1.The relationship between stress hyperglycemia and the prognosis of acute myocardial infarction without diabetes mellitus

Objective:To analyze the relationship between stress hyperglycemia (SHG) and the prognosis of acute myocardial infarction (AMI) without diabetes mellitus (DM).Methods:Using a retrospective cohort study method, 396 AMI patients without DM or impaired glucose tolerance (IGT) and admitted glycated hemoglobin A 1c (HbA 1c)≤6.0% from January 2018 to December 2020 in Xuanwu Hospital, Capital Medical University were selected. Among them, 238 patients did not have SHG at admission (group A), 85 patients had SHG at admission but their blood glucose level did not reach the diagnostic criteria for DM (group B), and 73 patients had SHG at admission and their blood glucose level reached the diagnostic criteria for DM (group C). The baseline data and the incidence of main adverse cardiovascular and cerebrovascular events (MACCE) were recorded. Multivariate Cox regression was used to analyze the independent risk factors of MACCE after discharge in AMI patients without DM. Results:The incidence of MACCE after discharge in group B and group C was significantly higher than that in group A: 29.4% (25/85) and 35.6% (26/73) vs. 18.5% (44/238), the incidence in group C was significantly higher than that in group B, and there was statistical difference ( P<0.05). Multivariate Cox regression analysis result showed that SHG on admission was an independent risk factor for MACCE after discharge in AMI patients without DM ( P<0.05), and LVEF on admission was an independent protective factor for MACCE after discharge in AMI patients without DM ( P<0.01). Conclusions:SHG on admission is the independent risk factor of MACCE in AMI patient without DM. Early detection, assessment and proper intervention measures based on clinical reality should be advocated for the AMI patients with SHG to further improve the prognosis.
2.Effects of daily mean temperature and other meteorological variables on bacillary dysentery in Beijing-Tianjin-Hebei region, China.

BACKGROUND:
Although previous studies have shown that meteorological factors such as temperature are related to the incidence of bacillary dysentery (BD), researches about the non-linear and interaction effect among meteorological variables remain limited. The objective of this study was to analyze the effects of temperature and other meteorological variables on BD in Beijing-Tianjin-Hebei region, which is a high-risk area for BD distribution.
METHODS:
Our study was based on the daily-scale data of BD cases and meteorological variables from 2014 to 2019, using generalized additive model (GAM) to explore the relationship between meteorological variables and BD cases and distributed lag non-linear model (DLNM) to analyze the lag and cumulative effects. The interaction effects and stratified analysis were developed by the GAM.
RESULTS:
A total of 147,001 cases were reported from 2014 to 2019. The relationship between temperature and BD was approximately liner above 0 °C, but the turning point of total temperature effect was 10 °C. Results of DLNM indicated that the effect of high temperature was significant on lag 5d and lag 6d, and the lag effect showed that each 5 °C rise caused a 3% [Relative risk (RR) = 1.03, 95% Confidence interval (CI): 1.02-1.05] increase in BD cases. The cumulative BD cases delayed by 7 days increased by 31% for each 5 °C rise in temperature above 10 °C (RR = 1.31, 95% CI: 1.30-1.33). The interaction effects and stratified analysis manifested that the incidence of BD was highest in hot and humid climates.
CONCLUSIONS
This study suggests that temperature can significantly affect the incidence of BD, and its effect can be enhanced by humidity and precipitation, which means that the hot and humid environment positively increases the incidence of BD.

3.Factors influencing HIV infection among college students who are young men who have sex with men in Tianjin

Objective:
To analyze associated factors of HIV infection among college students who are young men and have sex with men (YMSM) in Tianjin, providing reference for HIV prevention and control among YMSM college students.
Methods:
During Aug. 1st, 2018 to Dec. 31st, 2018, SHENLAN recruited college students who were YMSM aged 18-24 years from gay baths, gay bars, QQ, WeChat and gay dating app BLUED. HIV infection status and associated factors (general demographic characteristics, unsafe sexual behaviors, addictive substance using, basic knowledge of HIV) was collected and analyzed.
Results:
A total of 470 college students, including 21 HIV infected (4.47%), were enrolled in this study. Univariate Logistic regression analyses indicated that age, age at first sex behavior, HIV related knowledge, tobacco use, recreational drug usage, syphilis infection was associated with HIV infection among YMSM students (P<0.05). Multivariate analysis found age of first sex (OR=21.20,95%CI=3.09-145.43), recreational drug use (OR=5.07,95%CI=1.77-14.48), lack of HIV related knowledge (OR=3.38,95%CI=1.33-8.63)were associated with HIV infection (P<0.05).
Conclusion
College students who are YMSM in Tianjin have a high rate of HIV infection, who deserves further attention. Targeted campus HIV/AIDS prevention program should be developed combined with specific characteristics of this population.
4.Effect of auricular point press needle on improving sleep quality of postoperative patients with gastric cancer

Objective:To observe the effect of auricular point press needle on sleep quality of patients with gastric cancer after operation.Methods:From October 2017 to March 2019, a total of 80 patients underwent radical gastrectomy for gastric cancer in the First People's Hospital of Huzhou, Zhejiang Province were selected by using convenience sampling method, and divided into observation group and control group by random number table method, with 40 cases in each group. The control group received routine nursing, while the observation group received auricular point press needle on the basis of routine nursing. Sleep quality was monitored in both groups using sleep monitoring bracelets. In the course of the study, 2 patients in the observation group had surgical complications, and finally 35 patients completed the experiment; 2 patients in the control group had surgical complications, and a total of 33 patients completed the experiment.Results:From the 6th day of intervention (the 4th day after operation) , the sleep duration of the observation group was longer than that of the control group, and the difference was statistically significant ( P<0.05) , the group effect, time effect and interaction effect were statistically significant ( P<0.05) . From the 6th day of intervention (the 4th day after operation) , the proportion of deep sleep of the observation group was higher than that of the control group, and the difference was statistically significant ( P<0.05) , the group effect, time effect and interaction effect were statistically significant ( P<0.05) . Conclusions:Auricular point press needle can effectively improve the sleep quality of patients with gastric cancer after operation.
5.Application of metagenomic next-generation sequencing technology in pathogen detection in patients with burns and patients with acute or chronic wounds

Objective:To explore the value of using metagenomic next-generation sequencing (mNGS) technology to detect pathogens in patients with burns and patients with acute or chronic wounds.Methods:A retrospective observational study was conducted. From March 2019 to June 2020, 11 patients with burns and patients with acute or chronic wounds (including 10 males and 1 female, aged 23 to 85 years) in the Fourth Medical Center of PLA General Hospital met the inclusion criteria and were recruited. A total of 23 specimens were collected, including 6 whole blood specimens, 1 skin tissue specimen, 1 drained pus specimen, and 15 wound secretion swab specimens. Each specimen was divided into two parts, which were subjected for pathogen detection using microbial culture method and mNGS method, respectively. The number and types of pathogens detected by the 2 methods and the relative abundance detected by the mNGS method were recorded, and the consistency of the two methods were compared. Data were statistically analyzed with paired Wilcoxon rank sum test.Results:With the microbial culture method, no pathogen was detected in 5 of the 23 specimens, while 35 pathogens were detected in the remaining 18 specimens, belonging to 9 species of bacteria and 2 species of fungi. Five specimens had one pathogen while 9 specimens had 2 pathogens and 4 specimens had 3 pathogens detected in each specimen. With the mNGS method, no pathogen was detected in one of the 23 specimens, while 75 pathogens were detected in the remaining 22 specimens, belonging to 28 species of bacteria, 3 species of fungi, and 3 species of viruses. Eight specimens had one pathogen, 5 specimens had 2 pathogens, 2 specimens had 3 pathogens, 3 specimens had 4 pathogens, 2 specimens had 6 pathogens, and 1 specimen had 7 pathogens, and 1 specimen had 20 pathogens detected in each specimen. The number of pathogens detected in each specimen by microbial culture method was 2 (1, 2) types, which was significantly less than 2 (1, 4) types by mNGS method ( Z=3.359, P<0.01). In 5 specimens, no bacteria were detected by microbial culture method but mNGS method detected bacteria in 2 specimens and virus in 2 different specimens. The mNGS method detected two or more types of bacteria in 13 specimens, the relative abundance of bacteria with the 1st relative abundance ranking ranged from 28.8% to 95.9% in each specimen. Of the 23 specimens detected by two detection methods, 7 specimens (30.4%) showed identical detection results, 5 specimens (21.7%) showed totally different detection results, and 11 specimens (47.8%) had partially consistent detection results. Conclusions:Compared with the traditional microbial culture method, the mNGS method has higher detection sensitivity and stronger capacity to detect pathogens, and can determine the relative abundance of pathogens in mixed infections. As a supplement to the culture method, the mNGS method is expected to play an important role in the diagnosis of infectious pathogens in burns and acute or chronic wounds.
6.Ten cases of wound hemostasis with glove bandaging in hand skin grafting

From July 2018 to April 2019, 10 patients (8 males and 2 females, aged 5 to 53 years) who need skin grafting with 17 hands were admitted to the Fourth Medical Center of PLA General Hospital. The method of glove bandaging was used for wound hemostasis in skin grafting. After the initial hemostasis, sterile latex gloves were used as the inner cover of hand, and then pressure-wrapped with gauze. After 20 minutes, the gauze and gloves were removed in order from the near to the far, then thorough hemostasis was conducted again, and finally skin grafting was performed. The use of gloves as inner cover can avoid the adhesion of gauze fibers to the wound surface, so that it will not cause small blood vessels to re-bleed when the gauze is removed, therefore reducing the excessive use of electrocoagulation and secondary damage to the wound. As a result, blood loss can be reduced and hemostatic time can be shortened.
7. Clinical effects and mechanism of treating extensive deep burns by stage-Ⅱ Meek skin grafting on adipose tissue after tangential excision

Objective:
To observe the clinical effects of stage-Ⅱ Meek skin grafting on adipose tissue after tangential excision in patients with extensive deep burns, and to explore the functional mechanism.
Methods:
The medical records of 26 extensively burned patients who met the inclusion criteria and were admitted to the Department of Burns and Plastic Surgery of the Fourth Medical Center of PLA General Hospital from May 2015 to December 2017 were retrospectively analyzed. According to the treatment methods, 14 patients were enrolled in stage-Ⅰ skin grafting group (10 males and 4 females, aged 27 to 75 years), and 12 patients were enrolled in stage-Ⅱ skin grafting group (10 males and 2 females, aged 31 to 76 years). Patients in the 2 groups all underwent debridement of tangential excision, and their healthy adipose tissue was preserved. Meek skin grafting was performed just after tangential excision in patients in stage-Ⅰ skin grafting group. In patients in stage-Ⅱ skin grafting group, porcine acellular dermal matrix (ADM) was applied to cover the wound after tangential excision, and 3 days later, it was removed and Meek skin grafting was performed. The times of complement skin grafting and the wound basic healing time of patients in the 2 groups were observed and recorded. 9.Effect of alendronate on glucocorticoid-induced osteoporosis.

OBJECTIVE:
To determine the effect of alendronate on glucocorticoid-induced osteoporosis(GIO).
METHODS:
Thirty-five GIO patients were enrolled. Ten milligrams alendronate were prescribed daily for 6 months. Bone mineral density (BMD) was measured by dual-energy X-ray absorptiometer (DEXA) after and before the treatment.
RESULTS:
BMD at lumbar, neck and trochanter sites of the 35 patients after the treatment was significantly increased compared with those before the treatment (P<0.05). Urine Ca/Cr level was decreased (P<0.05). There was no severe side effect.
CONCLUSION
Alendronate is effective and well tolerated for GIO.

10.Study on Detection of Mycobacterium avium by Polymerase Chain Reaction

Objective To develop a rapid method with high sensitivity and specificity to detect Mycobacterium avium.Methods A polymerase chain reaction(PCR)was developed.Its sensitivity and specificity were verified by sequential dilution of DNA of M.avium and other17Mycobacterium spp.,respectively.Simulation of clinical infection was established by mixing normal skin tissue with M.avium.The treatment of the tissue specimens was optimized in order to improve the sensitivity of the PCR assay.Results A fragment of427bp was amplified by the PCR assay with the strains of M.avium at the sensitivity of1?10 2 cells/mL.The other17Mycobacterium spp.were all negative.The sensitivity of the PCR assay decreased to1?10 4 cells/mL when M.avium was mixed with the homogenized skin tissue.The sensitivity recovered to1?10 2 cell/mL when the skin tissue was diluted to≥1∶4.Conclusion It is suggested that PCR be a rapid and reliable method for detection of M.avium.
