1.Astragali Radix Polysaccharide Inhibits Proliferation and Migration of Gastric Cancer Cells by Targeting ID1 and Akt

ObjectiveTo explore the regulatory effects and mechanisms of Astragali Radix polysaccharide (APS) on inhibitor of differentiation1 (ID1) and protein kinase B (Akt) in gastric cancer. MethodsImmunohistochemical staining was used to detect the expression of ID1 and Akt in 61 gastric cancer tissue samples and 20 adjacent normal gastric tissue samples. Immunofluorescence was used to detect the localization of ID1 and Akt. The effects of APS at the concentrations of 0.625, 1.25, 2.5, 5, 10, 20 mg·L-1 on the proliferation of gastric cancer MGC-803 cells were examined by the cell counting kit-8(CCK-8) method and the colony formation assay. The target information of APS was retrieved from the Traditional Chinese Medicine Systems Pharmacology and Analysis Platform and Swiss Target Prediction. Keywords such as gastric cancer, gastric tumor, and stomach cancer were searched against GeneCards, UniProt, DisGeNET, and Online Mendelian Inheritance in Man (OMIM) for the screening of gastric cancer-related targets. The online tool jvenn was used to create the Venn diagram to identify the common targets, and STRING and Cytoscape were used to construct the protein-protein interaction network. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analyses were conducted via R 4.2.2 to predict the potential roles of APS in the development of gastric cancer. The cell scratch assay was employed to assess the effect of APS on the migration of MGC-803 cells. The protein and mRNA levels of ID1 and Akt in the cells treated with APS were determined by Western blot and Real-time PCR, respectively. ResultsCompared with the adjacent normal gastric tissue, the gastric adenocarcinoma tissue showed increased positive expression of ID1 (χ2 =81.00, P<0.01). Immunofluorescence detection showed that ID1 and Akt were mainly located in the cytoplasm of gastric adenocarcinoma cells. Bioinformatics analysis identified 14 common genes shared between APS and gastric cancer. The average degree of protein-protein interaction network nodes was 14.29. GO and KEGG pathway enrichment results showed that ID1 and Akt were significantly enriched in the Rap1 and phosphatidylinositol-3-kinase (PI3K) /Akt signaling pathways. Cell experiments demonstrated that 5-fluorouracil (0.1 mg·L-1) and APS (10, 20 mg·L-1) groups showed decreased cell proliferation, migration, and colony formation. Compared with the control group, 10, 20 mg·L-1 APS inhibited the proliferation of MGC-803 cells (P<0.01), with 10 mg·L-1 APS demonstrating stronger inhibitory effect. In addition, APS at 10, 20 mg·L-1 inhibited the migration (P<0.01) and colony formation (P<0.05, P<0.01) of MGC-803 cells. Compared with the control group, APS at 10, 20 mg·L-1 down-regulated the protein levels of ID1 (P<0.01) and Akt (P<0.05) and the mRNA levels of ID1 (P<0.05, P<0.01) and Akt (P<0.05, P<0.01) in MGC-803 cells. ConclusionID1 and Akt are highly expressed in the gastric adenocarcinoma tissue, which may be related to the development of gastric cancer. APS can down-regulate the protein and mRNA levels of ID1 and Akt to exert anti-tumor effects, which is expected to provide new therapeutic targets for gastric cancer treatment.
2.The application of a psychological nursing programme based on acceptance and commitment therapy in end-stage cancer patients

Objective To explore the effects of a psychological nursing programme based on acceptance and commitment therapy on patients with end-stage cancer.Methods The convenience sampling method was adopted to select 120 patients with end-stage cancer who were hospitalized in the radiotherapy department of a tertiary A maternity and general hospital in Yuncheng from June 2021 to December 2022 as the research subjects.The Research Randomizer was used to divide them into an experimental group and a control group,with 60 cases in each group.The experimental group adopted the psychological nursing programme based on acceptance and commitment therapy,and the control group received the routine psychological nursing.The differences of psychological resilience,psychological pain,experiential avoidance level and quality of life before and after intervention were compared between the 2 groups.Results 57 cases in the experimental group and 58 cases in the control group completed the study.The scores of the 10-item Connor-Davidson Resilience Scale were(35.14±4.76)points in the experimental group and(33.17±2.97)points in the control group,and the difference between the 2 groups was statistically significant(t=2.664,P=0.009).After intervention,the scores of the Distress Thermometer in experimental group and control group were(4.14±1.88)points and(5.69±2.13)points,and the difference between the 2 groups was statistically significant(t=-4.138,P<0.001).The scores of the Acceptance and Action Questionnaire-Ⅱwere(16.19±3.80)points in the experimental group and(23.93±2.46)points in the control group,and the difference between the 2 groups was statistically significant(t=-13.002,P<0.001).The EORTC QLQ-C30 score in the experimental group was(117.00±8.50)points,and that in the control group was(101.45±11.88)points,and the difference between the 2 groups was statistically significant(t=8.066,P<0.001).Conclusion The psychological nursing programme based on acceptance and commitment therapy can effectively reduce the level of psychological pain and experiential avoidance,improve psychological resilience and improve the quality of life of patients.
3.Summary of the best evidence for home-based exercise management in patients with mild-to-moderate Parkinson's disease

Objective:To retrieve, evaluate and summarize relevant evidence on home-based exercise management in patients with mild to moderate Parkinson's disease, so as to provide reference and guidance for clinical healthcare professionals to construct and improve patient home-based exercise intervention plans.Methods:According to the top-down principle of the "6S" evidence model, UpToDate, BMJ, National Institute for Health and Clinical Excellence, Guidelines International Network, National Guideline Clearinghouse, Medlive, Clinical Guidelines Network, Joanna Briggs Institute, Cochrane Library, PubMed, CINAHL, Web of Science, Embase, China National Knowledge Infrastructure, Wanfang Data, VIP and China Biology Medicine Disc were systematically searched for relevant literature on home-based exercise management in patients with Parkinson's disease, including guidelines, expert consensuses, systematic reviews, evidence summaries, clinical decisions, recommended practices and best practices. The search period was from the establishment of the databases to October 31, 2022. Two researchers independently evaluated the quality of the literature and extracted and integrated evidence.Results:A total of 27 articles were included, including three guidelines, five expert opinions and consensuses, three clinical practice suggestions, 15 systematic evaluations and one clinical decision. A total of 45 pieces of evidence were summarized from six aspects, such as organizational management, health education, exercise evaluation, exercise prescription, family and social support and monitoring guidance.Conclusions:This study summarized the best evidence of home-based exercise management for patients with mild to moderate Parkinson's disease, which can provide evidence-based basis for clinical staff to formulate home-based exercise management programs and standardize practice standards.
4.Correlation Between TCM Syndrome Types and Onset Solar Terms in Patients with Gastric Cancer

Objective To investigate the correlation between TCM syndrome types and solar term of onset in patients with gastric cancer.Methods A total of 359 patients with gastric cancer admitted to the Cancer Center of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from January 2020 to August 2023 were included in this study,and the frequency distribution method was used to analyze their information such as sex,age,solar term of onset,differentiation,metastasis and TCM syndrome type.The solar term of onset corresponding to the onset date was calculated,and then the association between solar term of onset and TCM syndrome type in gastric cancer was analyzed.The circular distribution method was used to explore the peak of solar term of onset and TCM syndrome type.Results Among the 359 patients with gastric cancer included in this study,male patients were more than female patients(1.69∶1).The age of onset was mainly between 60 and 70 years old(117 cases,32.6%),and male patients(85 cases,72.6%)were more than female patients(32 cases,27.3%)in this age group.The most common TCM syndromes were qi and blood deficiency syndrome(160 cases,44.6%)and phlegm damp condensation syndrome(94 cases,26.2%).The onset of the disease mainly occurred in winter(132 cases,36.8%),and the peaks were light snow(31 cases,23.5%),major cold(25 cases,18.9%),heavy snow(23 cases,17.4%)and the start of winter(22 cases,16.7%).Qi and blood deficiency syndrome and phlegm damp condensation syndrome in patients with gastric cancer were correlated with solar terms and seasons(P<0.05).The 285 patients with gastric cancer were mainly poorly differentiated gastric cancer(175 cases,61.4%),mainly concentrated in winter(66 cases,37.7%).The main route of gastric cancer metastasis is lymph node metastasis,followed by liver and abdominal cavity metastasis.Conclusion Qi and blood deficiency syndrome and phlegm dampness condensation syndrome are common in patients with gastric cancer.The onset time of gastric cancer is mostly in winter,and the syndrome type is significantly correlated with the onset solar term and differentiation degree.
5.Molecular Mechanism of Essential Oil from Chimonanthus nitens Leaves Against Acute Lung Injury

ObjectiveBased on network pharmacology and molecular docking techniques, the mechanism of essential oil from Chimonanthus nitens leaves (CLO) in the treatment of acute lung injury (ALI) was predicted, and a rat model of ALI was established to verify the mechanism of CLO. MethodThe composition of CLO was determined by gas chromatography-mass spectrometry (GC-MS). The component targets were obtained from PharmMapper and SwissTargetPrediction databases, ALI-related targets were obtained from GeneCards, Online Mendelian Inheritance in Man (OMIM) and DisGeNET, cross-over analysis with differential expressed genes (DEGs) of ALI obtained from Gene Expression Omnibus (GEO) on the Venny 2.1.0 platform yielded potential anti-ALI targets of CLO. Protein-protein interaction (PPI) analysis of potential targets was carried out by STRING 11.5. The tissue expression profiles of potential targets were obtained from the National Center for Biotechnology Information (NCBI) and the target tissue distribution maps were constructed. Potential targets were analyzed for Gene Ontology (GO) function and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment by RStudio 4.0.0 software. Composition-target-pathway network was constructed by Cytoscape 3.9.1 software, and key components and pathways were screened out and verified by molecular docking. ALI model was established by lipopolysaccharide (LPS) induction, levels of interleukin (IL)-6 and tumor necrosis factor (TNF)-α in serum of rats were measured, the expression levels of IL-17 protein in the lung tissue of ALI rats were analyzed by immunohistochemistry. ResultA total of 19 components of CLO were identified by GC-MS, and 18 potential targets were obtained by target screening. After PPI analysis, 15 target proteins with interaction relationship were obtained, further analysis showed that they were highly expressed in lung and thymus. The network diagram of component-target-pathway was analyzed to obtain the key components, including bornyl acetate, linalool, elemol, geranyl isobutyrate, and the core targets of matrix metalloproteinase 13 (MMP13), S100 calcium binding protein A9 (S100A9), spleen tyrosine kinase (SYK), as well as the main signaling pathways, such as IL-17 and TNF. The results of molecular docking showed that the key components were stably bound to MMP13 and S100A9 of IL-17 signaling pathway. The results of pharmacological experiment confirmed that CLO could significantly inhibit the expression of IL-6 and TNF-α in serum of ALI rats, and decrease the expression of IL-17 protein in lung tissue of ALI rats. ConclusionCLO can achieve the therapeutic effect on ALI and protect lung tissue, the mechanism may be related to the decrease of the expression of IL-6 and TNF-α in serum and the inhibition of the activation of IL-17 signaling pathway in lung tissue of ALI rats.
6.A clinical research on relationship between sepsis-induced coagulopathy and prognosis in patients with sepsis

Objective:To evaluate the prognostic value of sepsis-induced coagulopathy (SIC) in patients with sepsis.Methods:From January 2019 to December 2021, patients with sepsis admitted to the Intensive Care Unit of our hospital were retrospectively classified into the SIC group and non-SIC group according to SIC diagnostic criteria. The baseline clinical data, severity score, total length of hospital stay, length of ICU stay and 28-day survival were compared between the two groups. Kaplan-Meier was used to compare the 28-day survival of patients with sepsis between the two groups. Cox proportional hazard regression model was employed to analyze the risk factors of prognosis in patients with sepsis.Results:Totally 274 patients with sepsis were included in the analysis, including 139 patients in the SIC group and 135 patients in the non-SIC group. The two groups were compared in the perspectives of the Platelet count (PLT), prothrombin time (PT) , procalcitonin (PCT), D dimer, hematocrit, red blood cell distribution width, hemoglobin, acute kidney injury (AKI), the use of continuous renal replacement treatment (CRRT), the use of vasoactive drugs, sequential organ failure assessment (SOFA) score, acute physiology and chronic health evaluation (APACHEⅡ) score were compared between the two groups and the difference were statistically different (all P<0.05). Kaplan-Meier analysis showed that the 28-day mortality rate in the SIC group was significantly higher than that in the non-SIC group (32.4% vs. 14.1%, P<0.05). COX proportional hazard model showed that SIC score ( HR= 2.17, 95% CI: 1.15-3.91, P<0.05), APACHEⅡ score ( HR= 1.13, 95% CI: 1.09-1.17, P<0.05) and the use of vasoactive drugs ( HR=3.66, 95% CI: 1.53-8.75, P<0.05) were independent influencing factors for 28-day death in patients with sepsis. Conclusions:Patients with sepsis and SIC have more severe disease and increased mortality risk. SIC score exhibits good clinical value in predicting the prognosis of patients with sepsis.
7.Visual analysis of home-based rehabilitation exercises for patient with Parkinson's disease based on Web of Science

Objective:To explore the research hotspot and trends of home rehabilitation exercise for patients with Parkinson's disease.Methods:Literature related to home-based rehabilitation exercises for Parkinson's disease patients included in the core set of Web of Science database from establishment of databases to March 31, 2023 was retrieved. The source countries, institutions, authors, key words and references of the literature were visually analyzed by CiteSpace software, and the scientific knowledge map was drawn.Results:A total of 472 articles were included, and the annual publication volume was increasing year by year. The top three countries in terms of publication volume were the United States (176 articles) , the United Kingdom (54 articles) and Australia (54 articles) . The top three institutions with the highest number of publications were La Trobe University, Catholic University of Leuven and Radboud University. The research in this field mainly focused on randomized controlled trials and Meta-analysis. The initial stage of the research mainly focused on the effect of Parkinson's disease on the body, later transferred to gait research, and in recent years, began to focus on the application of virtual reality technology in the home-based rehabilitation of Parkinson's disease patients.Conclusions:Home-based rehabilitation exercises for Parkinson's disease patients can be carried out in many ways, but it is limited by disease symptoms and other influencing factors. Virtual reality technology, telemedicine, wearable devices and other technologies can promote the development of home-based rehabilitation exercises. It is necessary to increase the research efforts of such technology in the future and explore comprehensive home-based rehabilitation exercises programs.
8.Hand, foot and mouth disease in Hubei province, 2009-2015: an epidemiological and etiological study

Objective To clarify the age patterns and types of differences so as to provide reference on prevention and interventions of hand,foot and mouth disease (HFMD) cases,in Hubei province.Methods We collected the HFMD case information of Hubei province from the Chinese National Notifiable Infectious Disease Reporting System in 2009-2015 while the information on pathogens from the laboratory monitoring system of Center for Disease Control and Prevention at all levels in Hubei province.All the data were stratified by age,disease severity,laboratory confirmation status,and serotypes of enterovirus.Results There were 495 783 reported HFMD cases from 2009 to 2015,in Hubei province,of which 1 045 were severe with 99 fatal.The annual notification rate was 1 231.0/106.HFMD cases were concentrated mainly in 0.5-5 year olds,with highest severity and mortality seen in 6-11 month-olds.The predominated pathogen in mild laboratory-confirmed cases each year,in order during 2009-2015 as:EV71,Cox A16,Cox A16,Cox A16,EV71,Cox A16 and other EV.HFMD showed semiannual peaks in April-June,November-December,and with more cases in the even years than in the odd years.Conclusions Children aged 0.5 to 5 years with 6 to 11 month-olds in particular,were the focused groups of attention in Hubei province.Our findings provided evidence for the improvement on monitoring program.Targeted intervention approaches should be strengthened to reduce the mortality and morbidity of HFMD in the province.
9.Health related quality of life on severe hand, foot and mouth disease patients

Objective To evaluate the health related quality of life (HRQoL) for severe hand,foot and mouth disease (HFMD) patients with different complications.Methods A national telephone interview under the EQ-5D proxy2 questionnaire (EQ-SD and EQ-VAS),was conducted to obtain the HRQoL of lab-confirmed severe HFMD patients,aged between six months and five-year-olds from the national communicable disease surveillance system from January 1,2012 to December 31,2013.Results A total of 685 severe HFMD cases were included in the study.A total of 456 (66.6%) of them were males with 75.8% of them younger than three years old.A total of 337 (49.2%) and 407 (59.4%) of the participants reported that they had problems in mobility or daily activities.A total of 569 (83.1%) and 616 (89.9%) of the cases reported having problems in pain/discomfort or anxiety/depression.The average EQ-5D and EQ-VAS scores were 0.58 ± 0.23 and 53.6 ± 25.7,both were positively associated with the duration of illness.Mean quality adjusted life years loss during the HFMD episode for the severe patients was (15.45 ± 13.75) years/1 000 persons.The QALY losses for severe patients with each of below complication were:respiratory diseases (11.17 ± 8.83) years/1 000 persons,aseptic meningitis (13.56 ± 11.99) years/1 000 persons,encephalitis/brainstem encephalitis/acute flaccid paralysis (AFP) (15.31 ± 12.63) years/1 000 persons,Myocarditis (17.28 ± 18.16) years/1 000 persons,pulmonary hemorrhage/pulmonary edema (17.34 ±14.98) years/1 000 persons,cardiopulmonary failure (25.47 ± 20.53) years/1 000 persons.Among patients with lab confirmed Entero virus A71 (EV71) (16.51 ± 14.48) years/l 000 persons,the QALY loss was seen higher than Coxsackie virus A16 (Cox A16) (13.02± 11.45) years/1 000 persons and other Enter virus (14.74 ± 14.22) years/1 000 persons (Z=11.83,P=0.003).Conclusion The HRQoL loss for severe HFMD patients substantially increased under complications exacerbation and related to the duration of illness.
10.Joint application of artificial dermis and autologous split thickness skin graft in scar plastic surgery

Objective To observe the clinical effects of dermis combined autologous transplants in patients with abnormal scar thickness skin graft.Methods 13 cases of scar deformity were selected during August 2011 to December 2014 in our hospital.In stage 1 procedure,the scars that affected the function and induced deformities were resected and then the artificial dermis covering the wound was placed;in stage 2,two weeks after the artificial dermis vascularization,autologous skin grafts were removed to repair the wound edge.Meanwhile,therapy of inhibiting scar after conventional treatment was conducted with funcitional exercise.Results 13 cases were scheduled to repair scar wounds,underwent surgery with more than 6 months follow-up observation.The dysfunctions caused by scar contracture and deformity were significantly improved;appearance,elasticity and appearance of the skin grafts were similar to autologous skin graft with the desired clinical effect.Conclusions Artificial dermis combined autologous skin graft treatment of patients with scar deformity can achieve similar clinical effects to traditional treatment methods.
