1.Research advances in abnormal eye movements in multiple system atrophy

Multiple system atrophy (MSA) is a rare degenerative disease of the nervous system and has diverse and atypical clinical manifestations, and it overlaps with other diseases in α-synuclein spectrum disease. There are great challenges in the diagnosis and early differential diagnosis of the disease, and missed diagnosis and misdiagnosis occur from time to time, thereby delaying the treatment of the disease.Videonystagmography (VNG) is currently the main noninvasive test used to assess vestibular function and can provide different eye movement parameters. Studies have shown the presence of abnormal eye movements in patients with MSA. From the perspective of vision-eye movement, this article reviews the current status of research on eye movements in patients with MSA and reveals the internal connection between them, in order to provide an important reference for the early diagnosis of MSA.

2.Best evidences for postpartum follow-up in patients with gestational diabetes

Objective To summarise the best evidence for postpartum follow-up acquired from patients with gestational diabetes mellitus so as to provide an evidence-based reference for establishment of a postpartum follow-up program.Methods Literature on postpartum blood glucose follow-up in patients with gestational diabetes mellitus was retrieved from 17 major databases including BMJ Best Practice,UpToDate,Joanna Briggs Institute(JBI),International Guide Collaboration Network,National Institute for Health and Care Excellence(NICE),American Diabetes Association website,Medlive,Cochrane Library,Embase,OVID,PubMed,Web of Science,Quanfang local PubMed,CNKI,Wanfang Data,VIP,and Sinomed from the inception of databases to July 2023.The literature to be retrieved included guidelines,expert consensus,evidence summaries,systematic reviews,clinical decisions,and the original studies.Results A total of 9 articles,five practice guidelines,a systematic review,an evidence summaries,an expert consensus and a clinical decisions,were included.Totally,17 pieces of best evidence were summarised from the five aspects:postpartum blood glucose,lifestyle guidance,breastfeeding,drug treatment and health education.Conclusions The summarised best evidences can provide evidence-based references for postpartum follow-up of patients with gestational diabetes.Nursing staff in hospitals and community health centres should formulate relevant follow-up plans according to the actual postpartum conditions of patients in order to prevent the incidence of postpartum Type Ⅱ diabetes.
3.Study on Objective Recognition and Color Classification of Sublingual Veins

Objective To explore the method of objective identification of color information in sublingual veins diagnosis of TCM.Methods Combined with computer vision,compact fully convolution networks(CFCNs)and 19 deep learning classification models were used for study,and a double pulse rectangle algorithm was designed as a means of segmentation and recognition of sublingual veins and color information extraction.Results The accuracy of segmentation of tongue bottom obtained by the method of removing reflection + data expanding + data post-processing was 0.955 9,F1 value was 0.947 3,and mIoU value was 0.900 0.The accuracy of segmentation of sublingual veins obtained by the method of removing reflection + tongue input + data expanding + corrosion expansion was 0.778 4,F1 value was 0.738 3 and mIoU value was 0.585 1,which were obviously superior to the current classic or improved U-net model.On the color classification of sublingual veins,the best classification model was DenseNet161-bc-early_stopping with an accuracy rate of 0.803 7.Conclusion The deep learning method has a certain effect on identifying the color information of sublingual veins in TCM,which provides a new method for the research of quantitative color detection technology of sublingual veins diagnosis in TCM.
4.Non-pharmaceutical intervention of Traditional Chinese Medicine in stable chronic obstructive pulmonary disease: a best evidence summary

Objective:To systematically review, assess, extract, and summarize the most effective evidence on non-pharmacological traditional Chinese medicine interventions for promoting pulmonary rehabilitation in stable patients with chronic obstructive pulmonary disease, aiming to provide a theoretical foundation for clinical nurses to implement traditional Chinese medicine nursing interventions.Methods:According to the "6S" evidence model, relevant evidence on non-pharmacological traditional Chinese medicine interventions for stable chronic obstructive pulmonary disease was retrieved from domestic and foreign databases, guideline websites and professional society websites, including clinical decisions, guidelines, evidence summaries, systematic reviews, expert consensus, etc. The retrieval time was from January 1, 2018 to August 10, 2023. Two researchers evaluated the quality of the included literature, and extracted data and summarized evidence.Results:A total of 23 articles were included, including 6 guideline, 1 evidence summary, 7 systematic reviews, 8 Meta analysis and 1 expert consensus. The 37 evidences were summarized from 11 aspects, including target population, intervention focus, moxibustion therapy, acupoint sticking therapy, transcutaneous electrical acupoint stimulation, acupuncture therapy, acupoint injection therapy, traditional Chinese exercises, diet therapy, follow-up procedures, safety considerations.Conclusions:This study summarizes the evidences of traditional Chinese medicine non-drug pulmonary rehabilitation in the stable stage of chronic obstructive pulmonary disease. When applying the evidence in clinical practice, the traditional Chinese medicine nursing ability of nurses and the actual situation of patients should be fully considered to formulate an individualized traditional Chinese medicine nursing plan.
5.Qualitative research on influencing factors of hospice care carried out by clinical nurses in general hospitals under the mode of hospice shared care model

Objective:To explore the influencing factors of clinical nurses in general hospitals to carry out hospice care under hospice shared care model, and further improve the quality of hospice care in general hospitals under this mode.Methods:Using the descriptive phenomenological research method, 13 clinical nurses from Sir Run Run Shaw Hospital Affiliated to Medical School of Zhejiang University were selected by purposive sampling from June to September 2023. Semi-structured interviews were conducted based on the capacity opportunity-motivation-behavior model, and Colaizzi′s 7-step analysis method was used to summarize and refine the theme.Results:There were 3 males and 10 females, aged 24-38 years old. Three themes and 11 sub-themes were extracted in this study: ability factors (theoretical knowledge cannot meet clinical needs, lack of clinical practice ability, insufficient self emotional regulation ability), motivation factors (willingness to carry out hospice shared care model, sense of professional benefit, professional identity, perception of care pressure, hospice care self-efficacy), opportunity factors (the influence of Confucian culture, systematic construction of hospice shared care model in hospitals, and multiple external support).Conclusions:Nurses' hospice care under hospice shared care model is affected by internal and external factors. Nurses can learn through multiple channels, find positive work experience, and use multi-dimensional support system to provide high-quality hospice care services for patients.
6.Development of A Predictive Model for Adverse Inhalation Risk in COPD Inhaler Therapy Using Machine Learning Algorithms

Objective To construct and validate a risk prediction model for poor inhalation in chronic obstructive pulmonary disease(COPD)patients receiving inhaler therapy,providing a decision support tool for personalized prevention of poor inhalation.Methods A cross-sectional study was conducted to collect data related to COPD patients receiving inhaler therapy,forming a dataset.The dataset was randomly divided into a training set and a test set in a ratio of 4∶1.Four different methods for missing value imputation,3 methods for variable feature selection,and 18 machine learning algorithms were employed to successfully construct 216 models on the training set.The monte carlo simulation method was used for resampling in the test set to validate the models,with the area under curve(AUC),accuracy,precision,recall,and F1 score used to evaluate model performance.The optimal model was selected to build the poor inhalation prediction platform.Results A study involving 308 patients with COPD found that 135(43.8%)were at risk of adverse inhalation.Using 33 predictor variables,216 risk prediction models were developed.Of these models,the ensemble learning algorithm yielded the highest average AUC of 0.844,with a standard deviation of 0.058[95%CI=(0.843,0.845)].The differences in predictive performance among the 216 models were statistically significant(P<0.01).Under the ensemble learning algorithm,adherence to inhaler use(38.087 4%),inhaler satisfaction(25.680 1%),literacy(24.031 3%),number of inhalers(5.482 3%),age(4.204 5%)and number of acute exacerbations in the past year(2.184 7%)contributed most to the predictive model.The model exhibited superior performance,with an AUC of 0.869 3,an accuracy of 83.87%,a precision of 86.96%,a recall of 74.07%,and an F1 score of 0.8.Conclusion This study has developed a predictive model for poor inhalation risk in COPD inhaler therapy patients using machine learning algorithms,which exhibits strong predictive capabilities and holds potential clinical application value.
7.Study on the Correlation between Serum miR-24-3p and MARK4 Level Expressions and Cardiac Function in Patients with Coronary Atherosclerotic Heart Disease

Objective To investigate the correlation between serum microRNA(miR)-24-3p,microtubule affinity-regulating kinase 4(MARK4)level expression and cardiac function in serum of patients with coronary atherosclerotic heart disease(CHD).Methods From August 2021 to April 2023,138 CHD patients who visited Sichuan Provincial People's Hospital were collected as the study subjects(CHD group).According to the New York Heart Association(NYHA)grading,patients in the CHD group were separated into grade Ⅱ(n=39),grade Ⅲ(n=68),and grade Ⅳ(n=31).Additionally,123 individuals who underwent physical examinations were selected as the health group during the same period.Real-time fluorescence quantitative PCR(qRT-PCR)method was applied to detect the expression levels of miR-24-3p and MARK4.Cardiac function indicators in the CHD and health groups were detected.Pearson method was applied to analyze the correlation among serum miR-24-3p,MARK4,and cardiac function indicators.Results Compared with the healthy group,the levels of MARK4(1.19±0.21 vs 1.00±0.20),left ventricular end-systolic dimension(LVESd),left ventricular enddiastolic dimension(LVEDd),interventricular septal thickness(IVS),left ventricular end-diastolic volume(LVEDv),left ventricular end-systolic volume(LVESv),and left ventricular mass(LV mass)in the CHD group were increased(t=7.462,18.242,23.888,9.941,2.812,12.520,11.029),while the levels of miR-24-3p(0.62±0.11 vs 1.00±0.18),left ventricular ejection fraction(LVEF),and cardiac output(CO)were reduced(t=20.821,10.212,18.188),and the differences were statistically significant(all P<0.05).As cardiac function grading increased,MARK4 expression levels(1.09±0.19,1.18±0.17,1.32±0.22),LVESd,LVEDd,IVS,LVEDv,LVESv and LV mass were increased(F=13.025,10.606,11.920,57.956,29.680,21.253,12.954),and miR-24-3p expression levels(0.84±0.15,0.61±0.11,0.37±0.08),LVEF and CO were decreased(F=139.227,9.720,13.411),and the differences were statistically significant(all P<0.05).Pearson correlation analysis showed a negative correlation between miR-24-3p and MARK4 expression in the serum of patients with CHD(r=-0.540,P<0.05).The relative expression of miR-24-3p in the serum of patients with CHD was negatively correlated with LVESd,LVEDd,IVS,LVEDv,LVESv and LV mass(r=-0.656,-0.636,-0.617,-0.576,-0.492,-0.687,all P<0.05),but positively correlated with LVEF and CO(r=0.570,0.683,all P<0.05).The relative expression level of MARK4 was positively correlated with LVESd,LVEDd,IVS,LVEDv,LVESv,and LV mass(r=0.503,0.542,0.508,0.624,0.516,0.560,all P<0.05),but negatively correlated with LVEF and CO(r=-0.594,-0.525,all P<0.05).Conclusion The expression of miR-24-3p in the serum of CHD patients was decreased,while the expression of MARK4 was increased.There was an obvious correlation between the two and cardiac function indicators.
8.Clinical characteristics and prognosis of 8 cases of severe infant botulism

Objective:To summarize the clinical characteristics and prognosis of severe infant botulism and evaluate the therapeutic effect of botulinum antitoxin in the pediatric intensive care unit (PICU).Methods:The clinical data of 8 cases diagnosed with infantile botulism were retrospectively analyzed in the PICU of Beijing Children′s Hospital from October 2019 to August 2023. Data of basic demographic information, clinical manifestations, laboratory tests, treatment and prognosis of each child were collected and analyzed using descriptive statistical methods.Results:Eight laboratory-confirmed cases of infant botulism were included in this study, all of which were male infants with an age of 6.0 (3.3,6.8) months. Three of the children were from Inner Mongolia Autonomous Region, 2 of them were from Hebei, and the other 3 were from Beijing, Shandong and Xinjiang Uyghur Autonomous Region, respectively. All the patients were previously healthy. In 4 of these cases, the possible cause was the ingestion of either honey and its products or sealed pickled food by the mother or child before the onset of the disease. The first symptom was poor milk intake (4 cases), followed by shallow shortness of breath (7 cases), limb weakness (7 cases) and so on. The typical signs were bilateral dilated pupils (8 cases) and decreased limb muscle strength (8 cases). The main subtype was type B (7 cases), and only 1 case was classified as type A. Six of the children were treated with antitoxin therapy for a duration of 24 (19, 49) d. Seven of them had invasive mechanical ventilation. All the patients survived upon discharge with a follow-up period of 29 d to 3 years and 8 months. Six patients had fully recovered, and 2 recently discharged patients were gradually recovering.Conclusions:For infants with suspected contact or ingestion of botulinum and presented with bilateral pupillary paralysis, muscle weakness and clear consciousness, the stool should be collected for diagnostic testing using a mouse bioassay as soon as possible. Type B was the most common type. The antitoxin treatment was effectiveness and the prognosis was well.
9.Effect of granulocyte colony-stimulating factor combined with tadalafil on endometrial receptivity and pregnancy outcomes in patients with thin endometrium

Objective To investigate the effects of granulocyte colony-stimulating factor(G-CSF)combined with tadalafil(TD)on endometrial receptivity and pregnancy outcomes in patients with thin endometrium(TE).Methods Patients with TE in the department of reproductive medicine of the First Hospital of Qinhuangdao from May 2020 to March 2023 were selected as the study subjects.They were divided into a experimental group(G-GSF combined with TD)and a control group(G-GSF)according to the different treatment regimens.The endometrial receptivity[endometrial thickness(EMT),endometrial volume(EMV),endometrial blood flow peak systolic flow rate/end diastolic flow rate(EBF-S/D),endometrial fractionation,endometrial blood flow fractionation,uterine artery pulsatility index(AUPI),uterine artery resistance index(AURI)and uterine arterial peak systolic flow rate/end diastolic flow rate(AU-S/D)]of the TE patients before and after treatment(the endometrial transition day)were compared;the endometrial receptivity on endometrial transformation day,post-treatment pregnancy outcomes[embryo implantation rate(EIR),clinical pregnancy rate(CPR),and early miscarriage rate(ABR)],and the incidence of adverse reactions during treatment were compared between the two groups.Results A total of 60 patients were included in the study,with 30 in each group.Before treatment,the difference in endometrial receptivity between the two groups was not statistically significant(P>0.05).Compared with the pre-treatment period,the EMT,EMV,the proportion of type A endometrium and the proportion of type Ⅱ+Ⅲ endometrial blood flow significantly increased in the two groups after treatment on the endometrial transformation day increased significantly(P<0.05),while the EBF-S/D,AUPI,AURI and AU-S/D significantly decreased(P<0.05).EMT and EMV were greater in the experimental group than in the control group(P<0.05),whereas EBF-S/D,AUPI,AURI and AU-S/D were less than those in the control group(P<0.05).Compared with the control group,the differences in the proportion of endometrium,the proportion of endometrial blood flow,EIR,CPR and ABR in the experimental group were not statistically significant(P>0.05).No adverse reactions occurred during treatment in both groups.Conclusion G-CSF combined with TD can improve endometrial receptivity in TE patients with high safety,but there is no effect on pregnancy outcome.
10.The Circadian System Is Essential for the Crosstalk of VEGF-Notch-mediated Endothelial Angiogenesis in Ischemic Stroke.

Ischemic stroke is a major public health problem worldwide. Although the circadian clock is involved in the process of ischemic stroke, the exact mechanism of the circadian clock in regulating angiogenesis after cerebral infarction remains unclear. In the present study, we determined that environmental circadian disruption (ECD) increased the stroke severity and impaired angiogenesis in the rat middle cerebral artery occlusion model, by measuring the infarct volume, neurological tests, and angiogenesis-related protein. We further report that Bmal1 plays an irreplaceable role in angiogenesis. Overexpression of Bmal1 promoted tube-forming, migration, and wound healing, and upregulated the vascular endothelial growth factor (VEGF) and Notch pathway protein levels. This promoting effect was reversed by the Notch pathway inhibitor DAPT, according to the results of angiogenesis capacity and VEGF pathway protein level. In conclusion, our study reveals the intervention of ECD in angiogenesis in ischemic stroke and further identifies the exact mechanism by which Bmal1 regulates angiogenesis through the VEGF-Notch1 pathway.
