1.Treatment of Globus Hystericus Based on the Theory of "Regulating the Five Zang Organs and Harmonizing the Spleen and Stomach"

Based on the concept of "regulating the five zang organs and harmonizing the spleen and stomach", globus hystericus is believed to originate from dysfunction of the five zang organs and disharmony of the spleen and stomach. Treatment primarily focuses on regulating the spleen and stomach while also considering other affected organs, with a self-prescribed Anpiwei Jingyan Formula (安脾胃经验方) for harmonizing the spleen and stomach as the foundational treatment. Additionally, syndrome-based modifications are applied according to imbalances in the heart, lung, kidney, or liver. For heart-yang deficiency, modified Linggui Zhugan Decoction (苓桂术甘汤) could be combined; for heart-yin deficiency, modified Tianwang Buxin Pill (天王补心丹) could be combined. For lung failing to disperse and descend and fluid retention, modified Sanao Decoction (三拗汤) could be combined; for lung and stomach yin deficiency, modified Shashen Maidong Decoction (沙参麦冬汤) could be combined. For kidney-yang deficiency with ascending counterflow of cold water, modified Jingui Shensi Pill (金匮肾气丸) could be combined; for kidney-yin deficiency, modified Liuwei Dihuang Pill (六味地黄丸) could be combined. For liver constraint and spleen deficiency, modified Sini Powder (四逆散) could be combined; for liver-yin deficiency or liver stagnation transforming into fire and attacking the stomach, modified Yiguan Decoction (一贯煎) could be combined.
2.Treatment of Recurrent Aphthous Ulcers from the Perspective of "Heart"

Based on the traditional Chinese medicine theory that "all pain, itching, and sores are related to the heart", this paper proposes treating recurrent aphthous ulcers from the perspective of the heart. It suggests that excessive heart fire and tissue erosion due to flaming fire in the heart meridian constitute the core pathogenesis of this condition. Hyperactive heart fire is identified as the key pathogenic factor, while heart yin deficiency, obstruction of the heart collaterals, and malnourishment of the heart spirit are considered significant contributing factors. Clinically, the treatment follows the principle of clearing heart fire as the main strategy, supplemented by nourishing yin, activating collaterals, and calming the spirit. The self-formulated Qingxin Yuchuang Formulation (清心愈疮方) serves as the base prescription, with flexible modifications incorporating the Yuyin Formulation (育阴方), Huoxue Formulation (活血方), and Yu'an Formulation (郁安方) to address specific syndromes involving heart yin deficiency, collateral blockage, and emotional disturbance.
3.Zanubrutinib inhibits macrophage infiltration to ameliorate renal fibrosis after renal ischemia-reperfusion injury

Objective To explore the action mechanism of Bruton's tyrosine kinase (BTK) inhibitor zanubrutinib on renal fibrosis after renal ischemia-reperfusion injury (RIRI). Methods C57BL/6 mice were randomly divided into sham operation group, modeling group and modeling + zanubrutinib treatment group (zanubrutinib group), with 5 mice in each group. The groups underwent sham operation, RIRI modeling and RIRI modeling + zanubrutinib (5 mg/kg) treatment, respectively. Tissues were collected after 21 days. The morphological changes of the kidneys, histopathological changes, levels of M1 macrophages in the kidneys, inflammatory responses in the kidneys, and the expression of related inflammatory signaling pathways of macrophages induced by lipopolysaccharide(LPS) + interferon(IFN)-γ in vitro after lentivirus transfection were observed. Results Compared with the sham operation group, the kidneys of the modeling group mice shrank, the ratio of unilateral kidney weight to mouse body weight decreased, renal tubular interstitial fibrosis worsened, and the expression of α-smooth muscle actin (α-SMA) and type I collagen in the kidneys increased. The expression of F4/80 and CD86 in the kidneys increased, the lumen of the renal proximal convoluted tubules was significantly dilated, cellular debris accumulated in the lumen and inflammatory cell infiltration occurred, and the messenger RNA (mRNA) levels of CD86, tumor necrosis factor (TNF)-α, interleukin (IL)-6, inducible nitric oxide synthase (iNOS) and IL-1β in the kidneys increased. Compared with the modeling group, the kidneys of the zanubrutinib group mice enlarged after RIRI, the ratio of unilateral kidney weight to mouse body weight increased, renal tubular interstitial fibrosis was alleviated, and the expression of α-SMA and type I collagen in the kidneys decreased. The expression of F4/80 and CD86 in the kidneys decreased, the number of CD45+ lymphocytes and CD11b+ F4/80+ macrophages in the kidneys decreased, the infiltration of CD11b+ F4/80+ and CD86+ macrophages in the damaged tissue decreased, the degree of renal inflammatory pathological changes was milder, and the mRNA levels of CD86, TNF-α, IL-6, iNOS and IL-1β in the kidneys decreased. In vitro experiments using LPS+IFN-γ to induce M1-type macrophages found that the phosphorylation levels of phosphatidylinositol-3-kinase (PI3K), protein kinase B (Akt), and nuclear factor (NF)-κB increased, while the phosphorylation levels decreased after BTK knockdown, indicating that BTK knockdown may inhibit the PI3K/Akt and NF-κB related inflammatory signaling pathways, thereby reducing the pro-inflammatory effects of LPS+IFN-γ induced M1-type macrophages. Conclusions Zanubrutinib may alleviate renal fibrosis after RIRI by inhibiting the PI3K/Akt and NF-κB related inflammatory signaling pathways, reducing the infiltration of M1 macrophages and the expression of related inflammatory factors, providing potential evidence for its clinical application.
4.Research progress on association between macrophages and ischemia-reperfusion injury

Ischemia-reperfusion injury (IRI) is an extremely complicated pathophysiological process, which may occur during the process of myocardial infarction, stroke, organ transplantation and temporary interruption of blood flow during surgery, etc. As key molecules of immune system, macrophages play a vital role in the pathogenesis of IRI. M1 macrophages are pro-inflammatory cells and participate in the elimination of pathogens. M2 macrophages exert anti-inflammatory effect and participate in tissue repair and remodeling and extracellular matrix remodeling. The balance between macrophage phenotypes is of significance for the outcome and treatment of IRI. This article reviewed the role of macrophages in IRI, including the balance between M1/M2 macrophage phenotype, the mechanism of infiltration and recruitment into different ischemic tissues. In addition, the potential therapeutic strategies of targeting macrophages during IRI were also discussed, aiming to provide reference for alleviating IRI and promoting tissue repair.

In recent years,the infection of nontuberculous mycobacterium(NTM)has been increasing rapidly,which captivates widespread attention.The infection rate of NTM in kidney transplant recipients is more significantly elevated due to the impact of immunosuppressive drugs and other factors.However,due to the lack of sufficient research evidence,relevant guidelines for the diagnosis and treatment of NTM after kidney transplantation are still lacking.To further standardize the diagnosis and treatment of NTM disease in kidney transplant recipients,and deepen medical practitioners'understanding and diagnosis and treatment of NTM disease in organ transplantation in China,Branch of Organ Transplantation of Chinese Medical Association organized relevant experts to formulate this guideline by referring to the latest edition of"An official ATS/IDSA statement:diagnosis,treatment,and prevention of nontuberculous mycobacterial diseases","Expert Consensus on the Diagnosis and Treatment of Nontuberculous Mycobacterial Disease",and"Technical Specification for Clinical Diagnosis and Treatment of Nontuberculous Mycobacteria in Organ Transplant Recipients(2019 Edition)",and considering the characteristics of kidney transplant recipients.
6.Discussion on Treatment of Urethral Syndrome by Liu Qiquan Based on"Regulating Five Internal Organs to Calm Spleen and Stomach"

This article summarized Professor Liu Qiquan's clinical experience in treating urethral syndrome from the perspective of"regulating the five internal organs to calm the spleen and stomach".It is believed that urethral syndrome is located in the urethra,and its core pathogenesis is disharmony between the spleen and stomach,obstruction of dampness,and unfavorable water channel.Therefore,"tranquilizing the spleen and stomach"should be carried out throughout the treatment.At the same time,the occurrence of the disease is closely related to the disorder of the five internal organs.External sensation,internal injury,emotion,diet,fatigue and visceral disorders will affect the function of the five internal organs and then the disease will occur.In clinical practice,based on the idea of"regulating the five internal organs to calm the spleen and stomach",according to the symptoms and manifestations of the patients,the treatment can be guided by comprehensively using the methods of raising the clear and lowering the turbid to harmonize the stomach and dredge the drench,nourishing the heart and clearing the heart to purge heat and dredge the drench,benefiting the lung and purging heat to relieve dampness and dredge the drench,tonifying the kidney and warming the yang to dissipate qi and dredge the drench,dredging the liver and clearing the liver to promote the flow of qi and dredge the drench,which can make the five internal organs harmonious,benefit the waterways.Finally,the symptoms would be reduced and the disease would be cured.
7.Clinical analysis of allergen reactivity and atopic disease history in 168 patients with chronic inducible urticaria

Objective:To analyze the relationship between allergen reactivity, atopic disease history and clinical features in patients with chronic inducible urticaria (CIndU) .Methods:A retrospective analysis was conducted on clinical data and follow-up results from 168 patients with CIndU in the Department of Dermatology, Southwest Hospital of Army Medical University from June 2014 to June 2015. Associations were analyzed between allergen reactivity, atopic disease history and clinical characteristics (including patient global assessment [PGA] scores, pruritus intensity, dermatology life quality index [DLQI], proportions of cases with complicated angioedema, natural course, etc.) in patients with different CIndU subtypes. Chi-square test, Mann-Whitney U test and Kaplan-Meier survival analysis were used for statistical analysis. Results:Among the 168 patients with CIndU, 117 were diagnosed with symptomatic dermographism (SD) , 32 with cold contact urticaria (CCU) , 5 with heat contact urticaria (HCU) , and 14 with cholinergic urticaria (CholU) ; there were 46 (39.3%) , 14 (43.8%) , 3, and 9 patients with positive skin prick test (SPT) among the patients with SD, CCU, HCU and CholU respectively, and no significant difference was observed in the positive rate of SPT among patients with different CIndU subtypes ( χ2 = 3.86, P = 0.283) . The SPT-positive CIndU patients showed significantly increased PGA scores, pruritus scores, DLQI scores and proportions of cases with complicated angioedema compared with the SPT-negative patients (all P<0.05) ; the CIndU patients with a personal or family history of atopic diseases also showed significantly increased PGA and DLQI scores compared with those without (both P < 0.05) . For different CIndU subtypes, the pruritus scores, PGA scores, DLQI scores, and proportions of cases with complicated angioedema were significantly higher in the SPT-positive SD patients than in the SPT-negative SD patients (all P < 0.05) ; the DLQI scores were significantly higher in the SPT-positive CholU patients than in the SPT-negative CholU patients ( Z = -2.28, P = 0.019) ; the pruritus scores were significantly higher in the CCU patients with a personal or family history of atopic diseases than in those without ( Z =-2.41, P = 0.022) . Conclusion:The allergen reactivity and atopic disease history of CIndU patients were associated with disease severity, pruritus intensity, quality of life, and the proportion of cases with complicated angioedema, but their relationship with the natural course of CIndU needs to be confirmed by further studies.
8.Efficacy observation of acupuncture plus point application for pain after thoracoscopic radical lung cancer surgery

Objective:To observe the clinical efficacy of acupuncture plus point application in managing pain after thoracoscopic radical lung cancer surgery(TRLCS). Methods:A total of 120 patients undergoing TRLCS were randomized into a treatment group and a control group,with 60 cases in each group.Both groups received patient-controlled intravenous analgesia(PCIA)to relieve postoperative wound pain.The control group did not receive any other interventions.The treatment group started acupuncture treatments 4 h after the surgery with point application between two acupuncture sessions;the acupuncture treatment was conducted 4,24,48,and 72 h after the surgery.At the above 4 time points,the visual analog scale(VAS)score and additional PCIA drug consumption were recorded.The 5-hydroxytryptamine(5-HT)content in the peripheral blood was determined 4 h and 72 h after the surgery. Results:The treatment group was superior to the control group in comparing the total effective rate(P<0.05).After the intervention,the VAS score decreased in both groups(P<0.05);the VAS score presented different decreasing patterns at each time point in the two groups,and the score dropped more significantly in the treatment group than in the control group(P<0.05).The total PICA drug consumption varied in the two groups after the surgery;the additional analgesic consumption was notably smaller in the treatment group than in the control group(P<0.05).The decrease in the 5-HT content in the peripheral blood also varied in the two groups;the peripheral blood 5-HT content was significantly lower in the treatment group than in the control group(P<0.05). Conclusion:Acupuncture plus point application can significantly relieve wound pain after TRLCS.

10.Establishment and analysis of mouse model of acute antibody-mediated rejection in heart transplantation

Objective To establish a mouse model of acute antibody-mediated rejection (AMR) in heart transplantation and to analyze its characteristics. Methods Mouse models of heart transplantation and skin transplantation were established. According to different treatment methods, all animals were divided into the homologous control group, non-sensitized group, pre-sensitized group and pre-sensitized+ ciclosporin group (9 donors and 9 recipients in each group). The graft survival time, donor-specific antibody (DSA) level and pathological manifestations of each group were observed, and the characteristics of rejection were analyzed. Results In the homologous control group, the cardiac grafts of the mice survived for a long period of time during the 3-month observation period. The survival time of the cardiac grafts in the non-sensitized group, pre-sensitized group and pre-sensitized+ciclosporin group was (7.0±0.7) d, (2.6±0.5) d and (5.0±0.7) d, respectively.
