1.Construction of the quality evaluation scale of specification of management for humanistic caring in outpatients and its reliability and validity testing

Objective:To construct the quality evaluation scale of specification of management for humanistic caring in outpatients and test its reliability and validity.Methods:Referring to the group standards in Specification of Management for Humanistic Caring in Outpatients released by the China Association for Life Care,as well as relevant guidelines and literature,a pool of items for the quality evaluation scale of specification of management for humanistic caring in outpatients was formed.After expert consultation and expert argumentation,a quality evaluation scale of specification of management for humanistic caring in outpatients was constructed.From January to February 2024,243 hospital managers from 5 hospitals in Zhengzhou were selected as survey subjects to conduct item analysis,and reliability and validity testing on the scale.Results:Two rounds of expert inquiry and two rounds of expert argumentation were conducted,with questionnaire response rates of 92.00%and 100.00%,respectively,and expert authority coefficients of 0.952.In the second round of the expert inquiry scale,the mean importance score of the first-level indicators was 4.80 to 5.00,the full score ratio was 88.00%to 100.00%,the coefficient of variation was 0.04 to 0.17,and Kendall's coefficient of concordance was 0.857(P<0.001);the mean importance score of the second-level indicators was 4.60 to 5.00,the full score ratio was 80.00%to 100.00%,the coefficient of variation was 0.00 to 0.21,and Kendall's coefficient of concordance was 0.775(P<0.001);the mean importance score of the third-level indicators was 4.60 to 5.00,the full score ratio was 76.00%to 100.00%,the coefficient of variation was 0.00 to 0.21,and Kendall's coefficient of concordance was 0.830(P<0.001).Finally,a quality evaluation scale of specification of management for humanistic caring in outpatients was formed,including 5 first-level indicators,25 second-level indicators,and 58 third-level indicators.Exploratory factor analysis produced 5 common factors with a cumulative variance contribution rate of 74.628%.The Pearson correlation coefficients between the five-factor scores ranged from 0.648 to 0.798,and the correlation coefficients between the factor scores and the total score of the scale ranged from 0.784 to 0.938.The scale-level content validity index(S-CVI)of the scale was 0.945,the item-content validity index(I-CVI)was 0.725 to 1.000,the Cronbach's alpha coefficient of the total scale was 0.973,and the retest reliability coefficient was 0.934.Conclusion:The constructed quality evaluation scale of specification of management for humanistic caring in outpatients has good scientific validity and reliability,and can be used as an evaluation tool for specification of management for humanistic caring in outpatients.
2.Research advances of membranous nephropathy-associated antigens

The identification of pathogenic antigens in membranous nephropathy (MN) is a hot topic in the research field of kidney diseases. In recent years, the widespread application of mass spectrometry has brought a breakthrough in the identification of MN-pathogenic antigens. As the antigen spectrum continues to be refined, the diagnosis of MN has evolved from morphological level to molecular level. This article reviewed the research progress of currently identified antigens of MN, such as phospholipase A2 receptor (a major pathogenic antigen of primary MN), thrombospondin type 1 domain-containing 7A (a potential tumor-associated antigen), neural epidermal growth factor-like protein 1 (an antigen associated with various secondary factors), semaphorin 3B (an antigen specific to pediatric MN) and so on, and the pathogenic mechanisms and clinical significance of these antigens.

Mismatch repair-deficient/microsatellite instability-high(dMMR/MSI-H)gastric cancer represents a distinct molecular subtype of tumors characterized by pronounced sensitivity to immune checkpoint inhibitors(ICIs)attributed to its unique immune microenvironment and elevated mutation burden.Various clinical studies underscore the efficacy of ICIs in treating dMMR/MSI-H gastric cancer;however,chal-lenges such as primary and acquired resistance persist.Overcoming resistance and identifying optimal ICIs for its treatment remain critical clinical issues.This review delineates the mechanisms of ICIs,recent advances in their therapeutic application for dMMR/MSI-H gastric can-cer,and ongoing challenges in combating resistance,aiming to guide clinical practice effectively.
4.Prediction of risk of in-hospital death in patients with chronic heart failure complicated by lung infections using interpretable machine learning

Objective To predict the risk of in-hospital death in patients with chronic heart failure(CHF)complicated by lung infections using interpretable machine learning.Methods The clinical data of 1415 patients diagnosed with CHF complicated by lung infections were obtained from the MIMIC-IV database.According to the pathogen type,the patients were categorized into bacterial pneumonia and non-bacterial pneumonia groups,and their risks of in-hospital death were compared using Kaplan-Meier survival curves.Univariate analysis and LASSO regression were used to select the features for constructing LR,AdaBoost,XGBoost,and LightGBM models,and their performance was compared in terms of accuracy,precision,F1 value,and AUC.External validation of the models was performed using the data from eICU-CRD database.SHAP algorithm was applied for interpretive analysis of XGBoost model.Results Among the 4 constructed models,the XGBoost model showed the highest accuracy and F1 value for predicting the risk of in-hospital death in CHF patients with lung infections in the training set.In the external test set,the XGBoost model had an AUC of 0.691(95%CI:0.654-0.720)in bacterial pneumonia group and an AUC of 0.725(95%CI:0.577-0.782)in non-bacterial pneumonia group,and showed better predictive ability and stability than the other models.Conclusion The overall performance of the XGBoost model is superior to the other 3 models for predicting the risk of in-hospital death in CHF patients with lung infections.The SHAP algorithm provides a clear interpretation of the model to facilitate decision-making in clinical settings.

Outpatient humanistic care refered to providing a full process of caring medical services to outpatients. In order to standardize the human caring services for outpatients in medical institutions, promote the comprehensive service level of outpatient services, and improve the patient′s medical experience, Chinese Association for Life Care issued the group standard of Management Norms for Human caring of Outpatients in April 2023. This standard clarified the relevant terms and definitions of human caring for outpatients, specified the basic requirements for human caring, the humanistic quality and care responsibilities of outpatient staff, the outpatient care environment and facilities, the outpatient care process and measures, and quality management. It designed standardized and personalized full process care service norms, providing references for medical institutions at all levels to promote the development of human caring for outpatients.
7.Effect and Mechanism of Nongsuo Dangguiwan in Relieving Oxidative Stress in Premature Ovarian Insufficiency Rats Based on Nrf2/HO-1 Signaling Pathway

ObjectiveTo investigate the effect and mechanism of Nongsuo Dangguiwan in improving ovarian oxidative stress in rats with ovarian dysfunction. MethodThirty-six adult female SD rats were randomly divided into normal group, model group, positive drug group (Femoston, 0.3 mg·kg-1), and high-, medium-, and low-dose groups of concentrated Nongsuo Dangguiwan (2.08, 4.16, 8.32 g·kg-1), with six rats in each group. Rats, except for those in the normal group, were injected with 80 mg·kg-1 vinyl cyclohexene dioxide (VCD) per day for 14 consecutive days to induce ovarian dysfunction. From the 15th day, rats were treated with corresponding drugs by gavage, while those in the model group received 2 mL·kg-1 saline, once daily for 28 consecutive days. The ovarian index, levels of related hormones including estradiol (E2), follicle-stimulating hormone (FSH), luteinizing hormone (LH), and anti-mullerian hormone (AMH) in serum, as well as superoxide dismutase (SOD) activity and glutathione (GSH) content in serum were measured by enzyme-linked immunosorbent assay (ELISA). The malondialdehyde (MDA) content in serum was detected by the thiobarbituric acid (TAB) method. Ovarian morphology was observed by hematoxylin-eosin (HE) staining. The expression of nuclear factor E2-related factor 2 (Nrf2), heme oxygenase-1 (HO-1), SOD2, and SOD1 in ovarian tissues was detected by immunohistochemistry (IHC) and Western blot. ResultCompared with the normal group, the model group showed a significant reduction in growing follicles in the ovary, loose arrangement of granulosa cells in the follicle, decreased body weight, ovarian index, and serum AMH and E2 levels, increased LH and FSH levels (P<0.01), reduced levels of SOD and GSH in serum (P<0.01), and increased MDA level (P<0.01). Compared with the model group, the groups with drug intervention showed increased ovarian index (P<0.05, P<0.01), increased serum E2 level (P<0.05, P<0.01), decreased FSH, AMH, and LH levels (P<0.05, P<0.01), increased number of growing follicles in the ovary, potentiated SOD activity in serum, increased GSH content, decreased MDA content (P<0.05, P<0.01), and up-regulated expression levels of Nrf2, HO-1, SOD2, and SOD1 proteins in ovarian tissues (P<0.05, P<0.01). ConclusionNongsuo Dangguiwan can regulate serum hormone levels, increase the expression of Nrf2, HO-1, SOD2, and SOD1 in ovarian tissues, and improve ovarian antioxidant capacity to resist oxidative stress injury, thereby improving ovarian reserve function.
8.Effect of Jianpi Yishen Huazhuo Prescription on Ovarian Function in Rats with Polycystic Ovary Syndrome Based on NLRP3 Inflammatory Pathway

ObjectiveTo explore the mechanism of Jianpi Yishen Huazhuo prescription in the improvement of ovarian function in polycystic ovary syndrome (PCOS). MethodSeventy female SD rats in SPF grade were randomly divided into 6 groups, 15 in the blank group and 15 in the model group, 10 in the metformin group (0.1 g·kg-1·d-1), and 10 in the low (1.275 g·kg-1·d-1), medium (2.55 g·kg-1·d-1), and high-dose (5.10 g·kg-1·d-1) Jianpi Yishen Huazhuo prescription groups. The blank group was given normal saline (10 mL·kg-1·d-1) by gavage and ordinary feed, and the other groups were given letrozole (1 mg·kg-1·d-1) by gavage combined with high-fat feed for 21 days to induce the model of PCOS. After modeling, the blank group and model group were given equal volume normal saline by gavage, and each drug group was given the corresponding dose of the drug by gavage for 30 days. The changes in body mass and fasting blood glucose (FPG) of rats before and after modeling were compared. Hematoxylin eosin (HE) staining was used to observe the morphological change in the ovaries of rats in each group. The serum levels of follicle stimulating hormone (FSH), luteinizing hormone (LH), testosterone (T), anti-Mullerian hormone (AMH), and estradiol (E2) were measured by enzyme-linked immunosorbent assay (ELISA), and the LH/FSH ratio was calculated. Immunohistochemical staining (IHC) and Western blot were used to detect the protein expression levels of nucleoside binding oligomerization domain protein like receptor 3 (NALP3), apoptosis-associated speck-like protein (ASC), cysteine protease-1 (Caspase-1), nuclear transcription factor-κB (NF-κB), interleukin-1β (IL-1β), interleukin-18 (IL-18), and interleukin-6 (IL-6) in the rat ovaries. ResultAs compared with the blank group, large follicles with polycystic expansion were found in the ovaries of the model group, no dominant follicles were found, the granular layer of follicles decreased and arranged loosely, and the number of corpus luteum decreased significantly. Serum T, LH, AMH and LH/FSH increased in the model group (P<0.05, P<0.01), while FSH and E2 decreased (P<0.05, P<0.01). The relative protein expression levels of NALP3, ASC, Caspase-1, NF-κB, IL-1β, IL-18, and IL-6 increased (P<0.05, P<0.01) in the ovaries of the model group. Compared with the model group, the low, medium, and high-dose Jianpi Yishen Huazhuo prescription groups and the metformin group showed growing follicles and corpus luteum at all levels, the number of cystic expanding follicles decreased, the thickness of follicular granular layer increased, the number of follicular fluid increased, mature follicles were visible, and the local morphology of oocytes was complete. Serum T, LH, AMH, and LH/FSH in these groups decreased (P<0.05, P<0.01), while E2 and FSH increased (P<0.05). The relative protein expressions of NALP3, ASC, Caspase-1, NF-κB, IL-1β, IL-18, and IL-6 in the ovaries of these groups decreased (P<0.05, P<0.01). There was no significant difference among the treatment groups. ConclusionBy inhibiting the activation of NLRP3 inflammasome, Jianpi Yishen Huazhuo prescription reduces the release of NALP3, ASC, Caspase-1, NF-κB, IL-18, IL-1β, and IL-6 inflammatory factors in ovarian tissues, regulates endocrine level, and effectively reduces PCOS inflammatory statu, so as to play a role in improving ovarian function.
9.Value of diagnosis and efficacy evaluation of folate receptor-positive circulating tumor cells in patients with small cell lung cancer

Objective:To analyze the value of folate receptor-positive circulating tumor cells (FR +-CTC) in the diagnosis and efficacy evaluation of patients with small cell lung cancer (SCLC). Methods:The data of 59 patients with SCLC and 14 patients with benign pulmonary diseases treated in China-Japan Friendship Hospital from May 2017 to October 2019 were retrospectively analyzed. Folate receptor targeted detection was used to detect the level of FR +-CTC in the blood of SCLC patients. The levels of serum progastrin-releasing peptide (Pro-GRP), neuron-specific enolase (NSE), cytokeratin 19 fragment 21-1 (Cyfra21-1) , and carcinoembryonic antigen (CEA) were detected by using chemiluminescence. The median ( P25, P75) was used as all the detection indexes. Mann-Whitney U test was used for pairwise comparison, Spearman correlation test was used to analyze the correlation between two variables, and receiver operator characteristic (ROC) curve was used to evaluate the diagnostic efficacy. Results:The level of FR +-CTC in 59 patients with SCLC was 11.00 FU/3 ml (7.10 FU/3 ml, 14.50 FU/3 ml), and the positive rate of FR +-CTC in patients with SCLC was 66.10% (30/59); the level of FR +-CTC in 14 patients with benign pulmonary diseases was 6.75 FU/3 ml (5.03 FU/3 ml, 7.85 FU/3 ml), and the positive rate of FR +-CTC in 14 patients with benign pulmonary diseases was 14.29% (2/14). The level of FR +-CTC in patients with SCLC was higher than that in patients with benign pulmonary diseases, and the difference was statistically different ( U = 33.50, P < 0.01). The expression level of FR +-CTC was not related to age, gender and smoking history in SCLC patients (all P>0.05). The expression level of FR +-CTC in patients with extensive-stage was higher than that in patients with limited-stage, and the difference was statistically significant ( P < 0.05). Tumor markers Pro-GRP, NSE, Cyfra21-1 and CEA were compared with FR +-CTC, and the ROC curve was drawn; the results showed that FR +-CTC had better sensitivity (71.2%) and specificity (92.90%) in the diagnosis of SCLC. For SCLC patients who received chemotherapy, the decrease range of FR +-CTC in patients with partial remission and stable disease was greater than that in patients with the progression of disease, and the differences were statistically significant (all P < 0.05). Conclusion:FR +-CTC can assist the diagnosis and disease staging of SCLC. For patients receiving chemotherapy, continuous detection of circulating tumor cells can help to evaluate the efficacy of chemotherapy and provide a reference for the choice of clinical treatment.

Realtime xCELLigence analysis (RTCA) is a new cell detection technology to continuously monitor, record and analyze a variety of information generated by cell activity. In drug research, it plays an important role in assessing myocardial toxicity and cell biological activity. Here, we first introduce the underlying mechanisms and characteristics of RTCA. Then we review the applications of RTCA in the research of myocardial toxicity and cell biological activity, to provides the fundamental baseline for understanding and exploiting RTCA. With the real-time, unlabeled, non-invasive, high throughput, and high accuracy features, RTCA not only promotes drug research and development, but also has a broad and good application prospect in other fields.
