Clinical Medicine of China 2024;40(5):369-374
Objective:To explore the relationship between calcitonin receptor (CALCR) expression and clinicopathologic features and prognosis of gastric cancer patients. Methods:A retrospective analysis was performed to collect 108 samples of gastric cancer patients treated in Department of Gastrointestinal Surgery, First Affiliated Hospital of Bengbu Medical College from January 2014 to November 2017. Immunohistochemistry (IHC) was used to analyze the differential expression of calcitonin receptors in gastric cancer and adjacent tissues, and the relationship and significance between their expression and clinical pathological parameters were analyzed K-M and ROC curves were used to assess the K-M and ROC curves were used to evaluate the effect of CALCR expression on patients' postoperative survival and to predict the 5-year postoperative survival rate. The comparison of inter group rates in count data was performed using χ2 test, and the Kaplan Meier method was used to estimate overall survival and progression free survival. The comparison of survival differences was performed using the Log-rank method test. Results:CALCR was mainly expressed in the cytoplasm and its expression in gastric cancer tissues(62.04%(67/108)) was higher than that in paracancerous tissues (13.89% (15/108)), and the differences were statistically significant ( χ2=53.16, P<0.001). The tissue infiltration depth (91.04% (61/67)), lymph node metastasis (73.1% (49/67)), and peripheral blood CA19-9 (32.8% (22/67)) in gastric cancer patients with high expression of CALCR were higher than those in the low expression group of CALCR (63.41% (26/41), 53.7% (22/41), 12.2% (5/41)), and the differences were statistically significant ( χ2 values were 12.40, 4.28, and 4.78; P values were <0.001, 0.038, and 0.016). The results of K-M curve and Log-rank test showed that among gastric cancer patients, the postoperative 5-year survival rate of patients with high expression of CALCR was lower than that of patients with low expression (11.9% (8/67) and 43.9% (18/41), Log-rank χ2=29.92, P<0.001). The results of the ROC curve showed that the area under the curve was 0.772, sensitivity was 87.8%, specificity was 53.7%, indicating that CALCR is of high value in predicting the 5-year survival rate of gastric cancer patients after surgery. Conclusion:CALCR is highly expressed in gastric cancer tissue and is associated with the pathological characteristics of patients. High expression of CALCR suggests poor prognosis, indicating that it has the potential to become an important indicator for evaluating the survival prognosis of gastric cancer patients.

Venous thromboembolism (VTE) is a complication in children with acute lymphoblastic leukemia (ALL). The Chinese Children's Cancer Group-ALL-2015 protocol was carried out in China, and epidemiology, clinical characteristics, and risk factors associated with VTE were analyzed. We collected data on VTE in a multi-institutional clinical study of 7640 patients with ALL diagnosed in 20 hospitals from January 2015 to December 2019. First, VTE occurred in 159 (2.08%) patients, including 90 (56.6%) during induction therapy and 108 (67.92%) in the upper extremities. T-ALL had a 1.74-fold increased risk of VTE (95% CI 1.08-2.8, P = 0.022). Septicemia, as an adverse event of ALL treatment, can significantly promote the occurrence of VTE (P < 0.001). Catheter-related thrombosis (CRT) accounted for 75.47% (n = 120); and, symptomatic VTE, 58.49% (n = 93), which was more common in patients aged 12-18 years (P = 0.023), non-CRT patients (P < 0.001), or patients with cerebral thrombosis (P < 0.001). Of the patients with VTE treated with anticoagulation therapy (n = 147), 4.08% (n = 6) had bleeding. The VTE recurrence rate was 5.03% (n = 8). Patients with VTE treated by non-ultrasound-guided venous cannulation (P = 0.02), with residual thrombus (P = 0.006), or with short anticoagulation period (P = 0.026) had high recurrence rates. Thus, preventing repeated venous puncture and appropriately prolonged anticoagulation time can reduce the risk of VTE recurrence.

Chinese Acupuncture & Moxibustion 2023;43(5):565-568
With three representative types of gynecological diseases (dysmenorrhea, pelvic inflammation, polycystic ovary syndrome) as examples, the application methods of meridian and acupoint diagnosis for gynecological diseases treated with acupuncture and moxibustion are discussed. During clinical diagnosis and treatment, it is recommended to examine the patient's leg segment along the three yin meridians of foot, aiming to explore the positive reactions of the meridians and acupoints (color, shape, skin temperature, sensory abnormalities, etc.). Acupuncture and moxibustion treatment at this positive reaction place can improve the clinical efficacy. Meridian and acupoint diagnosis could provide basis for meridian syndrome differentiation, thus guiding the selection of acupoint prescriptions; it is also helpful to clarify the deficiency, excess, cold and heat of the disease nature, thus guiding the selection of acupuncture and moxibustion methods. In addition, it is an auxiliary method to estimate the prognosis and outcome of the disease.

Objective: To explore the clinical characteristics of various types of infected pancreatic necrosis(IPN) and the prognosis of different treatment methods in the imaging classification of IPN proposed. Methods: The clinical data of 126 patients with IPN admitted to the Department of Pancreatic and Biliary Surgery, the First Affiliated Hospital of Harbin Medical University from December 2018 to December 2021 were analyzed retrospectively. There were 70 males(55.6%) and 56 females(44.4%), with age(M(IQR)) of 44(17)years (range: 12 to 87 years). There were 67 cases(53.2%) of severe acute pancreatitis and 59 cases (46.8%) of moderately severe acute pancreatitis. All cases were based on the diagnostic criteria of IPN. All cases were divided into Type Ⅰ(central IPN)(n=21), Type Ⅱ(peripheral IPN)(n=23), Type Ⅲ(mixed IPN)(n=74) and Type Ⅳ(isolated IPN)(n=8) according to the different sites of infection and necrosis on CT.According to different treatment strategies, they were divided into Step-up group(n=109) and Step-jump group(n=17). The clinical indicators and prognosis of each group were observed and analyzed by ANOVA,t-test,χ2 test or Fisher exact test, respectively. Results: There was no significant difference in mortality, complication rate and complication grade in each type of IPN(all P>0.05). Compared with other types of patients, the length of stay (69(40)days vs. 19(19)days) and hospitalization expenses(323 000(419 000)yuan vs. 60 000(78 000)yuan) were significantly increased in Type Ⅳ IPN(Z=-4.041, -3.972; both P<0.01). The incidence of postoperative residual infection of Type Ⅳ IPN was significantly higher than that of other types (χ2=16.350,P<0.01). There was no significant difference in the mortality of patients with different types of IPN between different treatment groups. The length of stay and hospitalization expenses of patients in the Step-up group were significantly less than those in the Step-jump group(19(20)days vs. 33(35)days, Z=-2.052, P=0.040;59 000(80 000)yuan vs. 122 000(109 000)yuan,Z=-2.317,P=0.020). Among the patients in Type Ⅳ IPN, the hospitalization expenses of Step-up group was significantly higher than that of Step-jump group(330 000(578 000)yuan vs. 141 000 yuan,Z=-2.000,P=0.046). The incidence of postoperative residual infection of Step-up group(17.4%(19/109)) was significantly lower than that of Step-jump group(10/17)(χ2=11.980, P=0.001). Conclusions: Type Ⅳ IPN is more serious than the other three types. It causes longer length of stay and more hospitalization expenses. The step-up approach is safe and effective in the treatment of IPN. However, for infected lesions which are deep in place, difficult to reach by conventional drainage methods, or mainly exhibit "dry necrosis", choosing the step-jump approach is a more positive choice.
