1.Research Progress in Obesity Related Heart Failure

The prevalence rate of heart failure is increasing year by year,posing a significant threat to human health,partly due to the rapid growth of overweight and obese populations.Obesity,as an independent risk factor of heart failure,affects hemodynamics,cardiac structure and cardiac function through mechanisms such as adipocyte release of adipokines,insulin resistance and accumulation of epicardial adipose tissue.Although the results of whether weight loss can reduce heart failure incidence are inconsistent,weight loss is associated with benefits such as reduced symptoms and comorbidities,and improved quality of life.Weight loss thus plays an indispensable role in the prevention and management of cardiovascular diseases in obese individuals.Currently,effective weight loss strategies confirmed by evidence-based studies mainly include lifestyle interventions,medication therapy and weight loss surgery.
2.Simultaneous determination of six compounds in the alcohol extract from Mongolian medicine Qinggan Manu-4 and the establishment of fingerprints by HPLC

Objective To establish a HPLC method and fingerprints study for the simultaneous determination of loganic acid,gentiopicroside,paeoniflorin,vitexin,liquiritin,and ammonium glycyrrhizinatein,and other components in the alcohol extract of Mongolian medicine Qinggan Manu-4.Methods The Agilent Eclipse XDB-C18 column(250 mm×4.6 mm,5 μm)was used,with the detection wavelength of 230 nm for paeoniflorin,vitexin and liquiritin,237 nm for loganic acid,250 nm for glycyrrhizic acid,and 270 nm for gentiopicroside.The mobile phase was 0.1%phosphoric acid aqueous solution-acetonitrile with gradient elution.The flow rate was 1.0 mL/min,the column temperature was 30℃,and the injection volume was 10 μL.Results The mass concentrations of loganic acid,gentiopicroside,paeoniflorin,vitexin,liquiritin and ammonium gentiopicroside in the alcohol extract of Mongolian medicine Qinggan Manu-4 showed good linear relationships with the chromatographic peak area in the range of 0.020 3-0.121 0,0.053 3-0.317 7,0.021 3-0.127 0,0.011 5-0.069 0,0.014 1-0.083 8 and 0.035 1-0.209 1 mg/mL,respectively(r≥0.999 8).The average recovery rates of the six components were 94.75%,99.49%,92.32%,95.82%,101.29%and 98.04%,with the RSDs of 1.11%,0.76%,0.99%,2.75%,1.09%and 2.43%,respectively(n=6).The HPLC fingerprint of the alcoholic extract of Mongolian medicine Qinggan Manu-4 was established,using the chromatogram of sample S1 as the reference chromatogram,the control fingerprints were generated through multi-point correction and full-spectrum peak matching.A total of ten common peaks were identified,and after comparison with the reference substance,eight components were identified.Conclusion The established method and fingerprints are accurate,reliable,reproducible and specific,which provide a basis for the quality control and subsequent development of Mongolian medicine Qinggan Manu-4.
3.Retrospective cohort study on the efficacy of methotrexate combined with mifepristone in the treatment of ectopic pregnancy

OBJECTIVE To investigate the efficacy of Methotrexate (MTX) injection combined with Mifepristone tablets in the treatment of ectopic pregnancy. METHODS A retrospective cohort study method was used to collect data on patients with ectopic pregnancy who visited the Affiliated Hospital of Chengdu University from January 1, 2013 to December 31, 2021. Control group was given MTX injection alone, and exposure group was given MTX injection combined with Mifepristone tablets. Baseline variables were balanced between the two groups using a propensity score matching (PSM), and the outcomes of the matched cohorts were compared. RESULTS A total of 125 patients were included, involving 68 in the control group and 57 in the exposure group. After PSM, 46 cases in each group were successfully matched. There was no significant difference in the treatment response rate and the incidence of adverse drug reaction (ADR) between the exposure group and the control group, which were 73.91% and 63.04%, 21.74% and 13.04% (P>0.05). Compared with before treatment, serum levels of β-HCG were all significantly decreased in 2 groups after treatment (P<0.05); there was no significant difference in the serum level of β-HCG after treatment, the time to return to normal, the interval time of second pregnancy, second ectopic pregnancy and second intrauterine pregnancy between 2 groups (P>0.05). The mean diameter difference of B-ultrasound pregnancy mass before and after treatment, duration of clinical symptom remission and length of hospital stay in the exposure group were significantly higher than those in the control group (P< 0.05), but drug costs during hospitalization and total hospitalization costs were not significantly increased (P>0.05). There were no significant difference between single-dose group and multi-dose group of MTX in the treatment response rate and the incidence of ADR (P>0.05). The hospitalization time, drug cost and total cost of the single dose group were significantly less than those of the multi-dose group (P<0.05). CONCLUSIONS MTX injection combined with or without Mifepristone tablet can both effectively treat ectopic pregnancy, with comparable efficacy and safety. MTX single-dose regimen is similar to the multi-dose regimen in efficacy, but the multi-dose regimen increases the hospital stay and hospitalization costs.
4. Emerging therapeutic role of Prunella vulgaris in thyroid disease

Thyroid disease is characterized by unusual levels of thyroid hormones, which results in either hyperthyroidism or hypothyroidism. The pathology of a particular type or stage of thyroid disease is very complicated, and always linked to a variety of biological functions. Although the mortality rate is not high, thyroid dysfunction could lead to metabolic and immunological disorders that can subsequently cause discomfort. To date, many drugs are suggested to have curative effects on thyroid disease, however, drug toxicity and long treatment periods encourage the search for more promising ones. Prunella vulgaris L. (Labiatae) is a popular herb that has shown great potential for improving human immunity and organ protection. It has been extensively used in the treatment of many diseases but its ability to treat specific diseases has not been fully reported. In this review, a literature search regarding herbs and herbal recipes for treating thyroid disease were carried out, organized, and summarized. In addition, this study conducted a literature search on the current situation and progress of P. vulgaris treatment for various diseases. Finally, this study discussed studies regarding P. vulgaris treatment of goiter, and the mechanism of treatment through the regulation of apoptosis. Accordingly, a combination therapy of herbs and Western medicine can provide significant therapeutic effects in the clinical treatment of thyroid disease. Furthermore, the association between P. vulgaris and various diseases suggests that P. vulgaris is rich in a variety of active substances that can fight oxidation and participate in the regulation of apoptosis, thus having a protective effect on the thyroid. Here, a comprehensive literature review regarding the application of herbs or herbal recipes in the treatment of thyroid disease was presented. It is concluded that there is strong evidence for further research regarding the use of P. vulgaris in the treatment of thyroid diseases.
5.Regular characteristics of the effects of double-bridge and single-bridge exercises on the surface EMG activity of related core muscles in stroke patients with hemiplegia

Objective:To observe and analyze the effect of double and single bridge exercise on the electromyographic activities of related core muscle groups in stroke patients, and to explore its rules and characteristics.Methods:A prospective cohort study was conducted on 40 stroke patients hospitalized in the Department of rehabilitation medicine, Hefei second people's Hospital, Anhui Province from March 2020 to May 2021.The surface electromyography (sEMG) instrument was used to collect the surface electromyographic signals of erector spinalis, rectus abdominis, gluteus maximus and biceps femoris during double and single bridge exercise, and the root mean square (RMS) and integrated electromyography (iEMG) of the time domain indexes were analyzed. The measurement data conforming to normal distribution was expressed in xˉ± s.The non normal distribution data was expressed in M (Q1, Q3). Wilcoxon rank sum test was used to compare the difference between the two groups. Results:There was significant difference between RMS (30.0 (21.3, 45.5) μV vs. 24.0 (14.0, 35.8) μV) and IEMG (15.5. (10.0, 23.0) μV?s vs. 9.0 (5.0, 13.0) μV?s s) values of gluteus maximus on the healthy side and the affected side during double bridge exercise ( Z values were 2.07, 4.19; P values were 0.039, <0.001, respectively). There was significant difference in RMS (31.0 (15.3, 70.0) μV ratio of the healthy and affected biceps femoris 17.0 (11.0, 28.8) μV) and IEMG (14.5 (8.0, 26.5) μV?s vs. 7.0 (5.0, 10.8) μV?s) values of biceps femoris on the healthy side during double bridge exercise ( Z values were 3.44, 3.64; P values were 0.001 and <0.001, respectively ). There was significant difference between RMS(38.5(32.3, 46.0) μV vs. 35.0(22.3, 43.0) μV) and IEMG (16.5(12.0, 22.8) μV?s vs. 12.0(7.0, 21.0) μV?s) values of the gluteus maximus on the healthy side during single bridge exercise ( Z values were 2.24, 2.45; P values were 0.025, 0.014, respectively). There was significant difference between RMS (38.0 (15.3, 70.0) μV vs. 19.0 (12.0, 35.5) μV) and IEMG (16.0 (10.0, 27.0) μV?s vs. 6.5 (5.0, 12.5) μV?s s) values of biceps femoris on the healthy side during single bridge exercise ( Z values were 2.98,4.34; P values were 0.003 and <0.001, respectively). There was significant difference between RMS (24.0 (14.0, 35.8) μV vs. 35.0 (22.3, 43.0) μV) and IEMG (9.0 (5.0, 13.0) μV?s vs. 12.0 (7.0, 21.0) μV?s) values of double and single gluteus maximus on the affected side ( Z values were 2.24, 1.99; P values were 0.025,0.047, respectively). Conclusion:Double bridge and single bridge exercise could improve the related core muscle groups of stroke patients with hemiplegia, and single bridge was better than double bridge for the activation of gluteus maximus
6.Molecular docking study of "detoxification" mechanism of Chebulae fructus- Margarita intervention in stroke

Objective:To analyze the material basis, target and pathway of Chebulae fructus and Margarita in Mongolian medicine Garidi-13 Pill on stroke with the method of network pharmacology and molecular docking technology, so as to better understand its "detoxification" mechanism. Methods:TCMIP and BATMAN-TCM were used to predict the target of Chebulae fructus and Margarita composition, and GeneCards were used to search for the target of stroke. The overlapping targets of the two platforms were imported into the Metascape software for GO biological analysis and KEGG pathway analysis. The molecular docking of key molecules and targets was carried out with LEDOCK. Results:A total of 22 active components were collected and 217 targets related to stroke were predicted. Among them, 1-O-galloyl-glucose, cuprum, ellagicacid, arjungenin and corilagin were the key substances playing the role of "detoxification" of the Chebulae fructus and Margarita; IL6, TNF, HSP90AA1, PTGS2, CASP3, NR1I2, VKORC1 and ATP1A1 were the key targets playing the role of "detoxification" . These targets were significantly enriched in cell response, humoral level regulation, hemostasis, response to steroid hormones, steroid metabolism, coagulation and other biological processes, as well as nitrogen metabolism, IL-17 signaling pathway and other pathways. Molecular docking verified the accuracy of previous prediction results from computer simulation level. Conclusion:The process of Chebulae fructus and Margarita intervening strok is closely related to the elimination of harmful metabolites and calmingthe inflammatory reaction, which was not only consistent with the modern medicine on the pathological process of stroke, but also consistent with the interpretation of "evil and poison" with Mongolian medicine theory.
7.Progresses of ultrasound elastography in endometrial lesions

Endometrial lesions are common gynecological diseases in women,including submucosal fibroids,endometrial polyps,endometrial hyperplasia and endometrial cancer,etc.Endometrial hyperplasia is one of the precancerous lesions,and early diagnosis can effectively reduce the rate of malignancy.Ultrasound elastography has been gradually used in gynecology.The classification,principles of ultrasound elastography and its application in diagnosis of endometrial lesions were reviewed in the article.
8.Effect of seretide combined with singulair on lung function improvement and prognosis of moderate to severe AECOPD

Objective To study the effect improvation of lung function and prognosis of singulair and seretide for moderate to severe AECOPD . Methods 78 patients associated with moderate to severe AECOPD from January 2014 to June 2016 in our hospital were researched.They were divided randomly into observation and control groups according to the order of admission, 39 cases in each group.In the control group, they were treated by conventional anti-infective, expectorant, oxygen, suction seretide.In the observation group, using singulair on the basis of the control group.Clinical data, clinical efficacy, clinical symptoms begin to improve time, hospitalization time, blood gas analysis (PaO2, PaCO2) after five days and pulmonary function after treatment forced expiratory volume in one second ( FEV1 ) , call gas peak velocity ( PEF) , forced expiratory volume in one second than the forced vital capacity (FEV1/FVC), maximum mid-expiratory flow rate (MMEF) were compared with each other.Results Their clinical data of patients and other general information and basic condition had no significant difference between each other.The effective rate of observation group and control group were 97.44%and 87.18%, clinical effecicy of observation group was better than the control group, the difference was statistically significant (u=2.2805, P<0.05).Postoperative hospital stay, time of symptoms began to improve of the observation group were shorter than the control group, the difference was statistically significant (P<0.05), PaO2 of 5-day post-treatment of observation group was higher, and PaCO2 was lower, the difference was statistically significant (P<0.05).Lung function of two groups of patients had no significant difference before treatment, after treatment, pulmonary function in the observation group (including FEV1, PEF, FEV1/FVC, MMEF) was better than the control group, the difference was statistically significant (P<0.05).Conclusion Seretide combined with singulair to treat severe AECOPD has a good effect, it can significantly improve the therapeutic effect and improve the patient's lung function.
9.Study on Clinical Characteristics and Traditional Chinese Medicine Syndrome of Severe Pneumonia Associated Sepsis

This study was aimed to investigate therapies in combination of traditional Chinese medicine (TCM) and western medicine from the investigation of clinical characteristics and TCM syndromes of patients with severe pneumonia associated sepsis in ICU.A retrospective study was conducted to collect clinical data on 113 patients suffered from severe pneumonia associated sepsis.The basic information on hospital admission,TCM syndrome of deficiency of excess,APACHE Ⅱ score,CD3+ (%),CD4+ (%),and CD8+ (%) were analyzed.The total days of hospitalization and treatment outcome were recorded.A total of 113 patients who met the diagnostic criteria of severe pneumonia associated sepsis were divided into the survival group and the death group.There were 81 patients in the survival group while 32 patients in the death group.The total mortality rate was 28.32%.There was a statistical difference between two groups on the days of hospitalization,APACHE Ⅱ score,CD3+ (%),CD4+ (%),CD8+ (%) and CD4+/CD8+ (all P < 0.05).The days of hospitalization of the death group was longer than that of the survival group,with higher APACHE Ⅱ score (P < 0.05).The survival group showed a higher level of CD3+ (%),CD4+ (%),CD4+/CD8+ (%) than that of the death group (P < 0.05).The survival group showed a lower level of CD8+ (%) than that of the death group (P < 0.05).The deficiency syndrome group showed a higher mortality rate than that of the excess syndrome group (P < 0.05).APACHE Ⅱ score,the days of hospitalization,deficiency syndrome,and CD3+ (%) were related to the death outcome of sepsis patients.It was concluded that severe pneumonia associated sepsis predicates a higher mortality rate.The clinical data collected at the beginning of hospital admission shows a certain prediction effects to the disease prognosis.There is a certain correlation between TCM syndrome and the disease severity.The mortality rate of deficiency syndrome is higher.

The injury of anal sphincter complex during childbirth can result in long-term anal incontinence problems.Recently,several new techniques in ultrasound have been used to evaluate the function and injury of anal sphincter,such as tomographic ultrasound imaging,volume contrast imaging and speckle reduction imaging.The diagnosis rate of the injury of anal sphincter complex has been improved rapidly.The development of 2D and 3D ultrasound in evaluating function and injury of postpartum anal sphincter complex were reviewed in this article.
