1.Meta-analysis of the effect of probiotics on core symptoms of children with autism spectrum disorder

		                        			【Objective】 To evaluate the therapeutic effect of probiotics on core symptoms in patients with autism spectrum disorder (ASD), in order to provide theoretical basis for the treatment and intervention of ASD. 【Method】 Articles published from January 2000 to May 2023 on the effect of probiotics on core symptoms in ASD children were retrieved from 7 databases, including Chinese Biomedical Literature Service System (SinoMed), Wanfang Data Knowledge Service Platform, VIP Citation Database, and China National Knowledge Infrastructure (CNKI), PubMed, Embase, and Web of Science.Meta-analysis was performed using RevMan 5.4.Effect size, combine heterogeneity test sensitivity analysis and subgroup analysis were calculated. 【Result】 Totally 10 studies with 343 patients were included in this Meta-analysis.Meta-analysis showed that there was statistically significant difference in ASD core symptom score between the intervention group and the control group (SMD=-0.34, 95%CI: -0.55 - -0.12, P<0.05).Subgroup analysis showed that the efficacy of the trial conducted in Asian populations was not significant (SMD=-0.32, 95%CI: -0.63 - 0.00, P=0.05).In Caucasian populations, the therapeutic effect was significant (SMD=-0.35, 95%CI: -0.65 - -0.06, P<0.05).Grouped by age, it was found that the efficacy was not significant in trials involving adults (SMD=-0.12, 95%CI: - 0.57 - 0.33, P=0.61), but significant in trials involving only minors (SMD=-0.40, 95%CI: -0.65 - -0.16, P<0.05).According to the treatment course grouping, the intervention for less than 3 months (SMD=-0.35, 95%CI: -0.66 - -0.03, P<0.05) and more than 3 months (SMD=-0.33, 95%CI: -0.62 - -0.03, P<0.05) showed significant therapeutic efficacy.Grouped by bacterial strains, the efficacy of a single microbial community was not significant (SMD=-0.16, 95%CI: -0.46 - 0.15, P>0.05), while the efficacy of a composite microbial community was significant (SMD=-0.51, 95%CI: -0.81 - -0.21, P<0.05). 【Conclusions】 Probiotic therapy is effective in improving the core symptoms of ASD patients, but is influenced by factors such as race, age and probiotic strain. 		                        			Acute mitral regurgitation(MR)in the setting of myocardial infarction(MI)may be the result of papillary muscle rupture(PMR).The clinical presentation can be catastrophic,with refractory cardiogenic shock.This condition is associated with high morbidity and mortality.Transcatheter edge-to-edge repair(TEER)has become increasingly common in treating severe mitral regurgitation.This case details a successful TEER is feasible and safe in patients with acute MR following MI.TEER is an emerging treatment option in this clinical scenario that should be taken into consideration.
		                        		
		                        		
		                        		
		                        	
3.Status Investigation on Management of Off-label Drug Use in Tertiary Hospitals of Guizhou Province

		                        			Objective To investigate the current status of off-label drug use(OLDU)management in tertiary hospitals of Guizhou province and to provide baseline evidence for developing a unified administration regulation for OLDU in Guizhou province.Methods In line with the relevant policies and regulations,a questionnaire including basic information about the person filling out the form,basic information about the hospitals,and information about OLDU was developed.The questionnaire was sent to 84 tertiary hospitals in Guizhou province through the Wenjuanxing.Results A total of 84 questionnaires were distributed and recovered,with a response rate of 100.00%.Of the 84 hospitals,77 had OLDU,of which 68(88.31%)had established a management system for OLDU.Among the 77 hospitals with OLDU,65(84.42%),42(54.55%),58(75.32%),36(46.75%),15(19.48%),and 21(27.27%)hospitals respectively,required approval from the Committee on Drug Administration and Pharmacotherapy before OLDU,restricted the qualifications of doctors prescribing OLDU,required informed consent from patients or their families before OLDU,recorded the matters and reasons in the medical records of patients treated with OLDU,followed up patients in their files and evaluated the reasonableness of the OLDU,and carried out special reviews for OLDU.Only 30(38.96%)hospitals have set up a catalogue of OLDUs,and 58(75.32%)hospitals have urgent needs to set up a unified provincial catalogue of OLDUs.Conclusion The pharmacy administration level of OLDU in tertiary hospitals of Guizhou province is relatively low,so there is an urgent need to establish a unified OLDU management system and medication catalog.
		                        		
		                        		
		                        		
		                        	
4.Factors affecting the self-reported life quality of patients with acromegaly

		                        			Objective:To explore influencing factors of the self-reported brief life quality satisfaction score(Brief-QoL) in patients with acromegaly and understand the persistent low Brief-QoL scores in cases achieving biochemical remission.Methods:This study included 836 acromegaly patients who were hospitalized at Peking Union Medical College Hospital between January 2012 and December 2020. We retrospectively examined how clinical characteristics, biochemical parameters, comorbidities, and symptoms influenced Brief-QoL. Among patients who achieved biochemical remission, differences in clinical symptoms and comorbidities were analyzed between the high and low quality of life groups.Results:Patients with well-controlled biochemical indicators at the last follow-up had generally high Brief-QoL. However, patients with symptoms such as headaches (47.8% in the low-score group vs 14.9% in the high-score group, P<0.001) and joint pain (69.6% in the low-score group vs 19.0% in the high-score group, P<0.001) had low Brief-QoL despite biochemical remission. Receiving combined treatment(52.4% in the low-score group vs 27.5% in the high-score group, P=0.030) and having comorbid diabetes or hyperlipidemia were significant factors leading to decreased quality of life. Conclusion:Brief-QoL is suitable for follow-up of outpatient patients. 5.Mesothelin expression in gynecologic carcinosarcoma: clinicopathological significance and correlation with HER2expression

		                        			We retrospectively evaluated patients with uterine carcinosarcoma (UCS) and ovarian carcinosarcoma (OCS) who underwent surgery between 1997 and 2019.Immunohistochemical staining of formalin-fixed, paraffin-embedded specimens for MSLN (clone SP74) and HER2 (clone 4A5) was also performed. MSLN was scored using the H-score and 4-tired scoring system (0–3+). MSLN positivity was defined as any positive cell at any intensity, while high MSLN expression was defined as an intensity of ≥2+ in ≥30% of tumor cells. HER2 expression was scored according to modified 2018 American Society of Clinical Oncology/College of American Pathologists criteria.

		                        			A total of 128 patients were recruited, including 119 with UCS and 9 with OCS.All cases in UCS exhibited MSLN positivity, and 33.9% showed high-MSLN expression.Clinicopathological characteristics were not significantly associated with high or low-MSLN expression. However, the high-MSLN group showed more prolonged overall survival (OS) than the low-MSLN group (not assessed vs. 36.8 months; hazard ratio=0.48, 95% confidence interval=0.26–0.89, p=0.016). HER2-high patients had higher MSLN expression than HER2-negative patients. In high-MSLN and low-MSLN expression groups, HER2 status did not affect OS. OCS showed 100% MSLN positivity, with 66.6% high-MSLN.

		                        			MSLN expression is widely observed in gynecological carcinosarcomas.Moreover, high-MSLN expression is a favorable prognostic factor for UCS. 8.Deep Learning Model and its Application for the Diagnosis of Exudative Pharyngitis

		                        			 Objectives:
		                        			Telemedicine is firmly established in the healthcare landscape of many countries. Acute respiratory infections are the most common reason for telemedicine consultations. A throat examination is important for diagnosing bacterial pharyngitis, but this is challenging for doctors during a telemedicine consultation. A solution could be for patients to upload images of their throat to a web application. This study aimed to develop a deep learning model for the automated diagnosis of exudative pharyngitis. Thereafter, the model will be deployed online. 
		                        		
		                        			Methods:
		                        			We used 343 throat images (139 with exudative pharyngitis and 204 without pharyngitis) in the study. ImageDataGenerator was used to augment the training data. The convolutional neural network models of MobileNetV3, ResNet50, and EfficientNetB0 were implemented to train the dataset, with hyperparameter tuning. 
		                        		
		                        			Results:
		                        			All three models were trained successfully; with successive epochs, the loss and training loss decreased, and accuracy and training accuracy increased. The EfficientNetB0 model achieved the highest accuracy (95.5%), compared to MobileNetV3 (82.1%) and ResNet50 (88.1%). The EfficientNetB0 model also achieved high precision (1.00), recall (0.89) and F1-score (0.94). 
		                        		
		                        			Conclusions
		                        			We trained a deep learning model based on EfficientNetB0 that can diagnose exudative pharyngitis. Our model was able to achieve the highest accuracy, at 95.5%, out of all previous studies that used machine learning for the diagnosis of exudative pharyngitis. We have deployed the model on a web application that can be used to augment the doctor’s diagnosis of exudative pharyngitis. 
		                        		
		                        		
		                        		
		                        	
9.Drug resistance and genomic characteristics of a strain of O139 Vibrio cholerae isolated from human bloodstream infection.

		                        			Objective: To analyze the drug resistance and genomic characteristics of a strain of serogroup O139 Vibrio cholerae producing cholera toxin isolated from the bloodstream of a person with bacteremia. Methods: The broth dilution method and automatic drug sensitivity analyzer were used to determine the antibiotic sensitivity of the strain. The complete genome sequence of the strain was obtained by using second-generation gene sequencing and nanopore sequencing. BLAST software was used for comparison and analysis with CARD, Resfinder, ISfinder, VFDB, and other databases. The drug-resistant genes, insertion sequences and virulence genes carried by the strain were identified. MEGA 5.1 software was used to construct a genetic phylogenetic tree based on the core genomic single nucleotide polymorphisms. Results: V. cholerae SH400, as the toxigenic strain, carried multiple virulence-related genes and four virulence islands. The strain was resistant to streptomycin, tetracycline and cotrimoxazole, carrying corresponding drug-resistant genes. The strain also carried IncA/C plasmid with the size of 172914 bp and contained 10 drug-resistant genes. Combined with the genomic evolutionary relationship, this study found that the drug-resistant genes and drug-resistant plasmids carried among strains showed certain aggregation. The traditional ST type of strain SH400 was ST69, and the cgMLST type was a new type highly similar to cgST-252. Conclusion: This strain of serogroup O139 V. cholerae carries the ctxAB gene, multiple drug-resistant genes and IncA/C plasmid, and there are multiple drug-resistant islands.
		                        		
		                        		
		                        		
		                        	
10.Pregnancy stress and gestational diabetes mellitus

		                        			Objective:To observe the pressure level and hair cortisol level of pregnant women, and explore the influence of pregnancy stress on gestational diabetes mellitus(GDM) and underlying mechanism.Methods:The eligible pregnant women at People′s Hospital of Henan Province from January 2020 to December 2021 were selected. Pregnancy stress scale(PSS) was used for stress assessment at 10 th-12 th gestational weeks. The hair of pregnant women was collected to detect cortisol at 24 th-28 th gestational weeks. Oral glucose tolerance test was performed at the 24 th, 28 th, 32 nd and 36 th gestational weeks to diagnose GDM. The endpoint event of follow-up was GDM. Results:Among 498 pregnant women, 20(4.0%) had no stress, 321(64.5%) had mild stress, 128(25.7%) had moderate stress, and 29(5.8%) had severe stress. There was a significant positive correlation between maternal hair cortisol levels and the total PSS score. The hair cortisol level in severe stress group was significantly higher than that in other groups, and the hair cortisol level in GDM group was significantly higher than that in non-GDM group. There were significant differences in GDM-free survival curves among different stress groups( χ2=15.975, P=0.001). Cox multivariate regression showed that severe stress was an independent risk factor for GDM. The area under the receiver operating characteristic curve is 0.747(95% CI 0.545-0.950, P=0.028). Conclusions:Severe stress during pregnancy is an independent risk factor for the occurrence of GDM, and has a certain ability to predict GDM. Cortisol may play a role in the process through which stress mediates GDM.
