Objective:To analyze the expression levels and clinical significance of interferon (IFN)-α/β in the renal cortex and serum of children with lupus nephritis (LN).Methods:A total of 32 children with LN diagnosed in the pediatric nephrology department of the Second Xiangya Hospital of Central South University from December 2017 to September 2020 were selected as the study subjects (LN group). The normal kidney control group consisted of 3 normal kidney transplant volunteers who underwent biopsy of kidney tissue (normal kidney control group), while 14 healthy children who underwent physical examination were collected as the normal control group. According to the Systemic Lupus Erythematosus Disease Activity Index (SLEDAI), LN patients were divided into mild activity group ( n=8), moderate activity group ( n=9), and severe activity group ( n=15). According to the International Society of Nephrology/Society of Nephrology (ISN/RPS) 2003 LN classification criteria, pathological classification was performed (3 cases in the mild pathological damage group, 8 cases in the moderate pathological damage group, and 11 cases in the severe pathological damage group); Immunohistochemistry was used to detect the expression and distribution of IFN-α/β in glomeruli and renal interstitium; Enzyme linked immunosorbent assay (ELISA) was used to detect the concentration of IFN-α/β in serum samples and analyze its correlation with the pathological classification and disease activity of LN patients. Results:The serum and renal cortex IFN-α/β levels in the LN group were higher than those in the normal control group and normal kidney control group, respectively (all P<0.05). The average level of serum IFN-α/β in the heavy activity group was higher than that in the light and moderate activity groups (all P<0.05). The serum and renal cortex IFN-α/β levels in the severe pathological damage group were significantly higher than those in the mild and moderate pathological damage groups (all P<0.05). Conclusions:IFN-α/β in the renal cortex is closely related to renal injury in LN; Serum IFN-α/β can assist in evaluating the disease activity level of LN to a certain extent.

ObjectiveTo explore the amelioration of cognitive dysfunction in diabetes mellitus (DM) by Jianpi Qinghua prescription (JPQH) based on type 2 diabetes (T2DM) model rats. MethodFifty healthy male Wistar rats of SPF grade were randomly divided into control group (n=10) and experimental group (n=40). The rats in the control group were fed conventionally, while those in the experimental group were fed on a high-sugar, high-fat diet for six weeks and administered with streptozotocin (STZ) for the induction of the DM model. The model rats were randomly divided into model group, sitagliptin group (1.2 g·L-1), pioglitazone group (0.8 g·L-1), and JPQH group (1.3 g·mL-1), with 10 rats in each group. After six weeks of drug intervention, the changes in body weight, blood glucose, and other related indexes of each group were recorded. Enzyme-linked immunosorbent assay (ELISA) was performed to detect the levels of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), and interleukin-6 (IL-6) in the peripheral blood and brain. The Morris water maze test was used to evaluate the cognitive function in rats. Hematoxylin-eosin (HE) staining was used to observe the pathological morphology of the hippocampal CA region. The amyloid β-protein 40 (Aβ40) level was detected by immunohistochemistry. The protein expression of t-tau and p-tau in hippocampal neurons of rats was detected by Western blot. ResultCompared with blank group, the body weight of model group was significantly decreased (P<0.05), blood glucose level was significantly increased (P<0.01), inflammatory cytokines TNF-α and IL-1β were increased (P<0.05), learning and spatial ability were significantly decreased (P<0.01), the arrangement of hippocampal cells was loose and disordered, and the intercellular space was significantly increased. The number of cells decreased significantly, and the expression of Aβ40 increased significantly. and increased t-tau and p-tau protein content in the hippocampus (P<0.01). Compared with model group, the JPQH group showed reduced blood glucose (P<0.01), decreased TNF-α and IL-1β levels in the peripheral blood and cerebrospinal fluid (P<0.05), a downward trend of IL-6 without a statistical difference, improved learning and spatial memory ability (P<0.01), densely arranged cells in the hippocampal CA1 area, increased cell number, reduced Aβ40 expression, and decreased p-tau protein expression (P<0.05). ConclusionJPQH can prevent cognitive dysfunction in DM by reducing inflammatory factor levels, decreasing neurotoxicity caused by Aβ40 deposition, and inhibiting hyperphosphorylation of tau protein in DM rats.

Coronary heart disease (CHD) has become a major public health issue in China. Gut microbiota has gradually become a CHD research hotspot in terms of auxiliary diagnosis, treatment and prevention targets. Results from multiple studies have shown that gut microbiota dysbiosis may mediate the process of CHD directly or indirectly through their metabolites, and some intestinal probiotics could inhibit the progression of atherosclerosis. The research advances on the related mechanism and their diagnostic efficacy as biomarkers of gut microbiota and metabolites in CHD are reviewed here to provide a reference for current and future clinical application.

Endometrial cancer has been identified by The Cancer Genome Atlas program with four molecular subtypes by genome sequence analysis. Clinical trials to select suitable immunotherapeutic agents based on the different immune characteristics of each subtype have been conducted in several countries and have made important progress. The main clinical applications of immune checkpoint inhibitors include anti-programmed cell death protein-1/programmed death-ligand 1 antibodies and poly ADP-ribose polymerase inhibitors. Optimizing drug selection and drug combination based on the target characteristics of different immune checkpoint inhibitors may provide new opportunities for immunotherapy of endometrial cancer and bring new light to improve survival rates.

Objective:To explore the application value of bedside lung ultrasound score(LUS)in evaluating patients with primary graft dysfunction(PGD)after lung transplantation. Methods:A total of 35 PGD patients after lung transplantation in surgical intensive care unit from June 2018 to May 2020 were selected as research objects. After lung transplantation, one physician collected the clinical data, including age, gender, preoperative 24 h APACHE II score, postoperative hemodynamic parameters, lactate and respiratory parameters; another physician was responsible for LUS examination and LUS at 12/24/48/72 h post-operation. The correlation between LUS and oxygenation index was analyzed by bivariate correlation analysis. Receiver operating characteristic curve(ROC)was utilized for calculating the predictive value, sensitivity and specificity of LUS score for severe PGD.Results:A negative correlation existed between LUS and oxygenation index in single lung transplant( r=-0.536, P<0.01)and a negative correlation between LUS and oxygenation index ( r=-0.518, P<0.01)in double lung transplant. The area under ROC curve of LUS showed that the predictive value of LUS of severe PGD patients with single lung transplant was 7.0 with a sensitivity of 86.7% and a specificity of 72.1%, the predicted value of LUS of severe PGD in double lung transplant was 13.0 with a sensitivity of 83.3% and a specificity of 60.0%. Conclusions:Lung ultrasound is simple and easy to operate and LUS can evaluate the severity of PGD patients after lung transplantation. It has a high potency of guiding clinical diagnosis and treatment.
