1.Mechanism of negative pressure wound therapy in the auxiliary treatment of bone and soft tissue infection

		                        			BACKGROUND:As the population ages at an accelerated rate in China,the incidence of bone and soft tissue infections is likewise on the rise.Bone and soft tissue infections can involve all bone and surrounding soft tissues,including the periosteum,cortex,cancellous bone and bone marrow.Examples of such infections include diabetes foot,osteomyelitis,surgical incision infection,and infection around joint prostheses.Owing to the intricate pathogenesis and challenging treatment,it has become increasingly noteworthy in clinical settings.Negative pressure wound therapy is a modern wound treatment which has been gaining popularity,especially in the area of bone and soft tissue infection. OBJECTIVE:To analyze the global research progress in the utilization of negative pressure wound therapy for treating bone and soft tissue infection in recent years. METHODS:Relevant articles in Chinese and English published in PubMed,Web of Science,and CNKI from 1990 to 2022 were retrieved.Search terms were"negative pressure wound therapy,vacuum assisted closure,negative pressure,osteomyelitis,bone infection"in English and Chinese,separately.A total of 711 articles were initially retrieved,out of which 65 articles were included for further review. RESULTS AND CONCLUSION:Negative pressure wound therapy,as an auxiliary approach to treating wounds,has a range of positive effects such as stabilizing wounds,diminishing edema,decreasing bacterial load,encouraging granulation tissue and angiogenesis,enhancing tissue perfusion,modulating peripheral nerves,modulating biological immunity and promoting the growth and differentiation of osteoblasts.This is a more effective method for managing complex wounds,such as bone and soft tissue infections,than traditional dressings with one-dimensional benefits.Numerous studies,both basic and clinical,have demonstrated the safety and efficacy of negative pressure wound therapy in the auxiliary treatment of bone and soft tissue infections.
		                        		
		                        		
		                        		
		                        	
2.Clinical efficacy of semaglutide in the treatment of type 2 diabetes mellitus combined with non-alco-holic fatty liver disease and its effect on oxidative stress and inflammatory factors

		                        			Objective To explore the efficacy of semaglutide in the treatment of type 2 diabetes mellitus(T2DM)com-bined with non-alcoholic fatty liver disease(NAFLD)and its effect on oxidative stress and inflammatory factors.Methods Totally 80 patients with T2DM accompanied by NAFLD admitted to the First Affiliated Hospital of Xinxiang Medical University from July 2021 to December 2022 were selected and randomly assigned to the observation group and the control group,with 40 patients in each group.Patients in the control group were treated with pioglitazone metformin and dapagliflozin,while patients in the observation group were treated with pioglitazone metformin,dapagliflozin,and semaglutide.The levels of glycated hemoglobin(HbA1c),fasting blood glucose(FBG),2-hour postprandial blood glucose(2hPG),body mass,body mass index(BMI),waist circumference,alanine aminotransferase(ALT),aspartate aminotransferase(AST),gamma-glutamyl transferase(GGT),controlled attenuation parameter(CAP),liver stiffness measurement(LSM),malondialdehyde(MDA),glutathione peroxidase(GSH-PX),lipid peroxide(LPO),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),and interleukin-10(IL-10)before and after the treatment were compared.Results After 24 weeks of treatment,the overall response rate(ORR)in the observation group and control group was 92.5%(37/40)and 72.5%(29/40),respectively;and the ORR in the observation group was significantly higher than that in the control group(x2=5.541,P<0.05).Before treatment,there was no statistically significant difference in the body mass,BMI,waist circumference,HbA1c,FBG,2hPG,ALT,AST,GGT,CAP,LSM,MDA,GSH-PX,LPO,TNF-α,IL-6,and IL-10 of patients between the 2 groups(P>0.05);after 24 weeks of treatment,the body mass,BMI,waist circumference,HbA1c,FBG,2hPG,ALT,AST,GGT,CAP,LSM,MDA,LPO,TNF-α,IL-6,and IL-10 were significantly lower than before treatment,while GSH-PX was significantly higher than before treatment(P<0.05);after 24 weeks of treatment,the body mass,BMI,waist circumference,HbA1c,FBG,2hPG,ALT,AST,GGT,CAP,LSM,MDA,LPO,TNF-α,IL-6,and IL-10 of patients in the observation group were significantly lower than those in the control group,and GSH-PX was significantly higher than that in the control group(P<0.05).The incidence of adverse reactions in the observation group and the control group during the treatment period was 17.5%(7/40)and 12.5%(5/40),respectively;and the difference in the incidence of adverse reactions between the two groups was not statistically significant(P>0.05).Conclusion Semaglutide significantly downregulates the levels of FBG,2hPG and HbA1c in patients with T2DM combined with NAFLD and reduces the body mass,waist circumference,liver enzyme level,hepatic fat content,hepatic fibrosis,oxidative stress,and inflammatory indicators.
		                        		
		                        		
		                        		
		                        	
3.Bibliometric analysis of current status of global and Chinese anesthesiology research in 2023

		                        			Objective:To analyze the current status of anesthesiology research in the world and China in 2023 and to identify the anesthesiology research hotspots using bibliometrics.Methods:The literature related to anesthesiology published in PubMed in 2023 was searched, and the country and author of the literature, as well as the key words of the literature were visually analyzed by using the software CiteSpace6.2. R4.Results:A total of 22 473 articles were included, the country with the largest number of publications was the United States, and China ranked second. The author with the highest number of publications in the field of anesthesiology in the worldwide in 2023 was Kaye Alan D from the United States. Chronic pain, general anesthesia and pain management were the research hotspots in the field of anesthesiology worldwide in 2023. The research hotspots in the field of anesthesiology in China focused on general anesthesia, oxidative stress and neuropathic pain.Conclusions:The United States is the leader in the research in the field of anesthesiology, with China following behind. The keywords of the literature in the field of anesthesiology between China and the world are roughly the same, reflecting the convergence of Chinese scientific research with global scientific research. 4.Giant fibroma of left ventricular apex with T wave inversion: 2 cases report and literature review

		                        			Objective:To investigate the clinical features, diagnostic methods and treatments of left ventricular apical fibroma.Methods:The clinical manifestations, ECG, imaging features and treatment plans of 2 patients with giant fibroma of left ventricular apex diagnosed in September 2020 and May 2022 were analyzed retrospectively, and the related literature was reviewed.Results:Both patients had slight chest distress and discomfort after activities. The ECG showed T-wave inversion of different degrees, which were misdiagnosed as “myocarditis” and “coronary heart disease” respectively. The cardiac magnetic resonance imaging and echocardiography showed left ventricular apical mass. Coronary artery stenosis was not found in coronary angiography. One patient required conservative treatment, and there was no significant change in clinical symptoms and tumor size in the follow-up for half a year; Another patient underwent cardiac mass removal, and the pathological examination after operation confirmed that it was cardiac fibroma, and there was no recurrence in the follow-up 2 years.Conclusion:Fibroma of left ventricular apex is a rare cardiac tumor, which is easy to be missed and misdiagnosed, and is one of the rare causes of T-wave inversion. Cardiac magnetic resonance imaging, CT and echocardiography are commonly used imaging methods to diagnose cardiac fibroma, and surgical resection is an effective treatment.

Consumption of poisonous mushrooms has an incubation period, there is a false recovery period in the clinical process, and the early performance is slight and does not attract enough attention from doctors, and it is easy to miss the treatment opportunity. 7.Research and development of Chinese anti-COVID-19 drugs.

		                        			The outbreak and spread of coronavirus disease 2019 (COVID-19) highlighted the importance and urgency of the research and development of therapeutic drugs. Very early into the COVID-19 pandemic, China has begun developing drugs, with some notable progress. Herein, we summarizes the anti-COVID-19 drugs and promising drug candidates originally developed and researched in China. Furthermore, we discussed the developmental prospects, mechanisms of action, and advantages and disadvantages of the anti-COVID-19 drugs in development, with the aim to contribute to the rational use of drugs in COVID-19 treatment and more effective development of new drugs against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and the variants. Neutralizing antibody is an effective approach to overcome COVID-19. However, drug resistance induced by rapid virus mutation will likely to challenge neutralizing antibodies. Taking into account current epidemic trends, small molecule drugs have a crucial role in fighting COVID-19 due to their significant advantage of convenient administration and affordable and broad-spectrum. Traditional Chinese medicines, including natural products and traditional Chinese medicine prescriptions, contribute to the treatment of COVID-19 due to their unique mechanism of action. 8. Quality Evaluation of Andrographis Herba Based On Electronic-eye Technique

		                        			 Objective: Taking electronic-eye (visual analyzer) technique,based on the powder color of Andrographis Herba,to investigate the applicability of electronic-eye technique and evaluate the quality of Andrographis Herba with different commercial specifications. Method: HPLC was employed to determine contents of andrographolide,dehydroandrographolide,14-deoxyandrographolide,neoandrographolide in 50 batches of Andrographis Herba with different commercial specifications(stems,leaves and aerial parts).Color of these samples were measured by electronic-eye technique.The data were analyzed by principal component analysis(PCA) and Pearson correlation analysis.The ability of electronic-eye to distinguish the different commercial specifications of Andrographis Herba was investigated and the correlation of chroma space system parameters (L*,a*,b*) with active components was investigated. Result: There was remarkable difference in contents of 4 diterpenoids in Andrographis Herba from different parts,their contents in leaves was the highest,followed by the aerial parts(mixture of stems and leaves),and their contents in stems was the lowest.The results of PCA was divided into two classes,namely the stem part,leaf and aerial parts,indicating that electronic-eye could be used to distinguish the quality of Andrographis Herba.The correlation results showed that there were significant negative correlation(P<0.01) between L*(lightness value) and the contents of andrographolide,dehydroandrographolide,14-deoxyandrographolide,neoandrographolide and the total content of these 4 components.In addition,L* of samples that did not conform to the lower limit of determination in the 2015 edition of Chinese Pharmacopeia was ≥ 69.5,and the L* of more than 90% of the samples in accordance with the requirements was <69.5. 9. Ion Chromatography Method of Four Carboxylic Acids in Workplace Air

		                        			 Objective:
		                        			To assess psychological acceptance and occupational stress of medical staff, analyze the relationship among personality, psychological acceptance and occupational stress and discuss the direct or indirect effects of personality to occupational stress.
		                        		
		                        			Methods:
		                        			The gaseous four kinds of carboxylic acids in the workplace air were simultaneously collected by silica gel tube, and then desorbed by deionized water and eluted by ion chromatograph. The the content was detected by conductivity detector.
		                        		
		                        			Results:
		                        			The linear relationship was good in the concentration range of about 0~140 mg/L. The correlation coefficient r>0.999, and the maximum detection limit was 4.5 μg/mL. The sampling efficiency of the four carboxylic acids ranges from 96.10%~100.27%. Through the sample added recovery experiments, the low and high content of the silicone tube samples were detected; and the range of desorption efficiency was 82.18%~100.12%; the range of precision was 0.70%~3.71%.
		                        		
		                        			Conclusion
		                        			This method adopts deionized water to desorb samples, and the application of ion chromatography detection have reached the requirements of《Guidelines for the establishment of occupational health standards, Part 4: Determination of chemical substances in workplace air》, which can be used in four kinds of workplace air detection of carboxylic acid compounds. 
		                        		
		                        		
		                        		
		                        	
10.Effect of metformin on serum vitamin D in patients with background diabetic retinopathy

		                        			Objective To investigate the changes of visual acuity,fundus changes,and serum vitamin D concentration in diabetic retinopathy patients treated with metformin,and to evaluate its clinical efficacy.Methods Totally 80 cases (160 eyes) patients with background diabetic retinopathy were included in the study,and were divided into observation group (metformin + Pancreatic Kininogenase Enteric-coated Tablets) and control group (Pancreatic Kininogenase Enteric-coated Tablets) according to the different treatment methods used for 3 months.Fundus fluorescein angiography was used to evaluate the improvement of the fundus lesions.The serum levels of vitamin D were measured with electrochemiluminescence immunoassay (ECLIA).Results After 3 months of treatment,serum vitamin D concentration of the observation group increased,and the differences were statistically significant (P < 0.05) when compared to before treatment.However,3 months after treatment,serum vitamin D levels of the control group increased slightly,but the difference was not statistically significant (P > 0.05).The total effective rate of fundus improvement in the observation group and the control group was 70% and 55%,respectively.There was significant difference between the two groups in the improvement of fundus (P < O.05).Conclusions Metformin has an adjunctive effect on background diabetic retinopathy,and the increase of serum vitamin D may be one of the mechanisms of metformin in improving diabetic retinopathy.
