1.Treatment of Recurrent Aphthous Ulcers from the Perspective of "Heart"

Based on the traditional Chinese medicine theory that "all pain, itching, and sores are related to the heart", this paper proposes treating recurrent aphthous ulcers from the perspective of the heart. It suggests that excessive heart fire and tissue erosion due to flaming fire in the heart meridian constitute the core pathogenesis of this condition. Hyperactive heart fire is identified as the key pathogenic factor, while heart yin deficiency, obstruction of the heart collaterals, and malnourishment of the heart spirit are considered significant contributing factors. Clinically, the treatment follows the principle of clearing heart fire as the main strategy, supplemented by nourishing yin, activating collaterals, and calming the spirit. The self-formulated Qingxin Yuchuang Formulation (清心愈疮方) serves as the base prescription, with flexible modifications incorporating the Yuyin Formulation (育阴方), Huoxue Formulation (活血方), and Yu'an Formulation (郁安方) to address specific syndromes involving heart yin deficiency, collateral blockage, and emotional disturbance.

This article summarized Professor Liu Qiquan's clinical experience in treating urethral syndrome from the perspective of"regulating the five internal organs to calm the spleen and stomach".It is believed that urethral syndrome is located in the urethra,and its core pathogenesis is disharmony between the spleen and stomach,obstruction of dampness,and unfavorable water channel.Therefore,"tranquilizing the spleen and stomach"should be carried out throughout the treatment.At the same time,the occurrence of the disease is closely related to the disorder of the five internal organs.External sensation,internal injury,emotion,diet,fatigue and visceral disorders will affect the function of the five internal organs and then the disease will occur.In clinical practice,based on the idea of"regulating the five internal organs to calm the spleen and stomach",according to the symptoms and manifestations of the patients,the treatment can be guided by comprehensively using the methods of raising the clear and lowering the turbid to harmonize the stomach and dredge the drench,nourishing the heart and clearing the heart to purge heat and dredge the drench,benefiting the lung and purging heat to relieve dampness and dredge the drench,tonifying the kidney and warming the yang to dissipate qi and dredge the drench,dredging the liver and clearing the liver to promote the flow of qi and dredge the drench,which can make the five internal organs harmonious,benefit the waterways.Finally,the symptoms would be reduced and the disease would be cured.

4.Construction of "Expert consensus on diagnosis and treatment of chronic inducible urticaria in China (2023) " based on the Delphi method

Objective:To construct "Expert consensus on diagnosis and treatment of chronic inducible urticaria in China (2023) " based on the Delphi method, and to provide a methodological basis for consensus construction.Methods:After systematic search and evaluation of the literature related to chronic inducible urticaria, the first draft of "Expert consensus on diagnosis and treatment of chronic inducible urticaria in China (2023) " was written, and a questionnaire was designed for expert consultation. A representative sample of 25 experts was selected to conduct two rounds of correspondence consultation via electronic questionnaire in strict accordance with the Delphi method, and the content of the consensus was revised and improved according to the consultation results.Results:The response rates in the two rounds of questionnaire consultation were both 100%, and the expert authority coefficient was 0.92 ± 0.09. In the first round of consultation, the coefficients of variation (CV values) of 9 items were greater than 20%, and the mean agreement degree of 3 items was less than 7 points; in the second round of consultation, the CV values of all items were less than 15%, the agreement degree of the above 3 items whose mean agreement degree was less than 7 points in the first round of consultation all rose to over 7 points, and the median agreement degree of all items was greater than or equal to 8 points. Reliability analysis of the two rounds of questionnaire results showed that the Cronbach α coefficient and standardized Cronbach α coefficient were both greater than 0.9; the P values in the agreement tests by using Kendall′s coefficient of concordance for the two rounds of questionnaire results were both less than 0.001, and the Kendall′s coefficients of concordance were 0.170 and 0.219 in the first and second rounds of questionnaire consultation, respectively. Conclusion:The Delphi method-based "Expert consensus on diagnosis and treatment of chronic inducible urticaria in China (2023) " is highly representative, authoritative and reliable; this study also provides a methodological reference for the formulation and research of consensus.
5.Clinical analysis of allergen reactivity and atopic disease history in 168 patients with chronic inducible urticaria

Objective:To analyze the relationship between allergen reactivity, atopic disease history and clinical features in patients with chronic inducible urticaria (CIndU) .Methods:A retrospective analysis was conducted on clinical data and follow-up results from 168 patients with CIndU in the Department of Dermatology, Southwest Hospital of Army Medical University from June 2014 to June 2015. Associations were analyzed between allergen reactivity, atopic disease history and clinical characteristics (including patient global assessment [PGA] scores, pruritus intensity, dermatology life quality index [DLQI], proportions of cases with complicated angioedema, natural course, etc.) in patients with different CIndU subtypes. Chi-square test, Mann-Whitney U test and Kaplan-Meier survival analysis were used for statistical analysis. Results:Among the 168 patients with CIndU, 117 were diagnosed with symptomatic dermographism (SD) , 32 with cold contact urticaria (CCU) , 5 with heat contact urticaria (HCU) , and 14 with cholinergic urticaria (CholU) ; there were 46 (39.3%) , 14 (43.8%) , 3, and 9 patients with positive skin prick test (SPT) among the patients with SD, CCU, HCU and CholU respectively, and no significant difference was observed in the positive rate of SPT among patients with different CIndU subtypes ( χ2 = 3.86, P = 0.283) . The SPT-positive CIndU patients showed significantly increased PGA scores, pruritus scores, DLQI scores and proportions of cases with complicated angioedema compared with the SPT-negative patients (all P<0.05) ; the CIndU patients with a personal or family history of atopic diseases also showed significantly increased PGA and DLQI scores compared with those without (both P < 0.05) . For different CIndU subtypes, the pruritus scores, PGA scores, DLQI scores, and proportions of cases with complicated angioedema were significantly higher in the SPT-positive SD patients than in the SPT-negative SD patients (all P < 0.05) ; the DLQI scores were significantly higher in the SPT-positive CholU patients than in the SPT-negative CholU patients ( Z = -2.28, P = 0.019) ; the pruritus scores were significantly higher in the CCU patients with a personal or family history of atopic diseases than in those without ( Z =-2.41, P = 0.022) . Conclusion:The allergen reactivity and atopic disease history of CIndU patients were associated with disease severity, pruritus intensity, quality of life, and the proportion of cases with complicated angioedema, but their relationship with the natural course of CIndU needs to be confirmed by further studies.
6.Insights into the progress in targeted therapy of atopic dermatitis from the perspective of its pathogenesis

The pathogenesis of atopic dermatitis is complex, and mainly related to genetic background, environmental stimuli, epidermal barrier defects, immune disorders, and other factors. Using Th2-type inflammatory cytokines and the JAK-STAT pathway as therapeutic targets has become a hotspot in drug development for atopic dermatitis. By now, multiple monoclonal antibodies and small-molecule drugs have been successfully used in the treatment of atopic dermatitis, marking that the treatment of atopic dermatitis has entered the era of precision therapy and targeted therapy.
7.Treatment of chronic inducible urticaria

At present, the pathogenesis of chronic inducible urticaria (CIndU) is not completely clear, and it is often difficult to achieve satisfactory symptom control only by avoiding triggers. In recent years, some progress has been made in the treatment of CIndU in China and other countries, such as traditional antihistamines, non-antihistamines, biological agents, small-molecule targeted drugs and other drug treatments, as well as physical therapies such as trigger-point desensitization therapy. This review comprehensively and systematically outlines and summarizes relevant progress.

9.Investigation on the willingness of organ donation of the floating population in Guangdong Province and its ;influence factors

Objective To investigate the current status of the willingness of organ donation among floating population in Guangdong Province and explore its influence factors.Methods The questionnaire survey was conducted to investigate general information of the floating population and their willingness of organ donation,knowledge of organ donation,attitude towards organ donation and death as well as the willingness of family discussion (organ donation),etc.The correlation analysis of variables and the willingness of organ donation in floating population was conducted by Spearman rank correlation,and the multivariate analysis was conducted by multiple linear regression model.Results Among the floating population,the people who were female,from city,owned an average monthly household income per capita of RMB 4 001 -5 000 yuan,got acquaintance with the organ donors or their families,got acquaintance with accepted the organ donors or their families had higher willingness of organ donation (all in P <0.05 ).Scores of the willingness of organ donation,knowledge of organ donation,attitude towards organ donation and the willingness of family discussion were 1.95 ± 0.91 ,5.02 ±2.40,92.00 ±1 6.78 and 3.98 ±1 .28 respectively.The willingness of family discussion,knowledge of organ donation and recognition of the value of organ donation showed positive correlation with the willingness of organ donation (P <0.05-0.01 ).The cause hindering organ donation and fear of death showed negative correlation with the willingness of organ donation (all in P <0.01 ).Five variables were integrated into the multiple linear regression analysis model,including willingness of family discussion,knowledge of organ donation,recognition of the value of organ donation, cause hindering organ donation and fear of death.Conclusions The willingness of organ donation among the floating population in Guangdong Province is on or below the medium level,and its influence factors include willingness of family discussion,knowledge of organ donation,recognition of the value of organ donation,cause hindering organ donation and fear of death.
10.Implication of renal biopsy in donor and recipients with delayed graft function

Objective To investigate the renal pathologic changes in both donors and transplant recipients with delayed graft function (DGF).Methods The clinical and laboratory data were retrospectively analyzed in 144 renal recipients with DGF.All the patients received renal biopsy,and donors' biopsy was performed on 131 recipients.The pathological changes were examined under the light microscopy (LM),immunofluorescence (IF) and electron microscopy (EM).Results (1) The incidence of DGF was 10.16%-7.48% during 1994 to 2005,and decreased to 5.35 % during 2006 to 2009.(2) Anuria occurred in 24 cases (16.67 %),oliguria in 24 (16.67%) and hypertension in 68 cases (47.22 %).The enlargement of transplanting kidney and the increased vascular resistance was detected in 79.67 % (98/123 cases) and 45.53 % (56/123 cases) respectively by ultrasound examination.(3) The level of serum creatinine was ranged from 451 to 707 μmol/L.The high level of urinary NAG enzyme was found in 102 cases (70.83 %),proteinuria in 79 recipients (54.86 %) and hematuria in 77 cases (53.47 %).(4) The acute rejection was observed in 66 cases (45.83 %),toxicity of CNI in 22 (15.28 %),IgA nephropathy in 18 (12.50 %),acute tubular necrosis in 8 (5.56 %),and recurrent FSGS in 2 cases (1.39 %).(5) In most recipients (66/109 cases,60.55 %)immunosuppressive regimen altered and renal replacement therapy was given.Conclusion The causes of DGF are complicated.The quality of donors' kidney and early histological changes of recipients are contributed to the development of DGF.It is necessary to perform renal biopsy not only in donors but also in recipients with DGF.And kidney biopsy in transplanted patients was also beneficial to the treatment.
