidemia due to spleen deficiency. Strengthening the spleen and resolving dampness is the core therapeutic principle. From the perspective of modern medicine, this may involve promoting hepatic functional recovery related to lipid metabolism, thereby enhancing lipid processing and reducing the levels of abnormally accumulated lipids in the bloodstream.
4.Effects of wedelobata on apoptosis and secretion of inflammatory cytokines in the alveolar epithelium infected by Streptococcus pneumonia

Objective To investigate the regulatory effects of wedelobata on apoptosis and secretion of inflammatory factors in the alveolar epithelial cells infected by Streptococcus pn e um on i a e.Methods Alveolar epithelial cells A549 were divided into infection group(1×108/CFU/mL cultured cells of Streptococcus pneumoniae),control group(no treatment),infection+wedelolactone low-dose group,middle-dose group and high-dose group(pretreated with wedelolactone at 10,20 and 40 μmol/L and then cultured cells of Streptococcus pneumoniae at 1×108/CFU/mL).Alveolar epithelial cells A549 were divided into infection group(1×108/CFU/mL cultured cells of Streptococcus pneumoniae),control group(no treatment),infection+wedelolactone low-dose group,middle-dose group and high-dose group(pretreated with wedelolactone at 10,20 and 40 μmol/L and then cultured cells of Streptococcus pneumoniae at 1×108/CFU/mL).Results Compared with control group,the apoptosis rate,the relative expression levels of Bax,Caspase-3 protein,IL-6,IL-1β and TNF-α mRNA were higher in infection group,infection + wedelolide low dose group,medium dose group and high dose group,while the expression level of Bcl-2 protein was lower(P<0.05).Compared with the infected group,the apoptosis rate,the relative expression levels of Bax,Caspase-3 protein,IL-6,IL-1β and TNF-α mRNA and the expression levels of Bcl-2 protein were lower in the infected + wedelolide low dose,medium dose and high dose groups.Moreover,the apop-tosis rate,the expression levels of Bax,Caspase-3 protein,IL-6,IL-1β and TNF-α mRNA were the highest in the infected + wedelactone high-dose group,and the expression levels of Bcl-2 protein were the lowest(P<0.05).Conclusion The apoptosis rate of alveolar epithelial cells infected by Streptococcus pneumoniae decreased and the secretion of inflammatory factors decreased after the intervention of wedelia lactone.
5.Strategies for resolving difficulties in immunology teaching

Immunology courses are offered in the majors of biology,medicine,animal husbandry and food in Chinese colleges and universities.Due to the characteristics of multi-dimensional composition of immunology knowledge,most students are not easy to understand the logical relationship between key points and knowledge points in learning,and will encounter problems such as unable to keep up with the teaching pace,lack of confidence in learning,feeling complex association between the knowledge points of chap-ters,and not fully understanding the immunology knowledge points.This paper summarizes the teaching methods of heuristic,main line and case in Immunology teaching,so as to solve the difficulties in learning,clarify learning ideas,improve learning confidence and learning effectiveness.
6.Dynamic trajectory and cell communication of different cell clusters in malignant progression of glioblastoma

Objective:To delve deeply into the dynamic trajectories of cell subpopulations and the communication network among immune cell subgroups during the malignant progression of glioblastoma(GBM),and to endeavor to unearth key risk biomarkers in the GBM malignancy progression,so as to provide a more profound understanding for the treatment and prognosis of this disease by integrating tran-scriptomic data and clinical information of the GBM patients.Methods:Utilizing single-cell sequencing data analysis,we constructed a cell subgroup atlas during the malignant progression of GBM.The Mono-cle2 tool was employed to build dynamic progression trajectories of the tumor cell subgroups in GBM.Through gene enrichment analysis,we explored the biological processes enriched in genes that significant-ly changed with the malignancy progression of GBM tumor cell subpopulations.CellChat was used to identify the communication network between the different immune cell subgroups.Survival analysis helped in identifying risk molecular markers that impacted the patient prognosis during the malignant pro-gression of GBM.This methodological approach offered a comprehensive and detailed examination of the cellular and molecular dynamics within GBM,providing a robust framework for understanding the disease's progression and potential therapeutic targets.Results:The analysis of single-cell sequencing data identified 6 different cell types,including lymphocytes,pericytes,oligodendrocytes,macrophages,glioma cells,and microglia.The 27 151 cells in the single-cell dataset included 3 881 cells from the pa-tients with low-grade glioma(LGG),10 166 cells from the patients with newly diagnosed GBM,and 13 104 cells from the patients with recurrent glioma(rGBM).The pseudo-time analysis of the glioma cell subgroups indicated significant cellular heterogeneity during malignant progression.The cell interaction analysis of immune cell subgroups revealed the communication network among the different immune sub-groups in GBM malignancy,identifying 22 biologically significant ligand-receptor pairs across 12 key bio-logical pathways.Survival analysis had identified 8 genes related to the prognosis of the GBM patients,among which SERPINE1,COL6A1,SPP1,LTF,C1S,AEBP1,and SAA1L were high-risk genes in the GBM patients,and ABCC8 was low-risk genes in the GBM patients.These findings not only provided new theoretical bases for the treatment of GBM,but also offered fresh insights for the prognosis assessment and treatment decision-making for the GBM patients.Conclusion:This research comprehensively and pro-foundly reveals the dynamic changes in glioma cell subpopulations and the communication patterns among the immune cell subgroups during the malignant progression of GBM.These findings are of significant im-portance for understanding the complex biological processes of GBM,providing crucial new insights for precision medicine and treatment decisions in GBM.Through these studies,we hope to provide more ef-fective treatment options and more accurate prognostic assessments for the patients with GBM.
7.Prevalence and risk factors of cancer related muscle disorder in lung cancer patients

Objective:To Investigate the prevalence of cancer related muscle disorder (CRMD) in hospitalized lung cancer patients, and to identify the possible risk factors.Methods:This study was a cross-sectional study. The study enrolled 259 patients without missing data who met the inclusion and exclusion criteria from 347 lung cancer patients who were non-repeated hospitalized in the Department of Respiratory and Critical Care Medicine of Peking Union Medical College Hospital from March 2023 to August 2023. Bioimpedance analysis was used to measure muscle mass in patients, and dietary information was collected using the food frequency questionnaire. Disease information and laboratory test results were obtained by consulting Hospital Information System. According to AWGS 2019, patients were divided into five groups: possible sarcopenia (PS), sarcopenia (S), severe sarcopenia (SS), low muscle mass (LMM), and non-muscle disorder (non-MD). The analysis of variance, Kruskal-Wallis test, and χ 2 test were used to compare differences in baseline data, energy and macronutrient intake, and laboratory test indicators among different groups, and ordered logistic regression analysis was applied for multivariate analysis to identify the influencing factors of CRMD in lung cancer patients. Results:This study included 259 hospitalized lung cancer patients who met the inclusion and exclusion criteria. The prevalence of CRMD among 259 patients was 64.5%, PS was 27.4%, S was 13.5%, SS was 14.3%, and LMM was 5.4%. plant protein intake ( OR=0.969, 95% CI: 0.942-0.996) and regular exercise ( OR=0.485, 95% CI: 0.269-0.869) were found to be protective factors for CRMD in hospitalized lung cancer patients, while age ( OR=1.056, 95% CI: 1.013-1.101), weight loss of more than 5% in the last six months ( OR=4.546, 95% CI: 1.363-15.563), and diabetes ( OR=2.342, 95% CI: 1.137-4.866) were identified as risk factors. Conclusion:The prevalence of CRMD in hospitalized lung cancer patients is relatively high, and is closely related to age, weight changes, exercise, comorbidities, and dietary intake.
8.Impact of soy protein and whey protein on body composition in hospitalized lung cancer patients with cancer-related sarcopenia

Objective:This study aims to assess the effects of soy and whey protein on body composition and muscle function in hospitalized lung cancer patients with cancer-related sarcopenia.Methods:Paired comparison method was adopted for data collected before and after intervention. All enrolled non-small cell lung cancer patients with cancer-related sarcopenia were assigned to either the soy protein (30 g/d) or the whey protein (30 g/d) group to receive the 12-week intervention. Changes in muscle mass (MM) and muscle function before and after intervention were observed to assess intra-group and inter-group differences.Results:The study included 48 patients, 32 of whom completed the intervention and follow-up (16 from the whey protein group and 16 from the soy protein group). After 12 weeks of intervention with whey protein, significant increases were observed in trunk muscle mass (TMM, P=0.014) and TMM adjusted for height (TMM/Ht 2, P=0.011). After 12 weeks of intervention with soy protein, significant increases were noted in appendicular muscle mass (AMM, P=0.049), AMM adjusted for body mass index (AMM/BMI, P=0.044), fat free mass (FFM, P=0.041), and MM ( P=0.038). After adjustment using generalized estimating equations, only TMM/Ht 2 ( P=0.049) showed a significant increase in the whey protein group, while TMM ( P=0.040) and TMM/Ht 2 ( P=0.005) significantly increased in the soy protein group. Despite that all MM-related indices increased in both groups, there were no significant inter-group differences after adjusted by covariate analysis. Conclusions:Interventions with soy protein and whey protein can help maintain and improve muscle mass and muscle function in patients with lung cancer-related sarcopenia. Soy protein and whey protein may have comparable benefits in patients with lung cancer-related sarcopenia.
9.Vitexin exerts anti-prostate cancer effects by modulating macrophage polari-zation from M2 to M1

Objective:To investigate effect of vitexin on macrophage polarization and its impact on tumor growth in a mouse model of prostate cancer.Methods:C57BL/6J male mice were used to establish RM-1 prostate cancer xenograft model.Mice were ran-domly divided into model group,vitexin-low,medium and high doses groups(40,80,160 mg/kg),and cisplatin group as positive control.After continuous administration for 16 days,mice were euthanized and tumor mass was measured.HE staining was performed to observe tumor morphology.Immunohistochemistry was used to detect Ki67 positive rate.Flow cytometry was conducted to measure expressions of CD86+CD11b and CD206+CD11b in tumor-associated macrophages.CCK8 assay was performed to assess cytotoxic effect of vitexin on RAW264.7 macrophages to determine suitable concentrations.RT-qPCR was used to measure mRNA expressions of M2 macrophage markers,including arginase-1(ARG-1),Fizz1 and Ym1.Results:Vitexin inhibited tumor volume and weight,induced tumor tissue necrosis,suppressed Ki67 protein expression,increased expression of CD86+CD11b+M1 macrophages,and inhibited CD206+CD11b+M2 macrophage expression in mouse tumor tissues in vivo.Vitexin at concentrations of 10~20 μmol/L showed no cyto-toxicity on RAW264.7 macrophages in vitro,and promoted expression of iNOS in IL-4-induced M2 macrophages while inhibiting CD206 expression,as well as suppressed mRNA expressions of ARG-1,Fizz1 and Ym1.Conclusion:Vitexin effectively inhibits tumor growth in a mouse model of prostate cancer,possibly by regulating M2 macrophages towards an M1 phenotype and exerting immunomodulatory effects.
10.Influences of the copy number of SMN2 and transcript level of fl-SMN2 on the phenotype and survival of spinal muscular atrophy

Objective:To explore the distribution of the copy number of survival motor neuron gene 2 ( SMN2) and the transcript level of the full-length SMN2 ( fl-SMN2) transcript level in patients with type 1-3 spinal muscular atrophy (SMA), and to evaluate their influences on disease severity, progression, and prognosis. Methods:It was a retrospective study involving 78 therapy-naive SMA patients with SMN1 gene homozygous deletion who were diagnosed and treated in the Capital Institute of Pediatrics from January 2019 to December 2021.Cross-sectional clinical data, including age at onset, motor milestones, and complications were recorded.They were followed up for monitoring motor function degeneration and survival.The copy number of SMN2 and the transcript level of fl-SMN2 were detected.Differences between groups were compared by the Student′s t-test or One- Way ANOVA or Chi- square test.Kaplan-Meier analysis was used for survival analysis, and Kendall′ s tau- c was performed to assess the correlation of these two biomarkers with SMA phenotypes, age at onset, motor milestones, and survival. Results:Of the 78 SMA patients, there were 17 cases (21.8%) of type 1, 34 cases(43.6%) of type 2, and 27 cases(34.6%) of type 3.Seven cases(41.2%) type 1 SMA patients died, with a median survival time of 11 months, and no deaths were observed in type 2 and type 3 SMA patients.There was a significant difference in the median age at onset among SMA patients with 2, 3, and 4 copies of SMN2 (1.8, 12.0, and 24.0 months, respectively; F=4.943, P=0.01). The mean transcript level of fl-SMN2 in type 1, 2 and 3 SMA patients were 196.25±68.79, 331.21±108.79 and 455.69±122.27, respectively ( F=37.154, P<0.001). The survival rate of SMA with 2 SMN2 copies at 1, 2, and 5 years were 50.5%, 0, and 0, respectively, and their median survival age was 7 months.The survival rate of SMA with 3 and 4 SMN2 copies at 5 years were 97.4% and 100.0%, respectively.Moreover, a negative correlation was observed between the transcript level of fl-SMN2 and phenotype severity ( Kendall′ s tau- c=-0.444, P<0.001), and the transcript level of fl-SMN2 of the survival group was much higher than that of the death group (342.93±125.74 vs.212.14±92.31). More copies of SMN2 and higher transcript level of fl- SMN2 indicated more motor function acquisitions (head control, sitting and walking) ( P<0.001). In addition, there was a significant difference in the transcription level of fl-SMN2 between the undegenerated group and the degenerated group in sitting and standing ( F=5.432, P=0.023 and F=4.315, P=0.047, respectively). Conclusions:Both the copy number of SMN2 and the transcript level of fl-SMN2 are correlated with SMA severity, survival, and motor milestones, serving as valuable biomarkers for evaluating phenotypic severity of SMA.The transcript level of fl-SMN2 s may play an important role in the degeneration of sitting and standing.
