1.Clinical Efficacy and Mechanism of Danggui Liuhuang Tang in Reducing Cardiovascular Risk in Patients with Type 2 Diabetes Mellitus with Yin Deficiency and Fire Excess Syndrome

ObjectiveThis study aims to observe the clinical effect of Danggui Liuhuang Tang (DGLHT) on patients with type 2 diabetes mellitus (T2DM) complicated by atherosclerotic cardiovascular disease (ASCVD) at high risk, focus on evaluating the influence of DGLHT on cardiovascular risk indicators such as flow-mediated dilation (FMD), atherogenic index of plasma (AIP), and triglyceride-glucose index (TyG), and explore the regulatory effect of DGLHT on the myeloid differentiation factor 88/nuclear factor-kappa B (MyD88/NF-κB) signaling pathway. MethodsThe clinical study was a single-center, double-blind, and randomized controlled trial. A total of 68 patients with T2DM-ASCVD at high risk for cardiovascular events with Yin deficiency and fire excess syndrome were enrolled and randomly assigned to a treatment group and a control group. The treatment group was given atorvastatin calcium tablets and DGLHT, while the control group was given atorvastatin calcium tablets and placebos. The treatment course was 12 weeks, with a final study completion of 30 patients in the treatment group and 29 in the control group. Changes in cardiovascular risk indicators such as FMD, AIP, TyG, and small dense low-density lipoprotein cholesterol (sdLDL-C) index were compared. Human umbilical vein endothelial cells (HUVECs) were used to establish a vascular endothelial injury and inflammation model. The protective effect of DGLHT on endothelial injury was verified by reverse transcription polymerase chain reaction (Real-time PCR) and Western blot . ResultsAfter 12 weeks of treatment, the AIP in the treatment group significantly decreased compared with that before the treatment (P<0.05). Compared with the control group, the treatment group showed significant improvements in FMD and TyG (P<0.05). Additionally, the treatment group demonstrated significant reductions in two-hour postprandial glucose (2 hPG), glycated albumin (GA), triglycerides (TG), apolipoprotein E (Apo E), and sdLDL-C (P<0.05). Analysis of traditional Chinese medicine (TCM) syndrome efficacy indicated that in the treatment group, Yin deficiency and fire excess syndromes, including dry throat and mouth (P<0.05), excessive thirst (P<0.01), tidal fever and night sweats (P<0.05), and dry stools (P<0.05), improved. Compared with the control group, the treatment group showed significant improvements in symptoms of dry throat and mouth (P<0.05) and excessive thirst (P<0.01). TCM syndrome scores significantly decreased (P<0.01), and the overall efficacy rate was 56.67%, significantly higher than the 10.34% observed in the control group (P<0.01). At the cellular level, increasing concentrations of DGLHT led to decreased messenger ribonucleic acid (mRNA) levels of pro-inflammatory cytokines interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α), and interleukin-1-beta (IL-1β) in lipopolysaccharide (LPS)-stimulated HUVECs (P<0.01), with significant reductions in the high-concentration group (P<0.01). DGLHT may inhibit the expressions of MyD88 and phosphorylated (p)-NF-κB p65 proteins in a concentration-dependent manner. ConclusionDGLHT shows significant effects in reducing cardiovascular risks and may exert an anti-inflammatory effect by inhibiting the MyD88/NF-κB signaling pathway. This finding provides a new perspective for the prevention and treatment of cardiovascular diseases in high-risk individuals with T2DM-ASCVD.
2.Causal relationship of liposomes and circulating metabolomes to schizophrenia: a two-step, two-sample Mendelian randomization study

BackgroundSchizophrenia is a complex, chronic and severe mental disorder, and the pathogenesis of which has not been fully elucidated. The abnormalities in lipid metabolism and circulating metabolomes have already been implicated in the pathophysiology of schizophrenia. However, available studies have mainly focused on a few liposomes and circulating metabolites, failing to systematically reveal the mediating role of circulating metabolomes in the causal relationship between liposomes and schizophrenia. ObjectiveTo uncover mediating role of circulating metabolomes in the causal relationship between liposomes and schizophrenia, thereby providing biomarkers and potential therapeutic targets for the prevention and treatment of schizophrenia. MethodsData from Genome-Wide Association Studies (GWAS) were analyzed, taking data on 179 liposomes as exposure variables, data on 123 circulating metabolites as intermediate variables, and data on schizophrenia as outcome variable. A two-step, two-sample Mendelian randomization analysis was conducted using the inverse-variance weighted (IVW), MR- Egger, Weighted median, and Weighted mode methods to study the causal relationship of liposomes with schizophrenia and the mediating role of circulating metabolomes in the relationship. ResultsIVW model identified 8 lipids associated with schizophrenia without reverse causality. There were 5 circulating metabolomes strongly associated with schizophrenia. Acetate played a significant mediating role in the causal relationship between phosphatidylinositol (18:0_18:2) and schizophrenia (P=0.023, 95% CI: 0.036~0.532), accounting for 28.4% of the causal relationship. ConclusionThis study demonstrates a causal relationship between liposomes and schizophrenia, with phosphatidylinositol being a risk factor in the progression of schizophrenia, and acetate playing a mediating role in this process. 3.Study on the Knowledge-Attitude-Practice questionnaire for patients receiving oral anticoagulant therapy based on the Delphi method

OBJECTIVE To develop a questionnaire of the Knowledge-Attitude-Practice (KAP) for patients receiving oral anticoagulant therapy. METHODS Under the guidance of the theory of KAP, literature analysis and interview method were used to design the initial KAP questionnaire for patients treated with oral anticoagulants. Delphi method was adopted to consult the initial questionnaire and modify the questionnaire based on expert suggestions to form the final questionnaire. RESULTS Two rounds of consultation were conducted with 18 experts, and 18 questionnaires were sent out and recovered in each round, so the positive coefficient of experts was 100%. The expert authority coefficient was 0.94. The average importance scores for all dimensions, factors, and items of the questionnaire in both rounds were ≥4 points. The coefficient of variation was ≤0.25. The Kendall’s concordance coefficient for the overall questionnaire and the three dimensions of knowledge, attitude, and practice ranged from 0.09 to 0.34 (all P<0.05). Following the first round of expert consultation, four items were modified, two items were deleted, and five items were added; after the second round of expert consultation, ten items were modified. The final version of the questionnaire included three dimensions (knowledge, attitudes, and practice), 17 questionnaire factors, and 40 items. CONCLUSIONS The questionnaire has high reliability and scientific validity with relatively concentrated expert opinions. It is suitable for assessing the knowledge, attitudes, and practice status of patients receiving oral anticoagulant therapy.
4.Polydatin protects vascular endothelial cells by modifying the JAK2/STAT3 signaling pathway

Objective To study the protective effect of polydatin on lipopolysaccharide-induced injury of human umbilical vein vascular endothelial cells(HUVECs)through the protein Janus kinase 2(JAK2)/signal transducers and activators of transcription 3(STAT3)signaling pathway.Methods HUVECs were cultured in vitro,and 500 ng/ml LPS induced their injury and set as a model group;based on the model group,endothelial cells were inter-vened with different concentrations(10,20,and 40 μmol/L)of polydatin for 24 h,and set as polydatin low concentration group,polydatin medium concentration group,and polydatin high concentration group,respectively;a control group was set as another group.CCK-8,monocyte-endothelial cell adhesion,scratch and Transwell assays were used to detect cell viability,adhesion,migration and invasive ability;ELISA was used to detect interleukin-6(IL-6)and tumor necrosis factor-alpha(TNF-α)levels in the cell supernatant;Western blot was used to detect the expression of proteins related to the JAK2/STAT3 signaling pathway levels of JAK2/STAT3 signaling pathway relat-ed proteins.Results Compared with the control group,the model group showed decreased cell survival(P<0.01),increased cell adhesion,migration and invasion(P<0.001,P<0.05,P<0.01),increased levels of IL-6 and TNF-α in the cell supernatant(P<0.001),and increased levels of phosphorylation of JAK2 and STAT3 pro-teins in the cells(P<0.05).Compared with the model group,LPS damage to cells was attenuated after polydatin intervention,cell survival was increased in polydatin low-,medium-and high-concentration groups(P<0.05),cell adhesion,migration,and invasion decreased(P<0.05,P<0.05,P<0.001),IL-6 and TNF-α levels in cell supernatants decreased(P<0.05),and the levels of cellular JAK2 and STAT3 protein phosphorylation lev-els decreased(P<0.05).Conclusion Polydatin seems to reduce the inflammatory injury of human umbilical vein endothelial cells induced by LPS,reducing the secretion of inflammatory factors,and inhibiting the ability of cell ad-hesion,migration and invasion,which may be related to the down-regulation of JAK2/STAT3 signal pathway by polydatin.
5.A nomogram based on multimodal CT parameters predicts outcome after endovascular therapy in patients with vertebrobasilar artery occlusion stroke

Objective:To investigate the predictive value of a nomogram based on multimodal CT parameters for the outcome of endovascular therapy (EVT) in patients with acute vertebrobasilar artery occlusion (AVBAO).Methods:Patients with AVBAO underwent EVT at Xuzhou Central Hospital from January 2021 to March 2024 were included retrospectively. At 90 days after EVT, the modified Rankin Scale was used to evaluate clinical outcome. 0-3 points were defined as good outcome and 4-6 points were defined as poor outcome. Multivariate stepwise logistic regression model was used to screen for predictive variables. Then a nomogram was drawn and the prediction model was evaluated. Results:A total of 91 patients with AVBAO were included. There were 60 males (65.9%), aged 69.09±10.57 years. Thirty-eight patients (41.8%) had good outcome, 53 (58.2%) had poor outcome, and 35 (38.5%) died. Univariate analysis showed that there were significant differences in white blood cell count, neutrophil count, National Institutes of Health Stroke Scale (NIHSS) score, Glasgow Coma Scale (GCS) score, posterior circulation Alberta Stroke Program Early CT Score (pc-ASPECTS), Basilar Artery on Computed Tomography Angiography (BATMAN) score, core infarct volume, mismatched volume ratio, onset to door time between the poor outcome group and the good outcome group (all P<0.05). The above indicators were included in a binary multivariate stepwise logistic regression model. The results showed that higher NIHSS scores (odds ratio [ OR] 1.154, 95% confidence interval [ CI] 1.070-1.244; P<0.001), lower BATMAN scores ( OR 0.626, 95% CI 0.416-0.943; P=0.025), and larger core infarct volumes ( OR 1.147, 95% CI 1.046-1.258; P=0.004) on admission were the independent risk factors for poor outcome. A nomogram was plotted using the above three independent risk factors as predictor variables. Its area under the receiver operating characteristic curve for predicting poor outcome was 0.942 (95% CI 0.894-0.990). The sensitivity and specificity were 81.1% and 97.4%, respectively. The calibration curve fluctuates within a small range around the ideal curve. A mean absolute error was 0.027 and a mean square error was 0.001. The clinical decision curve suggested that the model had good clinical applicability. The dynamic nomogram is shown in: https://yuepeng.shinyapps.io/VBAO_model/. Conclusion:The nomogram prediction model based on multimodal CT parameters has good predictive performance for poor outcome in patients with AVBAO after EVT.
6.Research progress in TCM for the treatment of blood glucose fluctuation in diabetes

Modern TCM understands the pathogenesis of blood glucose fluctuation mainly from the viscera (spleen, kidney, liver, bile, small intestine), middle energizer, "yin fire theory", "Xuanfu depression" and "qi transformation". In clinic, blood glucose fluctuation is mainly divided into four syndrome types: qi-yin deficiency syndrome, yin-yang deficiency syndrome, exuberance of fire and heat syndrome, and heat stagnation in liver-stomach syndrome. TCM compounds can improve blood glucose fluctuation by improving insulin resistance, regulating intestinal microflora, promoting the secretion of glucagon-like peptide-1, and fighting oxidative stress.
7.Development and Application of Deep Learning-Based Model for Quality Control of Children Pelvic X-Ray Images

Objective A deep learning-based method for evaluating the quality of pediatric pelvic X-ray images is proposed to construct a diagnostic model and verify its clinical feasibility.Methods Three thousand two hundred and forty-seven children with anteroposteric pelvic radiographs are retrospectively collected and randomly divided into training datasets,validation datasets and test datasets.Artificial intelligence model is conducted to evaluate the reliability of quality control model.Results The diagnostic accuracy,area under ROC curve,sensitivity and specificity of the model are 99.4%,0.993,98.6%and 100.0%,respectively.The 95%consistency limit of the pelvic tilt index of the model is-0.052-0.072.The 95%consistency threshold of pelvic rotation index is-0.088-0.055.Conclusion This is the first attempt to apply AI algorithm to the quality assessment of children's pelvic radiographs,and has significantly improved the diagnosis and treatment status of DDH in children.

Objective To investigate the incidence rate,clinical characteristics,and prognosis of neonatal stroke in Shenzhen,China.Methods Led by Shenzhen Children's Hospital,the Shenzhen Neonatal Data Collaboration Network organized 21 institutions to collect 36 cases of neonatal stroke from January 2020 to December 2022.The incidence,clinical characteristics,treatment,and prognosis of neonatal stroke in Shenzhen were analyzed.Results The incidence rate of neonatal stroke in 21 hospitals from 2020 to 2022 was 1/15 137,1/6 060,and 1/7 704,respectively.Ischemic stroke accounted for 75%(27/36);boys accounted for 64%(23/36).Among the 36 neonates,31(86%)had disease onset within 3 days after birth,and 19(53%)had convulsion as the initial presentation.Cerebral MRI showed that 22 neonates(61%)had left cerebral infarction and 13(36%)had basal ganglia infarction.Magnetic resonance angiography was performed for 12 neonates,among whom 9(75%)had involvement of the middle cerebral artery.Electroencephalography was performed for 29 neonates,with sharp waves in 21 neonates(72%)and seizures in 10 neonates(34%).Symptomatic/supportive treatment varied across different hospitals.Neonatal Behavioral Neurological Assessment was performed for 12 neonates(33%,12/36),with a mean score of(32±4)points.The prognosis of 27 neonates was followed up to around 12 months of age,with 44%(12/27)of the neonates having a good prognosis.Conclusions Ischemic stroke is the main type of neonatal stroke,often with convulsions as the initial presentation,involvement of the middle cerebral artery,sharp waves on electroencephalography,and a relatively low neurodevelopment score.Symptomatic/supportive treatment is the main treatment method,and some neonates tend to have a poor prognosis.
9.Risk factors for postoperative mortality within 1 year in more than 90-year-old super advanced age patients with hip fractures

Objective To investigate the 1 year mortality after hip fractures in super advanced age patients and summarize the death associated risk factors in order to provide basis for targeted intervention countermeasures.Methods The clinical data of 332 super advanced age patients with femoral neck or intertrochanteric fractures treated by hip replacement or in-tramedullary femoral nail fixation from January 2015 to January 2023 were retrospectively analyzed.There were 128 males and 204 females with the mean age of(92.2±2.5)years ranging from 90 to 103 years old.Among them,92 cases died within 1 year after surgery.Correlation with the occurrence of death on age,gender,body mass index,fracture type,treatment method,timing of operation,preoperative hemoglobin and serum albumin level,operation time,combined medical diseases,pre-injury mobil-ityand American Society of Anesthesiology(ASA)classification were analyzed.The risk factors of death within 1 year after op-eration were screened by univariate analysis.The results were entered into the multivariate Logistic regression analysis,screen-ing the high risk factors for 1 year mortality after hip fractures.Results The mortality of super advanced age patients with hip fracture within 1 year after surgery accounted for 27.7%(92/332).Univariate analysis showed high body mass index,long in-terval from injury to surgery,low preoperative serum albumin levels,inability to walk independently before injury,accompanied by heart failure,pulmonary infection,obstructive pulmonary disease,stroke,and a higher proportion of ASA grades Ⅲ-Ⅳ.Multivariate Logistic regression analysis showed preoperative serum albumin below 30g g L-1[OR=2.973,95%CI(2.461,5.344),P=0.039],inability to walk independently before injury[OR=3.519,95%CI(2.224,5.413),P=0.018],heart function grade C-D[OR=4.213,95%CI(2.952,6.99),P=0.021],pulmonary infection[OR=3.927,95%CI(2.187,7.731),P=0.016]and ASA Ⅲ-1V[OR=5.124,95%CI(3.092,8.235),P=0.032]were the independent risk factors for death within 1 year in super advanced age patients with hip fractures.Conclusion Preoperative serum albumin below 30g.L-l,poor preinjury ac-tivity,heart function grade C-D,pulmonary infection,and ASA grade Ⅲ-Ⅳ are independent risk factors for postoperative mortality in super advanced age patients with hip fractures.
