1.Automated identification and localization of inferior vena cava based on ultrasound images

Objective To explore the automated identification and diameter measurement methods for inferior vena cava (IVC) based on clinical ultrasound images of IVC. Methods An automated identification and localization method based on topology and automatic tracking algorithm was proposed. Tracking algorithm was used for identifying and continuously locating to improve the efficiency and accuracy of measurement. Tests were conducted on 18 sets of ultrasound data collected from 18 patients in intensive care unit (ICU),with clinicians' measurements as the gold standard. Results The recognition accuracy of the automated method was 94.44% (17/18),and the measurement error of IVC diameter was within the range of±1.96s (s was the standard deviation). The automated method could replace the manual method. Conclusion The proposed IVC automated identification and localization algorithm based on topology and automatic tracking algorithm has high recognition success rate and IVC diameter measurement accuracy. It can assist clinicians in identifying and locating IVC,so as to improve the accuracy of IVC measurement.

3.Monitoring of foodborne pathogens in some ready-to-eat fruits and vegetables in Shanghai in 2021

ObjectiveThe contamination of foodborne pathogens in ready-to-eat fruits and vegetables in Shanghai was analyzed to provide a scientific basis for food safety, risk assessment and related supervision. MethodsFrom June to September 2021, a total of 143 batches of12 kinds of ready-to-eat fruits and vegetables such as lettuce, chicory, and cherry tomatoes were collected from farmers’ markets, supermarkets, and e-commerce platforms. The total number of bacterial colonies, Salmonella, Listeria monocytogenes, Staphylococcus aureus, Cronobacter spp. and Shiga toxin-producing Escherichia coli in the samples were tested according to National Food Contamination and Harmfulness Risk Monitoring Manual. ResultsAmong the 143 batches, foodborne pathogens were detected in 68 batches, with a total detection rate of 47.55% (68/143). A total of 79 strains of foodborne pathogens were detected. The detection rate of Staphylococcus aureus was the highest (32.87%, 47/143), followed by Cronobacter spp. (20.98%, 30/143), Salmonella (0.70%, 1/143), Listeria monocytogenes (0.70%, 1/143), Shiga toxin-producing Escherichia coli (0.00%). Furthermore, the detection rate was higher in different ready-to-eat fruits and vegetables: chicory (17.33%), cucumber (17.14%), cherry tomatoes (16.00%), and honeydew melon (15.38%), respectively. Meanwhile, the contamination rate of pathogens in ready-to-eat fruits and vegetables from farmers’ markets, supermarkets, and e-commerce platforms was relatively high. ConclusionReady-to-eat fruits and vegetables in Shanghai are contaminated by foodborne pathogens. The prevention and control of the contamination of post-harvest fruits and vegetables should be strengthened to reduce the risk of foodborne disease outbreaks.

Behçet’s syndrome is a kind of chronic systemic vasculitis with involvement of multiple organs. Intestinal involvement of Behçet’s syndrome is presently named as intestinal Behçet’s syndrome (disease). Recently, there is considering another kind of disease type with only typical intestinal ulcers. Since it is difficult to differentiate intestinal Behçet’s syndrome from Crohn’s disease, intestinal tuberculosis, intestinal lymphoma, as well as intestinal manifestations of many other autoimmune diseases, and there is limited evidence for the therapy of intestinal Behçet’s syndrome, proposing diagnosis and treatment recommendations for intestinal Behçet’s syndrome through evidence-based judgment will be of great significance for clinical practice.
5.Color Doppler ultrasonography and clinical characteristics of benign lymphoepithelial disease of lacrimal gland

Objective:To summarize the clinical manifestations and color Doppler ultrasonography of benign lymphoepithelial disease (BLL) in lacrimal gland, so as to improve the diagnostic accuracy of lacrimal BLL.Methods:Clinical manifestations and color Doppler flow imaging (CDFI) features of lacrimal BLL in 16 patients (2 males and 14 females, with 31 lesions) who visited the Second Hospital of Hebei Medical University from November 2014 to August 2019 were retrospectively analyzed.Results:The performance for lesions in patients with duration less than 3 months was scattered in irregular low echo, lesion blood flow signals within the rich (Adler Ⅱ level), pathologic examination results showed more lymphocytes were seen in the lacrimal matrix, and the myoepithelium of the lacrimal duct grew to form the epithelium-myocutaneous island. The ultrasonic feasures in patients with course of 3-6 months were in multiple categories such as circular low echo, "honeycomb" change, CDFI showed lesions with a lot of blood flow signals (Adler Ⅲ level), the pathological examination results indicated that there were a large number of lymphocytes and epithelial-musculocutaneous islands in the lacrimal matrix, and the lymphocytes were significantly increased compared with patients with the course of disease less than 3 months. The ultrasound results in patients with a course of more than 6 months showed lesions in multiple categories such as circular low echo, and large low echo, greater than 3 mm in diameter, CDFI showed lesions within the same large amounts of blood flow signals (Adler Ⅲ level), the pathological examination results were consistent with the course of 3 to 6 months. One patient had positive tuberculin test and 11 had higher IgG4 than normal.The number of lymphocytes increased with the prolongation of disease course.Conclusions:With the expert knowledge of color Doppler ultrasonographic characteristics of lacrimal gland BLL and with the serum IgG4 level helps to improve the accuracy of ultrasonic diagnosis.
6.Synergy of Centralized Drug Volume-based Purchasing and Medical Insurance Negotiation Policy

OBJECTIVE:To investigate the s ynergy between centralized drug volume-based purchasing (called“volume- based purchasing”for short )and medical insurance negotiation policy. METHODS :From the aspects of economic and social benefits , the synergy of volume-based purchasing and medical insurance negotiation was analyzed by using the methods of literature analysis,policy interpretation and interview. The corresponding suggestions were put forward for the problems of the supply and use of drugs. RESULTS & CONCLUSIONS :The synergy of volume-based purchasing and medical insurance negotiation in terms of economic benefits include medical insurance fund management optimization to improve fund utilization efficiency improvement ; medical insurance fund payment optimization to reduce transaction cost ;network promotion of medical security information platform to reduce the cost of information exchange. The synergy of volume-based purchasing and medical insurance negotiation in terms of social benefits includes relevant index assessment of drug quality evaluation to improve drug quality evaluation system ; medical service level improvement to form compound medical insurance payment mode under total budget management ;enterprise supervision driven by market to reduce the pressure of government supervision. The relevant departments should continue to strengthen the comprehensive management of medical institutions to reduce the phenomenon of irrational drug use ;strengthen policy convergence ,avoid the poor implementation of medical insurance drug adjustment policy ,so that the policy dividend of volume-based purchasing and medical insurance negotiation can benefit the people more through system reform ,optimal allocation of funds ,etc.
7. Research strategies and considerations on non-clinical pharmacokinetics of nanomedicine

With the rapid development of nanotechnology, the research and development of nanomedicine has become one of the current development directions of drug innovation. The pharmacokinetic characteristics of nanomedicine are significantly different from general drugs because of the scale effect based on nanostructures, and pharmacokinetics studies of nanomedicine may be different from the general drugs. This article focuses on the research strategies and considerations on non-clinical pharmacokinetics of nanomedicine, including test agents, in vivo/in vitro assays, biological sample analysis, data evaluation and analysis etc., providing references for developers.
8.Study on the relationship between Neutrophil lymphocyte ratio,platelet lymphocyte ratio and disease activity in patients with neuromyelitis optica spectrum disorders

Objective To explore the relationship between the neutrophil to lymphocyte ratio(NLR) and the platelet to lymphocyte ratio(PLR) and disease activity in patients with neuromyelitis optica spectrum disorders(NMOSD).Methods The clinical data,laboratory examination and imaging data of NMOSD patients admitted to Peking University Third Hospital from January 2015 to June 2019 were retrospectively analyzed to calculate the NLR and PLR. The NLR and PLR of 100 healthy subjects were compared.Results A total of 48 patients with NMOSD were enrolled. The white blood cell count,neutrophil absolute value and NLR of NMOSD in attack period were significantly higher than those in remission period and control group,while the lymphocyte absolute value was significantly lower than those in remission period and control group.There was no significant difference in platelet count and PLR between NMOSD group and control group.There were no statistically significant differences in WBC count,neutrophil absolute value,lymphocyte absolute value,platelet count,NLR and PLR between AQP4 Ab(+) patients and AQP4 Ab(-)patients in the NMOSD patients at the onset stage. The area under curve(AUC)the NLR in receiver operating characteristics was 0.806(P=0.000,95%CI 0.734~0.878).
9.An inter-observer consistency analysis for the evaluation of blood flow in breast lesions by super microvascular imaging

Objective:To analyze the inter-observer consistency of Smart 3D super microvascular imaging (Smart-3D SMI) in evaluating the blood flow of breast lesions.Methods:Prospective study on Smart-3D SMI images of 225 cases of continuous breast lesions in Peking Union Medical College Hospital from January to December 2018. Two sonographers conducted qualitative evaluation of vascular architecture(VA) under Smart-3D SMI, and took the mean value of three measurements evaluations of vascular index(VI). Weighted Kappa was used to test the consistency of the vascular architecture, and Bland-Altman scatter plot was used to calculate the 95% consistency limit of the measured data for the VI value. Spearman correlation coefficient was used to analyze whether the absolute difference of VI between observers was correlated with the mean value of VI.Results:The Weighted Kappa coefficient of Smart-3D SMI for the evaluation of VA by two sonographers was 0.839 (95% CI was 0.795-0.881, P<0.001), showing strong inter-observer consistency. Bland-Altman plot analysis of VI value showed that the value of 95.11% was within the clinically acceptable range and had good consistency, and there was a correlation between the difference of VI value between observers and the value of VI mean ( r s=0.639, P<0.001). Conclusions:There is a good inter-observer consistency in evaluating the blood vascular architecture and quantitatively measuring the blood flow abundance by Smart-3D SMI.
10.Progress in detection and modeling of quorum sensing molecules of foodborne pathogens.

Quorum sensing (QS) plays a major role in the outbreak mechanism of foodborne diseases caused by food poisoning and food spoilage. QS affects the formation of cell membrane and pathogenicity ofpathogenic bacteria. Through the in-depth understanding of QS molecules of food-borne pathogens, we describe here the types of signal molecules produced by Gram-negative and Gram-positive bacteria, and the differences in QS molecules. Meanwhile, we introduce the detection of QS molecules by different technologies. According to the influence of QS on food, we propose also future research needs for the control of foodborne pathogenic bacteria.
