1.Slicing Angle Recognition of Fritillariae Thunbergii Bulbus Based on Improved YOLOv7-tiny Algorithm

ObjectiveTo realize the automatic recognition of the slicing angles of Fritillariae Thunbergii Bulbus (FTB) based on the improved YOLOv7-tiny algorithm. MethodFirstly, a diverse dataset of FTB images, totaling 16 000 pictures, with various angles was constructed. Furthermore, improvements were made to YOLOv7-tiny by replacing standard convolutions with ghost convolution (GhostConv), incorporating the coordinate attention (CA) mechanism as a preferred addition, substituting some activation functions with HardSwish function for decreasing the floating point operations. Additionally, a penalty term for angle recognition error was integrated into the loss function, and modifications were made to the non-maximum suppression (NMS) strategy to address cases where multiple detection results were associated with the same target. In order to verify the effectiveness of different improvement points on the optimization of the algorithm model, ablation experiments were carried out on all the improvement points, and the effectiveness of the improvement points was proved by comparing the prediction results before and after the addition of a certain improvement point on the basis of the original model or the model with the addition of an improvement point that has been verified to be effective, in order to evaluate the improvement of the indexes. ResultThe number of parameters required for the improved slicing angle recognition algorithm of FTB was about 55.4% of the original algorithm, and the amount of computation was about 59.4% of the original algorithm. The mAP@0.5[mean average precision at an intersection over union(IoU) of 0.5] increased by 12.2%, the mean absolute error(MAE) of the recognized angle was 5.02°, representing a reduction of 4.58° compared to the original algorithm. In the experimental environment of this paper, the average recognition time per image was as low as 8.7 ms, significantly faster than the average human reaction time. ConclusionThis study, by utilizing the improved YOLOv7-tiny algorithm, achieves effective slicing angle recognition of FTB with high accuracy and more lightweight, which provides a novel approach for stable and precise automated slicing of FTB, thereby providing valuable insights into the automation of processing other traditional Chinese medicines.
2.Recognition of herpes zoster ophthalmicus from the diversity and complexity of clinical manifestations

Herpes zoster ophthalmicus(HZO)is caused by the involvement of the ophthalmic division trigeminal nerve after reactivation of varicella-zoster virus(VZV)latent in the trigeminal ganglia, which usually occurs in the elderly and people with low immune function. The clinical manifestations of HZO are complex and diverse, which show not only keratoconjunctivitis and uveitis, but also retinal necrosis, optic neuropathy, and rare central nervous system lesions. Some cases do not have typical skin lesions, which can easily lead to missed diagnosis, misdiagnosis, and mistreatment, seriously affecting People's life quality. This article aimed to evaluate the clinical manifestations of HZO cases reported in the literature in recent years and comprehensively understand their diversity and complexity to better diagnose and treat the disease. This study also aimed to improve the diagnosis and cure rates of the disease, reduce the maximum number of visual damage, and provide more evidence for the precise diagnosis and treatment of HZO.

The paper summarizes the imaging evaluation methods for assessing the degree of carotid artery stenosis and analyzes the unique advantages and limitations of various imaging techniques in vascular imaging based on existing guidelines and consensus.The paper focuses on reviewing the clinical applications of several novel ultrasound technologies,including the use of advanced hemodynamic parameters such as blood flow dispersion(Tur index)and wall shear stress(WSS).Carotid artery stenosis is closely associated with cardiovascular disease.Although non-invasive and radiation-free ultrasound technology has certain limitations in diagnostic accuracy to a certain extent,with the continuous emergence of advanced functions such as ultrasound hemodynamics and vascular elasticity,the combination of multi-modality and multi-parameter ultrasound is expected to become an important method for efficient diagnosis of arterial stenosis in the future.

In order to further improve the efficiency, based on the first edition, the second edition guideline was composed by incorporating experience of the past six years in conjunction with the latest international and domestic guidelines.
5.Expression and clinical significance of lncRNA NR2F2-AS1 and miR-129-5p in glioma

Objective:To detect the expression level of long non-coding RNA (lncRNA) nuclear receptor subfamily 2 group F member 2-antisense RNA 1 (NR2F2-AS1) and microRNA-129-5p (miR-129-5p) in glioma tissues and to explore its clinical significance.Methods:The glioma tissues of 103 patients with glioma who underwent surgical treatment in the neurosurgery department of Shanxi Provincial People′s Hospital from January 2015 to September 2016 and 50 normal brain tissues removed due to craniocerebral surgery in the same period were selected as the research objects. Real time fluorescence quantitative polymerase chain reaction (qRT-PCR) was used to detect the expression levels of lncRNA NR2F2-AS1 and miR-129-5p in tissue samples. The correlation between the two indexes and the relationship between the expression changes of the two and clinicopathological parameters of glioma patients were analyzed. Kaplan Meier curve was used to analyze the 5-year cumulative survival rate of glioma patients with different expression levels of lncRNA NR2F2-AS1 and miR-129-5p. Cox regression analysis was used to analyze the factors affecting the poor prognosis of glioma patients.Results:Compared with normal brain tissue, the relative expression of lncRNA NR2F2-AS1 in glioma was higher and the relative expression of miR-129-5p was lower (all P<0.05). There was significant negative relationship between the relative expression of lncRNA NR2F2-AS1 and miR-129-5p in glioma ( r=-0.756, P<0.05). The expression of lncRNA NR2F2-AS1 and miR-129-5p in glioma was related to World Health Organization (WHO) grade and tumor length (all P<0.05). The 5-year cumulative survival rate of patients in lncRNA NR2F2-AS1<2.89 group was higher than that in lncRNA NR2F2-AS1≥2.89 group, and the 5-year cumulative survival rate of patients in miR-129-5p<0.55 group was lower than that in miR-129-5p≥0.55 group (all P<0.05). Multivariate analysis showed that high expression of lncRNA NR2F2-AS1, low expression of miR-129-5p, WHO grade Ⅲ-Ⅳ and tumor length were the risk factors affecting the prognosis of patients with glioma (all P<0.05). Conclusions:The increased expression of lncRNA NR2F2-AS1 and the decreased expression of miR-129-5p in glioma tissues are involved in the clinical biological progress of glioma and are closely related to the prognosis of patients.
6.Application of parameter assignment method in the review of parenteral nutrition prescription in our hospital

OBJECTIVE To provide reference for improving the working efficiency of prescription review for pharmacists and ensuring the safe use of parenteral nutrition solution in patients. METHODS The review module of parenteral nutrition prescription was developed with the pharmacy intravenous admixture services(PIVAS) MATE system. According to the instructions and relevant information of parenteral nutrition solution, the parameters to the review module for parenteral nutrition solution prescriptions were assigned, thresholds and calculation formulas were set, and the prescription was automatically determined by the system. The type and quantity of the inappropriate prescriptions of parenteral nutrition solution in our hospital from January to August 2020 and from January to August 2022 were counted, and the review efficiency of inappropriate prescriptions was compared before and after the application of the review module of parenteral nutrition solution prescription. RESULTS The module realized the informatized review of basic attribute parameters and custom parameters of parenteral nutrition solution prescription, such as ion concentration, ratio of heat to nitrogen, ratio of sugar to lipid, glucose concentration, ratio of sugar to insulin, maximum dosage of compound preparation, necessary drug collocation, incompatibility, repeated drug use, age limit, etc. The module also shortened the review time greatly. It took about 33 minutes for two pharmacists to review 100 prescriptions, and only took 6 seconds after the module went online. Compared with January to August in 2020, the average approval rate of inappropriate prescriptions for parenteral nutrition solution increased from 0.582% to 1.416% during the corresponding period in 2022 (P<0.05). CONCLUSIONS The review module of parenteral nutrition solution prescription established by parameter assignment method in our hospital can greatly reduce the phenomenon of missed and wrong review by pharmacists, reduce the risk of parenteral nutrition solution dispensing, reduces the work pressure of pharmacists reviewing prescriptions, and ensures the drug safety of patients.
7.Effect of Xingnaojing injection on rifampicin concentration in cerebrospinal fluid and prognosis of patients with severe tuberculous meningitis

Objective: To observe whether an Xingnaojing 醒脑静 injection could improve the prognosis of patients,
by increasing rifampicin penetration through the blood-brain barrier. Methods: Patients with severe
tuberculous meningitis were enrolled in this study. The concentrations of Xingnaojing in cerebrospinal
fluid and blood in patients treated with Xingnaojing and control were determined by high performance
liquid chromatography. The changes in cerebrospinal fluid and the improvement of clinical symptoms
and signs, were evaluated two weeks after admission. The long-term prognosis of the patients in the
two groups were evaluated by the Glasgow Outcome Scale (GOS). Results: The concentration of
rifampicin in cerebrospinal fluid was significantly higher in the Xingnaojing group (1.77±0.17 μg/mL),
than in the control group (1.27±0.16 μg/mL, p<0.05). The difference in concentration of rifampicin
in the blood was not significant (P>0.05). The short-term effective rate of the Xingnaojing group was
92.5% (37/40), which was significantly higher than that of the control group (80%, 32/40, p<0.05).
After 6 months, 75% (30/40) of the Xingnaojing group had good prognosis according to the GOS
score, whereas that of the control group was 50% (20/40) showing significantly better long-term
treatment effect of the Xingnaojing group compared to the control group (P<0.05).
Conclusion: Xingnaojing injection improved rifampicin penetration into the central nervous system.
The increase in rifampicin concentration in cerebrospinal fluid improved outcomes in patients with
severe tuberculous meningitis.
8.Validation and comparison of risk prediction models in patients with cardiogenic shock complicating acute myocardial infarction

Objective:To externally validated the intra-aortic balloon pump (IABP) shockⅡ score and CardShock score for predicting in-hospital mortality in patients with acute myocardial infarction complicated by cardiogenic shock (AMI-CS) and compared them with the Acute Physiology and Chronic Health EvaluationⅡ (APACHEⅡ) score.Methods:According to the inclusion and exclusion criteria, patients admitted to the cardiac care center (CCU) of our center from December 2010 to May 2019 were enrolled in this study. Patients’ baseline characteristics, in-hospital interventions, and outcomes were collected. The APACHEⅡ score was calculated during hospitalization by clinicians and collected by researchers. Two researchers independently calculated the IABP-ShockⅡ score and CardShock score; any disagreement was discussed with the third researcher. The performance of risk scores was evaluated by discrimination and calibration. The discriminative ability of risk scores was evaluated using the area under the receiver operating characteristic curve (AUC) and compared by the Delong method. The calibration of these risk scores was examined by the Hosmer-Lemeshow goodness-of-fit test. The calibration plot was also built.Results:A total of 150 patients enrolled in our study, and the in-hospital mortality was 60%. According to the IABP-ShockⅡ score, patients scored as low risk (0-2), moderate risk (3-4), and high risk (5-9) had in-hospital mortality of 29%, 68%, and 80%, respectively. According to the CardShock score, patients scored as low risk (0-3), moderate risk (4-5), and high risk (6-9)had in-hospital mortality of 21%, 57%, and 82%, respectively. According to the APACHEⅡ score, patients scored<20, 20-30, and >30 had in-hospital mortality of 19%, 69%, and 93%, respectively. For predicting the in-hospital mortality, the APACHEⅡ score demonstrated excellent discrimination (AUC=0.90, 95% CI: 0.84-0.95). The IABP-ShockⅡ score and CardShock score showed good discrimination (AUC=0.76, 95% CI: 0.68-0.83 and AUC=0.79, 95% CI: 0.72-0.85). The discriminative ability did not significantly differ between the IABP-ShockⅡ score and the CardShock score (0.76 vs 0.79, P>0.05), but both of which were significantly lower than the APACHEⅡ score (0.76 vs 0.90, P<0.05, and 0.79 vs 0.90, P<0.05). At the same time, it was not significantly different between the IABP-ShockⅡ score and the CardShock score (0.76 vs 0.79, P>0.05). All of these three scores were adequately calibrated according to the Hosmer-Lemeshow goodness-of-fit test ( P>0.05).The calibration plot showed accurate calibration of these three scores. Conclusions:Although less accurate than the APACHEⅡ score, the IABP-ShockⅡ score and CardShock score can show accurate prediction for in-hospital mortality of AMI-CS patients.
9.Analysis of the whole genome characteristics of 9 hepatitis A virus strains in China

Objective:To analyze the characteristics of the whole genome of hepatitis A virus (HAV) strains from Nanchong, Sichuan and Hetian, Xinjiang.Methods:Serum samples of 9 patients with acute hepatitis A were selected for HAV whole genome sequencing. Among them, 3 were from the same outbreak in Nanchong, Sichuan (2006) and 6 were from Hetian, Xinjiang(2016). Nine full length HAV sequences were obtained by viral RNA extraction, reverse transcription, nested PCR and sequencing. Phylogenetic analysis, neutralizing epitope sites, recombination and natural selection analysis were also performed.Results:Genotyping of HAV VP1-2A region indicated that the 9 HAV sequences all belonged to sub-genotype IA and were divided into four different branches, with 3 Nanchong sequences and 3 Hetian sequences in the same branch. While the whole genome sequence showed that the nucleotide and amino acid homology of 3 Nanchong sequences were 99.99%~100% and 100%, respectively. The difference of nucleotides and amino acids were 0.50%~0.57% and 0.08%~0.17% between 3 Nanchong and 3 Hetian sequences, respectively. The nucleotide and amino acid identities of nine HAV genomes were 98.29%~100% and 99.62%~100%, which was closely related to hd9 in China and MNA12-001 in Mongolia (nucleotide and amino acid homology: with hd9 98.60%~99.50% and 99.75%~99.92%; and with MNA12-001 98.45%~99.32% and 99.71%~99.87%, respectively). The 9 HAV sequences did not change at the neutralizing epitope sites, and no recombination events were found. Natural selection analysis indicated that the HAV coding region was subjected to a negative selection.Conclusions:There are many HAV epidemic strains in China; the 9 HAV amino acid sequences were conserved at the major neutralizing epitope sites.
10.Diagnostic value of contrast-enhanced ultrasound in breast precancerous lesions

Objective To evaluate the diagnostic value of contrast-enhanced ultrasound in breast precancerous lesions . Methods Retrospectively analyzed the contrast-enhanced ultrasound model and angiographic predictive model of 465 cases of the A prospective multicenter study of breast nodules contrast-enhanced ultrasound" that led the Sichuan Provincial People′s Hospital from January 2016 to April 2017 ,which included 69 cases of breast precancerous lesions and 396 other types benign lesions ,and the sensitivity ,specificity and accuracy of the diagnosis of breast precancerous lesions were calculated . Results The sensitivity of ultrasound predictive model for the diagnosis of precancerous lesions was 60 .9% and AUC was 0 .681 . Precancerous lesions mainly showed non-concentricity , increased homogeneity , and increased lesions;other types of benign lesions mainly showed non-centripetal ,high uniformity enhancement and lesion size unchanged . Conclusions Contrast-enhanced ultrasound shows a potential value in the differential diagnosis of precancerous lesions and other types of benign lesions ,that can help clinicians to take early intervention measures for breast precancerous lesions ,but there are still many problems to be solved .
