1.Progress on pravastatin and its safety in prevention and treatment of preeclampsia

		                        			Preeclampsia is the main cause of poor maternal-fetal outcomes. A series of cell and animal experiments, and a small number of clinical studies have shown that pravastatin can prevent and treat preeclampsia by regulating angiogenesis, increasing the expression of heme oxygenase, and stimulating the production of nitric oxide without any reported adverse effects during pregnancy. 2.Effect of the operation process query system on alleviating the psychological anxiety of the family members of the surgical patients

		                        			Objective:To explore the effect of the operation process query system and the psychological impact on the surgical patient's family members.Methods:From April to October 2019, 198 family members of patients whose operation time> 1 hour were selected from the Shanghai East Hospital, Tongji University. The patients were divided into a control group and an observation group according to the order of operation, with 99 cases in each group. Both groups implemented preoperative visits, preoperative assessments, and preoperative education. The control group performed routine nursing of Operating Room and did not specifically inform family members of the information about the operation process during the operation. The family members of the observation group could view information about the operation process in the operation progress query system. The State-Trait Anxiety Inventory (S-AI) was used to assess the anxiety of the family members of the two groups of patients, and we compared the changes of pulse, respiration and blood pressure before and during the operation of the two groups of family members, and compared the satisfaction of the family members of the two groups after the operation.Results:The anxiety scores and vital signs of the family members of the two groups of patients during the operation were higher than those before the operation. The family members of the observation group had an intraoperative anxiety score of (49.01±7.91) , pulse (92.97±8.76) times/min, respiration (20.13±1.64) times/min, systolic blood pressure (122.06±11.69) mmHg, diastolic blood pressure (82.86±6.45) mmHg, all lower than those of the control group, the differences were statistically significant ( t=20.494, 6.749, 26.784, 4.387, 8.139; P<0.01) . The total satisfaction of the family members of the observation group after the operation was 95.96% (95/99) , and that of the control group was 63.64% (63/99) , and the difference was statistically significant (χ 2=34.932, P<0.01) . Conclusions:The application of the operation query system can effectively alleviate the psychological anxiety of the family members of the surgical patients and improve the satisfaction of the family members of the patients with the operation.
		                        		
		                        		
		                        		
		                        	
3.Application of antigen-free porcine peritoneum as carrier of autologous microskin grafting in patients with extensive burn

		                        			Objective:To investigate the effects of microskin transplantation with antigen-free porcine peritoneum (AFPP) as substitutive carrier for allogeneic skin graft in treating patients with extensive deep burns.Methods:Medical records of 32 patients with extensive deep burns, hospitalized in Changhai Hospital of Naval Medical University meeting the inclusion criteria were investigated from January 2014 to December 2017. Twenty patients [12 males and 8 females, aged (35.4±2.2) years]with microskin transplantation using allogeneic skin graft as microskin carrier were included in allogeneic skin graft group and 12 patients [6 males and 6 females, aged (32.1±4.8) years] with microskin transplantation using AFPP as microskin carrier were included in AFPP group. On post injury day 3-7, the vital signs of patients were stable and escharectomy and autologous microskin grafting of head were performed. The expansion ratio of microskin, the application time of albumin and antibiotics, the percentage of infectious autologous microskin grafting area, the survival rate of microskin, and dressing change times of patients in 2 groups were recorded. Data were statistically analyzed with t test. Results:The expansion ratio of microskin of patient in AFPP group was 14.8±0.6, which was close to 13.5±0.6 of allogeneic skin graft group ( t=1.531, P>0.05). The application time of albumin and antibiotics of patients in AFPP group were close to those of allogeneic skin graft group ( t=0.027, 1.121, P>0.05). The percentage of infectious autologous microskin grafting area of patients in AFPP group was (8.5±1.2)%, which was significantly lower than (18.1±0.6)% in allogeneic skin graft group in 4 weeks after surgery( t=7.593, P<0.01), the survival rate of microskin of patients in AFPP group was (82.5±1.1)%, which was significantly higher than (72.5±0.6)% in allogeneic skin graft group ( t=8.689, P<0.01). The dressing change time of patients in AFPP group was significantly less than that in allogeneic skin graft group ( t=4.743, P<0.01). Conclusions:Compared with allogeneic skin graft, microskin transplantation with AFPP as carrier can reduce wound infection, improve the survival rate of microskin graft, and reduce dressing change time, so that AFPP is a good carrier of microskin.
		                        		
		                        		
		                        		
		                        	
4.One case of atypical septic shock with acute pulmonary edema in a patient with extensive burn

		                        			A 25-year-old man with extensive burn due to industrial dust explosion was admitted to the First Affiliated Hospital of Naval Medical University on 16th October, 2018. Four days after the first skin grafting and vacuum sealing drainage surgery, the patient developed signs of uncontrolled severe inflammation and shock. However, several atypical manifestations interfered the diagnosis of septic shock. After giving emergency treatment including fluid resuscitation, broad-spectrum antibiotics, and administration of vasopressor agents, the patient′s condition was alleviated, but quickly relapsed and deteriorated, with acute pulmonary edema appeared in the evening of the same day. Finally, the condition was reversed by completely removing the negative pressure devices on upper limbs and thorough dressing change. This case suggests that the diagnosis and treatment of infection in patients with extensive burn need comprehensive analysis. Timely intervention of the wound is the key to control the exacerbation of sepsis. 5.Over-expression of ROBO4 reduces permeability of human renal glomerular endothelial cells in high glucose medium

		                        			Objective To investigate the effect of over-expression of ROBO4 on permeability of human renal glomerular endothelial cells (HRGECs) in high glucose medium.Methods HRGECs infected with recombinant lentiviral vector ROBO4 were cultured in high glucose or low glucose medium in vitro.The protein levels of ROBO4 and ARF6 in each group were detected by Western blotting.The endothelial permeability was measured by the effiux of fluorescein isothiocyanate-dextran (FITC-Dextran)permeated through the monolayer endothelial cells using Transwell cell model system.The cell viability after lentivirus transfection was measured by CCK8 assay.Results The transfection rate of lentiviruses in HRGECs reached 80% 72h after,and obvious overexpression of ROBO4 protein was in transformed cells compared with the empty vector group (P<0.05).The lentivirus-mediated ROBO4 transfection did not affect cell viability of HRGECs.Compared with the low glucose group,the expression of ROBO4 increased obviously after 12h,but declined after 24h (P<0.05),and reached to minimun after 72h (P<0.05).On the contrary,the expression of ARF6 increased after 12h,and the increase reached to the maximum after 72h (P<0.05).Furthermore,the vascular permeability increased gradually after 24h,and reached to the maximum after 72h (P<0.05) in high glucose group.Compared with the empty vector group,the over-expression of ROBO4 inhibited the expression of ARF6 significantly,and the FITC-Dextran permeability reduced obviously.Conclusion Over-expression of ROBO4 may significantly enhance the barrier functions of HRGEC in high glucose medium,and ROBO4 activation may be a potential therapeutic approach in diabetic nephropathy.

		                        			Objective:To detect the expression of tumor necrosis factor alpha in (TNF alpha) breast cacner and its relationship with imaging features.Methods:Using immunofluorescence and flow cytometry to detect the expression of TNF-α in MDA-MB-231 and MCF-7.Collect 82 patients with mammary gland disease,which was confirmed by pathological tissue,its pathological data,imaging data,and by immunohistochemistry to detect the expression of TNF-α in breast tissues,and analyze and the relationship between its expression and the pathological features and imaging characteristics.Results:TNF-α high expression in MDA-MB-231,the expression of TNF-oα in malignant breast tumor tissue significantly higher than that in benign tumor,the expression quantity associated with lymph node metastasis,TNM stages,strengthen uniform in MRI,the boundary and the shape of the X-ray Mammography (P=0.01),and color flow signal strength in ultrasound (P<0.05).Conclusions:TNF alpha in breast tumor tissue was unusually high expression,and is closely related to some of the imaging features of breast tumor.
		                        		
		                        		
		                        		
		                        	
7.Effects of hemodialysis combined with hemoperfusion on expression of β2-microglobulin and high-sensitivity C-reactive protein in patients with uremia

		                        			Objective To explore the effects of hemodialysis combined with hemoperfusion on the expression of β2-microglobulin (β2-MG) and high-sensitivity C-reactive protein (hs-CRP) in patients with uremia.Methods A total of 50 patients with uremia were randomly divided into control group and observation group.The control group was treated by single hemodialysis,while the observation group was treated by hemodialysis combined with hemoperfusion.After the first treatment and 2 months after treatment,the levels of β2-MG and hs-CRP were detected.Results After the first treatment and 2 months after treatment,the levels of β2-MG and hs-CRP in the observation group were significantly lower than those before treatment,and were significantly lower than those in the control group (P < 0.05).Conclusion Hemodialysis combined with hemoperfusion can reduce the levels of β2-MG and hs-CRP in patients with uremia.

		                        			Estrogen receptors (ERs) are members of nuclear receptors and related to several diseases such as cancer, inflammation and osteoporosis. ERs have two forms, ERα and ERβ, which have different functions and organism distributions. Compounds selectively targeting ERβ can regulate important physiological functions and avoid the side effects caused by targeting ERα. Therefore, selective ERβ ligands have received considerable research interest in recent years. 10.Application of inhaling 50%nitrous oxide on burn pediatric patients during and after dressing change

		                        			Objective To investigate the analgesic effects of inhaling 50% nitrous oxide on burn pediatric patients during and after dressing change. Methods A total of 120 burn pediatric patients received outpatient dressing and hospitalized from September 2012 to September 2014 were enrolled in our study, and they were all in accordance with the inclusion criteria. The 120 pediatric patients were divided into control group (n=30) treated with inhalation of oxygen during dressing change) and treatment group (n=90) treated with inhalation of 50% nitrous oxide during dressing change ) according to the computer-generated list of random number. The other treatments in control group and treatment group were the same. Before, during and after dressing change, degree of pain, heart rate, systolic blood pressure, diastolic blood pressure, oxygen saturation and adverse effects were observed at the same time points. Data were processed with analysis of chi-square test, covariance and Student′s t test. Results There were no significant differences between the two groups in levels of HR, SBP, DBP, and SpO2 before dressing change (t=0. 34, 0. 57, 0. 11, 0. 98, respectively;P>0. 05). Compared with those of control group, levels of HR, SBP, DBP, and SpO2 in treatment group were significantly ameliorated during dressing change (t=25. 96, 24. 11, 8. 37, 20. 29, respectively;P<0. 01). After dressing change, the levels of DBP in the two groups were close (t=1. 57,P>0. 05), but the levels of HR, SBP, and SpO2 showed statistical differences (t=5. 20, 8. 64, 3. 37, respectively;P<0. 01). Before dressing change, the pain scores were approximate between control group and treatment group (t=0. 18,P>0. 05). Compared with those in control group, the pain scores in treatment group during and after dressing change were (2. 82 ± 0.8) and (1.2 ±0. 84), which were significantly lower than those in the control group (t =23. 00, 4. 30, respectively;P<0. 01). There were no obvious adverse effects in two groups during and after dressing change. The results of the covariance analysis of pain scores during dressing change showed statistical differences ( F=867. 956,P <0. 01).
