1.Mechanism of agomelatine alleviating anxiety-and depression-like behaviors in APP/PS1 transgenic mice

BACKGROUND:Agomelatine is a clinically proven treatment for neuropsychiatric symptoms,such as anxiety and depression.Furthermore,our previous study has demonstrated that agomelatine ameliorates cognitive behaviors,hippocampal synaptic plasticity,and brain pathology in a mouse model of Alzheimer's disease.However,it remains unclear whether agomelatine can improve anxiety and depression-like behaviors in Alzheimer's disease model mice. OBJECTIVE:To investigate the improving effects of agomelatine on anxiety-and depression-like behaviors in APP/PS1 transgenic mice and its underlying molecular mechanisms. METHODS:(1)Eighteen APP/PS1 transgenic mice were randomly divided into model control group(n=9)and model intervention group(n=9).Another wild-type mice were randomized into control group(n=9)and intervention group(n=9).Model intervention group and intervention group were intraperitoneally injected with 10 mg/kg agomelatine per day for 31 continuous days.Behavioral experiments,including the elevated cross maze and forced swimming tests,and mRNA sequencing of the hippocampus were then performed.(2)Mouse hippocampal neuronal cell lines(HT22)and brain microvascular endothelial cell lines(bEnd.3)were cultured and divided into four groups:blank group without any drug,drug group with 20 μmol/L agomelatine,model group with 10 μmol/L β-amyloid 1-42,and experimental group with 10 μmol/L β-amyloid 1-42+20 μmol/L agomelatine.After 24 hours of incubation,protein expression of S416p-tau and S9p-GSK3β in HT22 cells was detected by immunoblotting,and protein expression of low-density lipoprotein receptor-related protein 1 and glycosylation end-product receptor in bEnd.3 cells was detected by immunoblotting. RESULTS AND CONCLUSION:In the elevated plus maze test,the time spent in the open arms(P<0.01)and the entries into open arms(P<0.05)in the mice of model control group were evidently lower than those in the control group,whereas those were obviously increased in the model intervention group compared with the model control group(P<0.05).Forced swimming test results showed that the immobile time exhibited a marked increase in the model control group compared with the control group(P<0.05),but it was significantly decreased in the model intervention group compared with the model control group(P<0.05).Hippocampal tissue mRNA sequencing showed that agomelatine enhanced the expression of low-density lipoprotein receptor-related protein 1 in the hippocampus of APP/PS1 mice.Western blot analysis revealed that the level of S416p-tau in HT22 cells was higher in the model group than the blank group(P<0.05),while it was markedly decreased in the experimental group compared with the model group(P<0.05);the level of S9p-GSK3β in HT22 cells was higher in the drug group than the blank group(P<0.05)as well as higher in the experimental group than the model group(P<0.05).Moreover,the expression of low-density lipoprotein receptor-related protein 1 in bEnd.3 cells was higher in the experimental group than the model group(P<0.05).To conclude,agomelatine can alleviate anxiety-and depression-like behaviors in Alzheimer's disease mice by promoting the clearance of β-amyloid and phosphorylated tau.
2.Signal mining and analysis of adverse events of oseltamivir,zanamivir and baloxavir marboxil

OBJECTIVE To provide reference for safe drug use in clinic by mining the adverse drug events (ADE) of 3 kinds of anti-influenza A virus drugs (oseltamivir, zanamivir, baloxavir marboxil). METHODS The ADE data of oseltamivir, zanamivir and baloxavir marboxil were collected from the FDA adverse event reporting system (FAERS) between the first quarter in 2004 and the third quarter in 2022, and mined by using reporting odds ratio (ROR) method. The designated medical events (DME) were estimated. The system organ class (SOC) in the Medical Dictionary for Regulatory Activities (MedDRA, version 25.0) was used for the classification and statistics of drug ADE terminology. RESULTS A total of 12 636, 1 749 and 1 283 ADE reports were retrieved for oseltamivir, zanamivir and baloxavir marboxil, involving 26, 16 and 17 SOCs, respectively. Oseltamivir was strongly associated with sleep terror, abnormal behavior, hallucination and delirium. Zanamivir was implicated in abnormal behavior, delirium, incoherence, and altered state of consciousness with prominent signal intensity. Baloxavir marboxil was strongly associated with ischemic colitis, hemorrhagic cystitis, erythema multiforme and melaena. Erythema multiform was detected in the DME of three drugs with strong signals. CONCLUSIONS When clinically administering the three drugs, it is crucial to pay close attention to both common adverse reactions and those ADEs that are not explicitly mentioned in the drug instructions. For oseltamivir, clinicians should exercise caution due to the potential risk of acute kidney injury and fulminant hepatitis, necessitating regular monitoring of the patient’s liver and kidney function. When prescribing zanamivir, caution should be exercised due to ADEs related to the respiratory system, including acute respiratory distress syndrome and respiratory failure, necessitating close monitoring of the patient’s respiratory status. Similarly, for baloxavir marboxil, clinicians should be vigilant for potential ADEs such as erythema multiforme and rhabdomyolysis.
3.Clinical efficacy analysis of interventional treatment of iatrogenic massive vaginal bleeding

Objective To investigate the clinical efficacy and value of interventional treatment of iatrogenic massive vaginal bleed-ing.Methods Retrospective analysis was performed on 35 patients with postoperative vaginal massive hemorrhage in obstetrics and gynecology who were admitted.Abdominal aorta and bilateral internal iliac arteries angiography and embolization of abnormal vessels were performed under digital subtraction angiography(DS A),and relevant clinical data were recorded and analyzed.Results After interventional treatment,the vaginal bleeding of 33 patients basically stopped within 3 days,and the average interventional operation time was(57.5±17.2)min.The hemoglobin value,hematocrit and blood pressure decreased and the heart rate increased significantly before and after interventional embolization in obstetrics and gynecology,with statistical significance(P<0.05).There were no sig-nificant changes in hemoglobin value and hematocrit between the completion of interventional embolization and 72 hours after interventional embolization(P>0.05).The increase of blood pressure and the decrease of heart rate were statistically significant(P<0.05).Two patients with cesarean section had poor hemostatic effect after interventional embolization,and the bleeding stopped after exploratory laparotomy and hysterectomy.Conclusion Interventional treatment has the advantages of small trauma,simple operation,signifi-cant curative effect,few adverse reactions,and rapid recovery.It plays an important role and clinical value in the diagnosis and treat-ment of iatrogenic vaginal bleeding.

Cryoablation therapy with explicit anti-tumor mechanisms and histopathological manifestations has a long history.A large number of clinical practice has shown that cryoablation therapy is safe and effective,making it an ideal tumor treatment method in theory.Previously,its efficacy and clinical application were constrained by the limitations of refrigerants and refrigeration equipment.With the development of the new generation of cryoablation equipment represented by argon helium knives,significant progress has been made in refrigeration efficien-cy,ablation range,and precise temperature measurement,greatly promoting the progression of tumor cryoablation technology.This consensus systematically summarizes the mechanism of cryoablation technology,indications for oral mucosal melanoma(OMM)cryotherapy,clinical treatment process,adverse reactions and management,cryotherapy combination therapy,etc.,aiming to provide reference for carrying out the standardized cryoablation therapy of OMM.
5.Clinical Study of Huatan Tongluo Decoction in the Treatment of Post-Stroke Cognitive Impairment Based on BDNF Pathway

OBJECTIVE To observe the clinical efficacy of Huatan Tongluo Decoction in the treatment of post-stroke cognitive impairment(PSCI)with phlegm stasis obstructing collaterals syndrome,and its influence on the serum brain-derived neurotrophic fac-tor(BDNF)pathway-related factors.METHODS Sixty patients who met the inclusion criteria were collected and randomly divided into a control group and a treatment group with 30 patients each.The control group was given basic treatment and nimodipine,while the treatment group was given Huatan Tongluo Decoction on the basis of the treatment in the control group.The treatment course for both groups was 4 weeks.Changes in Mini-Mental State Examination(MMSE),TCM syndrome scores and serum levels of BDNF,nuclear transcription factor κB(NF-κB),B lymphocyte tumor-2(Bcl-2)and Bcl-2-associated X protein(Bax)were compared between the two groups before and after treatment.The TCM clinical efficacy in the two groups of patients was evaluated after treatment,and the oc-currence of adverse reactions was observed during treatment.RESULTS After treatment,the MMSE scores of the patients in the two groups increased significantly,the total TCM syndrome scores decreased significantly(P<0.01),and the treatment group was better than the control group(P<0.01);the serum BDNF,NF-κB,and Bcl-2 levels of the two groups of patients were significantly in-creased(P<0.05,P<0.01),and the Bax level was significantly decreased(P<0.01).The treatment group was better than the control group(P<0.01).CONCLUSION Huatan Tongluo Decoction can improve the clinical symptoms of PSCI patients with phlegm stasis obstructing collaterals,and is safe and effective.Its therapeutic mechanism may be related to regulating the BDNF pathway to protect nerve cells and inhibit nerve cell apoptosis.
6.Therapeutic mechanism of aqueous extract of Semiliquidambar cathayensis Chang root for pancreatic cancer:the active components,therapeutic targets and pathways

Objective To explore the key targets and signaling pathways in the therapeutic mechanism of Semiliquidambar cathayensis Chang(SC)root against pancreatic cancer network pharmacology and molecular docking studies and cell experiments.Methods The targets of SC and pancreatic cancer were predicted using the network pharmacological database,the protein-protein interaction network was constructed,and pathways,functional enrichment and molecular docking analyses were performed.CCK-8 assay was used to test the inhibitory effect of the aqueous extract of SC root on 8 cancer cell lines,and its effects on invasion,migration,proliferation,and apoptosis of pancreatic cancer cells were evaluated.Western blotting was performed to verify the results of network pharmacology analysis.Results We identified a total of 18 active components in SC,which regulated 21 potential key targets in pancreatic cancer.GO and KEGG pathway enrichment analyses showed that these targets were involved mainly in the biological processes including protein phosphorylation,signal transduction,and apoptosis and participated in cancer signaling and PI3K-Akt signaling pathways.Among the 8 cancer cell lines,The aqueous extract of SC root produced the most obvious inhibitory effect in pancreatic cancer cells,and significantly inhibited the invasion,migration,and proliferation and promoted apoptosis of pancreatic cancer Panc-1 cells(P<0.05).Western blotting confirmed that SC significantly inhibited the phosphorylation levels of PI3K and AKT in Panc-1 cells(P<0.001).Conclusion The therapeutic effect of SC root against pancreatic cancer effects is mediated by its multiple components that act on different targets and pathways including the PI3K-Akt pathway.
7.Mining and analysis for ocular adverse event signals induced by ALK inhibitors based on FAERS database

Objective To provide references for clinical safe medication by mining and analyzing signals of ocular adverse events(ADE)related to anaplastic lymphoma kinase(ALK)inhibitors.Methods The data from the third quarter in 2011 to the first quarter in 2024 were downloaded from the U.S.Food and Drug Administration Adverse Event Reporting System(FAERS),ocular ADEs associated with ALK inhibitors reports were extracted.The suspicious risk signals were mined and analyzed by reporting odds ratio(ROR)and information component(IC)method.The median occurrence time of ocular ADE was analyzed,and Weibull shape parameter test was used to analyze the relationship between ADE occurrence time and ALK inhibitor treatment time.Results A total of 1 575 reports of ALK inhibitor-related ocular ADEs were collected,including 1 107 reports for crizotinib,50 reports for ceritinib,158 reports for alectinib,110 reports for brigatinib and 150 reports for lorlatinib.The proportion of female patients was higher(46.29%),and the main age distribution was between 18 and less than 65 years old(35.17%).No risk signal was detected for ceritinib.13 ADE signals were obtained for crizotinib,alectinib,brigatinib and lorlatinib.Crizotinib ranked first in the number of ADE reports and positive signals,and the signal intensity of crizotinib-induced photopsia was the highest(ROR=43.46,95%CI 36.38 to 51.91;IC=5.18,95%CI 4.89 to 5.40).The median time to onset of most ocular ADEs was within one month of medication initiation,and the median time to onset of blindness caused by ALK inhibitors was the longest at 154.00(114.50,225.50)days.The visual impairment,vision blurred,photopsia,vitreous floatres and diplopia often occurred in the early stage of medication.The photophobia,visual field defect and blindness occurred randomly and did not change with treatment time.Conclusion The risk of ocular ADEs was different in different ALK inhibitors,most of which occurred in the early stage of the treatment.The ocular toxicity of ALK inhibitors should be recognized and treated in time for clinical application.
8.Simultaneous determination of primary volatile components in Tibetan medicine Bawei Chenxiang powders by GC

Objective To establish a GC method for simultaneous determination of volatile index components of octyl acetate,benzylacetone,myristicin and dehydrocostus lactone in Tibetan medicine Bawei Chenxiang powders.Methods A GC method was used with octyl acetate,benzylacetone,myristicin and dehydrocostus lactone as indicator components,SH-WAX capillary column(30 m×0.25 mm,0.25 μm)and temperature programming were used,the carrier gas was nitrogen,the inlet temperature was 220 ℃,the detector temperature was 240 ℃,with split injection volume of 1 μL.Results The concentrations of octyl acetate,benzylacetone,myristicin and dehydrocostus lactone showed a good linear relationship with peak area(r>0.999 5)in the range of 12.44-124.43 μg·mL-1,2.90-29.01 μg·mL-1,15.95-159.45 μg·mL-1,15.62-156.15 μg·mL-1,respectively;the average recovery rates were 100.40%(RSD=1.55%),97.80%(RSD=1.41%),99.50%(RSD=0.77%)and 99.50%(RSD=0.85%)(n=6).Conclusion The method has good specificity,precision,repeatability and accuracy,and can be used for the determination of volatile index components in Bawei Chenxiang powders.

Objective To investigate the distribution and antimicrobial resistance profiles of the common pathogens isolated from urine from 2015 to 2021 in the CHINET Antimicrobial Resistance Surveillance Program.Methods The bacterial strains were isolated from urine and identified routinely in 51 hospitals across China in the CHINET Antimicrobial Resistance Surveillance Program from 2015 to 2021.Antimicrobial susceptibility was determined by Kirby-Bauer method,automatic microbiological analysis system and E-test according to the unified protocol.Results A total of 261 893 nonduplicate strains were isolated from urine specimen from 2015 to 2021,of which gram-positive bacteria accounted for 23.8%(62 219/261 893),and gram-negative bacteria 76.2%(199 674/261 893).The most common species were E.coli(46.7%),E.faecium(10.4%),K.pneumoniae(9.8%),E.faecalis(8.7%),P.mirabilis(3.5%),P.aeruginosa(3.4%),SS.agalactiae(2.6%),and E.cloacae(2.1%).The strains were more frequently isolated from inpatients versus outpatients and emergency patients,from females versus males,and from adults versus children.The prevalence of ESBLs-producing strains in E.coli,K.pneumoniae and P.mirabilis was 53.2%,52.8%and 37.0%,respectively.The prevalence of carbapenem-resistant strains in E.coli,K.pneumoniae,P.aeruginosa and A.baumannii was 1.7%,18.5%,16.4%,and 40.3%,respectively.Lower than 10%of the E.faecalis isolates were resistant to ampicillin,nitrofurantoin,linezolid,vancomycin,teicoplanin and fosfomycin.More than 90%of the E.faecium isolates were ressitant to ampicillin,levofloxacin and erythromycin.The percentage of strains resistant to vancomycin,linezolid or teicoplanin was<2%.The E.coli,K.pneumoniae,P.aeruginosa and A.baumannii strains isolated from ICU inpatients showed significantly higher resistance rates than the corresponding strains isolated from outpatients and non-ICU inpatients.Conclusions E.coli,Enterococcus and K.pneumoniae are the most common pathogens in urinary tract infection.The bacterial species and antimicrobial resistance of urinary isolates vary with different populations.More attention should be paid to antimicrobial resistance surveillance and reduce the irrational use of antimicrobial agents.

Objective To understand the distribution and changing resistance profiles of clinical isolates of Enterococcus in hospitals across China from 2015 to 2021.Methods Antimicrobial susceptibility testing was conducted for the clinical isolates of Enterococcus according to the unified protocol of CHINET program by automated systems,Kirby-Bauer method,or E-test strip.The results were interpreted according to the Clinical & Laboratory Standards Institute(CLSI)breakpoints in 2021.WHONET 5.6 software was used for statistical analysis.Results A total of 124 565 strains of Enterococcus were isolated during the 7-year period,mainly including Enterococcus faecalis(50.7%)and Enterococcus faecalis(41.5%).The strains were mainly isolated from urinary tract specimens(46.9%±2.6%),and primarily from the patients in the department of internal medicine,surgery and ICU.E.faecium and E.faecalis strains showed low level resistance rate to vancomycin,teicoplanin and linezolid(≤3.6%).The prevalence of vancomycin-resistant E.faecalis and E.faecium was 0.1%and 1.3%,respectively.The prevalence of linezolid-resistant E.faecalis increased from 0.7%in 2015 to 3.4%in 2021,while the prevalence of linezolid-resistant E.faecium was 0.3%.Conclusions The clinical isolates of Enterococcus were still highly susceptible to vancomycin,teicoplanin,and linezolid,evidenced by a low resistance rate.However,the prevalence of linezolid-resistant E.faecalis was increasing during the 7-year period.It is necessary to strengthen antimicrobial resistance surveillance to effectively identify the emergence of antibiotic-resistant bacteria and curb the spread of resistant pathogens.
