Objective To explore the expression of serum miR-410-3p in patients with rheumatoid arthritis(RA)and its relationship with knee soft tissue lesions.Methods A total of 89 RA patients admitted to our hospital were selected and divided into the active group(42 cases)and the remission group(47 cases)according to disease activity score in 28 joints(DAS28).In addition,52 healthy volunteers underwent physical examination during the same period in our hospital were selected as the healthy group.The expression level of serum miR-410-3p was detected by RT-PCR,the lesions of knee soft tissue was examined by ultrasound,and the relationship between the expression of serum miR-410-3p and knee soft tissue lesions was analyzed by Pearson.Results The expression levels of serum miR-410-3p of patients in the active group and the remission group were lower than that in the healthy group(P<0.05),and the expression level of serum miR-410-3p of patients in the active group was lower than that in the remission group(P<0.05).The cartilage thicknesses of medial and lateral ankle of patients in the active group and the remission group were smaller than those in the healthy group(P<0.05),and the above indexes in the active group were smaller than those in the remission group(P<0.05).The depths of suprapatellar bursa fluid and synovial thicknesses of patients in the active group and the remission group were greater than those in the healthy group(P<0.05),and the depth of suprapatellar bursa fluid and synovial thickness of patients in the active group were greater than those in the remission group(P<0.05).The level of serum miR-410-3p in RA patients was positively correlated with the depth of suprapatellar bursa fluid and synovial thickness(P<0.05),and negatively correlated with the cartilage thicknesses of medial and lateral ankle(P<0.05).Conclusion Serum miR-410-3p expression level in RA patients is decreased,which was closely related to knee soft tissue lesions,detecting the changes of serum miR-410-3p level may provide a reference for the evaluation of knee soft tissue lesions.

With the increasing proportion of human papilloma virus(HPV)infection in the pathogenic factors of oro-pharyngeal cancer,a series of changes have occurred in the surgical treatment.While the treatment mode has been im-proved,there are still many problems,including the inconsistency between diagnosis and treatment modes,the lack of popularization of reconstruction technology,the imperfect post-treatment rehabilitation system,and the lack of effective preventive measures.Especially in terms of treatment mode for early oropharyngeal cancer,there is no unified conclu-sion whether it is surgery alone or radiotherapy alone,and whether robotic minimally invasive surgery has better func-tional protection than radiotherapy.For advanced oropharyngeal cancer,there is greater controversy over the treatment mode.It is still unclear whether to adopt a non-surgical treatment mode of synchronous chemoradiotherapy or induction chemotherapy combined with synchronous chemoradiotherapy,or a treatment mode of surgery combined with postopera-tive chemoradiotherapy.In order to standardize the surgical treatment of oropharyngeal cancer in China and clarify the indications for surgical treatment of oropharyngeal cancer,this expert consensus,based on the characteristics and treat-ment status of oropharyngeal cancer in China and combined with the international latest theories and practices,forms consensus opinions in multiple aspects of preoperative evaluation,surgical indication determination,primary tumor re-section,neck lymph node dissection,postoperative defect repair,postoperative complication management prognosis and follow-up of oropharyngeal cancer patients.The key points include:① Before the treatment of oropharyngeal cancer,the expression of P16 protein should be detected to clarify HPV status;② Perform enhanced magnetic resonance imaging of the maxillofacial region before surgery to evaluate the invasion of oropharyngeal cancer and guide precise surgical resec-tion of oropharyngeal cancer.Evaluating mouth opening and airway status is crucial for surgical approach decisions and postoperative risk prediction;③ For oropharyngeal cancer patients who have to undergo major surgery and cannot eat for one to two months,it is recommended to undergo percutaneous endoscopic gastrostomy before surgery to effectively improve their nutritional intake during treatment;④ Early-stage oropharyngeal cancer patients may opt for either sur-gery alone or radiation therapy alone.For intermediate and advanced stages,HPV-related oropharyngeal cancer general-ly prioritizes radiation therapy,with concurrent chemotherapy considered based on tumor staging.Surgical treatment is recommended as the first choice for HPV unrelated oropharyngeal squamous cell carcinoma(including primary and re-current)and recurrent HPV related oropharyngeal squamous cell carcinoma after radiotherapy and chemotherapy;⑤ For primary exogenous T1-2 oropharyngeal cancer,direct surgery through the oral approach or da Vinci robotic sur-gery is preferred.For T3-4 patients with advanced oropharyngeal cancer,it is recommended to use temporary mandibu-lectomy approach and lateral pharyngotomy approach for surgery as appropriate;⑥ For cT1-2N0 oropharyngeal cancer patients with tumor invasion depth>3 mm and cT3-4N0 HPV unrelated oropharyngeal cancer patients,selective neck dissection of levels ⅠB to Ⅳ is recommended.For cN+HPV unrelated oropharyngeal cancer patients,therapeutic neck dissection in regions Ⅰ-Ⅴ is advised;⑦ If PET-CT scan at 12 or more weeks after completion of radiation shows intense FDG uptake in any node,or imaging suggests continuous enlargement of lymph nodes,the patient should undergo neck dissection;⑧ For patients with suspected extracapsular invasion preoperatively,lymph node dissection should include removal of surrounding muscle and adipose connective tissue;⑨ The reconstruction of oropharyngeal cancer defects should follow the principle of reconstruction steps,with priority given to adjacent flaps,followed by distal pedicled flaps,and finally free flaps.The anterolateral thigh flap with abundant tissue can be used as the preferred flap for large-scale postoperative defects.

Tranexamic acid is widely used in joint orthopedic surgery.At the same time,it has high safety and few adverse drug reactions.It can effectively improve intraoperative bleeding and promote early functional recovery of patients.This article reviews the mode of administration,safe dose,administration time and adverse drug reactions of tranexamic acid in the perioperative period of joint replacement and arthroscopic surgery,in order to provide reference for the clinical application of tranexamic acid.
5.Clinical trial of spironolactone combined with sacubitril/valsartan in the treatment of patients with hypertensive nephropathy

Objective To observe the clinical efficacy and safety of spironolactone combined with sacubitril/valsartan in the treatment of patients with hypertensive nephropathy.Methods The patients with hypertensive nephropathy were randomly divided into control group and treatment group.The control group was treated with sacubitril/valsartan(100-200 mg·d-1 in the morning),and treatment group was combined with low-dose spironolactone treatment(20 mg·d-1 in the morning)on the basis of control group.Both groups were treated continuously for 12 weeks.The clinical efficacy was compared;the blood pressure,urinary microalbumin(mAlb),urinary β2 microglobulin(β2-MG)and serum cystatin C(Cys-C),transforming growth factor-β1(TGF-β1),connective tissue growth factor(CTGF)and angiotensin Ⅱ(Ang Ⅱ)and adverse drug reactions were observed before and after treatment.Results There were 87 cases in treatment group and 86 cases in control group were included respectively.After treatment,the total effective rates in treatment group and control group were 95.40%(83 cases/87 cases)and 82.56%(71 cases/86 cases),with significant difference(P<0.05).After treatment,the systolic blood pressure values in treatment group and control group were(124.65±9.65)and(130.27±8.93)mmHg,the diastolic blood pressure values were(75.08±7.14)and(80.45±7.35)mmHg,urinary mAlb levels were(42.58±5.65)and(51.28±6.64)mg·L-1,urinary β2-MG levels were(0.46±0.17)and(0.75±0.25)mg·L-1,24 h urinary protein quantitation levels were(138.49±46.64)and(216.48±65.27)mg,serum Cys-C levels were(0.63±0.26)and(0.85±0.24)mg·L-1,TGF-β1 levels were(98.67±21.43)and(112.46±26.72)pg·mL-1,CTGF levels were(1 206.54±236.56)and(1 340.51±248.25)pg·mL-1,Ang Ⅱ levels were(101.55±17.62)and(115.65±20.08)pg·mL-1,all with significant difference(all P<0.05).The incidence of adverse drug reactions in treatment group and control group were 6.90%(6 cases/87 cases)and 2.33%(2 cases/86 cases),with no significant difference(P>0.05).Conclusion Compared with sacubitril/valsartan alone,spironolactone combined with sacubitril/valsartan can better reduce blood pressure,improve renal function and delay progression of renal fibrosis in the treatment of hypertensive nephropathy,and has definite efficacy,with good safety.
6.Expert consensus on the use of vitamins in parenteral nutrition for adults

Vitamins,as important micronutrients widely involved in human physiological functions,play a significant role in parenteral nutrition therapy due to their inability to be synthesized or insufficient synthesis in the body,requiring exogenous supplementation. However,a consensus does not exist in China regarding the dosage and monitoring of vitamins in parenteral nutrition. Experts in the field are recruited by Chinese Nutrition Society for Medical Food and Nutrition Support,Chinese Society for Parenteral and Enteral Nutrition (CSPEN),Shanghai Medical Association for Parenteral and Enteral Nutrition and the Shanghai Nutrition Society to formulate a consensus paper.The current paper aims to provide standardized guidance for clinical use of vitamins in parenteral nutrition. The consensus paper is a composite of systematically reviewing pertinent literature,Chinese clinical practice and multiple sessions of debate,discussion,and composition,with reference to guidelines from European Society for Parenteral and Enteral Nutrition (ESPEN),American Society for Parenteral and Enteral Nutrition (ASPEN),Australasian Society for Parenteral and Enteral Nutrition (AuSPEN) and CSPEN.This includes 37 recommendations regarding the recommended dosage and monitoring of fat-soluble vitamins (vitamins A,D,E,K) and water-soluble vitamins (vitamin C and vitamins B1,B2,B6,B12,niacin,pantothenic acid,biotin,and folate) to serve varied adult parenteral nutrition patients.
7.Raman Spectroscopy Combined with Partial Least Squares for Quantitative Analysis of Two Kinds of Microplastics in Water Samples

Microplastics(MPs)are emerging contaminants in aquatic environments characterized by their polar structure,small particle size(Typically less than 5 mm),large surface area,good stability,and resistance to biodegradation.They pose adverse effects on the normal physiological activities of aquatic organisms and can accumulate in biota,including humans.Therefore,there is an urgent need for rapid and accurate quantitative analysis of MPs in water environments.In this study,Raman spectroscopy combined with partial least squares(PLS)was employed for rapid and accurate quantitative analysis of polyethylene(PE)and polystyrene(PS)MPs in real water samples.Initially,33 simulated water samples containing different concentrations of MPs were prepared,and their Raman spectra were collected.Six spectral preprocessing methods(Normalization,multiplicative scatter correction,standard normal variate transformation,first derivative,second derivative,and wavelet transform)were investigated for their impact on the predictive performance of PLS calibration models.Subsequently,three variable selection methods including synergy interval partial least squares(SiPLS),variable importance in projection(VIP)and mutual information(MI)were employed to optimize the input variables of the PLS calibration model.The predictive capability of the PLS calibration model was evaluated and validated using leave-one-out cross-validation.Under the optimal conditions of spectral preprocessing,variable selection,input variables and latent variables,the wavelet transform-partial least squares(WT-PLS)calibration model based on distilled water was established,and the contents of PE and PS in real water samples were predicted with prediction correlation coefficients(R2p)of 0.9540 and 0.8472 for PE and PS,respectively,and prediction errors(Errorp)of 0.0690 and 0.1126,respectively.Furthermore,a mixed sample MI-PLS calibration model was developed,demonstrating the best predictive performance in real water samples(With R2p values of 0.9776 and 0.9755 for PE and PS,respectively,and Errorp values of 0.0360 and 0.0392,respectively).This method provided a novel approach and new methodology for quantitative analysis of MPs and other organic pollutants in real water samples.
8.Clinical Observation on the Warming-Needle Moxibustion at Back-Shu Points Combined with Blood-Letting Puncture in Treating Postpartum Depression of Liver Stagnation and Spleen Deficiency Type

Objective To observe the clinical efficacy of warming-needle moxibustion at back-shu points combined with blood-letting puncture in the treatment of postpartum depression(PPD)of liver stagnation and spleen deficiency type.Methods Ninety-eight patients with PPD of liver stagnation and spleen deficiency type were randomly divided into observation group and control group,49 patients in each group,and Sertraline Hydrochloride Tablets were given to the control group orally,while the observation group was given the combination use of warming-needle moxibustion with blood-letting puncture at back-shu points based on the treatment of the control group.Both groups were treated for two months.After two months of treatment,the clinical efficacy of the two groups was evaluated,and the changes of Hamilton Depression Scale(HAMD)and Edinburgh Postnatal Depression Scale(EPDS),as well as Pittsburgh Sleep Quality Index(PSQI)and World Health Organization Quality of Life Scale(WHOQOL)-BREF of the two groups of patients before and after treatment were observed.Changes in serum 5-hydroxytryptamine(5-HT),norepinephrine(NE),and dopamine(DA)levels,as well as levels of the sex hormones estradiol(E2)and progesterone(P)before and after treatment were compared between the two groups.Results(1)The total effective rate was 93.88%(46/49)in the observation group and 75.51%(37/49)in the control group.The efficacy of the observation group was superior to that of the control group,and the difference was statistically significant(P<0.05).(2)After treatment,the scores of HAMD and EPDS of the patients in the two groups were improved significantly(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,and the difference was statistically significant(P<0.05).(3)After treatment,the scores of PSQI and WHOQOL-BREF of patients in the two groups were improved significantly(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,with statistically significant differences(P<0.05).(4)After treatment,the serum levels of 5-HT,NE,DA,E2,and P of the two groups of patients were significantly improved(P<0.05),and the improvement in the observation group was significantly superior to that in the control group,and the difference was statistically significant(P<0.05).Conclusion Warming-needle moxibustion at back-shu points combined with blood-letting puncture in the treatment of PPD of liver stagnation and spleen deficiency type can help to alleviate the clinical symptoms of the patients,improve the serum neurotransmitter and sex hormone levels of the patients,and improve whose quality of sleep and quality of survival,so as to improve the quality of life of the patients,with remarkable clinical efficacy.
9.Parametric analysis of craniocerebral injury mechanism in pedestrian traffic accidents based on finite element methods

Purpose::The toughest challenge in pedestrian traffic accident identification lies in ascertaining injury manners. This study aimed to systematically simulate and parameterize 3 types of craniocerebral injury including impact injury, fall injury, and run-over injury, to compare the injury response outcomes of different injury manners.Methods::Based on the total human model for safety (THUMS) and its enhanced human model THUMS-hollow structures, a total of 84 simulations with 3 injury manners, different loading directions, and loading velocities were conducted. Von Mises stress, intracranial pressure, maximum principal strain, cumulative strain damage measure, shear stress, and cranial strain were employed to analyze the injury response of all areas of the brain. To examine the association between injury conditions and injury consequences, correlation analysis, principal component analysis, linear regression, and stepwise linear regression were utilized.Results::There is a significant correlation observed between each criterion of skull and brain injury ( p < 0.01 in all Pearson correlation analysis results). A 2-phase increase of cranio-cerebral stress and strain as impact speed increases. In high-speed impact (> 40 km/h), the Von Mises stress on the skull was with a high possibility exceed the threshold for skull fracture (100 MPa). When falling and making temporal and occipital contact with the ground, the opposite side of the impacted area experiences higher frequency stress concentration than contact at other conditions. Run-over injuries tend to have a more comprehensive craniocerebral injury, with greater overall deformation due to more adequate kinetic energy conduction. The mean value of maximum principal strain of brain and Von Mises stress of cranium at run-over condition are 1.39 and 403.8 MPa, while they were 1.31, 94.11 MPa and 0.64, 120.5 MPa for the impact and fall conditions, respectively. The impact velocity also plays a significant role in craniocerebral injury in impact and fall loading conditions (the p of all F-test < 0.05). A regression equation of the craniocerebral injury manners in pedestrian accidents was established. Conclusion::The study distinguished the craniocerebral injuries caused in different manners, elucidated the biomechanical mechanisms of craniocerebral injury, and provided a biomechanical foundation for the identification of craniocerebral injury in legal contexts.
10.Characteristic Chromatogram and Index Component Contents of Substance Benchmark of Classic Formula Qingwei Power

OBJECTIVE To establish an HPLC characteristic spectrum and content determination method for the reference mate-rial of the classic formula Qingwei Powder,and control its quality to ensure subsequent preparation.METHODS Following ancient books and combining with the investigation of the previous preparation process,15 batches of Qingwei Powder material standards were prepared.HPLC was used to establish the material standard feature map,and the similarity was calculated using the Chinese medicine chromatographic fingerprint similarity evaluation system(2012 version);seven indicator components from five medicinal herbs were used as the reference content determination indicators for Qingwei Powder,and an HPLC content determination method was established for 15 batches of samples.RESULTS The similarity of the characteristic spectra was≥0.975;13 common peaks were calibrated;and 7 common peaks were identified for chemical composition,including catalpol(peak 1),ferulic acid(peak 5),isoferulic acid(peak 6),berberine hydrochloride(peak 7),palmatine hydrochloride(peak 8),paeonol(peak 12),and ligustilide(peak 13).Two sets of indicator component content determination methods have been established.Firstly,the contents of catalpol,ferulic acid,isoferulic acid,and paeonol were determined,and the four component contents were specified to be 1.48-2.76、0.14-0.26、0.48-0.90、1.72-3.19 mg·g-1,respectively;secondly,the contents of coptisine hydrochloride,palmatine hydrochloride,and ber-berine hydrochloride was determined,and it was specified that the content of the three components should be 0.65-1.21、0.57-1.07、1.86-3.45 mg·g-1,respectively.CONCLUSION A quality control method for the reference material of Qingwei Powder has been established through the characteristic spectrum and content determination method.This method is simple and reliable,providing a basis and foundation for subsequent quality control and formulation development.
