1.Efficacy of sublingual immunotherapy with Dermatophagoides farinae drops in preschool and school-age children with allergic asthma and allergic rhinitis.

OBJECTIVETo evaluate the efficacy of specific sublingual immunotherapy (SLIT) with Dermatophagoides farinae drops on children with allergic asthma and allergic rhinitis of the preschool and school-age groups of children and adolescents.
METHODThis study analyzed the efficacy of SLIT in 122 children (aged 3-14 yr) with house dust mites-induced allergic asthma and allergic rhinitis. According to the age, patients were defined as the preschool group ( ≤ 6 years old, n = 59) and school-age group (> 6 years old, n = 63). All children were treated with Dermatophagoides farinae drops for at least 2 years. Clinical observation and follow-up study were conducted during the treatment. Before and after SLIT for half a year, 1 year and 2 years, asthma symptom scores (day and night), rhinitis symptom scores and medication scores were evaluated. The adverse events were assessed during the study.
RESULTAfter SLIT for half a year, 1 year and 2 years, there were no significant differences between the preschool group (0.3 ± 0.5,0.0 ± 0.1,0.0 ± 0.0) and school-age group (0.3 ± 0.4,0.0 ± 0.1,0.0 ± 0.0) in day scores of asthma (Z value was -1.687, -0.613,0.000, all P > 0.05). There were no significant differences between the preschool group (0.2 ± 0.5,0.1 ± 0.3,0.0 ± 0.0) and school-age group (0.2 ± 0.4,0.1 ± 0.3, 0.0 ± 0.0) in night scores of asthma (Z value was -0.496, -0.486,0.000, all P > 0.05). There was no significant differences between the preschool group (1.4 ± 0.9,0.4 ± 0.5,0.1 ± 0.3) and school-age group (1.3 ± 0.9,0.5 ± 0.6,0.2 ± 0.4) in symptom scores of allergic rhinitis (Z value was -0.394, -1.166, -1.075, all P > 0.05). There were no significant differences between the preschool group (1.6 ± 0.8,0.0 ± 0.0,0.0 ± 0.0) and school-age group (1.7 ± 0.7,0.0 ± 0.0,0.0 ± 0.0) in medication scores of allergic rhinitis (Z value was -0.655,0.000,0.000, all P > 0.05). After SLIT for 2 years, most children in the preschool and school-age groups were no longer using asthma controlling medication, with no significant difference between the two groups (Z value was 0.000, P > 0.05). The overall incidence of adverse reactions was only 7%, and there was no significant difference in the incidence of adverse reactions between the two groups (χ(2) = 0.000, P > 0.05). The local adverse events were mild gastrointestinal discomfort and rash, a majority of local adverse events in the preschool group were diarrhea. No severe adverse events happened in the treatment.
CONCLUSIONSLIT with Dermatophagoides farinae drops is safe and effective in children with allergic asthma and allergic rhinitis of the preschool and school-age groups of children and adolescents, which provides evidences for early SLIT intervention of the disease.

2.Fast onset of action of sublingual immunotherapy in dust mite caused allergic rhinoconjunctivitis.

OBJECTIVE:
To investigate whether sublingual immunotherapy(SLIT) can take effect during the early phase in allergic rhinoconjunctivitis.
METHOD:
Sixty subjects diagnosed as allergic rhinoconjunctivitis were enrolled and divided into the SLIT group and the control group. The daily symptom and medication scores were recorded and analyzed.
RESULT:
After 4 weeks treatments, the daily symptoms score had significantly reduced in SLIT group(P<0. 05). After 6, 8, 10 weeks treatments, the daily symptoms scores of subjects in the SLIT group had all shown continuous reduction, with no consumption of medication. The comparison between daily symptom score at 4th week and those in 6th week or 8th week or 10th week in the SLIT group had shown no significant difference (all P > 0.05). Whereas, the daily symptoms score and daily medication score in control group had shown changes with fluctuations. The correlation analysis had indicated that the daily medication score in the control group had a significant negative correlation with the daily symptom score (F = 554.9, P < 0.01). No serious adverse events occurred in this study.
CONCLUSION
SLIT could take effect in patients with allergic rhinoconjunctivitis sensitive to house dust mite as fast as the 4th week.

3.The analysis of the allergens in 576 patients with allergic rhinitis in Qingyang of Gansu Province.

OBJECTIVE:
To investigate the allergens distribution of 576 allergic rhinitis patients in Qingyang, and to provide basic epidemiologic information for the prevention and treatment of allergic rhinitis.
METHOD:
Skin prick test was done to all the 576 patients with allergic rhinitis with 28 kinds of allergens.
RESULT:
Four hundred and eighty cases (83.3%) showed positive reaction to at least one allergen of 28 allergens. The most common allergens were Magwort (73.3%), Giant Ragweed (55.0%) Tree II (51.7%), Tree I (48.3%) Dermatophagoides farinae (43.3%) and Dermatophagoides pteronyssinus (36.7%). Moreover. the positivity decreased with age. There was no difference between male and female.
CONCLUSION
The study shows that Magwort. Giant ragweed and tree II are the most important allergens on Qingyang district.

OBJECTIVE:
To investigate the variation on different regions of the forearm when taking the allergen skin prick tests(SPTs).
METHOD:
Forty-one patients with hyper-reactive nasal symptoms were included. SPTs were done on the proximal, telecentric, middle region of right forearm and telecentric region of left forearm with house dust mite and histamine. The average diameter of wheal and the reactive grade were calculated and analyzed.
RESULT:
The diameters of the wheal on equal position of two forearms were similar (P > 0.05). The intension of SPTs reaction decreased from proximal to telecentric region of the same forearm (P < 0.05).
CONCLUSION
The intension of SPTs reaction on different region of forearm are quite different, which should be noticed in clinical practice.

5.Allergen analysis of patients with allergic rhinitis in Quanzhou.

OBJECTIVE:
To understand the distribution of allergens in patients with allergic rhinitis (AR) in Quanzhou, for the diagnosis, prevention and treatment.
METHOD:
Twenty standardized allergen underwent skin prick testing (skin prick test, SPT) were used for 1236 patients with suspected AR patients in Quanzhou, and the different age, gender and severity in patients with allergen distribution differences of positive rate were analyzed.
RESULT:
The top 5 allergens with positive skin tests were D. pteronyssinus (66.7%), D. farinae (65.5%), Blattodea (32.8%), Sea crab (18.2%), and Sea lobster (16.8%). The positive rate of SPT in the minor groups and adult groups had significant difference (P < 0.05). Gender difference between the positive rate of SPT had no significant difference (P > 0.05). There was significant difference between the positive rate in patients with a family history and history of asthma and patients without above two history (P < 0.05).
CONCLUSION
Dust Mite, Blattodea, Sea crab. Sea lobster are the most important allergens in quanzhou. Different ages of the patients with SPT positive rates vary, a family history, history of asthma patients with positive rate is higher than patients without a family history of asthma history, high positive rate.

6.Effect and safety analysis of Allergovit standardized mite allergen immunotherapy in patients with allergic rhinitis.

OBJECTIVE:
To evaluate the effect and safety analysis of Allergovit standardized dust mite subcutaneous immunotherapy in patients with allergic rhinitis.
METHOD:
From Jan 2006 to Jan 2009,425 patients with persistent were treated with Allergovit standardized dust mite allergen in 3 years, which data were retrospectively analyzed. The scores of the rhinoconjunctivitis quality of life were collected per-treatment and three years treatment. VAS (visual analogue scale) of all nasal symptoms and drug use scores were evaluated before treatment and every 6 weeks. The frequency of local and systemic reactions were recorded in the duration of dose escalation and maintenance.
RESULT:
The Overall QOL were significantly improved after 3 years treatment [pre-treatment: 3.8 (2.9; 4.5); post-treatment: 1.1 (1.0; 2.0), P < 0.01]. VAS of all nasal symptoms and drug used scores decreased dramatically after escalation period [pro-treatment: 23. 7 (20.5; 27.8), 3.9 (3.2; 4.7); post-treatment: 2.1 (1.6; 2.2), 0(0;0), P < 0.01]. Local reaction's rate was 3.15%, the frequency systemic reaction's rate was 0.90%.
CONCLUSION
The standardized specific allergen immunotherapy for allergic rhinitis is safe and effective.

7.Analysis of mite-allergic characteristics in patients with allergic rhinitis.

OBJECTIVE:
To investigate the characteristics of allergic rhinitis sensitized with mite, providing the evidence for the treatment plan of allergic rhinitis in Tianjin area.
METHOD:
The medical records of skin prick test results on 2390 allergic rhinitis patients from March 2009 to February 2012 were retrospectively studied. The data of mite sensitivity in gender, age, season, and regional differences were analyzed.
RESULT:
There was no significant difference of the positive rate of Dermatophagoides pteronyssinus and Dermatophagoides farinae between male and female (P > 0. 05); The positive rate of dust mite decreased gradually with the growth of the age in the three groups of < 18,18-39,40-60 years old, but the positive rate of the patients over the age of 60 years old increased then. There were significant differences of positive rate to dust mite in different season (P < 0.05). The positive rate of Dermatophagoides pteronyssinus was the highest in the autumn (55.4%), and the lowest in winter (39.7%). The positive rate of Dermatophagoides farinae was the highest in winter (73.8%), and the lowest in spring (47.2%). There was no significant difference of the positive rate to dust mite between urban and suburb (P > 0.05).
CONCLUSION
There were some relationship of the positive rate of dust mite with age and season, but not gender and region among allergic rhinitis in tianjin.

8.Clinical survey and analysis of allergic rhinitis patients' allergens in Guangdong developed and developing regions.

OBJECTIVE:
To survey and analyze the allergic rhinitis patients' allergens in developed region of Pearl River Delta and developing region out of Pearl River Delta in Guangdong.
METHOD:
Sixteen standard allergens were adopted for skin prick test and correlation factors were studied on 858 cases suspected allergic rhinitis from developed region of Pearl River Delta and developing region out of Pearl River Delta. Two regions patients sorts of allergen and their detection rate were compared, and the relationship between allergen positive rate and age, gender, body weight, family history was analyzed.
RESULT:
The 6 kinds allergen with higher positivity in developed region of Pearl River Delta are dermatophagoides pteronyssinus (55.4%), dermatophagoides culinae (53.9%), tropical mite (38.3%), dog hair (11.8%), blattella germanica (11.5%) and cat hair (8.1%); that from developing region out of Pearl River Delta are dermatophagoides culinae (50.4%), tropical mite(48.0%), dermatophagoides pteronyssinus (47.2%), blattella germanica (13.4%), cat hair (8.7%) and dog hair (7.1%). There is no significant difference of allergen positivity between the patients from the two regions except tropical mite. The positive rate in 15-18 years old patients was also different in the two groups. The positive rate was highest in the age from 19 to 30 years old,with a decreasing trend in younger and elder patients. There is no significant difference of the allergen positive rate between the both groups with body weight, family history and gender (P > 0.05). A higher allergen positivity was found in the patients with family history (P < 0.05).
CONCLUSION
Dermatophagoides pteronyssinus, dermatophagoides culinae, Tropical mite, dog hair, blattella germanica and cat hair are the major allergens in developed region of Pearl River Delta and developing region out of Pearl River Delta in Guangdong, but their ranking has some difference in the two regions. Patients with family history had a higher allergen positive rate.

9.Anti-allergic effects of xuebijing and potential role of heme oxygenase-1 against ovalbumin-induced murine allergic rhinitis model.

OBJECTIVE:
In this study, we investigated the anti-inflammation effects of Xuebijing in OVA-induced murine allergic rhinitis model. Furthermore, we determined whether heme oxygenase (HO)-1 is required for the protective activity of Xuebijing.
METHOD:
Airways of OVA-sensitized mice exposed to OVA challenge developed eosinophilia, mucus hypersecretion and increased cytokine levels. Levels of interleukin IL-4, IL-5, IL-13, and tumor necrosis factor (TNF)-alpha in nasal lavage fluid were measured using enzyme-linked immunosorbent assays (ELISAs). Lung tissue and nasal mucosa sections were stained with Mayer's hematoxylin and eosin for assessment of cell infiltration and mucus production, Immunohistochemistry, Real-time PCR and Western Blot analyses for HO-1 protein expression.
RESULT:
Orally administered Xuebijing significantly inhibited the number of OVA-induced inflammatory cells and IgE production, along with reduced T-helper (Th) 2 cytokine levels, such as IL-4, IL-5 and IL-13, improved the level of IFN-gamma, in nasal lavage fluid. In addition, Xuebijing induced a marked decrease in OVA-induced inflammatory cell infiltration and mucus production in nasal and lung tissues. These effects were correlated with HO-1 mRNA and protein induction.
CONCLUSION
Our results indicate that Xuebijing protects against OVA-induced airway inflammation, at least in part, via HO-1 upregulation.

10.Comparison of serum specific IgE and skin prick test in allergic rhinitis patients sensitive to dust mite.

OBJECTIVE:
To investigate the correlation of serum specific IgE and skin prick test in allergic rhinitis patients sensitive to dust mite and the difference between the results.
METHOD:
Data of 349 allergic rhinitis patients who had positive result in either Dermatophagoides pteronyssinus(Der p) or Dermatophagoides farinae (Der f) by serum specific IgE and skin prick test were statistically analyzed.
RESULT:
Grades of Skin prick test and specific IgE levels were notably relevant in these two dust mites(Der p r= 0. 568, Der f r= 0. 506, P<0. 01). There was significant difference of positive rates between serum specific IgE and skin prick test in Der p(Chi2 = 11. 605, P<0. 01)but not in Der f(Chi2 =0, P>0.05). There was no correlation between positive degree of two methods and score of clinical symptoms in allergic rhinitis.
CONCLUSION
Serum specific IgE and skin prick test were notably relevant in allergic rhinitis patients sensitive to dust mites. The positive rates of the two methods were different due to different allergen. Therefore, they could not substitute for each other. The level of serum specific IgE and positive degree of skin prick test could not reflect the degree of symptom in allergic rhinitis.
