Four new prenylflavonoid glycosides, namely koreanoside H-K (1-4), together with eleven known ones (5-15) were isolated from the leaves of Epimedium koreanum Nakai. Their structures were elucidated by 1D NMR, 2D NMR, HR-ESI-MS, IR and UV. The identification of the sugar moieties was carried out by means of acid hydrolysis and HPLC analysis of their derivatives. It is worth noting that compound 3 and compound 4 were elucidated to contain fucose and quinovose moieties, which were two extremely rare sugar units from the genus Epimedium. The anti-pulmonary fibrosis activity of the new compounds was evaluated using A549 cell line. Compounds 1, 2 and 4 showed significant anti-pulmonary fibrosis activities.

OBJECTIVE: To understand the current state and problem of screening and management of chronic kidney disease (CKD) in the community, and to explore the improving strategies. METHODS: We established a community-CKD integrated data science platform based on medical information from 79 community health centers, in Xicheng District, Beijing. Patients who referred to 79 community health centers from 21 June 2015 to 20 November 2021 were retrospectively included in this study using the CKD data platform. The monitoring of the indicator of kidney injury, risk factor control, medicine use and device configuration in community were assessed in the study. RESULTS: In the study, 70.6% of the population were identified with high risk of CKD in the total 374 498 individuals who referred to the community health centers. Hypertension (62.3%), coronary heart disease (43.3%) and diabetes (30.4%) were the most common risk factors in high-risk CKD population. Only 17.2% of the patients with high risk of CKD were screened for kidney injury including at least one serum creatine (Scr) or albuminuria test, among which 10 992 (24.2%) individuals were defined as CKD. 22.7% (11 338/49 908) of the total patients with kidney screening in community were defined as CKD, of whom, 42.6% and 46.1% were identified by estimated glomerular filtration rate (eGFR) < 60 mL/(min·1.73 m²) and abnormalities of urinary proteins, respectively. The overall CKD detection rate in the community was 5.2% (19 299/374 498), and the miss-diagnosis rate of CKD was 38.1%. Of the 79 community health centers, 13 (16.5%) were equipped with ACR testing device, and eGFR was reported directly in 66 (83.5%) centers. Altogether 60.3% and 99.7% of the community CKD patients achieved glucose control and blood pressure control, respectively, and 59.3% of the CKD patients who had proteinuria was treated with renin-angiotensin-aldosterone system (RAAS) inhibitors. CONCLUSION High-risk CKD population account for a substantial proportion of patients who refer to the community. Early screening, prevention and management of CKD in the community are of great importance to improve the prognosis and decrease the burden of CKD. It's essential to establish a screening and monitoring system, strengthen standardized management and clinician training for improving the ability of CKD management in the community.

Objective To obtain biomechanical conditions of the tibia section with different osteotomy thickness under different activity states, so as to provide theoretical basis for osteotomy thickness in total knee arthroplasty (TKA) and activities of patients after operation. Methods The three-dimensional models of the lower limb were reconstructed and the proximal tibia was resected by 0 mm,5 mm,7 mm,9 mm respectively based on the principle of bone resection in TKA. The tibia model after osteotomy were assigned the material properties and the stress and strain of the tibia cross-section were analyzed under the following activity states: standing, walking, running and going upstairs. Results The stress and displacement of the tibia cross-section increased with the increase in osteotomy thickness under the same activity state. The maximum stress and displacement of tibia cross-section increased in general with the same osteotomy thickness, when the strength of activity increased. Conclusions During TKA in clinic, the stress and strain on tibia cross-section became larger as the tibia osteotomy became thicker and the postoperative activity became stronger. Excessive osteotomy in TKA and high intensive activity after TKA should be avoided, so as to reduce stress and strain of the tibia plateau and promote long-term life of the prosthesis.

Objectives: To explore the clinical feathers and prognosis of primary malignant pericardial mesothelioma (PPM) reported in inland China. Methods: We searched Chinese data bases and collected medical records of PPM from 1981 to 2015 for patients' clinical, imaging, pathological, diagnostic and therapeutic information to explore the possible impact factors for prognosis. Results: A total of 186 patients were enrolled including 122 (65.6％) male, the mean age was (40.9±14.5) years at the range of (2-79) years. There were 108/166 (65.1％) patients suffered from dyspnea which was the most common symptom, 17 (10.2％) were complicated with cardiac tamponade and 48 (28.9％) complicated with constrictive pericarditis. 65 patients were misdiagnosed and 40 (61.5％) of them were diagnosed for tubercular pericarditis. 94 patients died and 30 (31.9％) of them died during hospitalization. 86 patients were discharged and received the follow-up study at the median of 6 months, 64 (74.4％) of them died. Multivariate Cox regression analysis did not find the meaningful clinical, imaging or pathological parameters to predict patients' mortality. Conclusion: PPM is a rare and highly aggressive pericardial malignant tumor, mainly involving middle-age males. Pathological combining immunohistochemical examinations are helpful for PPM diagnosis. Effective therapy has been lacking and comprehensive individualized therapy may improve the patients' prognosis.

The aim of this study was to investigate the relationship between the acetylcholine concentration in the blood and gelsenicine-induced death in mice. Kunming mice were given intraperitoneal injections of normal saline, gelsenicine or different doses of acetylcholine chloride. Atropine was given to the mice which received gelsenicine or medium dose acetylcholine chloride injection. The blood was sampled immediately when the mice died or survived for 20 min after injection. The acetylcholine concentration and acetylcholinesterase activity in the blood were measured by the testing kits, and the mortality was calculated and analyzed. The results showed that half lethal dose of gelsenicine (0.15 mg/kg) reduced the acetylcholinesterase activity and increased the blood acetylcholine concentration. The blood acetylcholine concentration of the dead mice in the gelsenicine group was increased to 43.0 μg/mL (from 31.1 μg/mL in the control), which was lower than that (53.9 μg/mL) of the dead mice in the medium dose acetylcholine chloride group, but almost equal to that (42.7 μg/mL) of the survival mice in the medium dose acetylcholine chloride group. Atropine could successfully rescue the mice from acetylcholine poisoning, but its efficiency of rescuing the mice from gelsenicine intoxication was weak. These results suggest that gelsenicine can inhibit acetylcholinesterase activity and increase blood acetylcholine concentration, but the accumulation of acetylcholine may not be the only or main cause of the death induced by gelsenicine in mice.

Objective To evaluate the effect of self -efficacy training on hemiplegic patients after ischemic stroke. Methods Sixty cases were randomly divided into control group (n =30 )and self -efficacy training group (n =30).The self -efficacy group received rehabilitation training according to the self -efficacy theory (5 times /week, 40 min /per time,1 0 weeks of intervention).The movement and balance function were evaluated using Fugl -Meyer score (FMA)and the activities of daily living were evaluated by the task analysis scale and Barthel index.The self -efficacy level and quality of life were evaluated by the general self -efficacy scale (GSES)and medical outcomes study (SF -36).Results The scores of FMA,Barthel,SF -36 and GSES had no significant differences between the two groups before training (P >0.05 )and all indexes improved after training.Six scores of Barthel and all scores of FMA,SF -36 and GSES of self -efficacy group were significantly higher than those of control group after training (all P <0.05 ).Conclusion Self -efficacy application could improve the motor function on the rehabilitation of hemiplegic patients after ischemic stroke.

Objective To construct the head nurses performance evaluation system in order to measure their performance fairly and equitably and to enhance their responsibility,crisis and sense of competition.Methods Many methods were used such as the balanced scorecard four elements were used to construct the basic framework of the performance evaluation system and Delphi method was used to determine the various index,analytic hierarchy process was used to identify the index of value at all levels.Results Two wheels consultation were used and the enthusiasm of experts was high with the coefficient was 88％ and authoritative coefficient was 0.76.Four primary indexes of performance evaluation system were consisted,such as finance,client,interior process,learn and study. It included sixteen secondary indexes,such as nursing service projects,nursing equipments,patients and families,nurses,doctors,and so on.Conclusions Performance evaluation system construction of head nurses based on the Delphi method can measure their performance fairly and equitably and it is worth to applyingin nursing.
