1.The role of silent information regulator in periodontitis

		                        			BACKGROUND:Periodontitis is an inflammatory and destructive disease with plaque biofilm as the main pathogenic material,which occurs in the gingiva,periodontal ligament,alveolar bone and cementum.The antigen of bacterial complex and its secreted toxin and enzyme directly lead to the destruction of periodontal tissue and trigger the host's immune response,causing indirect damage to the body tissue.Silence information regulatory factors(Sirtuins,SIRTs)play an important role in anti-aging,anti-oxidative stress,regulating inflammation,and mediating autophagy,and are closely related to the occurrence and development of periodontitis. OBJECTIVE:To review the research status of Sirtuins in periodontitis. METHODS:The first author used the computer to search the relevant research regarding the role of Sirtuins in periodontitis in PubMed,Web of Scene,CNKI and WanFang databases.The key words were"Sirtuins,Sirtuin1-7,periodontitis"in English and Chinese.After literature screening,57 articles were included for review and analysis. RESULTS AND CONCLUSION:SIRT1,SIRT2,SIRT3,and SIRT6 participate in regulating the occurrence and development of periodontitis.Inhibition of SIRT1 expression may be the target of periodontitis treatment,while overexpression of SIRT1 can inhibit periodontitis and protect periodontal tissue.The activator of SIRT1 can reduce the inflammation of periodontal tissue and improve the systemic pathological changes caused by periodontitis.SIRT2 is involved in nicotinamide phosphoribosyltransferase-mediated periodontal inflammation and plays a role in the treatment and prognosis of periodontal diseases.SIRT3 can improve age-related periodontal disease.Gastrodin promotes the osteogenic differentiation of periodontal ligament stem cells through the up-regulation of SIRT3.The activator of SIRT3 reduces the damage of periodontitis to periodontal and renal tissues by regulating the level of autophagy in the cells.SIRT6 can inhibit the inflammatory reaction of periodontal tissue and inhibit the differentiation and mineralization of cementoblasts.SIRT6 is beneficial to the prognosis of periapical periodontitis.The relationship between SIRT4,SIRT5,SIRT7 and periodontitis is rarely reported.
		                        		
		                        		
		                        		
		                        	
2.Exploring the role and mechanism of armillariella tabescens polysaccharides interference in 5-FU-induced intestinal mucosal injury based on ARRB1

		                        			Objective    :
		                        			To  investigate  the  effect and mechanism of ARRB1  on Armillariella tabescens polysaccharides reversal  of 5-fluorouracil   ( 5-FU ) -induced  chemotherapeutic  intestinal mucosal injury.
		                        		
		                        			Methods       :
		                        			   Twelve  ARRB1 knockout  ( ARRB1 -/ - ) and wild-type  ( WT) C57BL /6  mice were randomly divided into Control,Model  and ATPS groups  (200  mg / kg) ,respectively.5-FU  (50  mg / kg)  was injected intraperitoneally for 7 days to establish a model of chemotherapeutic intestinal mucosal injury.The histopathological damage of jejunum was evaluated  by HE staining ; the activity of serum superoxide dismutase  (SOD) and diamine oxidase  (DAO) was measured by  kits ; the expression of tight junction protein  (TJ) markers ZO-1,Occludin,Claudin-1  and proliferation-associated  protein Ki-67 was detected by immunohistochemistry.Crypt isolation and organoid culture were used to detect the  growth status  of  small  intestinal  organoids.  
		                        		
		                        			Results       :
		                        			5-FU  chemotherapy reduced body  weight,aggravated histopathological damage in  small intestine,decreased  SOD level,TJ protein  and  Ki-67  protein  expression,increased  serum DAO level,decreased spherical structure formation rate and organoid formation rate ; compared with the model group,after ATPS treatment,WT  mice  recovered  body  weight,decreased  pathological  damage,increased  serum  SOD level,TJ protein and Ki-67 protein expression,DAO levels decreased,and the rates of spherical structure for- mation and organoid formation were significantly higher.However,ARRB1 -/ - mice failed to reverse the effect of 5- FU after  ATPS  treatment.
		                        		
		                        			Conclusion     
		                        			ATPS  reverses  5-FU-induced  intestinal  mucositis through the protective  effects of ARRB1 on intestinal barrier and organoid growth.
		                        		
		                        		
		                        		
		                        	
3.Effects of intimate social network and sexual network on HIV-infection in men aged 50 years and older in Pengzhou, Sichuan Province

		                        			Objective:To understand the impact of intimate social networks and sexual networks on HIV-infection in men aged ≥50 years in Pengzhou, Sichuan Province, and provide reference for the formulation of HIV/AIDS prevention and control strategies in middle-aged and elderly people.Methods:A case-control study was conducted by an interview-style questionnaire survey in a case group consisted of 114 men aged ≥50 years and a control group consisted of 423 healthy men matched by age frequency in Pengzhou, Sichuan Province, from April 2019 to October 2020, and the influencing factors of HIV infection were identified by unconditional stepwise logistic regression analysis.Results:Among the 114 cases, 63.16% (72/114) and 96.49% (110/114) of men had intimate social network size and sexual network size ≥4 people, which were higher than the control group of 23.40% (99/423) and 11.58% (49/423) (all P<0.05).The overlap of intimate social network and sexual network was 49.12% (56/114) in the case group, which was lower than that in the control group (82.27%, 348/423)( P<0.05). Logistic regression analysis showed that the size of the intimate social network ≥4 people ( OR=8.66, 95 %CI: 2.01-37.30), the size of the sexual network ≥4 people ( OR=1 121.11, 95% CI: 28.38-4 429.61), the material support from the first intimate network member ( OR=6.39, 95% CI: 1.03-39.75), and multiple sexual partners ( OR=55.50, 95% CI: 6.22-494.96) were the risk factors of HIV-infection in men aged ≥50 years. Receiving health education about AIDS ( OR=0.07, 95% CI: 0.01-0.37), high sexual satisfaction with the first sexual partner and good personal relationship ( OR=0.02, 95% CI: 0.01-0.59; OR=0.01, 95% CI: 0.01-0.26), and the overlap between intimate social network and sexual network ( OR=0.02, 95% CI: 0.01-0.38) were the protective factors of HIV-infection in men aged ≥50 years. Conclusions:The overlap of intimate social network and sexual network can reduce the risk of HIV-infection in men aged ≥50 years. It is important to help middle-aged and elderly men to build good and stable intimate social and sexual networks to meet their social communication and sex demands and prevent HIV infection by reducing high riak sexual behaviors.
		                        		
		                        		
		                        		
		                        	
4.Accurate localization and successful treatment of 23 cases of migrating pharynx and cervical esophageal foreign bodies

		                        			Objective:To explore clinical features, diagnosis, localization, and therapeutic strategy of migratory pharyngeal and cervical esophageal foreign bodies.Methods:A total 23 cases of pharyngeal and cervical esophageal migratory foreign bodies were admitted between January 2015 and December 2021. There were 14 females and 9 males with the age ranged from 35 to 82 (55.0±12.7)years. In all the cases, esophageal CT was taken to confirm the esophageal foreign body. Multiplanar reconstruction (MPR) was performed to locate the foreign body from the horizontal, coronal and sagittal dimensions as well as the corrected reconstructed MPR. According to the location of the foreign body, appropriate surgical method was selected.The symptoms, complications, types of foreign body, positioning, surgical methods, and relevant information were recorded.Data were analyzed using the descriptive method and SPSS 25.0 software.Results:The clinical symptoms of 23 migrating esophageal foreign bodies included pharyngodynia (20/23), foreign body sensation (6/23), hoarsenss (1/23), difficulty in turning neck(1/23), difficulty in opening mouth (1/23), fever (7/23), poor appetite (1/23), and abdominal pain (1/23). The foreign bodies included 19 fish bones, 2 wires, 1 embroidery needle and 1 chicken bone. There were 9 cases (39.1%) of foreign bodies located in extraluminal cervical esophagus, 2 cases (8.7%) of foreign bodies located in the muscular layer of the cervical esophagus and 12 cases (52.2%) of foreign bodies located in pharynx. Twenty-one cases of foreign bodies were removed by cervical lateral incision, in which 11 were removed by cervical lateral incision directly, 10 by the second lateral cervical incision after the foreign bodies were accurately located by MPR and/or corrected MPR, 1 foreign body was removed by incision of the pharyngeal mucosa under suspension laryngoscope, 1 foreign body was removed by tracheoscopy. Compared with patients with intraluminal foreign bodies ( n=308) treated in the same period, intake of fishbone [19 (19/23) vs. 133 (82.6% (43.2%, 133/308), OR=7.31] and first visit was more than 24 hours [20(87.0%, 20/23) vs. 77(25.0%, 77/308), OR=17.2] were the significant risk factors of migratory esophageal foreign bodies. Conclusions:MPR and the corrected MPR can accurately locate the migrating pharyngeal and cervical esophageal foreign bodies, by providing more intuitive imaging evidence for doctors, which provide imaging basis for formulation of surgical programs. Foreign bodies in pharyngeal and cervical esophagus need to be treated as soon as possible, otherwise they are easy to migrate, leading lead to serious complications.
		                        		
		                        		
		                        		
		                        	
5.Efficacy analysis of ultrasound-guided percutaneous radiofrequency ablation in the treatment of nonmetastatic pheochromocytoma

		                        			Objective:To investigate the safety and efficacy of ultrasound-guided percutaneous radiofrequency ablation in the treatment of nonmetastatic pheochromocytoma.Methods:A retrospective analysis was performed on 7 patients with nonmetastatic pheochromocytoma admitted to the First Affiliated Hospital of Zhengzhou University from January 2018 to June 2020, all of whom underwent ultrasound-guided percutaneous radiofrequency ablation. The changes of postoperative blood pressure, improvement of symptoms, intraoperative and postoperative complications were observed.Plasma free methoxypinephrine (MN) and normetanephrine (NMN) levels were recorded before and 2 weeks after operation. The reduction rate of ablation lesion volume at 1, 3 and 6 months after operation was calculated.Results:Postoperative blood pressure of all 7 patients was reduced to the normal range within 3 days, and symptoms such as headache were significantly relieved immediately after operation.No serious complications occurred during or after operation. Plasma free MN and NMN levels decreased to normal levels 2 weeks after operation. The mean reduction rates of the ablation lesions at 1, 3 and 6 months after operation were (46.61±13.42)%, (67.21±10.54)% and (85.73±4.15)%, respectively. Postoperative follow-up of 12-30 months showed that the blood pressure, plasma free MN and NMN levels of the patients were all in the normal range, and no symptoms such as headache and palpitation occurred again. All the tumors were completely ablated, and no recurrence was observed.Conclusions:Ultrasound-guided percutaneous radiofrequency ablation in the treatment of nonmetastatic pheochromocytoma is minimally invasive, safe and effective, and can retain adrenal cortex function, which is worthy of clinical promotion.
		                        		
		                        		
		                        		
		                        	
6.Painful tonic spasm and spinal cord injury in patients with neuromyelitis optica spectrum disorder

		                        			Objective:To investigate the relationship between painful tonic spasm (PTS) and spinal cord injury in patients with neuromyelitis optica spectrum disorder (NMOSD).Methods:The clinical data, serum AQP4-IgG antibody levels and magnetic resonance data of 138 patients with NMOSD were analyzed. Those with spinal cord involvement were assessed using the American Spinal Injury Association Impairment Scale (AIS) to investigate the relationship between PTS and spinal cord injury.Results:The prevalence of PTS among the NMOSD patients was 36% (51/138), and all of the 51 NMOSD patients with PTS showed spinal cord lesions, an incidence significantly different from those without PTS. However, there were no significant differences in the age of onset, gender, disease duration, AQP4-IgG levels, lesion location, range of spinal cord lesions, or AIS grade between the NMOSD patients with and without PTS.Conclusion:PTS is a prevalent concomitant of NMOSD. As a common symptom of remission and recurrent remission, PTS is associated with myelopathy. This study failed to find any correlation between PTS and the affected spinal cord site or segment range. 7.Prenatal diagnosis and genetic counseling in two pedigrees affected with infantile polycystic kidney disease due to PKHD1 gene mutations.

		                        			To detect potential mutations of the PKHD1 gene in two pedigrees affected with infantile polycystic kidney disease.
		                        		
		                        			METHODS:
		                        			Clinical data and peripheral venous blood samples were collected from the probands and their parents as well as fetal amniotic fluid cells. Genome DNA was extracted from the peripheral blood samples and amniotic fluid cells. Exons 32 and 61 of the PKHD1 gene were amplified with PCR and subjected to direct sequencing.
		                        		
		                        			RESULTS:
		                        			The proband of pedigree 1 was found to carry c.4274T>G (p.Leu1425Arg) mutation in exon 32 and c.10445G>C (p.Arg3482Pro) mutation in exon 61 of the PKHD1 gene, which were inherited from her father and mother, respectively. The fetus has carried the c.4274T>G (p.Leu1425Arg) mutation. In pedigree 2, the wife and her husband had respectively carried a heterozygous c.5979_5981delTGG mutation and a c.9455delA mutation of the PKHD1 gene. No chromosomal aberration was found in the umbilical blood sample, but the genetic testing of their fetus was failed. Based on software prediction, all of the 4 mutations were predicted to be pathogenic.
		                        		
		                        			CONCLUSION
		                        			PKHD1 c.4274T>G (p.Leu1425Arg), c.10445G>C (p.Arg3482Pro), c.5979_5981delTGG and c.9455delA were likely to be pathogenic mutations. 8.Clinical analysis of 81 children with urinary system injury after hematopoietic stem cell transplantation

		                        			Objectives To investigate the etiology, renal pathology, treatment, and prognosis of children's urinary system injury after hematopoietic stem cell transplantation (HSCT). Methods Clinical data of 81 children with urinary dysfunction after HSCT admitted to the Hematology Department in Children's Hospital of Soochow University were analyzed, and relevant literatures were reviewed. Results In 81 cases (50 males and 31 females), the age ranges from 8 months to 17 years old. Thirty cases (37%) with prerenal injury were recovered after active rehydration and other symptom specific treatment. There were 9 (11.1%) children with renal injury, four cases were given up therapy or transferred to other hospitals, thus lead to an unknown prognosis. Kidney biopsy was performed in the remaining five cases for pathological investigation. After active symptom-speific and etiology-based treatment, serum creatinine and glomerular filtration rate of four cases return to normal. But in the long-term follow-up,one case died of recurrence of primary disease, reinfusion of hematopoietic stem cell combined with renal failure. The remaining 3 patients were with chronic kidney disease (CKD). One case with renal thrombotic microangiopathy was in the chronic dialysis. Postrenal renal injuries were mainly hemorrhagic cystitis (28.4%) and urinary tract infection (16%). After a large dose of rehydration, urine alkalization and anti-infection therapy, they were recovered in the short term with a good prognosis. Conclusions Urinary injury after HSCT is mainly divided into three categories: prerenal, renal and postrenal, in which renal injury is prone to frequent recurrence.
		                        		
		                        		
		                        		
		                        	
9.Cross-sectional survey and analysis of cleaning quality of dental handpieces in Suzhou City

		                        			Objective To explore the cleaning status and cleaning quality of dental handpieces in various types of medical institutions in Suzhou City.Methods On October 26-31, 2015, dental clinics in the whole city were sampled according to cross-sectional survey and proportional sampling method, the cleaning quality of dental handpieces in each clinic was detected by ATP bioluminescence assay.Results 72 medical institutions, 201 handpieces, 402 samples in 10 administrative regions of the city were sampled, 42 samples was unqualified, unqualified rate was 10.45%, unqualified rate of cleaning of dental handpiece surface was higher than waterline of dental handpiece(17.91% vs 2.99%, P<0.05).Cleaning quality of dental handpieces in different grades of medical institutions was different(P<0.05), tertiary medical institutions were all ualified, medical institutions without grade was 14.45%.According to the classification based on name of different medical institutions, cleaning quality of handpieces was statistically significant(P<0.05), cleaning efficacy of dental handpieces in department of stomatology of public hospitals was best(unqualified rate was 4.31%), while private dental clinics had the worst cleaning efficacy(unqualified rate was 13.81%).Conclusion Education and training of dental handpieces cleaning in the whole city should be strengthened, especially the management of cleaning of dental handpieces in low grade and private dental clinics.
		                        		
		                        		
		                        		
		                        	
10.Current status of cleaning and disinfection of digestive endoscopes in medi-cal institutions in Suzhou City

		                        			cleaning and disinfection.Results All 28 surveyed medical institutions had separate endoscope disinfection rooms, 89.29% of which had integrated endoscopic cleaning station,17.86% had automatic endoscope washer/disinfector;100% used multi-enzymatic detergent,chose the right disinfectant,monitored disinfectant concentration every day, and implemented standard disinfection time.But only 39.29% changed multi-enzymatic detergent for each endo-scope,cleaning and disinfection personnel in 78.57% of medical institutions wore personal protective equipment correctly.77 digestive endoscopes were detected,the qualified rate was 88.31%.Conclusion Cleaning and disin-fection management of digestive endoscope in secondary and above medical institutions in Suzhou City is generally standardized,there are still some problems in the manipulation procedures,relevant national regulations should be strictly complied with,efficacy of cleaning and disinfection of digestive endoscope should be further improved.
