1.Testosterone and non-alcoholic fatty liver disease in men and women: A Mendelian randomization study

Objective:To investigate the causal association between testosterone and nonalcoholic fatty liver disease(NAFLD) in men and women using a two-sample Mendelian randomization(MR) approach.Methods:Genetic variation in testosterone(total testosterone, bioavailable testosterone) and sex hormone-binding globulin(SHBG) in females and males was used as an instrumental variable using the genome-wide association study(GWAS) pooled data, and the inverse variance weighting method was applied. Inverse variance weighted(IVW) was used as the main analytical method, along with six univariate MR methods based on other modeling assumptions to assess the causal relationship between testosterone(total testosterone, bioavailable testosterone) as well as SHBG and NAFLD in women and men. In addition, NAFLD data from Finnish Biobank(FinnGen) were applied to validate the results of the exploratory analysis. Further, sensitivity analyses were performed to assess the level of heterogeneity, genetic pleiotropy, and stability of the instrumental variables using Cochran′ s Q test, MR-Egger regression, and leave-one-out methods. Results:The results of exploratory analysis of IVW model showed that bioavailable testosterone and SHBG were causally associated with NAFLD in women, for each unit increase in bioavailable testosterone levels, the risk of developing non-alcoholic fatty liver disease(NAFLD) rose by 24%( OR=1.24, 95% CI 1.07-1.43, P=0.004); and with each unit decrease in women′s SHBG, the NAFLD risk increased by 31%( OR=0.69, 95% CI 0.57-0.83, P<0.001). However, testosterone(total testosterone, bioavailable testosterone) as well as SHBG in men and female total testosterone did not show a causal relationship with NAFLD. The results of the other six MR methods were generally consistent with the IVW method. The results of the external validation data provided further evidence of a causal relationship between female bioavailable testosterone and SHBG and NAFLD. Conclusion:Elevated levels of bioavailable testosterone along lower levels of SHBG may increase the risk of developing NAFLD in women.
2.Preliminary analysis of self-made false lumen embolization device in the treatment of chronic Stanford type B aortic dissection

Objective:To analyze the early outcomes of self-made false lumen embolization device for occlusion the false lumen in chronic Stanford type B aortic dissection.Methods:A retrospective study analyzed 10 patients, there were 9 males and 1 female, with an average age of (52±10) years, aged from 39-69 years. Those 10 patients were diagnosed with chronic Stanford type B aortic dissection between June 2020 and July 2023, which presented with persistent false lumen backflow in the thoracic aortic segment, and persistent dilation or rupture of dissected aneurysms in our center. False lumen occlusion was performed using self-made false lumen embolization device. Primary endpoints index were technical success rate and clinical success rate. Secondary endpoints observation index were 30-day mortality and morbidity.Results:Over a mean 6-month follow-up (range 0-32 months) in 10 patients, the technical success rate was 90%. One patient occurred minor false lumen backflow in the intraoperative angiography and it exceeds the embolization device, but it was disappeared through conservative treatment during follow up period. The clinical success rate was 90%, 9 patients attained completely thrombosed at the thoracic segment while another 1 patient was manifested as partially thrombosed in false lumen. There were no intraprocedural death cases and 1 patient occurred paraparesis during perioperative period.Conclusion:It was a safe, effective and easy to achieve in the treatment of chronic Stanford type B aortic dissection using self-made false lumen embolization device, especially it is suitable for patients with chronic Stanford type B aortic dissection required emergent handling in the setting of rupture.
3.Progress on obesity-induced hypothalamic GnRH neuron dysfunction

The declining fertility has emerged as a significant concern impacting national welfare, with obesity identified as a major contributing factor. In the hypothalamus, gonadotropin-releasing hormone(GnRH) neurons release GnRH, which regulates the secretion of estrogen and testosterone via the hypothalamic-pituitary-gonadal(HPG) axis, thereby influencing sexual development and fertility. The proper function of GnRH neurons is crucial for maintaining normal sexual development and fertility. Research indicates that obesity can impact GnRH neuron function through several pathways, such as disrupting pulsatile secretion core kisspeptin neurons, causing imbalance in energy sensing and metabolic factor secretion, and triggering hypothalamic inflammation. These pathways directly or indirectly influence GnRH neuron function in the hypothalamus, which ultimately affects sexual development and impairs fertility. Therefore, this review consolidates recent advancements in understanding the mechanisms by which obesity induces dysfunction in hypothalamic GnRH neurons, offering new perspectives for early clinical interventions aimed at enhancing fertility outcomes in obese individuals.
4.Epidemiological overview and periodic trends of hemorrhagic fever with renal syndrome in Jingzhou City, Hubei Province from 1962 to 2020

Objective:To analyze the epidemic characteristics and periodicity of hemorrhagic fever with renal syndrome (HFRS) in Jingzhou City, Hubei Province, and provide a basis for scientific prevention and control of HFRS in Jingzhou City.Methods:Retrospective analysis was used to collect HFRS case data and population data of Jingzhou City and 8 counties (cities, districts) within its jurisdiction, including Shashi District, Jingzhou District, Gongan County, Jianli City, Jiangling County, Shishou City, Honghu City, and Songzi City from 1962 to 2020, from the Archives of the Jingzhou Center for Disease Control and Prevention and the Infectious Disease Report Information Management System of the China Disease Control and Prevention Information System; and the epidemic characteristics of HFRS was analyzed in Jingzhou City and 8 counties (cities, districts) within its jurisdiction. The periodicity of HFRS onset was determined using wavelet analysis.Results:From 1962 to 2020, 18 936 HFRS cases were reported in Jingzhou City, with an average incidence rate of 5.95/100 000. There were a total of three epidemic peaks, namely from 1972 to 1973 (24.82/100 000, 24.84/100 000), 1983 (60.08/100 000), and 1995 (14.57/100 000). According to different regions, the high incidence areas of HFRS showed a phased transfer trend: from the 1960s to the 1970s, the Jiangbei area (Honghu City, Jianli City) was the highest incidence area; in the 1980s and 1990s, the high incidence areas were transferred to Jiangnan area (Songzi City, Shishou City, and Gongan County); after 2005, high incidence areas were relocated to Jiangbei area (Honghu City, Jianli City, Jiangling County). The wavelet analysis results showed that there were 12.30 and 21.77 years of HFRS epidemic cycles in Jingzhou City before 2000 ( P < 0.05); among them, the periodicity of Shashi District, Gongan County, Jiangling County, Shishou City, and Honghu City was relatively consistent with that of Jingzhou City, with epidemic cycles of about 12 or 22 years ( P < 0.05). Conclusions:Jingzhou City is currently at the peak of a 22-year epidemic cycle of HFRS, with Jiangbei area as the high incidence areas. The 12-year epidemic cycle in Jingzhou City has ended after 2000.
5.Management strategy of iliac artery difficulty in abdominal aortic aneurysm endovascular repair

Objective:To explore the strategies for the treatment of difficult iliac artery approach in endovascular repair of abdominal aortic aneurysm.Methods:The clinical data of 275 patients with abdominal aortic aneurysm who underwent endovascular surgery at the Department of Vascular Surgery, People′s Hospital of Xinjiang Uygur Autonomous Region from March 2010 to March 2019 were retrospectively analyzed, and the general clinical data such as age, gender, and comorbidities of the patients were recorded. The resource index was to carry out corresponding measures to perform surgery for patients with difficult access, analyze the incidence of the type of difficult access, treatment measures and effects.Results:Two hundred and seventy-five patients underwent endovascular repair, 78 of them (28.3%) had difficulty in accessing the iliac artery, including 29 cases (10.5%) with mild iliac artery stenosis, 7 cases (2.54%) with severe stenosis, and 3 cases with occlusion ( 1.09%), 39 cases (14.2%) were twisted. For patients with vascular twist, super-hard guide wire was used to correct iliac artery angulation. For patients with iliac artery stenosis, balloon dilation was performed. For severe stenosis, the artificial blood vessel was passed through the lateral peritoneum. After road transplantation, stent placement and other treatments were successfully performed endovascular repair.Conclusions:Pathway vascular disease can cause difficulties in endovascular treatment of abdominal aortic aneurysms. Endovascular repair can be successfully performed after corresponding treatments according to different difficulties, and the long-term patency rate is good.
6.Correlation between thyroid function and glucolipid metabolism in type 1 diabetic adults

To assess the correlation between thyroid function and glucolipid metabolism in type 1 diabetic adults. A retrospective analysis was conducted in 230 type 1 diabetic adults who were hospitalized in the Department of Endocrinology of Shandong Provincial Hospital Affiliated to Shandong University from January 2008 to January 2020. It showed that thyroid stimulating hormone(TSH) was significantly positively correlated with total cholesterol (TC) ( r=0.239), triglycerides (TG) ( r=0.166) and low-density lipoprotein cholesterol (LDL-C) ( r=0.249), respectively (all P<0.05). Free triiodothyronine (FT 3) was significantly negatively correlated with fasting plasma glucose (FPG) ( r=-0.272), glycated hemoglobin (HbA1c) ( r=-0.240), TC ( r=-0.197) and LDL-C ( r=-0.220), respectively (all P<0.05). Free thyroxine (FT 4) was negatively correlated with TC ( r=-0.171) and LDL-C ( r=-0.170), respectively (all P<0.05). TC was an independent predictor of TSH, FT 3 and FT 4, FT 3 and FT 4 were independent predictors of HbA1c. TSH was an independent predictor of TC, TG and LDL-C. Thyroid function is closely related to glucolipid metabolism in type 1 diabetic adults.
7.Application of percutaneous mechanical thrombectomy combined with catheter directed thrombolysis in the treatment of acute lower extremity deep venous thrombosis

Objective:To investigate and compare the efficacy and safety of percutaneous mechanical thrombectomy (PMT) combined with catheter directed thrombolysis and catheter directed thrombolysis alone in the treatment of acute lower extremity deep venous thrombosis.Methods:Retrospective analysis of clinical data of 69 patients with acute lower extremity deep venous thrombosis who met the inclusion criteria of this study from January 2018 to December 2018 in department of vascular surgery, People′s Hospital of Xinjiang Uygur Autonomous Region was performed. There were 35 males and 34 females, with an average age of 59 years and an age rang of 20 to 80 years. Patients were divided into PMT combined catheter directed thrombolysis treated group ( n=38) and catheter directed thrombolysis treated group ( n=31) according to different methods of intra-luminal thrombolysis. The thrombolysis time, dosage of urokinase, thrombotic clearance rate, perimeter difference of suffered limb, and total number of adverse events during perioperative period between two groups were recorded. At 1 month, 3 months, and 6 months after discharge, patients underwent an ultrasonographic examination of lower extremity vessels to check and evaluate residual thrombolysis and thrombosis recurrence by outpatient review. The quantitative data in the two groups were expressed as mean±standard deviation( Mean± SD), t test was used for comparison between groups. Enumeration data in the two groups were expressed as percentage (%) and χ2 test was used for comparison between groups. Results:The thrombolysis time of PMT combined catheter directed thrombolysis treated group and catheter directed thrombolysis treated group were (3.7±2.1) d and (5.2±2.1) d, respectively. The differences between the two groups were statistically significant ( P=0.005). The dosage of urokinase were respectively (225.0±122.3)×10 4 units and (315.8±108.6)×10 4 units, the differences were statistically significant ( P=0.001). PMTⅠ, Ⅱ, Ⅲ thrombus clearance rate in patients with PMT combined catheter directed thrombolysis treated group were 18.4% (7/38), 73.7% (28/38), 7.9% (3/38), while 16.1% (5/31), 77.4% (24/31), 6.5% (2/31) were observed in patients with catheter directed thrombolysis treated group, with no statistically significant difference between the two groups ( P= 0.803, P=0.720, P=0.818). After a period of 48h thrombolytic therapy, the cross-section diameters of suffered thigh and calf were (2.16±0.87) cm and (1.38±0.66) cm in PMT combined catheter directed thrombolysis treated group respectively. While the cross-section diameters of suffered thigh and calf were (1.21±0.59) cm and (1.02±0.49) cm in catheter directed thrombolysis treated group respectively. The differences in the cross-section diameters of suffered thigh and calf were statistically significant after a period of 48h thrombolytic therapy ( P<0.001, P=0.014). Three cases (7.89%) had occurred adverse events during perioperative period in PMT combined catheter directed thrombolysis treated group, while there were 4 cases (12.90%) in catheter directed thrombolysis treated group, with no statistically significant difference between two groups ( P=0.692). The six month follow-up rate of PMT combined with catheter directed thrombolysis treated group was 71.05%(27/38), while that of the catheter directed thrombolysis treated group was 64.52% (20/31), with no statistically significant difierence between the two groups ( P=0.532). During the six month of follow-up, the recurrence rate of thrombus in PMT combined catheter directed thrombolysis treated group (3/27, 11.11%) was lower than those patients in catheter directed thrombolysis treated group (3/20, 15.00%), with no statistically significant difference between the two groups ( P=0.693). Conclusions:Compared with catheter directed thrombolysis treated group, PMT combined catheter directed thrombolysis treated group has a similar clinical efficacy for treatment of acute lower extremity deep venous thrombosis. PMT combined catheter directed thrombolysis treated group has advantages of reducing dosage of urokinase, shortening duration of thrombolysis time and relieving the swelling of suffered limb in a short time rather than catheter directed thrombolysis treated group. It shows a great result in the short term follow up.

9.Application of double parallel stents in complicated aortic arch pathologies

Objective To discuss the application of double parallel stent for preserving arch branch vessels during thoracic endovascular aortic repair.Methods During the period from June 2015 to June 2016,double parallel stents were carried out in 9 cases at department of vascular surgery,the People's Hospital of Xinjiang Uygur Autonomous Region.Results One patient died of an acute ischemic stroke and subsequently resulted in multiple organ failure after thoracic endovascular aortic repair.Eight patients received an aorta CT angiography during follow up at 1,3,6 months.During follow up,the patency rate for double parallel stents was 100%.There were no endoleak in 8 cases.Conclusion Double parallel stents are suited to patients with emergency situation and unsuited to open surgery in complicated aortic arch pathologies.Surgical approach is determined by preoperative evaluation.Prevention of endoleak and stroke is important in all cases.
10.Exploration of establishing a metastatic nude mouse model of double-tagging papillary thyroid cancer

Objective To obtain a thyroid cancer cell line integrated with luciferase and fluorescence,and to establish a metastatic nude mouse model of papillary thyroid cancer monitored with in vivo imaging system. Methods Lentivirus carrying recombinant plasmid containing luciferase gene and mCherry gene infected high malignant papillary thyroid cancer cell line (BHP10-3SCmice). The stable thyroid cancer cell line (BHP10-3mluc) labeled with luciferase and mCherry selected by hygromycin, analyzed for fluorescence with fluorescence microscopy and bioluminescence with in vitro analysis and in vivo imaging. 0.25 ml(5×106/ml)BHP10-3mluc cell suspension were injected into the left thigh intramuscular tissue of 5-week-old nude mice to establish BALB/C nude mice metastatic tumor model. The growth and metastasis of the tumors were monitored weekly with in vivo imaging system. The nude mice were sacrificed on the 42th day and the tissues with metastatic tumors were analyzed by fluorescent imaging ex vivo. The ectopic implanted tumors and metastatic lesions were verified by tissue sections with Hematoxylin and Eosin staining. Results BHP10-3mluc cells showed similar growth characteristics to the original BHP10-3SCmice cells and stably expressed luciferase and mCherry. At the end of the first week after ectopic implantation,the xenograft tumors were found and in the 6th week,the tumors were found to metastasized to adjacent lymph nodes and lungs,which was consistent with the results of pathology.The growth and metastasis of tumors can be accurately monitored with in vivo imaging system. Conclusions A PTC cell line stably expressing bioluciferase and fluorescence was successfully established.The nude mouse model of PTC metastatic tumor generated by intramuscular inoculation of those cells can be well monitored with in vivo imaging system, which provides ideal models for researches on tumor growth, metastasis,and drug treatment.
