1.Advances in diffuse optical technology lenses for myopia control

Recent years have witnessed significant advancements in myopia control research through the application of diffuse optical technology(DOT)spectacle lenses. Myopia has emerged as a global public health challenge, affecting nearly half of the world's population, with childhood and adolescent myopia rates continuing to rise. DOT lenses represent an innovative myopia control intervention based on retinal contrast signal theory. These lenses incorporate micro-light scattering dots distributed across the lens surface to reduce retinal imaging contrast and modulate the influence of visual input on axial elongation, thereby slowing myopia progression. The core mechanism operates through refractive index differences between the lens substrate(1.53)and scattering dots(1.50), which generate optical scattering effects. This design maintains clear vision through a central 5 mm optical zone while effectively reducing contrast signal intensity in the peripheral retina. Large-scale randomized controlled trials, including the CYPRESS study, have demonstrated significant myopia control efficacy in children aged 6-10 years: 12-month follow-up data revealed a 74% reduction in myopia progression and a 50% reduction in axial elongation, with sustained safety and visual quality maintained over 4-year long-term follow-up. However, several aspects of DOT technology remain contentious and require further clinical validation, including its applicability across different age groups, optimal scattering dot density configurations, combined application effects with other myopia control methods, and long-term visual adaptation during extended use. This review systematically examines the theoretical foundations, design characteristics, clinical application progress, and future development directions of DOT technology, providing scientific evidence for clinical myopia prevention and control strategy formulation.
2.Early outcomes of anterior segment parameters in patients with high myopia after implantable collamer lens V4c implantation

AIM: To study the early outcomes of anterior segment parameters after implantation of an implantable collamer lens with a central hole(ICL V4c)in patients with high myopia.METHODS:A total of 82 cases(160 eyes)with high myopia, including 42 males(82 eyes)and 40 females(78 eyes), aged 26.0±4.6(21 to 37)years, who underwent ICL V4c implantation at our institution from February 2019 to September 2022 and were followed up for 1 a, were included. The general characteristics of the anterior segment of the eye were measured preoperatively: spherical equivalent, mean horizontal corneal curvature, white-to-white(WTW), and axial length(AL); intraocular pressure(IOP), endothelial cell density(ECD), central anterior chamber depth(CACD), anterior chamber volume(ACV)and anterior chamber angle(ACA)were measured preoperatively and at 1 d, 1 wk, 1, 3 and 6 mo postoperatively. Furthermore, the distance from the centre of the posterior surface of the ICL V4c optical zone to the anterior surface of the lens(vault)was measured at 1 d, 1 wk, 1, 6 mo, and 1 a after surgery.RESULTS: The mean preoperative spherical equivalent of the patients was -7.56±2.55 D, mean horizontal corneal curvature was 42.89±1.47 D, WTW was 11.64±0.37 mm, and AL was 26.64±0.93 mm. The baseline IOP was 15.97±2.13 mmHg, and the differences in IOP at each time point after ICL V4c implantation compared to preoperative were not statistically significant(F=0.875, P=0.504); ECD was 2 989.30±140.78 cells/mm2 at baseline, and ECD at 6 mo after ICL V4c implantation was not statistically significant compared with preoperative ECD(t=1.475, P=0.142); CACD was 3.19±0.21 mm at baseline, and ACV was 210.30±27.7 mm3, and CACD and ACV were significantly lower than preoperative at all postoperative time points(F=111.10, 288.38, all P<0.001). The baseline ACA was 35.44°±11.27°, and the ACA at each time point after ICL V4c implantation was significantly lower than preoperatively(F=21.23, P<0.001). The vault was 665.32±184.03 μm at 1 d postoperatively, and continued to be significantly reduced at 1 wk, 1, 6 mo, and 1 a postoperatively compared with 1 d(F=52.10, P<0.001). However, it remained stable at 6 mo and 1 a postoperatively, and the difference was not statistically significant compared with vault at 1 mo postoperatively(P>0.05).CONCLUSION: ICL V4c has certain safety and efficiency in 1 a postoperative follow-up, and the parameters of the anterior segment of the eye stabilized in the early period.
3.Safety of defocusing soft lenses and their research progress in myopia prevention and control

Myopia is a common ophthalmic disease that endangers the health of eyes all over the world. The incidence rate of myopia is high in China. Myopia has become the most prominent problem affecting the health of young people's eyes. Myopia prevention and control work has become urgent. Periretinal hyperopia defocusing is one of the main causes of myopia, and the defocusing soft lenses involved based on this principle have played a good role in myopia prevention and control. This article summarizes the working principle of defocusing soft lenses for controlling myopia and its impact on corneal and visual function, and evaluates the prevention and control effect of defocusing soft lenses on myopia based on current research.
4.Effects of Jin's three needles combined with neuromuscular electrical stimulation on electromyography, muscle strength, and lower limb motor function in patients with hemiplegia after stroke

Objective:To investigate the effects of Jin's three needles combined with neuromuscular electrical stimulation on electromyography (EMG), muscle strength, and lower limb motor function in patients with hemiplegia after stroke.Methods:A total of 102 patients with hemiplegia after ischemic stroke who received treatment in Dongyang People's Hospital from July 2021 to August 2022 were included in this randomized controlled study. They were divided into a treatment group and a control group ( n = 51 per group) using a random number table method. The treatment group was given Jin's three needles combined with neuromuscular electrical stimulation, and the control group was given neuromuscular electrical stimulation alone. The National Institutes of Health Stroke Scale (NIHSS) was used to assess the degree of neurological impairment. The modified Barthel Index (MBI) score was used to assess a patient's self-care ability. EMG was performed to record myoelectric signals. The Fugl-Meyer Assessment Scale was used to evaluate a patient's lower limb motor function. Manual Muscle Testing was performed to measure a patient's muscle strength. The degree of nerve defect, self-care ability, lower limb motor function, hip flexion muscle strength, knee extensor muscle strength, and ankle dorsalis extensor muscle strength were compared before treatment and after 2 weeks of treatment. Results:After treatment, the NIHSS score in the treatment and control groups significantly decreased compared with those before treatment [(10.14 ± 3.45) points vs. (3.80 ± 1.92) points, (10.10 ± 3.17) points vs. (6.18 ± 2.15) points, t = 17.31, 19.08, both P < 0.05]. After treatment, the NIHSS score in the treatment group was lower than that in the control group ( t = -5.87, P < 0.001). After treatment, the integral EMG values of the tibialis anterior muscle, gastrocnemius muscle, and quadriceps femoris in the treatment and control groups were increased compared with those before treatment [(20.43 ± 5.26) points vs. (48.14 ± 10.41) points, (16.20 ± 2.62) points vs. (24.69 ± 4.07) points, (32.62 ± 3.42) points vs. (43.40 ± 4.69) points; (20.40 ± 5.19) points vs. (35.45 ± 7.41) points, (16.32 ± 2.51) points vs. (20.91 ± 3.45) points, (32.59 ± 3.37) points vs. (37.39 ± 4.06) points, t = -23.79, -16.11, -19.89; -21.92, -10.86, -8.71, all P < 0.05]. After treatment, the integral EMG values of the tibialis anterior muscle, gastrocnemius muscle, and quadriceps femoris in the treatment group were significantly higher than those in the control group ( t = 7.09, 5.05, 6.92, all P < 0.001). After treatment, The Fugl-Meyer Assessment scale score in the treatment and control groups were significantly increased compared with that before treatment [(17.61 ± 8.93) points vs. (27.16 ± 5.84) points, (17.53 ± 7.59) points vs. (22.22 ± 6.66) points, t = -14.64, -10.72, both P < 0.05). After treatment, The Fugl-Meyer Assessment scale score in the treatment group was significantly higher than that in the control group ( t = 3.98, P < 0.001). After treatment, hip flexion muscle strength, knee extensor muscle strength, and ankle dorsalis extensor muscle strength in the treatment and control groups increased compared with those before treatment. After treatment, hip flexion muscle strength, knee extensor muscle strength, and ankle dorsalis extensor muscle strength in the treatment group were significantly higher than those in the control group [(3.75 ± 0.63) grades vs. (3.31 ± 0.65) grades, (3.51 ± 0.67) grades vs. (3.08 ± 0.87) grades, (2.98 ± 0.81) grades vs.(2.35 ± 1.06) grades, t = 3.41, 2.80, and 3.36, all P < 0.05]. Conclusion:Jin's three needles combined with neuromuscular electrical stimulation can improve EMG findings, increase muscle strength, and lower limb motor function in patients with hemiplegia after stroke
5.Study of the energy loss and vortex in the left ventricle in patients with mid-range and reduced left ventricular ejection fraction by vector flow mapping

Objective:To quantitatively evaluate the energy loss (EL) and vortex characteristics in the left ventricle by vector flow mapping (VFM) technique in heart failure patients with mid-range ejection fraction (HFmrEF) and reduced ejection fraction (HFrEF).Methods:One hundred and five patients in Qilu Hospital of Shandong University from October 2016 to November 2017 with heart failure and left ventricular ejection fraction (LVEF)<50% were enrolled and divided into HFmrEF Group (LVEF40%~<50%, 56 cases) and HFrEF group (LVEF<40%, 49 cases). Another 32 healthy people at the same period were matched as control group. The EL, vortex area and circulation of isovolumic relaxation phase (IVR), rapid filling phase (RFP), atrial contraction phase (ACP), isovolumic contraction phase (IVC) and rapid ejection phase (REP) in the left ventricle was measured by VFM technique.Results:The EL in HFmrEF group and HFrEF group was lower than that in the control group. In ACP, the EL was gradually decreased among the control group, HFmrEF group and HFrEF group( P<0.05). In ACP, the vortex area and circulation was larger in heart failure patients than those in the control group, and gradually increased from control group, to HFrEF group( P<0.05). Positive correlation between EL and E/e ′ was evidenced in the RFP ( r=0.524, P< 0.001). While in ACP, there was a positive correlation between EL and A peak ( r=0.492, P<0.001), and a negative correlation between EL and vortex area ( r=-0.235, P=0.040). Conclusions:VFM can be applied to evaluate the EL in left ventricle. The EL in the left ventricle of patients with HFmrEF and HFrEF is significantly lower than that in control group. El is correlated with cardiac systolic and diastolic function and vortex area.
6.Effects of gestational diabetes mellitus and mid-pregnancy blood glucose levels on gestational weeks based on birth cohort study

【Objective】 To explore the effects of gestational diabetes mellitus (GDM) and mid-pregnancy blood glucose levels on gestational weeks in Northwestern China. 【Methods】 For this prospective cohort study, we recruited the first-trimester pregnant women who underwent obstetrical examinations at Northwest Women’s and Children’s Hospital from July 2018 to July 2019 as the cohort. We investigated their socio-demographic characteristics and risk factors of adverse pregnancy outcomes. Additionally, we collected the OGTT test results of pregnant women in mid-pregnancy and followed up their pregnancy outcomes. The relationship of GDM and blood glucose levels with gestational weeks was analyzed by using the generalized linear model. 【Results】 A total of 2 434 subjects were included in this study. There were 668 pregnant women with GDM, with a ratio of 27.44%. GDM in pregnant women shortened the gestational weeks (β=-0.17, 95% CI: -0.28—-0.05). Fasting blood glucose, OGTT-1 h blood glucose, and OGTT-2 h blood glucose increased by 1 mmol/L; gestational weeks were shortened by 0.17 (95% CI: -0.28—-0.05), 0.05 (95% CI: -0.09—-0.02) and 0.07 (95% CI: -0.12—-0.03). Fasting blood glucose and OGTT-1 h blood glucose abnormalities shortened gestational weeks by 0.18 (95% CI: -0.31—-0.05) and 0.28 (95% CI: -0.47—-0.10) respectively. An increase by 1 mmol/L or abnormality of fasting blood glucose would increase preterm delivery risk by 1.44 (95% CI: 1.01-2.06) and 1.73 times (95% CI: 1.10-2.69), respectively. 【Conclusion】 GDM in pregnant women may shorten their gestational weeks; the abnormal and elevated fasting blood glucose, in particular, would increase the risk of preterm delivery. Therefore, we should carry out active health education to control the blood glucose and other risk factors of GDM patients and promote healthy pregnancy.
7.Association of gestational diabetes mellitus and blood glucose level during the second trimester with neonatal birth weight: A study based on birth cohort

【Objective】 To explore the association of gestational diabetes mellitus (GDM) and blood glucose level with birth weight of the newborns in Northwest China. 【Methods】 Pregnant women in their first trimester who joined the birth cohort of Northwest Women and Children’s Hospital from July 2018 to July 2019 were consecutively enrolled. We collected their basic demographic characteristics, lifestyle behavior and other data. We followed up the results of the OGTT test and pregnancy outcomes. The generalized linear model was used to analyze the effects of GDM and blood glucose levels on the birth weight of newborns. 【Results】 A total of 2 422 subjects were included in this study, and 656 (27.09%) pregnant women had GDM during pregnancy. Pregnant women with GDM increased the birth weight (β=50.00, 95% CI: 17.48-82.51), Z-value (β=0.11, 95% CI: 0.03-0.19) and Z Centile value (β=3.22, 95% CI: 0.88-5.55), and increased the risk of macrosomia (OR=1.54, 95% CI: 1.04-2.28). Abnormal FPG value during the second trimester would increase the risk of macrosomia and LGA. With the increase of OGTT blood glucose value in the second trimester, the birth weight value and the incidence of macrosomia and LGA showed an upward trend. 【Conclusion】 Pregnancy in women with GDM might increase the birth weight of newborns and the risk of macrosomia, especially FPG. We should vigorously control the blood glucose level of GDM patients to promote the health of mothers and infants.
8.Targeted nano-vector for gene delivery into human bone marrow mesenchymal stem cells and cellular MR imaging in vitro

Objective To explore the feasibility and efficacy of an MRI-visible,targeted,nano-vector which is synthesized by attaching a targeting ligand,the GD2 single chain antibody (scAb GD2),to the distal ends of PEG-g-PEI-SPION as a carrier for gene delivery into human bone marrow mesenchymal stem cells (hBMSCs) and in vitro cellular MR imaging.Methods scAbGD2-PEG-g-PEI-SPION was synthesized as previously reported.Gel electrophoresis was performed to assess the pDNA condensation ability of scAbGD2-PEG-g-PEI-SPION.The particle size and Zeta potential of scAbGD2-PEG-g-PEI-SPION/pDNA nanocomplexes were observed by dynamic light scattering.Cytotoxicity of scAbGD2-PEG-g-PEI-SPI-ON was evaluated by CCK-8 assay using hBMSCs.Gene transfection efficiency of scAbGD2-PEG-g-PEI-SPION in hBMSCs was quantified by flow cytometry,PEG-g-PEI-SPION,scAbGD2-PEG-g-PEI-SPION,scAbGD2-PEG-g-PEI-SPION+ free AbGD2 and scAbIgG2a-PEG-g-PEI-SPION group was established.The cellular internalization of scAbGD2-PEG-g-PEI-SPION/pDNA nanocomplexes was observed by confocal laser scanning microscopy and Prussian blue staining.MRI of scAbGD2-PEG-g-PEI-SPION was performed by cellular MRI scanning in vitro.Results scAbGD2-PEG-g-PEI-SPION condensed pDNA to form stable nanocomplexes of 80-100 nm in diameter and showed low cytotoxicity to hBMSCs.At the same N/P ratio,the transfection efficiency of scAbGD2-PEG-g-PEI-SPION group was significantly higher than those of other groups (P<0.001).At the optimal N/P ratio of 20,scAbGD2-PEG-g-PEI-SPION/pDNA obtained the highest transfection efficiency of (59.60 ± 4.50) % in hBMSCs.Furthermore,hBMSCs labeled with scAbGD2-PEG-g-PEI-SPION showed sensitive low signal intensity on MRI T2/T2 *-weighted images in vitro.Conclusion scAbGD2-PEG-g-PEI-SPION is an efficient MRL visible targeted nano vector for gene delivery into hBMSCs.
9.Metabolic syndrome increases Framingham risk score of patients with type 2 diabetes mellitus.

OBJECTIVETo assess the impact of metabolic syndrome(MS) on Framingham risk score(FRS) in patients with type 2 diabetes mellitus (T2DM).
METHODSThe anthropometric and biochemical data of 1708 patients with T2DM admitted in hospital from May 2008 to April 2013 were retrospectively analyzed, including 902 males and 806 females with a mean age of 57.1±11.8 years (20-79 years). Diagnosis of MS was made according to the criteria of the Adult Treatment Panel Ⅲ Criteria modified for Asians.
RESULTSCompared to non-MS/T2DM patients, MS/T2DM patients had higher waist circumference, body weight, body mass index, systolic and diastolic blood pressure, fasting C peptide, total cholesterol, triglyceride, and LDL-C (P<0.05), while lower HDL-C (P<0.01). Both FRS [13.0(10.0, 15.0) vs 11.0(9.0, 13.0) in male,15.0(12.0, 18.0) vs 12.0(6.0, 14.8) in female,P<0.01)] and 10-year cardiovascular risk [12.0%(6.0%, 20.0%) vs 8.0%(5.0%,12.0%) in male,3.0%(1.0%, 6.0%) vs 1.0%(0.0%, 2.8%) in female,P<0.01] were higher in MS/T2DM patients than those in non-MS/T2DM patients.Both FRS and 10-year cardiovascular risk were increased with the components of MS.
CONCLUSIONT2DM patients with MS have more cardiovascular risk factors, higher FRS and 10-year cardiovascular risk.
10.Interventional treatment for ischemic-type biliary lesion after liver transplantation

Objective To investigate the safety and efficacy of interventional treatment for ischemic-type biliary lesion (ITBL)after liver transplantation (LT). Methods The clinical and imaging data of 76 patients with ITBL after LT,who received interventional treatment in the Department of Interventional Vascular Radiology of the Third Affiliated Hospital of Sun Yat-sen University from January 2006 to February 2014,were retrospectively analyzed. On the basis of the cholangiographic appearance,patients were classified into 3 groups:hilar biliary stricture group (n=28),multifocal biliary stricture group (n=42),and biloma group (n=6). The modalities of interventional treatment were percutanous transhepatic biliary drainage (PTBD), PTBD combined with balloon dilation,PTBD combined with balloon dilation and plastic stent implantation. The methods of biliary drainage included external drainage and external-internal drainage. All the patients were followed up after treatment. The curative effect and biliary complication was observed. Results The first successful rate of PTBD was 97% (74/76). The total curative rate,improvement rate and ineffective rate of interventional treatment were 21% (16/76),51% (39/76)and 28% (21/76). In hilar biliary stricture group,the cure,improvement and inefficacy rates were 36% (10/28),57% (16/28)and 7% (2/28).The efficacy rate was 93% (22/28 ). In multifocal biliary stricture group,the cure,improvement and inefficacy rates were 14% (6/42),50% (21/42)and 36% (15/42). The efficacy rate was 64% (27/42). In biloma group,2 cases (2/6)were cured and treatment of4 cases was ineffective. The efficacy ofhilar biliary stricture group was better than that of multifocal biliary stricture group (P<0.05 ). The efficacy of multifocal biliary stricture group was better than that of biloma group (P <0.001 ). The main biliary complication was biliary tract infection during drainage. The rates of bile tract infections were 20% (13/64) and 67% (8/12)in patients with external drainage and external-internal drainage,respectively. There was significant difference between these two items (P <0.001 ). Conclusions PTBD is a safe and effective therapeutic modality for ITBL after LT,which combined with balloon dilation and biliary stent implantation can improve patients’clinical symptoms,elevate patients’quality of life. The biliary external drainage can decrease the rate of biliary tract infection significantly.
