1.Micromorphological characteristics of the pedicle of the lower cervical vertebra

BACKGROUND:The lower cervical vertebral pedicle is the main stress site of the posterior column of the spine,which is of great significance for the maintenance of the stability of the human center of gravity and the reduction of shock.At present,there are few reports on the characteristics of the internal bone trabeculae,and the characteristics of the joint site of the vertebral pedicle with the articular process and the vertebral body.It is urgent to understand the fine anatomical structure of the vertebral pedicle and the relationship and function of each part. OBJECTIVE:To observe the microanatomical morphology of the vertebral pedicle by Micro-CT scanning of cervical vertebra specimens,and to measure and analyze the microstructure and morphometric parameters of the bone trabecula in the cervical pedicle under normal conditions to evaluate the safety performance of the cervical spine. METHODS:Micro-CT scanning was performed on 31 sets of cervical vertebrae C3-C7.By checking and reconstructing the areas of interest in the bone trabecular within the vertebral pedicle,the morphological characteristics and distribution direction of the bone trabecular within the cervical pedicle were observed,and the bone microstructure parameters were detected,and the differences in the bone microstructure of the C3-C7 vertebral pedicle were analyzed and compared. RESULTS AND CONCLUSION:(1)The Micro-CT images showed that the honeycomb bone trabeculae of the pedicle of the lower cervical spine presented a complex network of microstructures.The trabeculae near the cortical bone were lamellar and relatively compact,extending forward toward the vertebral body and backward toward the articular process lamina.Abatoid bone trabeculae extended into the medullary cavity and transformed into a network structure,and then into rod-shaped bone trabeculae.The rod-shaped bone trabeculae were sparsely distributed in the medullary cavity.(2)Statistical results of morphological parameters of bone trabeculae showed that bone volume fraction values in C4 and C5 were higher than that in C7(P<0.05).The bone surface/bone volume value in C7 was higher than that in C3,C4 and C6(P<0.05).The bone surface density of bone trabeculae in C7 was higher than that in C3,C4,C5 and C6(P<0.05).Trabecular thickness in C7 was higher than that in C3,C4 and C5(P<0.05).Bone surface/bone volume and bone surface density of the left pedicle bone trabecular were greater than those on the right side(P<0.05).(3)The microstructural changes of C3-C7 were summarized,in which the load capacity and stress of the C7 pedicle were poor,and the risk of injury was high in this area.
2.Characteristics and significance of age-related changes in cervical uncinate process-related angle

BACKGROUND:As a unique structure of the cervical spine,the occurrence,development and progression of the uncovertebral joint directly affect the stability and range of motion of the cervical spine,and are also closely related to the pathogenesis of cervical spondylosis.A thorough understanding of the developmental characteristics of the uncovertebral joint is of great significance for the pathogenesis,diagnosis,and treatment of cervical spondylosis. OBJECTIVE:By using imaging and three-dimensional reconstruction technology to measure and observe the cervical uncinate process-related angle in a large sample of different age groups,the aim is to reveal the characteristics of its changes with age and vertebral growth,as well as its relationship with cervical spine stability. METHODS:Using a retrospective research design,we collected 1 447 cases of raw CT imaging data that meet the study requirements for complete cervical spine segments.The raw data were imported into Mimics 21.0 software in DICOM format for post-processing and measurement of angle of uncinate process and sagittal angle of uncinate process.The data were grouped based on gender,age,and side. RESULTS AND CONCLUSION:(1)With the increase of vertebral sequence,the angle of uncinate process increased in a V-shaped shape,and the lowest peak was at C5.The overall population showed a sharp peak with the increase of age,and the peak value mostly occurred in the age range of 30-39 years.(2)The sagittal angle of the uncinate process increased like a fishhook with the increase of the vertebral sequence,and the overall angle of the uncinate process increased with age,and the peak value mostly occurred in the age range of 20-29 years.The uncinate process angle and sagittal angle showed only partial significant differences between sides and genders(P<0.05).(3)It is concluded that the angle of the uncinate process increased with the increase of vertebral sequence in a V-shaped manner.The sagittal angle of the uncinate process increases like a fish hook with increasing vertebral order,while the two angles generally peak with increasing age.The angle of the uncinate process is about 131°,which may be closely related to the stability of the cervical spine,while the sagittal angle of the uncinate process is about 14°,and its function may play a certain role in limiting the excessive rotation of the cervical spine.
3.Reconstruction and analysis of K-Clip surgery process based on finite element method

Objective To investigate the effects of different types of tricuspid regurgitation, implantation positions, and device models on the treatment outcomes of K-Clip for tricuspid regurgitation using numerical simulations. Methods Three-dimensional reconstruction of the heart model was performed based on CT images. Two different regurgitation orifices were obtained by modifying the standard parameterized tricuspid valve leaflets and chordae tendineae. The effects of different K-Clip models at different implantation positions (posterior leaflet midpoint, anterior-posterior commissure, anterior leaflet midpoint, posterior septal commissure) were simulated using commercial explicit dynamics software Ls-Dyna. Conclusion For the two types of regurgitation in this study, clipping at the posterior leaflet midpoint resulted in a better reduction of the regurgitation orifice (up to 75% reduction in area). Higher clamping forces were required for implantation at the anterior leaflet midpoint and posterior septal commissure, which was unfavorable for the smooth closure of the clipping components. There was no statistical difference in the treatment outcomes between the 18T and 16T K-Clip components, and the 16T component required less clamping force. Therefore, the use of the 16T K-Clip component is recommended.
4.Construction and reflection on the selection system for medical consumables in the context of high-qual-ity development of public hospitals

Objective To establish a selection system for medical consumables that identifies high-quality,cost-effective products meeting hospital and operational development needs while ensuring patient satisfaction,thus promoting the scientific and rational use of medical consumables in clinical settings.Methods The current status and challenges of medical consumables se-lection management in healthcare institutions were analyzed.The approach involved clarifying overall requirements,determining management structure and regulations,establishing standardized selection processes,choosing appropriate categories for selec-tion,and setting selection criteria.This system was built upon a medical consumables procurement transaction platform and lever-aged the SPD model to create a scientifically sound and comprehensive selection system for medical consumables.Results The number of similar consumable products was reduced,with an average price reduction of over 14.63%,and clinical department satisfaction reached 90%.Conclusion Constructing a scientifically sound selection system for medical consumables helps to lower patient expenses and hospital costs,improve service quality,and align with the requirements for high-quality development in public hospitals.
5.Outcomes of using long head of biceps tendon transposition to augment arthroscopic massive rotator cuff repair

Objective:To investigate the outcome of using long head of biceps tendon (LHBT) transposition to augment arthroscopic massive rotator cuff repair.Methods:A retrospective case series study was performed on 22 patients with massive rotator cuff tear treated in Zhongshan Hospital, Xiamen University from June 2019 through July 2020, including 12 males and 10 females, aged 54-79 years [(63.9±6.8)years]. LHBT transposition was performed to augment arthroscopic repair of massive rotator cuff tear. The active range of motion (forward flexion, abduction, external rotation), visual analog scale (VAS), University of California Los Angeles (UCLA) score and American Shoulder and Elbow Surgeons (ASES) score were compared preoperatively and at 3 months and 12 months postoperatively. The cuff integrity was evaluated using MRI following Sugaya classification at 12 months postoperatively. LHBT dislocation or distal retraction was recorded at the last follow-up.Results:All patients were followed up for 12-24 months [(17.0±3.8)months]. The postoperative 3-month active forward flexion [162.5(160.0, 170.0)°] and abduction [170.0(160.0, 170.0)°] were improved compared with preoperative measurements [90.0(73.8, 120.0)°,85.0(70.0, 112.5)°](all P<0.05). However, no statistically significant difference was found between the preoperative and postoperative 3-month external rotation [50.0(37.5,60.0)° vs. 60.0(48.8,70.0)°] ( P>0.05). The postoperative 12-month active forward flexion, abduction and external rotation were 170.0(160.0, 175.0)°, 170.0(170.0, 177.8)° and 60.0(48.8, 70.0)°, showing no significant improvement from those at 3 months postoperatively (all P>0.05). The postoperative 3-month VAS [1.0(0.8, 2.0)points], UCLA score [23.0(23.0, 25.0)points] and ASES score [79.1(72.9, 83.3)points] were improved significantly compared with preoperative measurements [7.0(8.0, 9.0)points, 9.0(10.0, 14.0)points, 25.0(16.6, 31.6)points] (all P<0.05). The postoperative 12-month UCLA score [33.0(31.0, 35.0)points] and ASES score [91.6(86.6, 93.3)points] were further improved compared with those at 3 months postoperatively (all P<0.05). However, the postoperative 12-month VAS [0.0 (0.0, 1.0)points] showed no statistically significant difference with that at 3 months postoperatively ( P>0.05). The UCLA score was excellent in 6 patients and good in 16 at 12 months postoperatively.MRI revealed healed tendons with continuity in 16 patients, with the healing rate of 72.7%, and partially retears with good shoulder function in 6 patients, with the retearing rate of 17.3%. No LHBT dislocation or distal retraction was found at 12 months postoperatively in regardless of mild anterior shoulder pain in 2 patients. Conclusion:Using LHBT transposition to augment arthroscopic massive rotator cuff repair has yielded excellent shoulder range of motion, shoulder function recovery, pain relief and high tendon healing rate with rare postoperative complication.
6.Research on the protective effect of hearing protective device for workers exposed to noise in a motor manufacturing enterprise

Objective:To understand the current situation of noise hazard in a motor manufacturing enterprise, and to explore the protective effect of workers wearing hearing protective device and its possible influencing factors.Methods:In November 2021, a total of 179 noise workers wearing hearing protective devices in a motor manufacturing company in a city were selected as research objects. Personal attenuation rating (PAR) of workers wearing hearing protective devices was measured. Baseline PAR was analyzed for different subgroups of basic demographic information, noise exposure, and the use of hearing protective devices to evaluate the effect of the intervention. Baseline PAR was compared using nonparametric tests.Results:There were 179 workers from 35 positions in 4 types of work, and the over-standard noise rate was 51.2% (42/82), among which the noise exposure intensity of motor equipment debugging workers was the highest [94.4 dB (A) ]. Compared the baseline PAR of different characteristics, it was found that the baseline PAR of male workers, workers whose daily noise exposure time were <8 h, workers who had used the hearing protective devices for 10 to 14 years, and workers who thought the hearing protective devices were comfortable were all higher, and the differences were statistically significant ( P<0.05). Baseline PAR passing rate was 43.0% (77/179), and PAR of 102 workers who did not pass baseline test increased from 0 (0, 3) dB before intervention to 14 (12, 16) dB after intervention, with statistical significance ( P<0.05) . Conclusion:The noise hazard in this motor manufacturing enterprise is serious, and the protective effect of workers wearing hearing protective devices is not good. Gender, daily noise exposure time, years and comfort of wearing hearing protective device are the possible influencing factors of poor protective effect.

8.Uphold the original intention and practice the mission: 2022 Fudan university public health doctorate academic forum

The 2022 Fudan university doctorate academic public health forum hosted by the graduate school of Fudan university, the graduate work department of the party committee of Fudan university, and the school of public health of Fudan university was successfully held on December 10, 2022 in the school of public health of Fudan university. In the early stage, a total of 53 manuscripts from nationwide universities and research institutes were received, and 10 were selected as excellent papers to participate in the forum report. More than 100 teachers and students attended the forum. Focusing on the theme of " Adhering to the original ambition of scientific research to serve the country and practicing the mission of public health youth", the major contents of the forums included excellent paper reports in the morning and round-table discussion in the afternoon. Experts and students conducted discussions and communications on prevention, control and management of public health emergencies. It is of great benefit for students to uphold the original intention,practice the mission and further improve public health research.
9.The effect of hypoglycemic drugs on bone metabolism and dental implantation in type 2 diabetes mellitus patients

Patients with type 2 diabetes mellitus (T2DM) have a large demand for dental implants, but the pathologic state of T2DM patients could compromise the efficacy of implant treatment. Glycemic control can improve the success rate of implants in the T2DM population, but the early osseointegration of individuals still needs to be improved. Strengthening early osseointegration in patients with T2DM is one of the urgent problems for clinicians. The pharmacological mechanisms of hypoglycemic drugs on the market for bone metabolism are different and may require different interventions on the bone around the implant, but there is a lack of direct clinical evidence of the protective effect of hypoglycemic drugs. This review integrated the bone metabolic effect of drugs in clinical medical research and dental implant research. The aim was to provide medication guidance for T2DM patients who require implant surgery, and it is recommended to avoid the use of drugs with negative effects on bone as far as possible without violating the clinical medication guidelines, including SGLT-2 inhibitors and thiazolidinediones. Instead, they should choose glucose-lowering drugs that are beneficial to bone metabolism, such as insulin, metformin and GLP-1 receptor agonists. However, the comparative clinical effects of these drugs on periimplant bone need to be further elucidated. The researcher should select appropriate drugs (incretin drugs) to enhance the early osseointegration of implants in patients with T2DM.
10.Research progress of superhydrophilic implants

Through a review of the literature on surface treatment of superhydrophilic implants and its clinical application, this paper discusses the shortening of load time, the improvement of the planting success rate and its long-term effect. Additionally, attention should be paid to the nonindication of superhydrophilic implants and issues requiring attention. The literature review showed that healthy patients could carry out an early load 21 days after implantation of superhydrophilic implants, and the load could be completed as soon as 6 weeks after implantation with superhydrophilic short implants when the residual alveolar bone height of the posterior dental area was repaired. Even if the residual alveolar bone density of the patient is low, the application of superhydrophilic implants can shorten the healing period to 8 weeks. Notably, some studies have reported that superhydrophilic implants have no significant effect on patients with a history of radiotherapy and the use of anticoagulants. Because the adhesion of the superhydrophilic implant to the bacteria is also improved to some extent, it is very important to prevent the use of antibiotics when using the superhydrophilic implant. Finally, this paper discusses and anticipates the future research direction of superhydrophilic implants: longer periodic follow-up and more in-depth molecular mechanism studies.
