Polycystic ovary syndrome(PCOS) is a common reproductive endocrine and metabolic disorder, affecting about 6%-10% of women of childbearing age worldwide, and has always been a major challenge in clinical treatment. Guizhi Fulingwan, derived from the Synopsis of the Golden Chamber(《金匮要略》), has the effect of promoting blood circulation, removing blood stasis, and alleviating blockages. In recent years, many studies have found that it has a good therapeutic effect on PCOS. By sorting out the research literature on the mechanism of Guizhi Fulingwan treating PCOS, it is found that this formula can decrease the autophagy and apoptosis of ovarian granulosa cells, correct endocrine hormone disorder, improve the inflammatory environment, alleviate oxidative stress. Moreover, it regulates phosphatidylinositide 3-kinase(PI3K)/protein kinase B(Akt)/mammalian target of rapamycin(mTOR), lncRNA H19/miR-29b-3p, nuclear factor E2-related factor 2(Nrf2)/heme oxygenase-1(HO-1), PI3K/Akt/nuclear factor-κB(NF-κB), and other signaling pathways. And also effects the regulator expression of matrix metallopeptidase-9(MMP-9), matrix metalloproteinase inhibitor-1(TIMP-1), cytochrome P45019A1 enzyme(CYP19A1), glucose transporter protein 4(GLUT4), regulated on activation normal T cell expressed and secreted(RANTES), monocyte chemoattractant protein-1(MCP-1). Thus, Guizhi Fulingwan is confirmed to treat PCOS based on these molecular mechanisms. Besides, the main active ingredients of the formula such as cinnamic aldehyde, (+)-catechin, stigmasterol, also have the effect of anti-oxidant, anti-inflammatory, regulating mitochondrial function, regulating endocrine, and regulating metabolism. According to the summary of clinical literature on Guizhi Fulingwan, it is found that the use of the formula alone or in combination with other drugs can significantly alleviate the clinical symptoms and complications of PCOS. Thus, it is safe with no adverse reactions in long-term use, which is worthily further promoted in clinical application. This paper comprehensively analyzed the current research status of Guizhi Fulingwan for the treatment of PCOS through the mechanism and clinical studies, summarized the limitations in current research, and put forward suggestions, aiming to provide the strong literature support for the future in-depth research on and clinical application of the formula.

Objective:To investigate the quantification assessment of segmental volume after thigh liposuction utilizing three-dimensional(3D) scanning technology. Methods:This retrospective study was performed with the analysis of 3D scanning images of patients who had undergone bilateral thigh liposuction in Body Contouring & Fat Grafting Center, Plastic Surgery Hospital, Chinese Academy of Medical Sciences from January 2018 to September 2022. Preoperative and postoperative 3D scanning were performed to build visual 3D models of bilateral thighs. From top to the bottom, bilateral thighs were segmented into seven sections with a space of 5 cm in acquired 3D model. Certain measurements and calculation of preoperative and postoperative volume and volume change rate of the overall thigh and each segment were conducted, to validate the efficiency of liposuction (EOL). Additionally, EOL of each thigh segment was computed and the symmetry of bilateral thighs was analyzed before and after surgery. The volume differences were computed using the paired Wilcoxon rank-sum test, and intraclass correlation coefficient (ICC) was exerted to assess the symmetry of bilateral thighs before and after surgery. Results:A total of 36 female patients were included in the study, with an age range of 18 to 49 years and a mean age of (28.6±7.6) years. Follow-up duration ranged from 23 to 1 133 days postoperatively, with a mean follow-up period of 274.7 days. The results of 3D scanning measurements indicated significant changes ( P<0.01) in both the overall volume of the thigh and the volumes of each segment before and after surgery. The highest volume change rate and EOL were observed in the uppermost segment, and the volumetric change rate and EOL exhibited a descending trend across the segments of the thigh from the uppermost to the lowermost segments. The ICC of the volume of each segment consistently surpassed 0.950 whether preoperatively or postoperatively, indicating a high level of symmetry between the bilateral thighs, and the ICC of overall volume showed a notable increase from 0.992 preoperatively to 0.997 postoperatively. Conclusion:3D scanning technology can be exerted to quantify the volume changes before and after thigh liposuction. This study provided quantitative and objective evidence to confirm the efficacy of thigh liposuction procedure, elucidating that the most significant liposuction effects observed in the uppermost segment of the thigh. Moreover, postoperative assessments reveal a further enhancement in bilateral thigh symmetry.

Obesity and related metabolic syndromes have been recognized as important disease risks,in which the role of adipokines cannot be ignored.Adiponectin(ADP)is one of the key adipokines with various beneficial effects, including improving glucose and lipid metabolism, enhancing insulin sensitivity, reducing oxidative stress and inflammation, promoting ceramides degradation, and stimulating adipose tissue vascularity.Based on those,it can serve as a positive regulator in many metabolic syndromes, such as type 2 diabetes(T2D),cardiovascular diseases,non-alcoholic fatty liver disease(NAFLD),sarcopenia, neurodegenerative diseases, and certain cancers. Therefore,a promising therapeutic approach for treating various metabolic diseases may involve elevating ADP levels or activating ADP receptors. The modulation of ADP genes, multimerization, and secretion covers the main processes of ADP generation, providing a comprehensive orientation for the development of more appropriate therapeutic strategies. In order to have a deeper understanding of ADP,this paper will provide an all-encompassing review of ADP.

Objective:To investigate the levels of sex hormone and fertility in female patients after hematopoietic stem cell transplantation (HSCT), as well as their correlation with conditioning regimens, and analyse the effect of hormone replacement therapy (HRT) in young women after HSCT.Methods:Retrospective case series study. The clinical data of 147 women who underwent HSCT in the First Affiliated Hospital of Soochow University from January 2010 to January 2021 were retrospectively analyzed. The sex hormone levels were measured and followed-up, and the survival, menstrual fertility and the use of HRT of the patients were also followed-up. The sex hormone levels were measured after transplantation, and the ovarian function was evaluated. Independent sample t test and χ2 test were used for comparison between the two groups. Results:The median age of the 147 patients was 26 (range, 10-45) years. Of them, 135 patients received allogeneic HSCT and 12 patients received autologous HSCT. Furthermore, 129 patients received myeloablative conditioning, and 18 patients received reduced conditioning dose. The median follow-up time was 50 months (range, 18-134 months). Five patients died of disease recurrence during follow-up. Of the 54 patients with subcutaneous injection of zoladex, three recovered menstruation spontaneously after transplantation, and all of them were myeloablative conditioning patients, one patient gave birth to twins through assisted reproductive technology. Ninety-three patients did not use zoladex before conditioning, two patients with aplastic anemia with non-myeloablative transplantation resumed menstruation spontaneously, and conceived naturally. The level of follicle stimulating hormone after transplantation in patients receiving myeloablative conditioning regimen was significantly higher than that in patients receiving reduced-dose conditioning regimen [(95.28±3.94) U/L vs. (71.85±10.72) U/L, P=0.039]. Among 147 patients, 122 patients developed premature ovarian failure, 83 patients received sex hormone replacement therapy after transplantation, and 76 patients recovered menstruation and improved endocrine function. Conclusions:The incidence of premature ovarian failure is high in female patients after HSCT, and patients have a chance at natural conception. Reducing the dose of conditioning regimen and the application of zoladex before transplantation can reduce ovarian of conditioning drugs. HRT after transplantation can partially improve the endocrine function of patients.

In recent years, two novel proteins in the ribosomes of mycobacteria have been discovered by cryo-electron microscopy. The protein bS22 is located near the decoding center of the 30S subunit, and the protein bL37 is located near the peptidyl transferase center of the 50S subunit. Since these two proteins bind to conserved regions of the ribosome targeted by antibiotics, it is speculated that they might affect the binding of related drugs to these targets. Therefore, we knocked out the genes encoding these two proteins in wild-type Mycolicibacterium smegmatis mc2155 through homologous recombination, and then determined the growth curves of these mutants and their sensitivity to related antibiotics. The results showed that compared with the wild-type strain, the growth rate of these two mutants did not change significantly. However, mutant ΔbS22 showed increased sensitivity to capreomycin, kanamycin, amikacin, streptomycin, gentamicin, paromomycin, and hygromycin B, while mutant ΔbL37 showed increased sensitivity to linezolid. These changes in antibiotics sensitivity were restored by gene complementation. This study hints at the possibility of using ribosomal proteins bS22 and bL37 as targets for drug design.

Objective:To evaluate the effect of continuous pump ropivacaine in rectus sheath in the postoperative analgesia after abdominoplasty. Methods:Patients with abdominoplasty who were admitted to the Plastic Surgery Hospital of the Chinese Academy of Medical Sciences from August 2020 to December 2021 were retrospectively analyzed. They were divided into 2 groups: the control group received patient-controlled intravenous analgesia (PCIA) after the operation, sufentanil 100 μg + normal saline diluted to 100 ml, the rate was 2 ml/h, lasted for 48 h, locking time 15 min, Bolus: 0.5 ml; the study group underwent postoperative rectus abdominis intrathecal intubation and continuously infused 0.262 5% ropivacaine saline mixture 100 ml, the rate was 2 ml/h, lasted for 48 h, the locking time is 15 min, Bolus: 0.5 ml. The method of catheter placement in the study group: a catheter was placed unilaterally in the anterior sheath of the rectus abdominis under direct vision, and an incision was made 2 cm below the umbilical plane. The other end is pierced from the umbilicus and connected to the analgesic pump. The visual analogue scale (VAS) for pain and the number of analgesic pump boluses at 24 h and 48 h after operation, the occurrence of postoperative adverse drug reactions and the patient satisfaction at 1 month after operation were collected. The normally distributed measurement data were expressed as Mean±SD, and the comparison between the two groups was analyzed by t-test; the count data was expressed by the frequency (percentage), and the Fisher’s test was used for analysis. Results:A total of 56 patients were included, including 9 males and 47 females, ranging in age from 27 to 53 years old, with ASA graded from 1 to 2, body mass index (BMI) ranged from 26 to 33 kg/m 2. Their rectus abdominis separation ranged from 3 to 7 cm. Compared with the control group, the pain VAS score of the study group at 48 hours after the operation was lower (3.9±1.5 vs. 5.2±1.4, P<0.05), and the number of analgesic boluses was reduced (2.9±0.3 vs. 3.9±0.4, P<0.05), the incidence of adverse reactions such as nausea, vomiting decreased( P<0.05). The satisfaction rate 1 month after operation in the study group was significantly higher than that in the control group [100.0% (30/30) vs. 42.3% (11/26), P<0.05]. Conclusions:Continuous infusion of ropivacaine through anterior rectus intrathecal cannula can effectively relieve abdominal pain after total abdominoplasty, reduce adverse reactions caused by postoperative analgesics, improve patient comfort during hospitalization, and improve overall surgical satisfaction.

To describe the general and histological features of the full-length superficial fascia of the circumferential upper limb.

		                        			Methods:
Fresh frozen arm specimens were dissected, and then MRI imaging in vivo, enhanced CT angiography and HE histological staining were used to describe the characteristics of the full-length superficial fascia of the circumferential arm and its relationship with important blood vessels.

		                        			Results:
The four typical structures of the superficial fascia of the arm were divided into subcutaneous superficial fat, membrane-like substance, deep fat and deep fascia from superficial to deep. The thickness and stratification, fusion degree and histological characteristics of the superficial fascia of these four layers were obviously different in different levels and regions of the arm. MRI confirmed that the total thickness of superficial fascia gradually decreased from shoulder to wrist. Venography showed that the cephalic vein ran below the second layer of superficial fascia and above the deep fascia. The basilic vein originated from the dorsal vein network of the hand and always lied below the second layer of membranous material until the basilic vein penetrates below the deep fascia of the upper arm.

		                        			Conclusions
The deep understanding of the circumferential full-length of superficial fascia structure of the upper limb provides an important theoretical basis for improving the surgical safety and fine operation for the Dynamic Arm Circumferential Liposuction.
