1.Yinqiao Powder affects macrophage polarization-mediated herpes simplex keratitis through the cGAS-STING-IRF3 molecular pathway

		                        			 AIM: To investigate the specific molecular mechanism of Yinqiao Powder in affecting macrophage polarization in herpes simplex keratitis(HSK)through the cyclic GMP-AMP synthetase(cGAS)-stimulator of interferon genes(STING)-interferon regulatory factor 3(IRF3)molecular pathway.METHODS:Human corneal epithelial cells(HCE-T)were divided into control, HSK, and HSK + Yinqiao Powder groups. M0 macrophages were grouped as Ctrl, HSV-1, HSV-1+oe-cGAS, HSV-1+Yinqiao Powder, and HSV-1+oe-cGAS+Yinqiao Powder. Conditional medium(CM)from each group of M0 macrophages was collected to intervene in HCE-T cells and divided into Ctrl-CM, HSV-1-CM, HSV-1+oe-cGAS-CM, and HSV-1+Yinqiao Powder-CM groups. Cell viability was detected by MTT assay, and apoptosis was detected by TUNEL assay. ELISA was used to detect the concentrations of Arg-1 and iNOS in cell supernatants, and Western blotting was used to detect the relative protein expressions of cGAS, STING, and IRF3. Balb/c mice were divided into control, model, and drug groups. The model and drug groups were inoculated with HSV-1 on the cornea of Balb/c mice using the corneal scratch method to construct an HSK mouse model, and the drug group was treated with Yinqiao Powder. The incidence and mortality of the three groups were compared on days 1, 3, 5, 7, and 14 after modeling.RESULTS:Compared with the control group, the HCE-T cell viability in the HSK group was decreased but apoptosis was increased, which was reversed by Yinqiao Powder intervention. Compared with the Ctrl group, the Arg-1 concentration in the cell supernatant of the HSV-1 group was decreased, the iNOS concentration was increased, and the protein expressions of cGAS, STING, and IRF3 were decreased. Compared with the HSV-1 group, the Arg-1 concentration was increased, the iNOS concentration was decreased, and the protein expressions of cGAS, STING, and IRF3 were enhanced in the HSV-1+oe-cGAS group and the HSV-1+Yinqiao Powder group, and the same results were obtained in the HSV-1+oe-cGAS+Yinqiao Powder group. Compared with the Ctrl-CM group, the HCE-T cell viability was decreased and apoptosis was increased in the HSV-1-CM group, which was reversed by overexpressing cGAS in macrophages or intervening with Yinqiao Powder. In vivo experiments found that Yinqiao Powder intervention could improve the pathological progression of keratitis.CONCLUSION:Yinqiao Powder inhibits M1 polarization of macrophages through the cGAS-STING-IRF3 molecular pathway, thereby delaying the progression of HSK. 
		                        		
		                        		
		                        		
		                        	
2.Safety analysis of Yttrium-90 resin microsphere selective internal radiation therapy on malignant liver tumors

		                        			Objective To explore the safety of Yttrium-90 resin microsphere selective internal radiation therapy (90Y-SIRT) on malignant liver tumors. Methods A retrospective analysis was conducted on 64 patients with malignant liver tumors who underwent 90Y-SIRT from February 2023 to November 2024 at Weifang People’s Hospital. The clinical characteristics of the patients and the occurrence of adverse reactions after treatment were analyzed to assess the safety of 90Y-SIRT. Results Among the 64 patients, there were 52 males (81.25%) and 12 females (18.75%); the average age was (56.29±11.08) years. Seven patients (10.94%) had tumors with maximum diameter of less than 5 cm, 38 patients (59.38%) had tumors with maximum diameter of 5-10 cm, and 19 patients (29.68%) had tumors with maximum diameter of greater than 10 cm. There were 47 cases (73.44%) of solitary lesions and 17 cases (26.56%) of multiple lesions; 53 cases (82.81%) were primary liver cancers and 11 cases (17.19%) were metastatic liver cancers. Of the 64 patients, 63 successfully completed the Technetium-99m macroaggregated albumin (99mTc-MAA) perfusion test and received the 90Y-SIRT; one patient received 90Y-SIRT after the second 99mTc-MAA perfusion test due to a work error. The most common adverse reactions included grade 1 alanine aminotransferase (ALT) elevation in 26 cases (40.62%) and grade 2 in 2 cases (9.37%), grade 1 aspartate aminotransferase (AST) elevation in 27 cases (42.18%) and grade 2 in 7 cases (10.93%); grade 1 nausea in 17 cases (26.56%) and grade 2 in 6 cases (9.37%); grade 1 abdominal pain in 12 cases (18.75%), grade 2 in 5 cases (7.81%), and grade 3 in 1 case (1.56%); grade 1 vomiting in 11 cases (17.18%), grade 2 in 5 cases (7.81%), and grade 3 in 1 case (1.56%). 3.Association between Self-rated Health and Age-adjusted Charlson Comorbidity Index in the Elderly of Different Genders

		                        			ObjectiveTo explore the relationship between self-assessed health and age-adjusted Charlson comorbidity index (AICC) in older adults, and to further analyze the differences in this relationship across gender groups. MethodsBased on the China health and retirement longitudinal study (CHARLS) database, this study selected data on basic characteristics, chronic disease status, depressive symptoms, and self-assessed health of older adults aged ≥60 years. Chi-square tests were used to perform a preliminary analysis of the association between these factors and AICC scores. A multifactorial ordered logistic regression model was constructed to assess the effects of each influencing factor on AICC, while multiple linear regression analysis was used to explore the linear relationship between self-rated health and AICC scores. Additionally. Stratified analysis by gender was performed to evaluate gender differences. ResultsA total of 10 911 participants were included, with a mean age of 67.40±5.94 years; 6 249 (57.3%) were male and 4 662(42.7%) were female. The distribution of AICC scores was categorized into low-risk, moderate-risk, higher-risk, and high-risk groups, accounting for 23.5%, 50.2%, 20.6%, and 5.7%, respectively. Multifactorial logistic regression analysis revealed that self-rated health was negatively associated with AICC in the total population and in the male geriatric group (OR=0.843, 95% CI: 0.776, 0.917, P=0.001), (OR=0.886, 95% CI: 0.796 , 0.987, P=0.028), but did not reach statistical significance in the female geriatric group . Linear regression analysis further indicated a significant negative linear relationship between self-rated health and AICC (b=-0.485, 95% CI: -0.516,-0.455, P<0.001).This relationship was consistent in both male (b=-0.356, 95% CI: -0.406,-0.305, P<0.001) and female (b=-0.373, 95% CI: -0.435,-0.310, P<0.001) subgroups, with a stronger negative association in females. ConclusionSelf-rated health is significantly negatively associated with AICC, and attention should be given to self-rated health in the female geriatric population. 4.Correlation of circulating ceramides and residual cholesterol with intracranial atherosclerotic stenosis in elderly stroke patients

		                        			Objective To investigate the correlation of ceramides(Cer),triglyceride-glucose(TyG)index,residual cholesterol(RC)and intracranial atherosclerotic stenosis(ICAS)in elderly with ischemic stroke(IS).Methods A total of 209 elderly IS patients admitted in the Affiliated Hospi-tal of Xuzhou Medical University from January to December 2022 were recruited,and according to intracranial artery stenosis,they were divided into stenosis group(n=122)and non-stenosis group(n=87).TyG index and RC levels were calculated after testing different types of Cer.Spearman correlation analysis and logistic regression analysis were used.Results Compared with the non-stenosis group,the stenosis group had significantly larger proportions of higher levels of LP(a),HbA1c,TyG index,RC and Cer16∶0,Cer18∶0,Cer24∶1,Cer16∶0/Cer24∶0,Cer24∶1/24∶0 and Cer18∶0/24∶0,and lower Cer24∶ 0 level(P<0.05,P<0.01).The levels of Cer16∶0,Cer18∶0,Cer24∶1,Cer16∶0/24∶0,Cer18∶0/24∶0,Cer24∶1/24∶0,LP(a),HbA1c,TyG index and RC were positively correlated with(r=0.290,0.343,0.383,0.436,0.434,0.481,0.166,0.248,0.140,0.204,P<0.05,P<0.01),and Cer24∶0 was negatively with ICAS(r=-0.247,P<0.01).Cer18∶0,Cer24∶1,Cer16∶0/Cer24∶0,Cer18∶0/Cer24∶0 and RC were important influencing factors for ICAS(P<0.05,P<0.01).Conclusion The levels of Cer,TyG index and RC are higher in elderly IS patients,and these indicators might be the risk factors for ICAS.

Critically ill patients are at high risk for hospital acquired infections, which can significantly increase the mortality rate and treatment costs for these patients. Therefore, in the process of treating the primary disease, strict prevention and control of new hospital infections is an essential component of the treatment for critically ill patients. The treatment of critically ill patients involves multiple steps and requires a concerted effort from various aspects such as theory, management, education, standards, and supervision to achieve effective prevention and control of hospital infections. However, there is currently a lack of unified understanding and standards for hospital infection prevention and control. To address this, in March 2024, a group of experts in critical care medicine, infectious diseases, and hospital infection from China discussed the current situation and issues of hospital infection control in the intensive care unit together. 6.A survey of sarcopenia in hospitalised elderly patients with comorbidity and its factors and countermeasure

		                        			Objective To investigate status,the influencing factors and nursing measures for sarcopenia in the elderly patients with comorbidity.Methods A total of 385 elderly patients with comorbidity hospitalised in our hospital from January 2020 to March 2023 were included as the research objects by convience sampling.General data questionnaire and electronic medical record system were used to collect relevant data.According to a presence or absence of sarcopenia the elderly comorbid patients were divided into a sarcopenia group and a control group.Logistic regression was used to analyse the influencing factors of sarcopenia in the elderly patients and the relevant nursing measures.Results It was found that a total of 134(34.81%)elderly comorbid inpatients had developed sarcopenia.where male was 53.73%,female was 46.27%.Logistic regression analysis indicated that old age(OR=1.106),high asthenia score(OR=2.827),high CCI(Charlson Comorbidity Index)score(OR=1.507),low BMI(OR=0.314)and low ADL(activity of daily life)were the factors of sarcopenia(all P<0.05).Conclusions This study has identified that the influencing factors of sarcopenia in hospitalised elderly patients with comorbidity were old age,low BMI,low ADL,high weakness score and high CCI.Medical staff should take appropriate measures that are pertinent to the influencing factors in order to reduce the occurrence of sarcopenia among the hospitalised elderly patients with comorbidity.
		                        		
		                        		
		                        		
		                        	
8.Effects of strengthening night professional nursing in patients undergoing open fracture surgery of lower extremities

		                        			Objective:To explore the effects of strengthening night professional nursing of patients with open fractures of the lower extremities.Methods:From January 2019 to June 2020, convenience sampling was used to select 397 patients with open fractures of the lower extremities from the First Orthopedic Trauma, the Third Hospital of Hebei Medical University. Patients were randomly assigned to a control group and an observation group, respectively, to receive general nursing (control group) and strengthening night professional nursing (observation group). The control group was equipped with a regular number of nurses. In the observation group, an additional night nurse was assigned for patients undergoing surgery for open fractures of the lower extremities. The nighttime period was from 18∶00 to 8∶00 the next day. The night nurse arranged compensatory time off for the rest of the time or performance rewards. The hospital stay, hospitalization expenses, incidence of complications during hospitalization and half a year after surgery of the two groups were compared.Results:In the control group, there were 196 patients with open fractures of the lower extremities, and 46 patients underwent night surgery. In the observation group, there were 201 patients with open fractures of the lower extremities, and 40 patients underwent night surgery. There were no statistically significant differences between two groups in terms of surgical time, incidence of complications during hospitalization and half a year after surgery ( P>0.05). The hospital stay in the observation group was shorter than that in the control group, and the hospitalization expense was lower than that in the control group, with statistically significant differences ( P<0.05) . Conclusions:Strengthening night professional nursing can help shorten the hospital stay of patients undergoing surgery for open fractures of the lower extremities and reduce medical expenses.
		                        		
		                        		
		                        		
		                        	
9.Latent profile analysis of subtypes and influencing factors of professional quality of life among Operating Room nurses

		                        			Objective:To explore the subtypes of professional quality of life for Operating Room nurses from an "individual centered" perspective, so as to compare the differences in negative emotions and turnover intentions among different subtypes of nurses.Methods:This article was a cross-sectional survey study. Using the convenient sampling method, a total of 409 Operating Room nurses from three tertiary hospitals in Shandong Province were selected as the research objects in November 2021. The survey was conducted using General Information Questionnaire, Professional Quality of Life Scale, Depression Anxiety Stress Self-assessment Scale and single item of resignation intention. The latent profile analysis was used to explore the potential categories of professional quality of life for operating room nurses.Results:Finally, 350 valid questionnaires were collected, and the effective recovery rate was 85.57% (350/409). 350 operating room nurses scored (50.35±9.82) in the dimension of compassionate satisfaction, (49.01±10.43) in the dimension of burnout, and (49.18±9.53) in the dimension of secondary trauma, all of which were at a moderate level. The professional quality of life of operating room nurses was divided into three potential categories, namely good type (25.14%, n=88), contradictory type (17.43%, n=61) and low satisfaction type (57.43%, n=201). The scores of depression dimension were (3.89±7.69), (14.39±11.13) and (13.18±9.50) in good, contradictory and low-satisfaction class nurses, respectively, and the differences were statistically significant ( P<0.01). The scores of anxiety were (5.82±8.45), (16.92±10.87) and (14.45±9.29), respectively, and the difference was statistically significant ( P< 0.01). The scores of stress dimension were (5.64±8.27), (17.18±11.36) and (15.01±9.10), respectively, and the difference was statistically significant ( P<0.01). The scores of turnover intention were (1.45±0.52), (2.07±1.09), and (2.42±1.06), respectively, and the differences were statistically significant ( P<0.01) . Conclusions:There are different characteristic groups of professional quality of life in operating room nurses, and the population distribution is closely related to negative emotion and turnover intention. 10.Survival efficacy of MDS/AML patients with TP53 abnormal received allogeneic hematopoietic stem cell transplantation

		                        			Objective:TP53-abnormal MDS/acute myeloid leukemia (AML) patients’ allogeneic hematopoietic stem cell transplantation (allo-HSCT) treatment’s effectiveness and influencing factors should be studied.Methods:42 patients with TP53 gene status change MDS/AML who underwent allo-HSCT from 2014.8.1 to 2021.7.31 at the Hematology Hospital of the Chinese Academy of Medical Sciences were the subject of a retrospective analysis. The 42 patients were divided into three groups: the TP53 deletion group (group A) , TP53 mono-alle mutation group (group B) , and TP53 multi-hit group (group C) . The differences in clinical features and prognostic factors after transplantation were analyzed.Results:There were 42 MDS/AML patients, including 21 patients with MDS, and 21 patients with AML. The median follow-up period was 34.0 (7.5-75.0) months and the median patient age at the time of transplantation was 41.5 (18-63) years old. The total OS was 66.3% (95% CI 53.4%-82.4%) in 3 years after transplantation, and EFS was 61.0% (95% CI 47.7%-78.0%) in 3 years. For 3 years after receiving hematopoietic stem cell transplantation, there were no statistically significant differences in 3-year OS and EFS in groups A, B, and C ( P≥0.05) . The 3 years OS was 82.5% (95% CI 63.1%-100.0%) in group A, 60.6% (95% CI 43.5%-84.4%) in group B, and 57.1% (95% CI 30.1%-100.0%) in group C. Univariate analysis revealed that the number of co-mutant genes, pre-HSCT treatment, and disease type did not affect prognosis, while age, karyotype, co-mutation, positive blast cell before transplantation, and positive blast cell after transplantation were common prognostic factors for OS and EFS ( P<0.1) . MRD levels before transplantation were found to be independent risk factors for OS ( P=0.037, HR=33.40, 95% CI 1.24-901.17) in a multivariate analysis. Conclusion:Patients with MDS/AML who have TP53 mutations can benefit from allo-HSCT, but patients with complex karyotypes have a worse prognosis. Meanwhile, the final flow cytometry (FCM) monitoring blast cell test before HSCT has a certain guiding significance for prognostic assessment.
