1.Efficacy of flexible ureteroscope lithotripsy with flexible vacuum-assisted urethral access sheath for 1-2 cm lower renal calyceal stones

[Objective] To compare the clinical efficacy and safety of flexible ureteroscope lithotripsy (FURL) combined with flexible vacuum-assisted urethral access sheath (FV-UAS) and traditional UAS in the treatment of 1-2 cm lower renal calyceal stones, so as to provide reference for clinical practice. [Methods] Clinical data of 157 patients with 1-2 cm lower renal calyceal stones treated with FURL during Mar.2021 and Oct.2023 were retrospectively analyzed, including 80 treated with traditional UAS, and 77 with FV-UAS.General and clinical information of the two groups were compared. [Results] The immediate stone-free rate (SFR) (84.4% vs.67.5%, P=0.013) and final SFR (88.3% vs. 75.0%, P=0.032) of the FV-UAS group were significantly higher than those of the traditional UAS group, with significant difference.The incidence of postoperative complications such as fever, renal colic, and perirenal hematoma was significantly higher in the traditional UAS group than in the FV-UAS group (15.0% vs.5.2%, P=0.042). After treatment with anti-infective and analgesic drugs, both groups were improved, and no severe sepsis or septic shock occurred after surgery.The hospitalization expenses of the FV-UAS group were significantly lower than those of the traditional UAS group [ (18 341±1519)yuan vs.(19 152±1826)yuan, P=0.003]. [Conclusion] Compared to the traditional UAS, the combination of FURL and FV-UAS for the 1-2 cm lower renal calyceal stones has a high SFR and low incidence of complications.Patients experience less pain, recover faster and spend less.It's a new treatment option for inferior calyceal calculi.
2.Efficacy of FURL with FV-UAS for 2-3 cm upper urinary tract stones

[Objective] To explore the efficacy of disposable flexible ureteroscopic lithotripsy (FURL) with flexible vacuum-assisted ureteral access sheath (FV-UAS) in the treatment of 2-3 cm upper urinary tract stones, so as to provide reference for the treatment selection. [Methods] Clinical data of 178 patients with upper urinary tract stones who received FURL or minimally invasive percutaneous nephrolithotomy (MPCNL) at our hospital during Apr. 2022 and Oct. 2023 were retrospectively analyzed. The patients were divided into FV-UAS group (n=90, received FV-UAS combined with diaposable FURL treatment) and MPCNL group (n=88, received MPCNL). The general information, perioperative data, and postoperative stone-free rate (SFR) of the two groups were compared. [Results] All operations were successfully completed. The operation time was significantly longer in the FV-UAS group than in the MPCNL group [(66.5±6.7) min vs. (63.9±7.4) min, P=0.015]. However, the intraoperative hemoglobin reduction [(7.3±3.1)g/L vs.(11.4±5.9)g/L], postoperative hospital stay (P<0.001) [(2.2±0.7)d vs.(5.4±1.3)d], and visual analogue score (VAS) [(2.7±0.9)vs.(5.6±1.1)] were significantly lower in the FV-UAS group than in the MPCNL group (P<0.001). The incidence of persistent gross hematuria was significantly higher in the MPCNL group than in the FV-UAS group (12.5% vs. 3.3%, P=0.023). The FV-UAS group had a similar postoperative immediate (83.3%) and final SFR (95.6%) to those of the MPCNL group (89.8%, 96.6%, P>0.05). [Conclusion] The combination of FURL with FV-UAS for 2-3 cm upper urinary tract stones has a higher SFR and a lower complication rate.Patients experience endurable pain and fast recovery, which is worth promoting and applying in clinical practice.
3.Effect of transurethral plasmakinetic resection of prostate in treating benign prostatic hyperplasia complicated with hypertension

Objective To compare the effects of transurethral plasmakinetic resection of prostate (TUPKRP) and drug therapy on blood pressure and change of blood pressure rhythm in patients with benign prostatic hyperplasia complicated with hypertension. Methods A total of 103 patients with benign prostatic hyperplasia in the hospital from June 2021 to June 2023 were retrospectively enrolled as study objects. According to different treatment protocols, they were divided into drug therapy group with 47 cases (treated with telmisartan combined with finasteride) and surgical treatment group with 56 cases (treated with telmisartan combined with TUPKRP). Blood pressure levels (24-hour mean diastolic blood pressure, 24-hour mean systolic blood pressure, daytime mean diastolic blood pressure, daytime mean systolic blood pressure, nighttime mean diastolic blood pressure, nighttime mean systolic blood pressure), change of blood pressure rhythm (dipper blood pressure), prostate symptoms, prostate volume, residual urine volume, and sexual function were compared between the two groups before treatment and at 3 and 6 months after treatment. 4.Role of sphingomyelinases in hepatocellular carcinoma

Sphingomyelinases (SMase) are the main enzymes that regulate the signaling pathway of sphingomyelin and the metabolism of related products, and they are involved in the key steps of the complex metabolic process of sphingomyelin. In recent years, many studies have shown that SMase is involved in the biological processes such as cell cycle arrest, cell migration, and inflammation and promotes the development and progression of hepatocellular carcinoma by regulating the apoptosis and proliferation of tumor stem cells. SMase has an important potential biological value in the development, progression, diagnosis, and treatment of hepatocellular carcinoma. This article summarizes the exact role of SMase in the development and progression of hepatocellular carcinoma, in order to provide new ideas and strategies for the clinical treatment of hepatocellular carcinoma and the development of new drugs.
5.Mechanism of action of Jieduhuayu granules for remission of oxidative stress in hepatocytes

Objective:To study and explore the effect and mechanism of action of Jieduhuayu granules on oxidative injury of human liver L02 cells.Methods:Human liver L02 oxidative injury model was established with 0.1 mmol/ L H 2O 2 intervention for 1 h, and treated with different concentrations of Jieduhuayu (JDHY) solution. Hepatocytes were divided into five groups: normal, H 2O 2, H 2O 2 + JDHY (0.5 mg/ml), H 2O 2 + JDHY (1 mg/ml), and H 2O 2 + JDHY (1.5 mg/ml). MTT assay was used to detect hepatocytes activity. Transmission electron microscope was used to observe mitochondrial morphology in hepatocytes. Biochemical test was used to detect the levels of superoxide dismutase, lactate dehydrogenase, alanine aminotransferase, aspartate aminotransferase, malondialdehyde, and reduced glutathione and albumin in hepatocytes. Western blot was used to detect the expression levels of rabbit anti-phosphatidylinositol 3-kinase (PI3K), AKT and mTOR in hepatocytes. One-way analysis of variance was used for comparison between multiple groups, and the LSD method was used for pairwise comparison. Results:Compared with the normal group, the cell proliferation activity ( P < 0.05), mitochondrial vacuolization, superoxide dismutase activity, reduced albumin and glutathione content, and PI3K, AKT, and mTOR protein expression levels in the H 2O 2 group were all significantly reduced ( P < 0.05), while the content of malondialdehyde and the activities of alanine aminotransferase, aspartate aminotransferase and lactate dehydrogenase were significantly increased ( P < 0.05). Compared with H 2O 2 group, the cell proliferation activity ( P < 0.05), alterations in morphological remission of mitochondria, superoxide dismutase activity, reduced albumin and glutathione content, and PI3K, AKT and mTOR protein expression levels in the H 2O 2 + JDHY (1 mg/ml) and H 2O 2 + JDHY (1.5 mg/ml) group ( P < 0.05) were all significantly increased ( P < 0.05), while malondialdehyde content and alanine aminotransferase, aspartate aminotransferase and lactate dehydrogenase activities were significantly decreased ( P < 0.05). Conclusion:Jieduhuayu granule can effectively improve oxidative stress and mitochondrial injury in hepatocytes, and its effect may be related to the promoting expression of PI3K/AKT/mTOR signaling pathways.
6.An Analysis of Hearing Screening Results from 616 Preschool Children

Objective To explore the feasibility and effectiveness of otoacoustic emission (OAE) in hearing screening for preschool children .Methods A total of 616 preschool children aged 3~6 (mean age 4 .6 years old) were included in this study .All the subjects received transiently evoked otoacoustic emission (TEOAE) test by trained personnel .The children who failed the hearing screening received diagnostic examination in the hearing ex-amination center .Results Of 616 children ,66 (10 .71% ) failed the hearing screening and 16 children received audi-ological assessment .In these 16 children ,8 had otitis media with effusion (5 with mild conductive hearing loss ) ,5 had impacted cerumen ,3 had normal hearing ,and none had sensorineural hearing loss .Conclusion OAE can be used effectively in the hearing screening for preschool children .But how to improve the referral rate of children who failed the hearing screening needs further discussion .
7.Production of L-citrulline by a recombinant Corynebacterium crenatum SYPA 5-5 whole-cell biocatalyst.

Arginine deiminase (ADI) was first high-efficient expressed in Corynebacterium crenatum SYPA 5-5. The ADI was purified by Ni-NTA affinity chromatography and SDS-PAGE analysis showed the molecular weight (MW) was 46.8 kDa. The optimal temperature and pH of ADI were 37 ℃ and 6.5 respectively. The Michaelis constant was 12.18 mmol/L and the maximum velocity was 0.36 μmol/(min·mL). Under optimal conditions, 300 g/L of arginine was transformed and the productivity reach 8 g/(L·h). The recombinant strain was cultivated in a 5-L fermentor and used for whole-cell transformation of 300 g/L arginine, under repeated-batch bioconversion, the cumulative production reached 1 900 g/L.
8.Analysis on causes of death among children under 5 years old in a class 3A hospital during 2007-2015

Objective To investigate and analyze the death situation of children aged under 5 year old in a class 3A hospital during 2007-2015 to provide a basis for formulating corresponding interventional measures to reduce the disease mortality of children.Methods The medical records of children death in this hospital during 2007 2015 were retrospectively analyzed.Results The mortality rate of children under 5 years old in this hospital during 2007-2015 showed a decreasing trend year by year.There was the statistically significant difference in the mortality rates among different years,between male and female (3.58 %vs.2.10%) and between rural family registers and non-rural family registers(P<0.05);the neonatal mortality rate was highest (50.45 %);the top 5 places of death causes were premature delivery or low birth weight newborns,respiratory system diseases,nervous system diseases,congenital diseases and accidents.Conclusion The key for reducing the children mortality rate is to strengthen the perinatal health care,strengthen prenatal examinations and pregnant examinations,prevent the occurrence of respiratory system disease,pay attention to the influence of various factors on children health and prevention and treatment of accidents.
9.Early enteral nutrition in postoperative patients of gastric cancer

Objective:To investigate the effects of early postoperative enteral nutrition on recovery of patients with gastric cancer.Methods:Sixty-five cases of patients with gastric cancer were randomized into early enteral nutrition (EEN) group and enteral nutrition (EN) group.Serum total protein (TB),albumin(ALB),Prealbumin,white blood cell count (WBC),C-reactive protein (CRP),immunoglobulin,peripheral T-lymphocyte subsets,gastrointestinal recovery time,hospital stay and cost were recorded.The nutritional and cellular immunity parameters of the EEN group on 7th day after operation were higher than those of the EN group.Inflammatory response of the EEN group on 3th day after operation was lower than EN group.EEN group showed better immunological response and clinical recovery than the EN group (P < 0.05).Conclusion:Early enteral nutrition in postoperative gastric cancer patients can improve early postoperative nutritional status and immune function,alleviate inflammatory response,promote the recovery of intestinal function and shorten hospital stay.
10.Correlation between HLA-G gene polymorphism and gastric cancer in Han nationality

Objective To explore the correlation between HLA-G gene polymorphism and gastric cancer in Han nationality.Methods A total of 123 gastric cancer patients in our hospital as the research objects (confirmed as gastric carcinoma by pathological diagnosis).At the same time,a total of 150 healthy controls were selected in our hospital,HLA-G and HLA-DRB1 allele subtypes were detected by polymerase chain reaction sequence specific primers (PCR SSP) detection method,and the allele distribution frequencies of subtypes of HLA-G and HLA-DRB1 were observed and analyzed.Results Compared with the control group,the observation group had higher detection rate in HLA-DRB1 * 03 and HLA-DRB1 * 15 (P < 0.05),and RR were 1.427 and 2.209,respectively.And there were significant differences in HLA-G * 01012 distribution in two groups (P < 0.05).Conclusion The incidence of gastric cancer may possibly correlated with HLA-G * 01012,HLA-DRB1-03 and HLA-DRB1-15 frequencies,and they are all protective genes.
