Journal of Prevention and Treatment for Stomatological Diseases 2025;33(8):659-665
Objective:
To explore the clinical characteristics, diagnosis, and treatment of ectopic thyroid gland in the parotid gland area, and to provide clinical ideas for the diagnosis and treatment of ectopic thyroid gland.
Methods:
A case of a normal thyroid gland with ectopic thyroid gland tissue in the parotid gland area in the neck was reported. The male patient was 20 years old. The chief complaint was the discovery of a painless mass gradually increasing under the left earlobe for one month. Clinical examination showed obvious bulging of the tissue under the left earlobe. A strip-shaped mass approximately 3.0 cm long could be palpated. It was soft in texture, with a clear boundary, and located under the skin. The skin was pale red and of normal temperature. The body position movement test was negative. Color Doppler ultrasound of the thyroid gland in the neck showed that the shape and size of the thyroid gland were normal. CT images of the head and neck showed a band-like soft tissue density shadow at the area of the parotid gland behind and below the left earlobe, with a clear boundary. The CT value was approximately 30 HU, and further enhancement yielded no additional findings. The admitting diagnosis was a mass in the left parotid gland area. The tumor was incised using a conventional surgical method for the parotid gland area. During the operation, it was found that the tumor was located under the skin, and the contents were bright-red granulomatous tissue without a capsule and adhesive to the skin tissue. The parotid gland capsule was not involved. After the tumor was completely scraped off, intermittent suturing was performed. The resected tumor was sent for pathological examination. A retrospective analysis of the diagnosis and treatment of this type of case was conducted in combination with a literature review.
Results:
The wound of the patient failed to heal in the first stage after the operation. By applying iodoform gauze for pressurized dressing changed weekly, the wound gradually healed about 2 months later. The postoperative pathological report showed an ectopic thyroid gland in the left parotid gland area. The results of the literature review indicate that ectopic thyroid glands can be partial or complete. In the former, normal thyroid gland tissue exists in the neck, and some thyroid gland tissue appears in other locations, mostly at the base of the tongue and mediastinum. In the latter, the thyroid gland in the neck is absent. Both can present with abnormal thyroid gland function and local compression symptoms, and the symptoms are more obvious in patients with a complete ectopic thyroid gland. Ectopic thyroid glands are mainly diagnosed and differentiated through physical examination and imaging examination. Ectopic thyroid glands occurring subcutaneously in the parotid gland area are extremely rare. Physicians should design personalized treatment plans based on clinical examinations and surgical indications.
Conclusion
A subcutaneous ectopic thyroid gland in the parotid gland area is rare. For ectopic thyroid gland surgery, a reasonable surgical plan should be designed considering the patient's aesthetic needs and prognosis. Puncture biopsy should be performed when necessary to formulate the surgical plan.

ObjectiveTo investigate the protective effect and mechanism of verbenalin on mouse mononuclear macrophage leukemia cells (RAW264.7) damaged by human coronavirus (HCoV)-229E infection, thereby providing experimental evidence for its development and application. MethodsRAW264.7 macrophages were infected with different concentrations of HCoV-229E to establish a coronavirus-induced macrophage injury model using the cell counting kit-8 (CCK-8) assay for assessing cell proliferation and viability. Cells were randomly divided into four groups: normal control, verbenalin group (125 μmol·L-1), model group (HCoV-229E), and HCoV-229E + verbenalin group (HCoV-229E + 125 μmol·L-1 verbenalin). Cell viability was measured using the CCK-8 assay, and the maximum non-toxic concentration (CC0), half-maximal cytotoxic concentration (CC50), half-maximal effective concentration (EC50), and selectivity index (SI) of verbenalin were calculated. Calcein/PI double staining was used to assess cell viability and cytotoxicity, and JC-1 staining was applied to evaluate changes in mitochondrial membrane potential (MMP). mito-Keima adenovirus labeling was used to assess mitophagy levels in each group. ResultsA macrophage infection model was successfully established by infecting RAW264.7 cells with the original concentration of HCoV-229E for 36 h. The CC0 of verbenalin was 125 μmol·L-1. The CC50 was 448.25 μmol·L-1. The EC50 against HCoV-229E-infected cells was 46.28 μmol·L-1, and the SI was 9.68. Compared with the normal group, the model group showed significantly reduced cell survival rate (P<0.01), increased cell death rate (P<0.01), decreased MMP (P<0.01), and suppressed mitophagy (P<0.01). In contrast, verbenalin treatment significantly improved cell survival rate (P<0.01), reduced cell death rate (P<0.01), alleviated MMP loss (P<0.01), and enhanced mitophagy levels (P<0.01) compared with the model group. ConclusionVerbenalin can enhance the survival rate of macrophages following HCoV-229E infection. The underlying mechanism may be associated with the activation of mitophagy, maintenance of MMP stability, and alleviation of mitochondrial damage.

Chinese Critical Care Medicine 2024;36(10):1063-1068
Objective:To investigate the risk factors and predictive value on acute kidney injury (AKI) following surgery, and to elucidate the relationship between platelet/white blood cell ratio (PWR) and AKI.Methods:A retrospective case-control study was conducted. The patients who underwent surgery admitted to Honghui Hospital of Xi'an Jiaotong University from January 2019 to January 2023 were enrolled. The patients who developed AKI during the postoperative hospital stay were categorized as the AKI group, while those with normal renal function or not meeting AKI criteria were categorized as the non-AKI group. Demographic information, clinical characteristics, perioperative medication usage, surgical related information, and preoperative and postoperative laboratory indicators were collected. Binary multivariate Logistic regression analysis was used to identify risk factors for postoperative AKI. Receiver operator characteristic curve (ROC curve) was plotted to assess the predictive efficacy of postoperative PWR for postoperative AKI.Results:A total of 420 patients were enrolled finally, with 72 developing AKI during hospitalization and 348 not developing AKI. Compared with the non-AKI group, the patients in the AKI group had a higher proportion of hypertension and coronary heart disease, a more usage of angiotensin converting enzyme inhibitor (ACEI), mannitol and vancomycin but a less usage of hydroxyethyl starch, a longer duration from injury to surgery and postoperative hospital stay, a greater intraoperative blood transfusion volume, a lower preoperative albumin (Alb), blood sodium, blood chlorine but a higher serum creatinine (SCr), blood urea nitrogen (BUN)/Alb ratio and N-terminal pro-brain natriuretic peptide (NT-proBNP), and a higher postoperative white blood cell count (WBC), BUN, SCr, BUN/Alb ratio, blood potassium, and blood phosphorus but a lower platelet count (PLT), PWR, and Alb. Binary multivariate Logistic regression analysis revealed that perioperative usage of hydroxyethyl starch [odds ratio ( OR) = 8.595, 95% confidence interval (95% CI) was 4.112-17.964, P < 0.001], prolonged duration from injury to surgery ( OR = 1.084, 95% CI was 1.034-1.137, P = 0.001), increased intraoperative blood transfusion volume ( OR = 1.001, 95% CI was 1.000-1.002, P = 0.017) were risk factors for AKI following surgery, and increased postoperative PWR was protective factor ( OR = 0.930, 95% CI was 0.894-0.967, P < 0.001). ROC curve analysis indicated that the area under the ROC curve (AUC) of postoperative PWR for predicting postoperative AKI was 0.684 (95% CI was 0.615-0.754); at the optimal cut-off value of 19.34, the sensitivity was 63.8%, and the specificity was 69.8%. Conclusions:Postoperative PWR is an independent risk factor for AKI in patients undergoing surgery. Postoperative PWR reduction in surgical patients can assist in predicting the occurrence of postoperative AKI.

Journal of Interventional Radiology 2024;33(7):780-784
Objective To establish a nomogram model which is used for predicting lower extremity deep venous thrombosis(DVT)after interventional treatment of lower extremity arteriosclerosis obliterans(LEASO),and to make an external validation of the model. Methods A total of 434 LEASO patients receiving intraluminal therapy between January 2020 and December 2022,who were retrieved from the database of patients with limb ischemia(JCLIMB)established by the Japanese Society of Vascular Surgery, were collected and used as modeling group, and other 60 LEASO patients, who received interventional treatment at the Hefei Binhu Hospital of China between January 2020 and May 2023,were collected and used as validation group.Through the electronic medical record system, the clinical data of the patients were obtained, which included gender, age, coexisting diabetes mellitus, coexisting hypertension, coexisting coronary heart disease, body mass index(BMI),Fontaine stage,D-dimer,prothrombin time(PT),activated partial thromboplastin(APTT),platelet lymphocyte ratio(PLR),and neutrophil-to-lymphocyte ratio(NLR).The nomogram model was established by using R software, and the prediction model was evaluated by the calibration curve, the decision curve analysis(DCA)and the receiver operating characteristic(ROC)curve. Results The differences in BMI,Fontaine stage,D-dimer,PLR and NLR between DVT patients and non-DVT patients were statistically significant(P<0.05).Multivariate logistic regression analysis showed that BMI,Fontaine stage,D-dimer,PLR and NLR were the independent influencing factors for DVT in LEASO patients(P<0.05).The nomogram model was established based on the results of the logistic regression analysis. The calibration curve analysis showed that the predicted values of the model was basically consistent with the actual values, indicating that this nomogram model carried a good accuracy.Taking the net income as the ordinate and the threshold as the abscissa, the none line represented that DVT would not occur in all LEASO patients. The nomogram model represented the actual curve of DVT,and the threshold range was 0.09-1.00,indicating that using this model for decision-making had good clinical benefits, suggesting the clinical utility of this model.ROC analysis revealed that using this nomogram model, which was established in this study,to predict the area under the DVT curve(AUC)of LEASO patients, the AUC in the modeling group and in the validation group was 0.912 and 0.834 respectively, the sensitivity was 96.50 and 99.20 respectively, and the specificity was 76.60 and 66.70 respectively. Conclusion The prediction model of DVT in LEASO patients that is established on the BMI,Fontaine stage,D-dimer,PLR and NLR has a good predictive value for the occurrence of DVT,therefore, this model is worth popularizing in clinical practice.
