1.Simultaneous content determination of eleven constituents and their inhibitory effects on pancreatic lipase for Sanzi Yangqin Decoction

AIM To simultaneously determine the contents of 3,6'-disinapoyl sucrose,sibiricose A5,sibiricose A6,ferulic acid,sinapic acid,caffeic acid,luteolin,progoitrin,apigenin,isorhamnetin and 3,4-dihydroxybenzaldehyde in Sanzi Yangqin Decoction,and to investigate their inhibitory effects on pancreatic lipase.METHODS The high-performance liquid chromatography-triple quadrupole tandem mass spectrometry(HPLC-QQQ-MS/MS)analysis was performed on a 25 ℃ thermostatic Halo-C18column(2.1 mmx100 mm,2.7 μm),with the mobile phase comprising of water(containing 0.05%formic acid)-acetonitrile(containing 0.05%formic acid)flowing at 0.3 mL/min in a gradient elution manner,and electron spray ionization source was adopted in negative ion scanning with multiple reaction monitoring mode.The simulation was performed by molecular docking,after which 4-nitrophenyl butyrate method was adopted in the evaluation of inhibitory effects.RESULTS Eleven constituents showed good linear relationships within their own ranges(R2>0.992 0),whose average recoveries were 85.79%-117.52%with the RSDs of 0.78%-3.73%.3,6'-Disinapoyl sucrose,caffeic acid,sinapic acid and 3,4-dihydroxybenzaldehyde demonstrated good affinities with pancreatic lipase,the IC50 values of Sanzi Yangqin Decoction,caffeic acid,sinapic acid and 3,4-dihydroxybenzaldehyde were 41.16 mg/mL,4.006 mmol/L,6.798 mmol/L and 10.48 mmol/L,respectively.CONCLUSION This stable and reliabe method can provide theoretical basis for the quality control of Sanzi Yangqin Decoction with inhibitory effects on pancreatic lipase.
4.CIC-rearranged sarcoma with rhabdoid features: a clinicopathological analysis.

Objective: To investigate the histopathologic, immunohistochemical, molecular genetic characteristics of CIC-rearranged sarcoma (CRS) with rhabdoid features. Methods: The clinical and pathologic data of two cases of CRS diagnosed between 2019 and 2021 at the Department of Pathology, Jiangsu Province Hospital were analyzed. Immunohistochemical study and fluorescence in situ hybridization (FISH) were performed. The relevant literature was reviewed. Results: Both patients were female, one was 58 years old, with tumor located in left thigh; the other was 43 years old, with tumor located in left pelvic cavity. Microscopically, both tumors were composed of small to medium-sized round, oval cells, arranged in nodules or sheets. The tumor cells showed irregular nuclear outline, coarse chromatin with prominent nucleoli and brisk mitotic activity. Both cases showed rhabdoid phenotype with myxoid stromal changes. Immunohistochemically, both cases were positive for CD99 and c-myc. High WT1 reactivity was seen in classic area, with low reactivity in rhabdoid area. There was no INI1 lost in both cases. Both were negative for NKX2.2 and NKX3.1. By FISH both cases demonstrated convincing break-apart signals of CIC gene. One patient died of disease after 1 month, and the other died of disease after 3 months. Conclusions: CRS is a small round cell undifferentiated sarcoma of the bone and soft tissue defined by molecular genetic characteristics, and may show atypical morphologic and immunophenotypic characteristics such as rhabdoid features. A correct understanding of its rare morphologic and immunophenotypic characteristics, combined with molecular pathologic detection, is conducive to correct diagnosis.

5.Diagnostic value of rapid antigen testing for the detection of SARS-CoV-2 infection.

Objective: To investigate the diagnostic value of rapid antigen test based on colloidal gold immunochromatographic assay for the detection of SARS-CoV-2 infection in symptomatic patients. Methods: From May 20 to June 5 2022, 76 hospitalized children and their 55 accompanying family members with confirmed SARS-CoV-2 infection in the COVID-19 isolation unit of the Children's Hospital of Fudan University (designated referral hospital for SARS-CoV-2 infection in Shanghai) enrolled. Their nasopharyngeal swab specimens were consecutively collected. The samples were tested for SARS-CoV-2 nucleic acid by real-time quantitative. SARS-CoV-2 antigen was tested by immunochromatography. The correlation between the antigen detection results and the change of the cycle threshold (Ct) values were evaluated, as well as the sensitivity and specificity of SARS-CoV-2 antigen detection at different periods after the onset of the disease. Kappa consistency test was conducted to investigate the consistency between the 2 diagnostic methods. Results: Of the enrolled SARS-CoV-2 symptomatic infections, 76 were children, including 41 males and 35 females, with an age of 5 (2, 9) years; 55 were accompanying families, including 8 males and 47 females, with an age of 38 (32, 41) years. All 478 samples were simultaneously tested for SARS-CoV-2 antigen and nucleic acid. In any period from disease onset to negative conversion of viral nucleic acid, the overall sensitivity of the rapid antigen test was 48.2% (119/247), the specificity was 98.3% (227/231), and antigen test and nucleic acid test showed moderate consistency (κ=0.46, P<0.05). The sensitivity of antigen test was 100% (82/82) when the Ct value was ≤25. And the sensitivity of antigen test was 8/10, 4/15 and 8.3% (3/36) when the Ct value was 26, 30 and 35, respectively. All antigen tests were negative when Ct value was >35. During the period of 1-2 days, 3-5 days, 6-7 days, 8-10 days and >10 days after onset, the sensitivity and specificity of SARS-CoV-2 antigen test were 5/8 and 5/5, 90.2% (37/41) and 5/5, 88.9% (24/27) and 2/5, 45.0% (36/80) and 94.1% (32/34), 18.7% (17/91) and 98.9% (183/185) respectively. The Ct values of nasopharyngeal swabs were<26 during 2 to 7 days after onset, 28.7±5.0 on day 8, 34.5±2.9 on day 13 and > 35 after 14 days, respectively . Conclusion: SARS-CoV-2 antigen test in the patients with SARS-CoV-2 infection shows acceptable sensitivity and specificity within 7 days after onset of disease, and the sensitivity was positively correlated with viral load and negatively correlated with onset time.

6. Advances in effect of peroxisome proliferators-activated receptors in ischemic stroke

Ischemic stroke(IS)is one of the leading causes of mortality and disability with a high incidence and recurrence rate.However, effective therapy for treating IS is still unavailable in clinic.Peroxisome proliferation-activated receptors(PPARs)is a type of ligand-activated nuclear transcription factors that play a key role in a variety of biological processes.PPARs are close to IS, hence this study reviews that PPARs exerts the protective effect on IS through mediating neuroinflammation, oxidative stress, apoptosis, autophagy, demyelination, blood brain barrier function, encephaledema and lactic acid metabolism, which hopes to provide novel strategies for the prevention and treatment of IS.

OBJECTIVE:
To compare the clinical effects of different anterior surgical methods in treating single segment cervical disc herniation.
METHODS:
The clinical data of 46 patients with single-segment cervical disc herniation underwent surgical treatment from September 2013 to September 2018 were retrospectively analyzed. The patients were divided into three groups according to different surgical methods. RESULTS:
All 46 patients were followed up for 12 to 24 (17.57±3.15)months. The follow-up time of APECD, CDR, ACDF groups were (17.30±3.25), (17.80±3.16), (17.85±2.88) months, and operation time were (95.48 ±13.85), (58.50±7.09), (76.00±15.72) min, respectively, there were no significant differences in follow-up time and operation time between two groups(
CONCLUSION
The three anterior surgical approaches can achieve satisfactory clinical results for the treatment of single-segment cervical disc herniation. However, the improvement rate of the CDR group and the activity of the retained responsibility segment are better than those of the other two groups. APECD surgery may have recurrence.

9.Effect of Sishenwan, Baitouweng Tang, and Lianlitang on Ulcerative Colitis in Rats

Objective:To explore the mechanism of Sishenwan, Baitouweng Tang, and Lianlitang in the treatment of ulcerative colitis (UC), and compare their efficacies on UC in rats. Method:Ninety SD rats of SPF grade were randomly divided into blank group (distilled water, 2 mL·d-1) and experimental group. The rats in the experimental groups were administered with 2,4,6-trinitrobenzene sulfonic acid (TNBS) by clysis to induce the UC model. Subsequently, the model rats were divided into a model group (distilled water, 2 mL·d-1), positive group [sulfasalazine (SASP), 0.4 g·kg-1·d-1], Sishenwan group (1.76 g·kg-1·d-1), a Baitouweng Tang group (1.40 g·kg-1·d-1), and Lianlitang group (2.13 g·kg-1·d-1) according to the random number table. The rats in each group were dosed at 2 mL·d-1 for 14 days. The pathological score for colonic mucosa was detected. Cytokines were detected by the cytokine chip. The enzyme-linked immunosorbent assay (ELISA) was used to detect the free triiodothyronine (FT3), glucagon-like peptide-1 (GLP-1), and corticosterone (CORT) in plasma, and neurotensin (NT), substance P (SP), vasoactive intestinal peptide (VIP), and somatostatin (SST) in colon tissues. Objective:
Several COVID-19 patients have overlapping comorbidities. The independent role of each component contributing to the risk of COVID-19 is unknown, and how some non-cardiometabolic comorbidities affect the risk of COVID-19 remains unclear.
Methods:
A retrospective follow-up design was adopted. A total of 1,160 laboratory-confirmed patients were enrolled from nine provinces in China. Data on comorbidities were obtained from the patients' medical records. Multivariable logistic regression models were used to estimate the odds ratio (
Results:
Overall, 158 (13.6%) patients were diagnosed with severe illness and 32 (2.7%) had unfavorable outcomes. Hypertension (2.87, 1.30-6.32), type 2 diabetes (T2DM) (3.57, 2.32-5.49), cardiovascular disease (CVD) (3.78, 1.81-7.89), fatty liver disease (7.53, 1.96-28.96), hyperlipidemia (2.15, 1.26-3.67), other lung diseases (6.00, 3.01-11.96), and electrolyte imbalance (10.40, 3.00-26.10) were independently linked to increased odds of being severely ill. T2DM (6.07, 2.89-12.75), CVD (8.47, 6.03-11.89), and electrolyte imbalance (19.44, 11.47-32.96) were also strong predictors of unfavorable outcomes. Women with comorbidities were more likely to have severe disease on admission (5.46, 3.25-9.19), while men with comorbidities were more likely to have unfavorable treatment outcomes (6.58, 1.46-29.64) within two weeks.
Conclusion
Besides hypertension, diabetes, and CVD, fatty liver disease, hyperlipidemia, other lung diseases, and electrolyte imbalance were independent risk factors for COVID-19 severity and poor treatment outcome. Women with comorbidities were more likely to have severe disease, while men with comorbidities were more likely to have unfavorable treatment outcomes.
