1.Correlation between serum uric acid/high density lipoprotein cholesterol ratio and type 2 diabetic peripheral neuropathy

Objective To explore the correlation between blood uric acid/HDL-C ratio(UHR)and peripheral neuropathy(DPN)in T2DM.Methods A total of 127 T2DM patients admitted to the Endocrinology Department of Wujin Traditional Chinese Medicine Hospital in Changzhou City from August 2022 to August 2023 were selected.They were divided into a simple T2DM group(n=62)and a combined DPN group(DPN,n=65)based on whether or not they had DPN.Compare two groups of general information,biochemical indicators,and UHR.Results Compared with the T2DM group,DPN group DM course of disease,HbA1c,FPG,FIns,HOMA-IR,TG,vibration sensation threshold(VPT),hypersensitive C-reactive protein(hs-CRP),blood uric acid(SUA),and UHR(P<0.05),HDL-C,tibial nerve motor nerve conduction velocity(mNCV),and superficial peroneal nerve sensory nerve conduction velocity (sNCV)decreased(P<0. 05). Spearman correlation analysis showed that UHR was positively DM duration of disease,HbA1c,FPG,HOMA?IR,TG,VPT,hs?CRP,and SUA(P<0. 05),negatively correlated with mNCV,sNCV,and HDL?C(P<0. 05). Logistic regression analysis showed that UHR,DM duration, hs?CRP,and HbA1c were the influencing factors of DPN. Conclusion Elevated UHR is a influencing factor for the occurrence of DPN in T2DM patients and has good predictive value for DPN.

Gastrointestinal complications after cardiac surgery are rare, but they are associated with significant morbidity and mortality. The mechanisms of gastrointestinal complications after cardiac surgery may be unique, as the abdominal cavity is not involved. This review summarizes the current evidence of the pathophysiology, clinical manifestations, risk factors, and management of gastrointestinal complications after cardiac surgery, aiming to improve the recognition of gastrointestinal complications after cardiac surgery.

We present a 46 years old female patient who was dignosed with Ebstein's anomaly, then suffered from cardiac arrest because of LVOTO secondary to cone reconstruction in ICU.

Objective:Guiding hospitals to strengthen the emphasis on clinical medical research, promote hospital discipline construction, to improve the quality of medical services and clinical innovation capacities.Methods:Starting from different user groups, combines the experience of the knowledge base of the library construction institution of Grade-A Tertiary Hospital and the cooperation experience with various functional departments, this paper analyzes the functional structure of the institutional repository according to the problems faced in the actual work to explore the difficulties during the construction process.Results:The construction of the Institutional Repository of hospitals for different users is imminent. In addition to the use of Institutional Repository to improve the utilization of intelligence and information resources, the construction of institutional repository on the evaluation of science and technology influence can also promote the development of hospital disciplines.Conclusions:A solution method is proposed, based on the background of scientific and technological influence evaluation, the future construction direction of the institutional repository.
5.Effect of bilateral vagal nerves transection on the inflammatory reaction of rats induced by lung ischemia and reperfusion

Objective To explore the effect of bilateral vagal nerves transection on the inflammatory reaction of rats induced by lung ischemia and reperfusion.Methods Twenty-four adult male SD rats weighed 250-320g were randomly divided into 3 groups (8 each): Sham group (S group), ischemia reperfusion group (IR group) and bilateral vagal nerves transaction and ischemia-reperfusion group (NIR group). Blood gas analysis with arterial blood was performed to detect the arterial partial pressure of oxygen (PaO2) and arterial-alveolar oxygen pressure gradient (A-aDO2) before ischemia and 0.5 and 4 hours after reperfusion. At the end of the experiment, the left lung tissues were collected to measure wet/dry ratio, the pathological changes were observed by light microscopy, and inflammatory indicators, including tumor necrosis factor-α (TNF-α), interleukin-10 (IL-10) levels and myeloperoxidase (MPO) activity were detected.Results Compared with S group, ischemia and reperfusion injury sharply decreased PaO2 and increased A-aDO2; the MPO activity, the contents of TNF-α and IL-10 and the wet/dry ratio obviously increased in IR group; more infiltration of inflammatory cells, hemorrhage, thickening and swelling of alveolar walls were observed. Compared with IR group, PaO2 further decreased while A-aDO2 had no obvious change in NIR group, the inflammatory reaction and pathologic lesions in lung tissues were getting worse.ConclusionBilateral vagal nerves transaction may aggravate lung ischemia and reperfusion injury, suggesting that the integrity of vagal nerves play a significant role in regulating inflammatory reaction of lung ischemia and reperfusion injury in rats.
6.Dynamic analysis of treatment of advanced schistosomiasis in Qianjiang City

Objective To understand the dynamics of the treatment and outcomes of advanced schistosomiasis patients in Qianjiang City so as to provide the evidence for improving the future work. Methods All advanced schistosomiasis patients were investigated in Qianjiang City during the period of 2009-2013. Results Totally 1 284 person?times of advanced schistoso?miasis patients received the treatment. The rate of clinical improved was 89.88% and 82 patients were cured. The type of asci?tes was the majority and the megalosplenia type was the second. Conclusion The prevalence of advanced schistosomiasis showed a decreasing tendency but the incidence of new advanced schistosomiasis patients showed a slightly increasing.
7.Prospective randomized controlled study between gastrodin and remifentanil for controlled hypotension in endoscopic sinus surgery

Objective To compare the changes in postoperative pain, complications and clinical effect between gastrodin and remifentanil for controlled hypotension in endoscopic sinus surgery.Methods Eighty patients undergoing endoscopic sinus surgery were enrolled in the study.Patients were randomized into two groups of 40 to receive gastrodin (Group G) or remifentanil (Group R) infusion.Two groups were administered with propofol of 2 mg/kg to induce anaesthesia, which was maintained using sevoflurane.Group R received a continuous infusion of remifentanil titrated between 3 and 4 ng/mL using target-controlled infusion, whereas Group G received an i.v.gastrodin bolus of 2 mg/kg followed by a 20 mg/kg continuous infusion to maintain a mean arterial pressure ( MAP) between 60 and 70 mmHg.Haemodynamic variables, surgical conditions, postoperative pain, and adverse effects, such as postoperative nausea and vomiting ( PONV) and shivering were recorded.Results Controlled hypotension was well maintained in both groups.MAP and heart rate were higher in Group R than Group G after operation(P<0.05).Surgical conditions scores were not different between two groups.Postoperative pain scores were significantly lower in Group G than Group R (P<0.05).Shivering and the use of antiemetic and analgesic drugs were also less frequent in Group G than Group R (P<0.05).Conclusions Both gastrodin and remifentanil when combined with sevoflurane provides adequate controlled hypotension and proper surgical conditions for endoscopic sinus surgery.However, patient administered gastrodin has a more favourable postoperative course with better analgesia and less shivering and PONV.

Objective To investigate whether monocyte chemoattractant protein-1 mediates lupus nephritis (LN) in BXSB mice and whether methylprednisolone ameliorates LN by inhibiting MCP-1 expression. Methods 18-week-old male BXSB mice (n=6) displaying clinical symptoms of glomerulonephritis were treated (i.p.) for 3 weeks with MPS (25 mg?kg-1?d-1) dissolved in N.S. BXSB controls were age-and sex-matched BXSB mice (n=6) that received N.S. alone. Age- and sex-matched BALB/C mice (n=6) were used as normal controls that were treated in the same way as the BXSB controls. MCP-1 expression was investigated by RT-PCR and immunohistochemical examination. The heaviness of proteinuria was also evaluated. Medical imaging analysis was performed to detect the relationship between MCP-1 expression and proteinuria. Results MCP-1 was strongly expressed in kidneys of all BXSB mice, stronger staining was found in cytoplasm of tubular epithelial cells than glomerular, while no expression was found in kidneys of BALB/C mice. MCP-1 expression in tubular cells in BXSB control group was closely correlated with proteinuria. MPS significantly reduced proteinuria and MCP-1 expression and down-regulation of MCP-1 expression in tubular cells was also closely correlated with that of proteinuria. Conclusion MCP-1 over-expression may mediate LN in BXSB. MPS ameliorates LN partially by inhibiting MCP-1 expression.
