1.Intratumoral and peritumoral CT radiomics for evaluating KRAS gene status in patients with colorectal adenocarcinoma

Objective To observe the value of intratumoral and peritumoral CT radiomics for evaluating KRAS gene status in patients with colorectal adenocarcinoma.Methods Totally 245 patients with colorectal adenocarcinoma were retrospectively enrolled and divided into mutant group(n=139)and wild group(n=106)according to KRAS gene status,also divided into training set(n=171)and test set(n=74)at a ratio of 7∶3.Clinical data were compared between groups,and clinical factors were screened with logistic regression analysis to establish a clinical model.Based on enhanced venous phase CT images,intratumoral volume of interest(VOI),peritumoral VOI,and intratumoral+peritumoral VOI were delineated,radiomics features were extracted,and radiomics models were constructed.The combination model was constructed based on the best radiomics model combined with clinical factors.The value of each model for evaluating KRAS gene status in patients with colorectal adenocarcinoma was analyzed.Results Significant differences of patients’gender and carcinoembryonic antigen(CEA)were found between mutant group and wild group(both P<0.05),which were independent impact factors of KRAS gene status in patients with colorectal adenocarcinoma(both P<0.05).The area under the curve(AUC)of clinical model for evaluating KRAS gene status in patients with colorectal adenocarcinoma in training set and test set was 0.633 and 0.658,respectively.Intratumoral+peritumoral 3 mm model was the best radiomics model,with AUC of 0.921 and 0.894 in training set and test set,respectively.AUC of the combination model in training set and test set was 0.949 and 0.956,respectively.In training set,significant differences of AUC were found between clinical model and intratumoral+peritumoral 3 mm model,also between clinical model and combination model(both P<0.001),while in test set,significant differences of AUC were found between each two models(all P<0.05).Conclusion Intratumoral+peritumoral 3 mm radiomics based on enhanced venous phase CT could help to evaluate KRAS gene status in patients with colorectal adenocarcinoma.Combining with patients’gender and CEA could further improve efficacy of this model.
2.Contrast-enhanced CT radiomics combined with clinical and hematology indicators for diagnosing lymph node metastasis of esophageal squamous cell carcinoma

Objective To observe the value of contrast-enhanced CT radiomics combined with clinical and hematology indicators for predicting lymph node(LN)metastasis(LNM)of esophageal squamous cell carcinoma(ESCC).Methods Totally 218 ESCC patients were retrospectively enrolled.Stage pN1 and pN2 were clustering as LNM(n=90),while stage pN0 were taken as non-LNM(n=128).The patients were divided into training set(n=174)and test set(n=44)at the ratio of 8∶2.In training set,clinical and LN imaging features which could be used to independently judge LNM were screened and a clinical-imaging model was constructed.The hematological indicators that might be associated with ESCC LNM were screened,and a hematological model was constructed.Radiomics features in LN ROI and ESCC volume of interest(VOI)were extracted based on venous-phase contrast-enhanced CT images,and those might be associated with LNM were screened,and a radiomics model was constructed.Finally a combined model was constructed based on all the above features.The efficacy of each model for diagnosing LNM was evaluated with the area under the curve(AUC)of receiver operating characteristic curves,and the clinical net benefit was evaluated using decision curve analysis(DCA).Results Body mass index(BMI)and internal necrosis of target LN were both independent judging factors for ESCC LNM(both P<0.05),and AUC of clinical-imaging model for diagnosing LNM in training and test sets was 0.747 and 0.687,respectively.Seven hematological indicators were included in hematological model,and AUC in training and test sets was 0.623 and 0.583,respectively.Ten LN radiomics features and 15 ESCC radiomics features were included in radiomics model,and AUC in training and test sets was 0.769 and 0.745,respectively.AUC of the combined model for diagnosing LNM in training and test sets was 0.822 and 0.739,respectively,better than other models in training set(all P<0.05),but no significantly different in test set(all P>0.05).DCA showed that combined model had higher net gain than the other models in 0.55-0.80 threshold probability interval.Conclusion Combined model based on venous-phase contrast-enhanced CT radiomics and clinical and hematology indicators could relatively effectively evaluate ESCC LNM,which might bring some promotions in clinical benefit.
3.Characteristics of pretreatment drug resistance mutations in people living with human immunodeficiency virus type 1 in Henan Province, 2023

Objective:To monitor the subtype distributions and drug resistance mutations of human immunodeficiency virus (HIV)-1 in people living with human immunodeficiency virus-1 (PLWH) who had not experienced anti-retroviral therapy (ART) in Henan Province in 2023, so as to provide valuable data for understanding the transmission of HIV-1 resistant strains and selection of ART regimens in Henan Province.Methods:The clinical data and drug resistance of PLWHs who had not experienced ART in the Sixth People′s Hospital of Zhengzhou from January to December 2023 were collected. This study was a cross-sectional study. Plasma samples were collected from patients, and the partial HIV pol gene sequence and the full integrase gene sequence were amplified by reverse transcription-nested polymerase chain reaction. The subtypes of HIV-1 isolates were determined using the online REGA HIV-1 Subtyping Tool. Drug resistance mutations and antiviral drug susceptibility were analyzed by submitting the determined sequences to the Stanford HIV-1 drug resistance database.Results:Among the 1 073 PLWHs who had not experienced ART, sequences in 1 042 were successfully amplified, giving a success rate of 97.11%. A total of 12 subtypes were detected, and the top five subtypes were circulating recombinant form (CRF)07_BC (43.76%, 456/1 042), CRF01_AE (25.91%, 270/1 042), B (20.92%, 218/1 042), CRF55_01B (5.28%, 55/1 042) and CRF08_BC (1.25%, 13/1 042). The incidence of drug resistance mutation was 28.89% (301/1 042). Drug resistance mutation of non-nucleoside reverse transcriptase inhibitors (NNRTI), nucleotide reverse transcriptase inhibitors (NRTI), protease inhibitors (PI) and integrase inhibitors (INSTI) were 20.92%(218/1 042), 4.03%(42/1 042), 3.84%(40/1 042) and 2.98%(31/1 042), respectively. V179 (12.96%, 135/1 042), M184 (2.40%, 25/1 042), Q58 (2.78%, 29/1 042), and E157 (1.44%, 15/1 042) were the most common drug resistance mutation for NNRTIs, NRTIs, PIs and INSTIs, respectively.Conclusions:The distribution of HIV-1 subtypes is diverse, and the incidence of drug resistance mutation is moderate prevalent in PLWHs who have not experienced ART. Drug resistance testing in PLWHs before ART should be closely monitored.
4.Mechanism of Flemiphilippinin D Regulating Inflammatory Response in CIA Rats Through TLR2/MyD88/NF-κB Signaling Pathway

ObjectiveTo observe the effect of Flemiphilippinin D on collagen-induced arthritis (CIA) in rats and explore its mechanism. MethodForty rats were randomly divided into normal group, CIA group, methotrexate (MTX) group (1.35 mg·kg-1), low-dose Flemiphilippinin D group (1.5 mg·kg-1), and high-dose Flemiphilippinin D group (3.0 mg·kg-1), with eight rats in each group. Except for the normal group, the CIA model was induced by type Ⅱ collagen. Each group was given corresponding liquid medicine or normal saline, once a week in the MTX group, and once a day in the Flemiphilippinin D groups for a total of 28 days. The arthritis score and joint swelling degree of rats were experimentally recorded. Pathological changes in the ankle joint of rats were observed by hematoxylin-eosin (HE) staining. Serum levels of inflammatory cytokines interleukin (IL)-1β, IL-6, IL-8, and tumor necrosis factor (TNF)-α were detected by enzyme-linked immunoabsorbent assay (ELISA), and the mRNA expression of Toll-like receptor 2 (TLR2), myeloid differentiation factor 88 (MyD88), and nuclear transcription factor-κB (NF-κB) p65 were detected by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR), and the protein expressions of TLR2, MyD88, and NF-κB p65 were detected by Western blot. ResultCompared with the normal group, the ankle joint of the CIA group was significantly swollen, and the clinical score of arthritis and the degree of joint swelling were significantly increased (P<0.01). The ankle joint tissue structure was significantly damaged, and the levels of inflammatory factors IL-1β, IL-6, IL-8, and TNF-α in serum were significantly increased (P<0.01). The mRNA levels and protein levels of TLR2, MyD88, and NF-κB p65 were significantly increased(P<0.01). Compared with the CIA group, arthritis clinical score and joint swelling of rats in each administration group were significantly reduced (P<0.05, P<0.01), and the pathological changes in the ankle joint were significantly improved. The contents of serum IL-1β, IL-6, IL-8, and TNF-α were significantly decreased (P<0.05, P<0.01). The mRNA levels and protein levels of TLR2, MyD88, and NF-κB p65 in the ankle joint were significantly decreased (P<0.05, P<0.01). ConclusionTo a certain extent, Flemiphilippinin D can reduce the expression of inflammatory factors in rheumatoid arthritis rats and play a good therapeutic effect. It works perhaps by inhibiting the activation of the TLR2/MyD88/NF-κB signaling pathway and thus shows an anti-inflammatory effect.

6.Efficacy of Transaxillary Approach Endoscopic Thyroid Surgery for the Treatment of Single-Sided Thyroid Cancer

Objective To investigate the safety and efficacy of endoscopic thyroid surgery through axillary approach in the treatment of unilateral papillary thyroid carcinoma.Methods A total of 135 patients admitted to Yunnan Cancer Hospital from April 2022 to November 2022 were selected.According to the surgical method,69 patients were in the endoscopic group and 66 patients were in the open surgery group.The clinical data,operation time,intraoperative blood loss,postoperative drainage volume,postoperative hospitalization stay,complications,cosmetic satisfaction,scar evaluation,postoperative swallowing pain and blood calcium,blood phosphorus,PTH levels of the two groups were observed.Results There were no significant differences in general data,pathological features,number of lymph node dissection,and perioperative complications between the two groups(P>0.05).The operation time and postoperative drainage volume in the endoscopic group were significantly higher than those in the open surgery group.The intraoperative blood loss,postoperative hospital stay,pain score,serum calcium,serum phosphorus,PTH levels and cosmetic satisfaction in the endoscopic group were significantly better than those in the open group(P<0.05).Conclusion Transaxillary approach endoscopic thyroid surgery for the treatment of single-sided papillary thyroid carcinoma is safe,feasible,and provides better protection of the parathyroid glands,postoperative swallowing pain,and cosmetic satisfaction.
7.Effects of coronavirus disease 2019 vaccination on seizures in patients with epilepsy

Background::Given that seizures may be triggered by vaccination, this study aimed to evaluate the risk and correlative factors of seizures in patients with epilepsy (PWE) after being vaccinated against coronavirus disease 2019 (COVID-19).Methods::This study retrospectively enrolled PWE who were vaccinated against COVID-19 in the epilepsy centers of 11 hospitals in China. We divided the PWE into two groups as follows: (1) patients who developed seizures within 14 days of vaccination were assigned to the SAV (with seizures after vaccination) group; (2) patients who were seizure-free within 14 days of vaccination were assigned to the SFAV (seizure-free after vaccination) group. To identify potential risk factors for seizure reccurence, the binary logistic regression analysis was performed. Besides, 67 PWE who had not been vaccinated were also included for elucidating the effects of vaccination on seizures recurrence, and binary logistic regression analysis was performed to determine whether vaccination would affect the recurrence rate of PWE who had drug reduction or withdrawal.Results::The study included a total of 407 patients; of which, 48 (11.8%) developed seizures within 14 days after vaccination (SAV group), whereas 359 (88.2%) remained seizure-free (SFAV group). The binary logistic regression analysis revealed that duration of seizure freedom ( P < 0.001) and withdrawal from anti-seizure medications (ASMs) or reduction in their dosage during the peri-vaccination period were significantly associated with the recurrence of seizures (odds ratio= 7.384, 95% confidence interval = 1.732–31.488, P = 0.007). In addition, 32 of 33 patients (97.0%) who were seizure-free for more than three months before vaccination and had a normal electroencephalogram before vaccination did not have any seizures within 14 days of vaccination. A total of 92 (22.6%) patients experienced non-epileptic adverse reactions after vaccination. Binary logistic regression analysis results showed that vaccine did not significantly affect the recurrence rate of PWE who had the behavior of ASMs dose reduction or withdrawal ( P = 0.143). Conclusions::PWE need protection from the COVID-19 vaccine. PWE who are seizure-free for >3 months before vaccination should be vaccinated. Whether the remaining PWE should be vaccinated depends on the local prevalence of COVID-19. Finally, PWE should avoid discontinuing ASMs or reducing their dosage during the peri-vaccination period.
8.Construction of the standardized preventive nursing training scheme for venous thromboembolism based on the Delphi method

Objective:To construct the standardized preventive nursing training scheme for venous thromboembolism (VTE) , so as to provide a basis for hospitals to carry out the standardized preventive nursing training scheme for VTE.Methods:Under the guidance of the National Medical Quality and Safety Improvement Goal, the standardized preventive nursing training scheme for VTE was initially developed through preliminary investigation, literature review and group discussion. Using the convenient sampling method, a total of 20 experts were selected from March to June 2022 for two rounds of Delphi expert consultation. After modification, the VTE standardized preventive nursing training scheme was finally determined.Results:In the two rounds of correspondence, the effective response rates of the questionnaire were 90.00% and 100.00%, respectively. The expert authority coefficients of two rounds were both 0.95 and Kendall coordination coefficients were 0.20 and 0.14, respectively ( P<0.01) . After two rounds of expert consultation, a standardized preventive nursing training scheme for VTE was finally established, which included 4 primary indicators, 12 secondary indicators and 78 tertiary indicators. Conclusions:The VTE standardized preventive nursing training scheme constructed in this study has strong reliability and scientific nature, which can provide a basis for hospitals to carry out VTE standardized preventive nursing training scheme.
9.Status of knowledge, attitude and practice of perioperative prevention of venous thromboembolism among Operating Room nurses: a survey and analysis study

Objective:To investigate and analyze status of knowledge, attitude and practice of perioperative prevention of venous thromboembolism (VTE) among Operating Room nurses, so as to provide guidance for VTE prevention.Methods:Using the convenient sampling method, a total of 2 728 Operating Room nurses from 27 provinces, municipalities and autonomous regions in China were investigated with an electronic questionnaire from February to April 2020. The questionnaire included Basic Information Questionnaire, VTE Prevention Status Questionnaire and Perioperative VTE Prevention Knowledge, Attitude and Practice Questionnaire. A total of 2 728 questionnaires were issued, and 2 512 were effectively returned, with an effective recovery rate of 92.08%.Results:79.50% (1 997/2 512) of nurses said that their hospitals had VTE prevention and treatment system and 79.78% (2 004/2 512) of nurses said that their hospitals had relevant training on intraoperative VTE prevention. 67.08% (1 685/2 512) of the nurses' departments had VTE prevention work and 59.95% (1 506/2 512) of the nurses' departments had adequate VTE prevention equipment. The knowledge score, attitude score and behavior score of perioperative VTE prevention questionnaire of Operating Room nurses were (16.18±4.26) , (45.64±4.95) and (78.69±16.36) , respectively. There were statistically significant differences in scores of knowledge dimension of the questionnaire among nurses with different educational background, professional title, position, post and follow-up surgery conditions ( P<0.05) . There were statistically significant differences in scores of attitude dimension of the questionnaire among nurses with different professional titles, positions and follow-up surgery conditions ( P<0.05) . There were statistically significant differences in practice dimension scores of nurses with different ages, working years, educational background, professional title and position ( P<0.05) . Conclusions:Managers should pay attention to the nursing work of preventing VTE in the Operating Room. The Operating Room nurses have insufficient knowledge about perioperative prevention of VTE prevention. Therefore, their prevention attitude needs to be improved and prevention behavior needs to be improved.
10.Analysis of basic medical insurance and disease burden of the disabled people under the background of health poverty alleviation

Objective To explore the problems of health poverty faced by a special group of people with disabilities and the difficulties in the practice of health poverty alleviation, so as to provide a scientific basis for the health poverty alleviation of the disabled. Methods A self-made questionnaire was used for one-to-one survey, and a database was established by Excel. SPSS was used for descriptive analysis and horizontal comparison. Results The participation rate of basic medical insurance for the disabled was relatively high (93.40%), and the medical insurance payment was mainly paid by individuals (70.13%). The satisfaction of medical insurance was low (43.12%), and 84.64% of the disabled thought that their medical expenses were high. 45.22% of the families of disabled patients met the universal standard of catastrophic health expenditure. Compared with Shandong Province, the basic medical insurance coverage rate of the disabled in Hubei Province was slightly lower, the satisfaction rate of medical insurance was higher, and the proportion of catastrophic health expenditure of families was larger. The analysis of the results showed that the disabled people with a lower disability level, children and middle-aged with disabilities, the disabled people with less or more family members, and the disabled people without the minimum living subsistence allowances were not satisfied with the medical insurance. Conclusion The basic medical insurance in the two places has alleviated the difficulty of medical treatment for the disabled to a certain extent, but the family burden of diseases of the disabled was still heavy. The level of medical security for people with disabilities should be improved, and their economic burden of disease should be reduced, so as to improve the satisfaction of medical insurance.
